Neoadjuvant radiation therapy for the management of myoepithelial carcinoma of the upper extremity.

Myoepithelial tumors of the soft tissue are a rare tumor displaying myoepithelial elements and lacking obvious ductal differentiation. The rarity of these precludes any evidence-based consensus regarding optimal management. Nevertheless, the current approach to these lesions begins with amputation or complete excision. The efficacy of neoadjuvant or adjuvant radiation therapy or chemotherapy has not been established. Here, we present the first report to the authors' knowledge of neoadjuvant radiation therapy for the treatment of this rare soft tissue neoplasm and review the management and outc

Successful treatment of neoadjuvant therapy for papillary tumor of the pineal region.

We report a boy who in 1994, at the age of 11, presented with headache and vomiting. The fi nal diagnosis was papillary tumor of the pineal region (PTPR). Magnetic resonance imaging (MRI) revealed a heterogeneous mass. Hydrocephalus was addressed by immediate ventricle drainage; subsequently, we attempted tumor removal. As the intraoperative diagnosis of the hemorrhagic tumor was primitive neuroectodermal tumor (PNET), we did not proceed to total removal. After the delivery of radiotherapy (50.4 Gy) and one course of Nimustine hydrochloride (ACNU) chemotherapy, the residual tumor was completel

Reassessing perioperative risks: impact of neoadjuvant therapy on mortality in pneumonectomy.

BACKGROUND: The INT0139 trial [2009] raised concerns about the safety of pneumonectomy after chemoradiation due to high perioperative mortality rates. Recent clinical trials have supported the use of neoadjuvant treatment as a viable option for stage Ib or greater lung cancer. We hypothesized that neoadjuvant treatment does not increase perioperative mortality risk in patients undergoing pneumonectomy. METHODS: Adult patients with nonmetastatic non-small cell lung cancer (NSCLC) from the 2004-2021 National Cancer Database undergoing pneumonectomy were analyzed in a retrospective cohort study.

Updates on Neoadjuvant Therapy for Resectable and Borderline Resectable Pancreatic Adenocarcinoma.

Mortality after the diagnosis of pancreatic ductal adenocarcinoma remains high for all patients. The role of multimodal therapy, including surgical resection and systemic chemotherapy, has been well established. Neoadjuvant treatment strategies continue to be used in resectable and borderline resectable PDAC. Results from ongoing or recently completed trials will continue to provide valuable insight into the management of patients with resectable and borderline resectable PDAC. Advances in the outcomes for this difficult disease and therefore the focus of futures studies should include a bette

Association of Neoadjuvant Therapy on Pathologic and Clinical Outcomes for Pancreatic Adenocarcinoma.

INTRODUCTION: The use of total neoadjuvant chemotherapy (TNC) for pancreatic adenocarcinoma (PDAC) is increasing. This study evaluated the association between chemotherapy timing [TNC, perioperative (PERIOP), adjuvant (ADJ)] and total duration of chemotherapy on survival in PDAC. METHODS: Single-institution, retrospective study (2020-2023) of PDAC patients that underwent surgical resection and chemotherapy with an intended 6-mo course. We compared overall survival and chemotherapy completion rates between TNC, PERIOP, and ADJ groups. RESULTS: Of 115 patients, 10 received TNC, 69 PERIOP, and

Stakeholder views on integrating prehabilitation in neoadjuvant therapy for breast cancer.

BACKGROUND: Neoadjuvant systemic therapy (NAST) for breast cancer can cause fatigue, neuropathy, and sleep disturbance, affecting treatment adherence and recovery. Early supportive care-including exercise, psychological support, and integrative therapies-may reduce side effects and improve outcomes. However, it remains unclear how best to deliver such care to optimise patient participation and effectiveness. This study explored the perspectives of women with breast cancer and healthcare professionals (HCPs) on the design and delivery of a multimodal supportive care program during NAST. METHOD

Total Neoadjuvant Therapy for the Treatment of Locally Advanced Rectal Cancer.

The treatment of locally advanced rectal cancer has been transformed over the last century. While initially treated with surgery alone, adjuvant therapy with chemotherapy and radiation became common for locally advanced disease to prevent local and distant recurrence. Several trials subsequently showed benefit to moving this therapy into the neoadjuvant setting which led to improved oncologic outcomes, greater compliance, and similar surgical morbidity. In this article, we will explore this historical transition to a total neoadjuvant approach that can lead to cure in select patients and bring

[Application and challenges of neoadjuvant therapy in gastric cancer in the immunotherapy era].

The emergence of neoadjuvant immunotherapy and neoadjuvant immunotherapy combined with chemotherapy has further elevated the status of neoadjuvant therapy in the treatment of gastric cancer. These new approaches have shown potential in improving the pathologic complete response rate and patient survival rate, providing more treatment options for patients with locally advanced gastric cancer. However, the application of neoadjuvant immunotherapy in gastric cancer still faces some challenges. On the one hand, only a portion of patients can benefit from immunotherapy, and how to accurately screen thi

Predictive factors for histological response to neoadjuvant therapy in gastric adenocarcinomas.

