IMPORTANCE: Child abuse and neglect are associated with risk of negative health outcomes, and children in minoritiy racial and ethnic groups and female children are disproportionately affected. It is unclear whether the incidence of child maltreatment has changed over time.
OBJECTIVE: To determine whether incidence of child maltreatment and its subtypes changed across sex, race, and ethnicity in the US from 2012 through 2023.
DESIGN, SETTINGS, AND PARTICIPANTS: In this serial, population-based, cross-sectional study, all Child Protective Services cases in the US from January 2012 through Dec

Little is known about ecstasy/MDMA use among Asian American and Pacific Islander populations. Research is important because AAPIs face unique cultural factors that may influence use. We estimated the prevalence and correlates of lifetime ecstasy/MDMA use based on a representative sample of US AAPI adults aged ≥18 from the 2015-2020 National Survey on Drug Use and Health. An estimated 5.1% of AAPI adults used ecstasy in their lifetime. Compared to males, females had higher odds of use (aOR = 1.45, 95% CI: 1.08-1.98). Compared to those aged 18-25, those aged 26-34 were at increased odds for use

Electronic Health Records (EHR) contain detailed information about a patient's medical history and can be helpful in understanding clinical outcomes among populations generally underrepresented in research, including pregnant individuals. A cesarean delivery is a clinical outcome often considered in studies as an adverse pregnancy outcome, when in reality there are circumstances in which a cesarean delivery is considered the safest or best choice given the patient's medical history, situation, and comfort. Rather than consider all cesarean deliveries to be negative outcomes, it is important to

The substantial underrepresentation of American Indian, Alaskan Native, and Native Hawai'ian (AI/AN/NH) students in Science, Technology, Engineering and Mathematics (STEM) majors at United States colleges and universities results from systemic barriers in higher education. Efforts to address inequities have thus far not led to increases in AI/AN/NH student participation in STEM at the national level. In fact, while the AI/AN/NH population in the United States is increasing, there was a 17.0% decrease in the number of bachelor's degrees awarded to Indigenous students in STEM during the period 2

INTRODUCTION: Health disparities in the Asian and Pacific Islander Americans (APIAs) community have not been well described, unlike non-Hispanic Black and Hispanic communities. However, there has been a rise in violence against the APIA community. This study explores and characterizes violent death by incident (e.g., homicide, suicide), weapon (e.g., firearm, strangulation), and location types among APIAs as they compare with other racial or ethnic groups.
METHODS: We used the National Violent Death Reporting System from 2003 to 2018 to characterize violent deaths among APIA and compared them

BACKGROUND: Racial and ethnic diversification of the physician and nurse workforce is recommended as a leverage point to address the impact of structural racism in maternal care, but empirical evidence supporting this recommendation is currently lacking.
OBJECTIVE: This study aimed to assess the association between state-level registered nurse workforce racial and ethnic diversity and severe adverse maternal outcomes during childbirth.
STUDY DESIGN: This population-based cross-sectional study analyzed 2017 US birth certificate data. Severe adverse maternal outcomes included eclampsia, blood

BACKGROUND: Recent analyses have shown increases in combined annual HIV diagnosis rates for Asians and Pacific Islanders (API).
METHODS: Using surveillance data from 33 states and 4 dependent areas we investigated the epidemiology of HIV among API during 2001-2008.
RESULTS: HIV diagnoses for API during 2001-2008 were predominantly among persons age 30-39 years (40%) and males (78%). The primary risk factor for males was sexual contact with males (78%) and for females, heterosexual contact (86%). API were the only racial/ethnic groups with a statistically significant estimated annual percenta

OBJECTIVE: To evaluate whether there are individual- and population-level associations between chronic hypertension and pregnancy complications, and to assess differences across seven racial-ethnic groups.
METHODS: This population-based study used linked vital statistics and hospitalization discharge data from all live and stillbirths in California (2008-2018), Michigan (2008-2020), Oregon (2008-2020), Pennsylvania (2008-2014), and South Carolina (2008-2020). We used multivariable log-binomial regression models to estimate risk ratios (RRs) and population attributable risk (PAR) percentages w

