Correlation between Elbow Flexor Muscle Strength and Needle Electromyography Parameters after Musculocutaneous Nerve Injury.

OBJECTIVES: To explore the changes of elbow flexor muscle strength after musculocutaneous nerve injury and its correlation with needle electromyography (nEMG) parameters. METHODS: Thirty cases of elbow flexor weakness caused by unilateral brachial plexus injury (involving musculocutaneous nerve) were collected. The elbow flexor muscle strength was evaluated by manual muscle test (MMT) based on Lovett Scale. All subjects were divided into Group A (grade 1 and grade 2, 16 cases) and Group B (grade 3 and grade 4, 14 cases) according to their elbow flexor muscle strength of injured side. The bice

Musculocutaneous nerve injury after coracoid process transfer for clavicle instability. Report of three cases.

During the last 10 years, we have treated 54 cases of fracture or dislocation of the lateral clavicle by transposing the coracoid process onto the clavicle. Recently, a postoperative musculocutaneous nerve injury was observed in 3 cases, all of which recovered spontaneously within 5 months. Our technique seems to entail a certain risk of causing stretch injury to the musculocutaneous nerve. Apart from resting the affected arm for a month or two, the condition does not require specific treatment.

Ulnar nerve to musculocutaneous nerve transfer in an ulnar ray-deficient infant with brachial plexus birth palsy: case report.

In Oberlin's nerve transfer, a fascicle of the ulnar nerve is sutured end-to-end to the branch of musculocutaneous nerve to the biceps muscle in the arm. This transfer is commonly used in adult traumatic C5-C6 avulsion injuries of the brachial plexus. We report the successful use of Oberlin nerve transfer in an ulnar ray-deficient infant with brachial plexus birth palsy.

Physiological and clinical advantages of median nerve fascicle transfer to the musculocutaneous nerve following brachial plexus root avulsion injury.

OBJECT: Loss of biceps muscle function is a significant disability after brachial plexus root avulsion injuries. Nerve grafting techniques to reestablish anatomical and functional continuity between the spinal cord and the avulsed root have not proven successful. Using nerve transfers for functional restoration of root avulsion injuries appears to be effective and has physiological advantages for reducing regeneration distances. Since the early 1990s, the Oberlin technique of transferring ulnar nerve fascicles to the motor branch of the musculocutaneous nerve has been the preferred operative t

The variable communicating branches between musculocutaneous and median nerves: a morphological study with clinical implications.

BACKGROUND: The course of the brachial plexus, its relations with surrounding structures and unique primary and secondary divisions result in its wide range of anatomical variations. Most of these variations were detected during anatomical dissections and studies. It has been found that 53% of studied brachial plexuses contained variations. The communication between musculocutaneous and median nerves is the most common variation of infraclavicular part of brachial plexus. METHODS: During gross anatomical dissections of peripheral nerves, we observed neuronatomical variations in upper limbs of

Is the musculocutaneous nerve really in the coracobrachialis muscle when performing an axillary block? An ultrasound study.

BACKGROUND: In reference textbooks describing axillary block, the ulnar, radial, and median nerves are located in a common sheath surrounding the axillary artery. In contrast, the musculocutaneous nerve is described as lying outside this sheath in the coracobrachialis muscle. In a recent case report of ultrasound-guided axillary block, the musculocutaneous nerve was joined to the median nerve outside this muscle. Our study evaluated the prevalence of atypical musculocutaneous nerve localizations during axillary block. METHODS: All patients undergoing ultrasound-guided axillary block were incl

Transfer of brachialis branch of musculocutaneous nerve for finger flexion: anatomic study and case report.

Based on an anatomic study, a transfer of the brachialis muscle branch of the musculocutaneous nerve (BMBMCN) to finger flexor functional fascicles of the median nerve was designed. Preliminary results of clinical application of this new procedure are reported. Dissection of 32 cadaver upper limbs revealed that BMBMCN derives from the musculocutaneous nerve at the distal 1/3 upper arm level. Mostly it is of single-branch type, with an average dissectable length of 5.2 cm. At this level, functional fascicles of finger flexors are located at the posterior 1/3 of the median nerve. BMBMCN can be d

Axonal damage to the left musculocutaneous nerve of the left biceps muscle during laparoscopic surgery.

The probable etiology of axonal damage to the left musculocutaneous nerve (motor branch) of the left biceps branchii during a laparoscopic procedure was the position in which the patient was maintained. As a result of unintentional change in the angle of the arm from 90 to approximately 120 degrees while in steep Trendelenburg position, the arm might have suffered hyperextension, resulting in pressure on and stretching of the brachial plexus nerve. This in turn might be the cause of neurologic damage. We recommend taking steps to prevent such occurrences, such as tying the patient's arms paral

Musculocutaneous Nerve Compression by a Humeral Osteophyte: An Unusual and Difficult-to-Diagnose Cause of Peripheral Neuropathy.

Isolated neuropathy of the musculocutaneous nerve is an uncommon condition, and compression by a bony outgrowth represents an exceptionally rare cause. We describe the case of a 58-year-old man who presented with motor and sensory deficits consistent with musculocutaneous nerve dysfunction. Cross-sectional imaging and nerve conduction studies revealed a proximal humeral osteophyte responsible for nerve compression. The patient underwent surgical excision of the osteophyte, which resulted in a favorable postoperative outcome. This case highlights the rarity of osteophyte-related nerve compressi

Case report and rehabilitative management of a complete musculocutaneous nerve injury caused by shoulder dislocation.

