[Unilateral nasal obstruction in children: Pai syndrome].

Unilateral obstruction of the nasal cavity in children is mainly caused by the introduction of foreign bodies further stated with rhinorrhea and fetid odor. Less commonly, it can be traumatic, neoplastic, due to congenital malformation or iatrogenic. Symptoms of congenital intranasal mass may present at birth, or go unnoticed and be a finding in a routine pediatric examination. Patient evaluation should include imaging studies to guide the diagnosis and rule out intracranial extension. A syndrome associated with congenital nasal tumor should be suspected when other abnormalities are present. P

Nasal obstruction and headache. A real correlation?

OBJECTIVE: to evaluate the relationships between headache and nasal obstruction or nasal allergy on a group of Italian school children. METHODS: One hundred twenty-six children suffering from headache, were enrolled in the study. All patients underwent an accurate clinical history, a complete ENT objective examination, an Anterior Active Rhinomanometry, an analysis of nasal mucociliary transport time and a skin test on the inner forearm. RESULTS: Nasal resistances were increased in 101 out of 105 of children (96.2%) affected by severe headache (group A) and in 5 out of 21 (23.8%) of those af

Sarcoidosis with nasal obstruction and septal perforation.

Although sarcoidosis is predominantly a pulmonary disorder, it often produces disease in the head and neck. For this reason, otolaryngologists should be comfortable in evaluating and treating the disease. The diagnosis of sarcoidosis often can be made with a biopsy of conjunctival or oral mucosa, even when disease manifestations are not apparent in those parts of the body. In a case of sarcoidosis involving the nose, a 54-year-old black man had a proliferation of small lumps on the tip of his nose. For many years, he had had daily epistaxis, anosmia and severe difficulty breathing through this

Nasolacrimal obstruction after inferior meatus nasal antrostomy.

Transient epiphora following rhinoplasty or intranasal procedures is a common occurrence. Permanent nasolacrimal duct obstruction, however, is rare. This article documents four cases of nasolacrimal duct obstruction following intranasal antrostomy. Three patients were cured by dacryocystorhinostomy and a fourth refused surgery. The anatomy of the nasolacrimal duct in the inferior meatus has considerable variation. Although the duct typically opens in the inferior meatus immediately under the insertion of the inferior turbinate, the orifice can be a single hole, a slit, multiple holes, or a tro

Pseudonasolacrimal duct obstruction caused by nasal allergy.

While mechanical obstruction of the nasolacrimal duct is the most common cause of acquired unilateral epiphora, it is not the only one. Of the 94 patients evaluated with this symptom, 22 cases (23%) were caused by primary nasal changes in the form of nasal allergy. Many of the characteristic findings of mechanical obstruction were present. Several patients had previous nasolacrimal duct surgery, which failed to eliminate the epiphora. Nasal allergy was not initially considered either by previous examiners or the patients themselves, because of the absence of typical allergic symptoms. All of t

Total inferior turbinectomy for nasal airway obstruction.

A variety of surgical procedures are performed to open the nasal airway chronically obstructed by hypertrophic inferior turbinates. Because the results are universally unsatisfactory, we suggest bilateral total inferior turbinectomy to patients in whom medical therapy fails. One hundred fifty patients were followed up for one to seven years (mean, 2 1/2 years) and the results of the follow-up were assessed clinically via questionnaire and chart review. Patent nasal airway resulted in 91% of the patients. Eighty percent of the patients reported improvement in nasal breathing, and 14 (27%) of th

Surgery for nasal obstruction--evaluation by rhinomanometry.

Nasal obstruction is predominantly a subjective patient complaint. The physical examination of the nose for nasal obstruction is a subjective evaluation as well. While the history and physical examination are reasonable diagnostic indicators, an objective means of evaluation is needed. Rhinomanometry measures nasal obstruction objectively and reliably. Thirty-six patients with nasal obstruction were evaluated by rhinomanometry before and after nasal surgery. The results are reported here. Rhinomanometry confirms the effectiveness of intranasal surgery. Using rhinomanometry, various techniques

Understanding Nasal Valve Compromise in Nasal Airway Obstruction: A New Classification System.