OBJECTIVES: To assess racial/ethnic differences in gestational diabetes mellitus (GDM) prevalence in Hawai'i.
METHODS: This retrospective cross-sectional study included hospital deliveries at Kapi'olani Medical Center for Women and Children (Honolulu, Hawai'i) from 2009 to 2019. For birthing people with multiple deliveries during the study interval, only the last delivery was included. Birthing people with pregestational diabetes, delivery < 29 weeks, and multifetal gestations were excluded. GDM was identified through ICD-9 and -1 0 codes. Race and ethnicity were self-reported. Maternal body

BACKGROUND: Substance use including opioids, methamphetamines, benzodiazepines, and barbiturates during pregnancy is harmful for the pregnant person and the fetus. Routine screening using validated questionnaires is recommended, but often biologic sampling is done instead. There is often bias in urine drug screening on labor and delivery units.
OBJECTIVE: This study aimed to compare characteristics of people who did and did not receive urine drug screening during labor and delivery and to examine the relationship of maternal results to neonatal results.
STUDY DESIGN: This was a retrospective

Pacific Islanders represent a minority population with a disproportionate amount of risk factors for cholangiocarcinoma, including chronic liver disease, obesity, and diabetes mellitus, compared to other populations in the United States, but are poorly studied independently from Asians. Thus, this study aimed to characterize cholangiocarcinoma in a group of Pacific Islanders compared to Asians. This study retrospectively assessed a population of 40 Pacific Islander and 215 Asian cholangiocarcinoma patients from Hawai'i's primary liver transplant center from 1993 to 2020. Overall, Pacific Islan

Hypertension and type 2 diabetes (T2D) are major public health issues that disproportionately affect minority communities, including Native Hawaiians and Pacific Islanders (NHPI). Minority communities are also more likely to have undiagnosed hypertension and T2D. Marshallese Pacific Islanders have been shown to have high proportions of diagnosed and undiagnosed hypertension and T2D. Using survey and biometric data collected from 378 overweight/obese Marshallese Pacific Islander adults, this study documents the prevalence of hypertension and T2D, as well as the prevalence of undiagnosed hyperte

INTRODUCTION: American Indian and Alaskan Natives (AIAN) and Native Hawaiian and Pacific Islanders (NHPI) research is limited, particularly in postoperative surgical outcomes. This study analyzes disparities in AIAN and NHPI surgical complications across all surgical types and identifies factors that contribute to postoperative complications.
METHODS: This retrospective cohort study examined all surgeries from 2011 to 2020 in the National Surgical Quality Improvement Program, queried by race. Multivariable models analyzed the association of race and ethnicity and 30-day postoperative complica

Epigenetic differences across racial/ethnic groups can provide insights into health disparities, including likely mechanisms and multifactorial upstream exposures at play. We compared the blood DNA methylome of Native Hawaiian (NatH) women, an understudied group with high chronic disease burden, with that of White women. Blood genome-wide DNA methylation profiling was performed in generally healthy, non-smoking NatH (n = 143) and White (n = 181) postmenopausal women in the Multiethnic Cohort. CpGs showing significant (Bonferroni p < 0.05) and substantial (delta-beta > 0.1) differential methyla

Prediabetes disproportionately affects racial and ethnic minority groups in Hawai'i. The National Diabetes Prevention Program lifestyle change program (National DPP LCP) decreases the risk of developing diabetes. However, enrolling and retaining participants is a challenge for program providers. This evaluation aimed to understand factors that influence racial and ethnic minority groups in Hawai'i to enroll in and complete the program. From 2018 through 2023, two federally qualified health centers (FQHCs) in rural Hawai'i administered 6 year-long cohorts. Trained lifestyle coaches, who were FQ

Past research has examined the complex reasons for the apparent reluctance of East Asian Americans (ie, Chinese, Japanese, Koreans) to seek mental health services when needed. The current study analyzed East Asian American (EAA) mental health, utilizing inpatient hospitalization data from the Hawai'i Health Information Corporation (HHIC) database. Frequency of inpatient hospitalizations for specific mental health diagnoses (depression, bipolar disorder, schizophrenia, and suicide attempts/ideation) in EAA patients was examined. White, Native Hawaiian, and Filipino patients were included for co