Anterior shoulder dislocation can secondarily cause nerve injury and/or rotator cuff tear. An elderly male with a recent shoulder dislocation and multiple medical comorbidities was transferred to a rehabilitation inpatient service in order to maximize his functional status. Physical exam suggested a brachial plexus injury. Electrodiagnostic testing confirmed a complete musculocutaneous and a partial axillary nerve lesion. This specific combination of nerve lesions is a previously unreported complication of anterior shoulder dislocation. Prognosis and treatment are discussed, particularly the u

Nerve transfer from the median to musculocutaneous nerve to induce active elbow flexion in selected cases of arthrogryposis multiplex congenita.

BACKGROUND: Arthrogryposis multiplex congenita (AMC) is a rare disease which affects mainly upper and lower extremities. Affected patients are not able to eat unassisted due to elbow contracture and nonexistent active elbow flexion. In traumatic brachial plexus palsies, a nerve transfer from either median or ulnar nerve to the musculocutaneous nerve has proved to induce active elbow flexion, and we report our results of such a procedure in a nontraumatic condition, that is, arthrogryposis. METHODS: We selected four patients with AMC type 1 (6 extremities, 2 males, 2 females) diagnosed with AM

Intercostal Nerve Transfers to the Musculocutaneous-A Reliable Nerve Transfer for Restoration of Elbow Flexion in Birth-Related Brachial Plexus Injuries.

Introduction  There is consensus on the need for early microsurgical reconstruction in birth palsies involving three or more roots, that is, extensive partial palsies and total palsies. The fundamental principles of these operations are complete exploration and judicious use of the ruptured stumps by nerve grafting to suitable distal targets. The frequent observation of root avulsions in such cases makes it imperative to look for extraplexual nerve donors for some functions. Intercostal nerves are readily available in such patients. Materials and Methods  This is a study of 50 patients of exte

Complications after open Latarjet procedure: influence of arm positioning on musculocutaneous and axillary nerve function.

BACKGROUND: Alterations in the anatomical relationships of the musculocutaneous (MCN) and axillary nerves and the influence of arm positioning on these relationships after a Latarjet procedure have been demonstrated in the cadaver, but there are no studies in the literature that establish if there is any neurophysiological repercussion. METHODS: We retrospectively identified 24 patients with a primary or revision open Latarjet procedure. A prospective clinical (Constant-Murley, Rowe and Walch-Duplay and active range of motion), radiographic (with CT), and electrodiagnostic evaluation was made

Intercostal to musculocutaneous nerve transfer in patients with complete traumatic brachial plexus injuries: case series.

BACKGROUND: To recover biceps strength in patients with complete brachial plexus injuries, the intercostal nerve can be transferred to the musculocutaneous nerve. The surgical results are very controversial, and most of the studies with good outcomes and large samples were carried out in Asiatic countries. The objective of the study was to evaluate biceps strength after intercostal nerve transfer in patients undergoing this procedure in a Western country hospital. METHODS: We retrospectively analyzed 39 patients from 2011 to 2016 with traumatic brachial plexus injuries receiving intercostal t

Functional magnetic resonance imaging and control over the biceps muscle after intercostal-musculocutaneous nerve transfer.

OBJECT: Recent progress in the understanding of cerebral plastic changes that occur after an intercostal nerve (ICN)-musculocutaneous nerve (MCN) transfer motivated a study with functional magnetic resonance (fMR) imaging to map reorganization in the primary motor cortex. METHODS: Eleven patients with traumatic root avulsions of the brachial plexus were studied. Nine patients underwent ICN-MCN transfer to restore biceps function and two patients were studied prior to surgery. The biceps muscle recovered well in seven patients who had undergone surgery and remained paralytic in the other two p

Ultrasound-Guided Percutaneous Peripheral Nerve Stimulation of the Musculocutaneous Nerve for Refractory Antecubital ElbowPain-Brief Technical Report and Illustrative Case Report.

Chronic pain following distal biceps rupture (DBR) is often nonspecific in that it may arise due to the injury, subsequent surgical repair, or a combination of factors, making the painful symptoms challenging to treat. Peripheral nerve injury in the setting of DBR most commonly affects the musculocutaneous nerve or one of its terminal branches and may lead to chronic neuropathic pain involving the elbow and lateral/radial aspect of the forearm. In this brief technical report, we describe an ultrasound-guided (USG) technique for percutaneous implantation of a peripheral nerve stimulator (PNS) t

Correlation between electromyography findings and plasticity grading score after long-term phrenic nerve to musculocutaneous nerve transfer in brachial plexus injuries.

OBJECTIVE: Patients with complete traumatic brachial plexus injuries often require nerve transfers to restore lost function. Phrenic nerve (PN)-musculocutaneous nerve (MCN) transfer can be used to restore elbow flexion. The degree to which reinnervated muscles achieve independent control requires central plastic changes. The central program of the PN donor nerve needs to disconnect and establish connections with that of the recipient MCN. This study aimed to improve understanding of the brain's changes after PN-MCN transfer by correlating electromyography (EMG) findings with clinical plasticit
