Incontinence of urine due to instability of micturition reflexes. Part II. Pudendal nucleus instability.

Overinhibition or underfacilitation of the pudendal nucleus may cause profound involuntary relaxation of the striated muscles of the pelvic floor and perineum, including the "external" striated urethral sphincter. Pudendal nucleus instability may adversely affect urethral closure pressure and preclude a successful voluntary "recovery" effort to limit stress incontinence. It may also result in the involuntary loss of an important source of inhibition to the sacral micturition reflex center, which can lead to detrusor instability. The bilateral reciprocal relationship which exists between the de

[Spa treatment of sciatic pains at Swieradów].

The authors report therapeutic results obtained in 190 patients with sciatic pains (110 men and 80 women). In 115 cases discopathy was the cause of sciatica, in 75 degenerative vertebral disease was present. The duration of the disease ranged from 6 months to 36 years. Four sets of procedures were used depending on the manifestations of the disease. In evaluation of therapeutic effects the mobility range of the spine, the tone of paravertebral muscles and disappearance of pain were taken into account. Complete improvement was obtained in 76% of cases on the average. It was observed that radon

Muscle and tendon stiffness in patients with upper motor neuron lesion following a stroke.

The objective of this study was to investigate muscle and tendon stiffness in the triceps surae muscles in patients who had previously had a stroke. The participants were 12 men showing slight to moderate degrees of muscle tonus in the affected leg. All patients showed minimal or no overt clinical motor symptoms, and all walked without mechanical aid. Muscle strengths in isometric and isokinetic activities were measured, as was passive resistance during plantarflexion in each leg. Walking speed was also measured. Evaluations of physical performance and muscle tone were made. Muscle and tendon

[Subclinical swallowing disorder in patients with abnormal sensation in the throat].

Measurement of swallowing pressure was done in 75 patients who complained of abnormal sensation such as obstructive sensation, difficulty in swallowing, etc. in the throat without noticeable cause. Abnormal swallowing pressure curves were observed in 33 patients (44%), and 76% of them showed abnormal elevation of the pressure at the hypopharyngeal or cervical esophageal region. These results indicated that subclinical swallowing disorder, particularly abnormal high swallowing pressure, can be a possible cause of their complaints. The author assumed that abnormal elevation of swallowing pressur

Ritodrine-induced rhabdomyolysis, infantile myotonic dystrophy, and maternal myotonic dystrophy unveiled.

A primiparous pregnant woman in remission of myositis suffered very acute-onset ritodrine-induced rhabdomyolysis. At 29 gestational weeks, ritodrine was administered for threatened preterm labor. Just 3 h later, she complained of severe limb muscle pain, with serum creatinine phosphokinase elevated to 32 019 U/L and myoglobinuria. The muscle pain disappeared immediately after ceasing administration of ritodrine. At 31 weeks, premature rupture of the membranes occurred, necessitating cesarean section, yielding a baby with weak tonus, and the presence of infantile muscle diseases was suspected.

THE MASTICATORY SYSTEM OF THE OBESE: CLINICAL AND ELECTROMYOGRAPHIC EVALUATION.

UNLABELLED: Masticatory performance is determined not only through the speed of mastication, or by the quantity of food ingested; it also depends on the structures and functional integration of the stomatognathic system (SS). OBJECTIVES: This study investigated differences in the SS and orofacial motricity between obese and normal--weight women. METHOD: A total of 18 obese women, with an average age of 28 ± 7.3 years and an average body mass index (BMI) of 37.4 ± 5.1 Kg/m2, and 18 normal--weight women, with an average age of 26 ± 7.6 years and an average BMI of 20.7 ± 1.8 kg/m2, took part in

The role of laryngeal hypersensitivity in dysphagia: A scoping review with embedded stakeholder involvement.

BACKGROUND: Based on stakeholder conversations, speech and language therapists have anecdotally observed cases of dysphagia with suspected laryngeal hypersensitivity. These presentations have been coined as 'muscle tension dysphagia' often with concurrent laryngeal sensitivity symptoms. However, the role of laryngeal hypersensitivity in dysphagia is not fully understood. Knowledge user engagement highlights a lack of clarity around diagnosis and investigative approaches, limited clinical guidance and inconsistent practice, which leads to inequity in service delivery and care. AIM: To identify

Acoustic and Physiologic Correlates of Vocal Effort in Individuals With and Without Primary Muscle Tension Dysphonia.