BACKGROUND: Perioperative chemotherapy is the standard curative treatment for resectable gastric adenocarcinoma, significantly improving both overall and recurrence-free survival. The histological response to neoadjuvant therapy is a critical prognostic factor, commonly assessed through grading systems such as Mandard's tumor regression grade (TRG). AIMS: The aim of the study was to identify predictive factors for histological response to neoadjuvant therapy in gastric adenocarcinoma. METHODS: A retrospective study was performed on patients with gastric adenocarcinoma who underwent surgery f

Genomic and transcriptomic determinants of response to neoadjuvant therapy in rectal cancer.

The incidence of rectal cancer is increasing in patients younger than 50 years. Locally advanced rectal cancer is still treated with neoadjuvant radiation, chemotherapy and surgery, but recent evidence suggests that patients with a complete response can avoid surgery permanently. To define correlates of response to neoadjuvant therapy, we analyzed genomic and transcriptomic profiles of 738 untreated rectal cancers. APC mutations were less frequent in the lower than in the middle and upper rectum, which could explain the more aggressive behavior of distal tumors. No somatic alterations had sign

Narrative Review: Predictive Biomarkers of Tumor Response to Neoadjuvant Radiotherapy or Total Neoadjuvant Therapy of Locally Advanced Rectal Cancer Patients.

Background/Objectives: Treatment of locally advanced rectal cancer (LARC) very often requires a neoadjuvant multimodal approach. Neoadjuvant treatment (NAT) encompasses treatments like chemoradiotherapy (CRT), short-course radiotherapy (SCRT), radiotherapy (RT) or a combination of either of these two with additional induction or consolidation chemotherapy, namely total neoadjuvant treatment (TNT). In case of complete radiological and clinical response, the non-operative watch-and-wait strategy can be adopted in selected patients. This strategy is impacted by a regrowth rate of approximately 30

Validation of the Neoadjuvant Rectal Cancer (NAR) Score for Prognostication Following Total Neoadjuvant Therapy (TNT) for Locally Advanced Rectal Cancer.

PURPOSE: The neoadjuvant rectal cancer (NAR) score is a prognostic tool for locally advanced rectal cancer (LARC) treated with total neoadjuvant therapy (TNT). It has been previously validated as an endpoint that predicts survival more accurately than pathologic complete response (pCR) and is the primary endpoint of the ongoing NRG-GI002 Phase II trial. Using the National Cancer Database (NCDB), we aimed to validate the NAR score's ability to predict survival in a large hospital-based dataset. METHODS: We queried the NCDB to identify locally advanced rectal cancer patients from 2004 to 2015 t

Real-world comparison between neoadjuvant FLOT therapy and DCF therapy for resectable esophagogastric junction adenocarcinoma.

BACKGROUNDS: In Western countries, the standard perioperative treatment for resectable locally advanced esophagogastric junction adenocarcinoma (EGJ-AC) is 5-fluorouracil, oxaliplatin, and docetaxel (FLOT) therapy based on the results of the FLOT4 and ESOPEC trials. On the other hand, there was little evidence based on optimal perioperative treatment for resectable locally advanced EGJ-AC in Japan. Our previous report showed that neoadjuvant docetaxel, cisplatin, and 5-fluorouracil (DCF) therapy demonstrated modest efficacy for resectable locally advanced EGJ-AC. Therefore, we compared neoadju

Breast-Conserving Surgery Versus Mastectomy After Neoadjuvant Therapy: A Survival Analysis.

BACKGROUND: Breast cancer management has increasingly incorporated neoadjuvant therapy (NAT), particularly to improve rates of pathologic complete response (pCR) and expand eligibility for breast-conserving surgery (BCS) instead of mastectomy. Using SEER database data, we evaluated whether BCS versus mastectomy after NAT is associated with different survival outcomes in patients with pCR. PATIENTS AND METHODS: We performed a population-based study including women with nonmetastatic invasive breast cancer diagnosed between 2010 and 2015 who received NAT followed by BCS or mastectomy and achiev

MRI‑based neoadjuvant therapy indication in middle and low rectal cancer.

OBJECTIVE: The aim of this study was to evaluate the indication of neoadjuvant therapy in patients with middle and low rectal cancer based on MRI examination. BACKGROUND: In spite of noticeable advances in the diagnosis of rectal cancer, the optimal treatment remains highly debated. Current guidelines advise the use of neoadjuvant therapy in UICC stage II patients or higher. However, in clinical praxis, there is gradual implementation of new criterions and variables used in rectal cancer stage evaluation, the fact of which influences the treatment choice. The most important emerging variables
