Achilles tendon injuries in athletes.

Two-thirds of Achilles tendon injuries in competitive athletes are paratenonitis and one-fifth are insertional complaints (bursitis and insertion tendinitis). The remaining afflictions consist of pain syndromes of the myotendineal junction and tendinopathies. The majority of Achilles tendon injuries from sport occur in males, mainly because of their higher rates of participation in sport, but also with tendinopathies a gender difference is probably indicated. Athletes in running sports have a high incidence of Achilles tendon overuse injuries. About 75% of total and the majority of partial ten

Amputation of finger by horse bite with complete avulsion of both flexor tendons.

Amputation of fingers with tendon avulsion occurs through a traction injury, and most occur through a ring avulsion mechanism. Usually the flexor digitorum profundus is torn out with the amputated finger. Replantation usually is recommended only when the amputation is distal to the flexor digitorum superficialis insertion. Animal bites are relatively common, with a decreasing order of frequency of dogs, cats, and humans. Horse bites are relatively infrequent but are associated with crush injuries and tissue loss when they occur. This article describes a 23-year-old man with amputation of his m

Rectus femoris myotendinous lesion treated with PRP: a case report.

BACKGROUND AND AIM OF WORK: Musculoskeletal injuries are the most common cause of severe, chronic pain and physical disability for the majority of all sport-related injuries. Platelet-rich plasma is being used more frequently to promote healing of muscle injuries. We report a case of 39 years old non professional soccer player who came to our attention for a quadriceps muscle pain onset after kicking the ball during a match. METHODS: Clinical and instrumental evaluation revealed a myotendinous junction rupture of the rectus femoris with retraction of 1.5 cm from the anterior inferior iliac sp

The use of the anterior tendinous portion of the supraspinatus muscle as a central point for rotator cuff repair.

INTRODUCTION: Double-row repairs with knots on the medial row carry a risk of secondary lesions at the myotendinous junction. Knotless double-row repairs imply dependence between rows, sometimes resulting in a failure to properly apply the medial row, and the systematic use of 4 anchors. The anterior tendinous portion of the supraspinatus muscle appears to be a promising anatomical landmark and support point for the medial row in an optimized independent double-row technique. HYPOTHESIS: This technique provides results equivalent to those published about rotator cuff repair and reduces the ri

The peroneus longus muscle and tendon: a review of its anatomy and pathology.

This article will review the anatomy and common pathologies affecting the peroneus longus muscle and tendon. The anatomy of the peroneus longus is complex and its long course can result in symptomatology referable to the lower leg, ankle, hindfoot, and plantar foot. Proximally, the peroneus longus muscle lies within the lateral compartment of the lower leg with its distal myotendinous junction arising just above the level of the ankle. The distal peroneus longus tendon has a long course and makes two sharp turns at the lateral ankle and hindfoot before inserting at the medial plantar foot. A s

Post-Levator Aponeurosis Fat Pad.

The aim of this study was to examine the superoinferior extension of the post-levator aponeurosis fat pad in the upper eyelids of Japanese cadavers. Twenty-six upper eyelids (12 right and 14 left) from 15 Japanese cadavers (6 men and 9 women; average age at death, 82.1 years) were used in this experimental anatomical study. After an orbital exenteration, the tissues were cut sagittally through the center of the orbit, and the superoinferior extension of the post-levator aponerosis fat pad was microscopically examined on the sections. Consequently, the fat pad superiorly reached the myotendinou

Identification of FHL1 as a therapeutic target for Duchenne muscular dystrophy.

Utrophin is a potential therapeutic target for the fatal muscle disease, Duchenne muscular dystrophy (DMD). In adult skeletal muscle, utrophin is restricted to the neuromuscular and myotendinous junctions and can compensate for dystrophin loss in mdx mice, a mouse model of DMD, but requires sarcolemmal localization. NFATc1-mediated transcription regulates utrophin expression and the LIM protein, FHL1 which promotes muscle hypertrophy, is a transcriptional activator of NFATc1. By generating mdx/FHL1-transgenic mice, we demonstrate that FHL1 potentiates NFATc1 activation of utrophin to ameliorat

Development of a Rabbit Chronic-Like Rotator Cuff Injury Model for Study of Fibrosis and Muscular Fatty Degeneration.

Rabbit rotator cuff (RC) pathophysiology can lead to progressive and highly degenerative changes in its associated musculature and tendons, which negatively influences clinically relevant parameters, such as strength and retraction of the muscle-tendon/myotendinous unit, ultimately causing loss of shoulder function and negatively affecting RC repair outcomes. Animal models that mimic aspects of human RC anatomy and pathophysiology are crucial for advancing the conceptual understanding of injury progression and developing effective tissue engineering and regenerative medicine-based therapeutics

The anatomical variant of high soleus muscle may predispose to tendinopathy: a preliminary MR study.

PURPOSE: This study aimed to examine the anatomic variations at the level of the distal soleus musculotendinous junction and the possible association between the length of the free tendon and the development of symptomatic Achilles tendinopathy. METHODS: We retrospectively assessed 72 ankle MRI studies with findings of Achilles tendinopathy (study group, 26 females/46 males, mean age 52.6 ± 10.5 years, 30 right/42 left) and 72 ankle MRI studies with normal Achilles tendon (control group, 32 females/40 males, mean age 35.7 ± 13.7 years, 42 right/30 left side). We measured the distance from the

Interactions between intrinsic regulation and neural modulation of acetylcholinesterase in fast and slow skeletal muscles.