OBJECTIVES: A key aspect of rhinoplasty is integrity of nasal valves area because of its fundamental role in patent nasal airway. furthermore, nasal obstruction is a dominant feature in patient with nasal valves dysfunction (NVD) which can be devastating outcome after rhinoplasty. METHODS: Nonetheless, NVD has been widely investigated regarding their etiologies and management. However, it has been assumed that the anatomical complexity and interplay between its structures causing a confusion for a beginner surgeon to diagnose and treat NVD. This study was approved by the institutional review

A surprising cause of unilateral nasal obstruction and epistaxis: nasal septal schwannoma.

Schwannomas of the head and neck are the most common form of benign nerve sheath tumours, most commonly arising in the form of vestibular schwannomas. Schwannoma of the nasal cavity is an uncommon presentation of this pathology and specifically Schwannoma of the nasal septum is a rare presentation of this well understood disease process. We present the case of a 31-year-old Eastern European male who presented with unilateral nasal obstruction, congestion and epistaxis of 3 months duration. After imaging and biopsy, the diagnosis of nasal septal schwannoma was made on histological examination.

Nasal congestion and nasal obstruction: An unusual presentation of renal cell carcinoma.

Renal cell cancer (RCC) is among the 10 most common cancers affecting both genders in the United States. Advanced RCC often remains clinically silent for much of its natural history. This can make the diagnosis challenging, especially when presenting symptoms arise from a metastasis. Sinonasal malignancies are rare, accounting for <1% of all malignant tumors and 3% of malignant tumors of the upper aerodigestive tract. RCC is the most common infraclavicular malignant primary tumor that metastasizes to the nasal cavity and paranasal sinus, followed by breast and lung. We describe a case of a 59

Congenital nasal pyriform aperture stenosis: a rare cause of neonatal nasal obstruction.

Congenital nasal pyriform aperture stenosis has been described as an unusual cause of neonatal nasal obstruction. Clinical suspicion is based on respiratory distress, cyclic cyanosis, apneas, and feeding difficulties. A bony overgrowth of the maxillary nasal processes is thought to be responsible for this deformity. This anomaly has been reported as an isolated feature or can be associated with craniofacial or central nervous system anomalies. Surgery is indicated in cases of severe respiratory distress, feeding difficulties, and when conservative methods fail.

Correlation between nasal airflow characteristics and clinical relevance of nasal septal deviation to nasal airway obstruction.

Since the imbalance of the nasal cavities due to nasal septal deviation (NSD) is a commonly observed anatomic variation in healthy adults, clinicians must often decide whether or not it is clinically relevant to the symptoms of nasal airway obstruction (NAO). Main reason for this is a lack of data correlating the symptoms of NAO with objective findings. The aim of our study is to find the correlation between fluid dynamic parameters and the anatomy of nasal cavity with NSD by numerical simulation. We generated 6 computational models of nasal cavities with NSD were created from computed tomogra

Nasal nerve ablation, nasal swell body and inferior turbinate reduction for nasal obstruction and congestion relief.

OBJECTIVE: Nasal obstruction and congestion can occur because of turbinate and septal variations with or without rhinitis. A combined treatment for nasal obstruction and congestion was examined retrospectively in cases where the nasal swell body was addressed with inferior turbinectomy, with or without posterior nasal nerve ablation. METHODS: A 940 nm laser was utilised for contact (nasal swell body, septum and inferior turbinate) and non-contact (posterior nasal nerve) ablation. Total Nasal Symptoms Score, visual analogue scale pain score, complications and procedure location (office vs oper

[Anatomy of nasal obstruction. The recognition of the role of septum. Its application to the surgical treatment of nasal obstruction].

A retrospective study of the records of 392 consecutive patients prospectively operated for non-vasomotor nasal obstruction, has allowed us to identify a syndrome that we would call morphological syndrome. This is caused by anatomical deformities of the nasal septum, inferior turbinate hypertrophy frequently found being the most often only a compensation for the septal deviation. This endonasal abnormality, which can be congenital or post-traumatic, is most frequently of developmental origin and therefore most often found in Caucasian men, whose facial and septal growth is considerable.