OBJECTIVES: The aims of this study were to determine relationships between vocal effort and (a) acoustic correlates of vocal output and (b) supraglottic compression in individuals with primary muscle tension dysphonia (pMTD) and without voice disorders (controls) in the context of a vocal load challenge. METHOD: Twenty-six individuals with pMTD and 35 vocally healthy controls participated in a 30-min vocal load challenge. The pre- and postload relationships among self-ratings of vocal effort, various acoustic voice measures, and supraglottic compression (mediolateral and anteroposterior) were

Visceral and somatic disorders: tissue softening with frequency-specific microcurrent.

Frequency-specific microcurrent (FSM) is an emerging technique for treating many health conditions. Pairs of frequencies of microampere-level electrical stimulation are applied to particular places on the skin of a patient via combinations of conductive graphite gloves, moistened towels, or gel electrode patches. A consistent finding is a profound and palpable tissue softening and warming within seconds of applying frequencies appropriate for treating particular conditions. Similar phenomena are often observed with successful acupuncture, cranial-sacral, and other energy-based techniques. This

INTRODUCTION: Occupational or professional dystonia is a focal motor dystonic disorder which affect motor programs necessaries for the exercise of the patient's profession. Clinical case. A patient with writer's cramp in the childhood presented at the age of 22 years a new dystonia, which we name 'butcher's spasm', giving rise to a laboral incapacity. Neurological exam was normal except for right arm distal (writer's cramp) and proximal dystonias (butcher's cramp), with intentional tremor in the upper extremities. We classified, after complementary exams, the disease as idiopathic. CONCLUSION

Quantification of False Vocal Fold Hyperfunction During Quiet Breathing in Muscle Tension Dysphonia.

BACKGROUND/OBJECTIVES: False vocal fold (FVF) hyperfunction during phonation is thought to be a diagnostic sign of primary muscle tension dysphonia (pMTD). However, hyperfunctional patterns with phonation are also observed in typical speakers. This study tested the hypothesis that FVF posturing during quiet breathing, as measured by the curvature of FVF, could differentiate patients with pMTD from typical speakers. METHODS: Laryngoscopic images were collected prospectively in 30 subjects with pMTD and 33 typical speakers. Images were acquired at the end of expiration and maximal inspiration d

Differential diagnosis of muscle tension dysphonia and spasmodic dysphonia.

PURPOSE OF REVIEW: Muscle tension dysphonia (MTD) can mimic the voice features of adductor spasmodic dysphonia (ADSD) leading to diagnostic confusion. Researchers have begun to compare characteristics of MTD and ADSD to determine whether there are markers which reliably distinguish the two disorders and lead to improved differential diagnosis. RECENT FINDINGS: Differences between MTD and ADSD have been identified during fiberoptic laryngoscopy, phonatory airflow measurement, acoustic analysis, and variable sign expression based upon phonatory task. In general, evidence of task-dependent sign

Crown-root Fracture Restoration on a Patient with Autism Spectrum Disorder.

Children with intellectual and physical disabilities including autism are susceptible to dental trauma as a sequela from falls due to poor muscular coordination. In addition, their altered muscle tonus often results in an open bite with labial flaring of the maxillary incisors and lip incompetence, predisposing these teeth to fractures. This case report describes an alternative approach of restoring a fractured maxillary permanent central incisor with a composite strip crown during surgical repositioning of the periodontium on an autistic patient. The prognosis of the incisor is guarded due to

Masseteric muscle spasm as a normal response to suxamethonium.

The myotonic response of the masseter muscles following various doses of suxamethonium was measured using a unique myotonometer in 50 apparently healthy patients, the majority of whom showed a myotonic response which lasted less than 100 s. A maximum increase in masseter tone was found to be greater than 1 kg in five patients, and 12 patients developed a maximum tone of greater than 500 g. An increased masseter tone of this degree might be interpreted as "masseteric muscle spasm" by the anaesthetist. The value of masseteric muscle "spasm" as an early sign of malignant hyperthermia is questione

Stiffness of the Masseter Muscle in Children-Establishing the Reference Values in the Pediatric Population Using Shear-Wave Elastography.

In children, the quality and muscle function are altered in many pathologic conditions, including temporomandibular disorders. Although several methods have been used to evaluate muscle tonus, none became a golden standard. Moreover, the masseter muscle characteristics in children have not been investigated to date. This study aimed to measure the stiffness of the masseter muscle using shear-wave elastography in healthy children. We enrolled 30 healthy children (mean age 10.87 ± 3.38 years). The stiffness of masseter muscles was measured with shear wave elastography. Stiffness for the total sa