1. Initiation of subsynaptic sarcolemmal specialization and expression of different molecular forms of AChE were studied in fast extensor digitorum longus (EDL) and slow soleus (SOL) muscle of the rat under different experimental conditions in order to understand better the interplay of neural influences with intrinsic regulatory mechanisms of muscle cells. 2. Former junctional sarcolemma still accumulated AChE and continued to differentiate morphologically for at least 3 weeks after early postnatal denervation of EDL and SOL muscles. In noninnervated regenerating muscles, postsynaptic-like sa

Localization and function of Xinα in mouse skeletal muscle.

The Xin repeat-containing proteins were originally found in the intercalated discs of cardiac muscle with implicated roles in cardiac development and function. A pair of paralogous genes, Xinα (Xirp1) and Xinβ (Xirp2), is present in mammals. Ablation of the mouse Xinα (mXinα) did not affect heart development but caused late-onset adulthood cardiac hypertrophy and cardiomyopathy with conductive defects. Both mXinα and mXinβ are also found in the myotendinous junction (MTJ) of skeletal muscle. Here we investigated the structural and functional significance of mXinα in skeletal muscle. In additio

Transgenic isolation of skeletal muscle and kidney defects in laminin beta2 mutant mice: implications for Pierson syndrome.

Pierson syndrome is a recently defined disease usually lethal within the first postnatal months and caused by mutations in the gene encoding laminin beta2 (LAMB2). The hallmarks of Pierson syndrome are congenital nephrotic syndrome accompanied by ocular abnormalities, including microcoria (small pupils), with muscular and neurological developmental defects also present. Lamb2(-/-) mice are a model for Pierson syndrome; they exhibit defects in the kidney glomerular barrier, in the development and organization of the neuromuscular junction, and in the retina. Lamb2(-/-) mice fail to thrive and d

Myotonic dystrophy and myotonic dystrophy protein kinase.

Myotonic dystrophy protein kinase (DMPK) was designated as a gene responsible for myotonic dystrophy (DM) on chromosome 19, because the gene product has extensive homology to protein kinase catalytic domains. DM is the most common disease with multisystem disorders among muscular dystrophies. The genetic basis of DM is now known to include mutational expansion of a repetitive trinucleotide sequence (CTG)n in the 3'-untranslated region (UTR) of DMPK. Full-length DMPK was detected and various isoforms of DMPK have been reported in skeletal and cardiac muscles, central nervous tissues, etc. DMPK

Expression of full-length utrophin prevents muscular dystrophy in mdx mice.

Duchenne muscular dystrophy (DMD) is a lethal, progressive muscle wasting disease caused by a loss of sarcolemmal bound dystrophin, which results in the death of the muscle fiber leading to the gradual depletion of skeletal muscle. The molecular structure of dystrophin is very similar to that of the related protein utrophin. Utrophin is found in all tissues and is confined to the neuromuscular and myotendinous junctions in mature muscle. Sarcolemmal localization of a truncated utrophin transgene in the dystrophin-deficient mdx mouse significantly improves the dystrophic muscle phenotype. There

Subpectoral biceps tenodesis with BicepsButton fixation in the young population: which technique works best?

BACKGROUND: Injuries of the long head of the biceps (LHB) tendon are a prevalent source of anterior shoulder pain and are commonly treated with tenodesis. Not only a stable fixation of the LHB but also anatomic restoration of the length-tension relationship plays a central role in providing satisfactory functional and cosmetic outcomes, especially in young patients. We report the clinical outcomes of 2 different subpectoral tenodesis techniques using unicortical button fixation. METHODS: Patients aged ≤ 50 years who were treated between April 2015 and January 2020 with 1 of the 2 following su

A Novel Dynamic Ultrasound Assessment of T-Junction Hamstring Injury during Gym-Based Exercises: A Case Report in an Elite Football Player.

BACKGROUND: "T-Junction" or distal musculotendinous junction (DMTJ) hamstring injuries have become a true enigma for clinicians as demonstrated by the high recurrence rate. Magnetic Resonance Imaging (MRI) can sometimes fail to provide an accurate assessment of these injuries, probably because of the dynamic function that this structure provides to offer stability at the synchronous movement of the two heads of biceps femoris. The aim of the present case report was to describe a novel method for dynamic ultrasound evaluation of the DMTJ performed during functional loading exercises and to expl

The Independent Effect of Nerve Injury and Tendon Injury on Rotator Cuff Fatty Infiltration.

BACKGROUND: Muscular fatty infiltration (FI), atrophy, and fibrosis commonly occur after chronic rotator cuff tear and are barriers to successful rotator cuff repair. It is unclear if these phenomena result from tendon injury alone, neurological injury, or a combination. PURPOSE: To determine the role of suprascapular nerve mediated FI with and without chronic rotator cuff tear in a rabbit model. STUDY DESIGN: Controlled laboratory study. METHODS: A total of 22 rabbits underwent bilateral shoulder procedures. Extremities were randomized to surgical procedures: suprascapular neurotomy, supra

Beyond tendon interfaces - Functional divergence of the matricellular protein SPARC.

Musculoskeletal tissue interfaces (TIs), including osteotendinous, myotendinous, and osteochondral junctions, are specialized regions that enable effective force transmission, mechanical stability, and long-term tissue integration. These interfaces are defined by gradients in cellular composition, extracellular matrix (ECM) organization, and mechanical properties, and their maintenance relies in part on tightly regulated cell-matrix interactions. Beyond structural ECM components, matricellular proteins such as Secreted Protein Acidic and Rich in Cysteine (SPARC), thrombospondins, osteopontin,
