Moral tensions at the bedside: A survey of neonatal nurses' perceptions of trisomy 18 care.

We sought to characterize bedside nurses' perceptions of providing intensive care to patients with trisomy 18 (T18) in a level IV neonatal intensive care unit (NICU). NICU nurses were anonymously surveyed with an electronic, mixed-methods survey. Items included quantitative questions about whether T18 is "incompatible with life," whether interventions should be offered, open-ended free-text questions, and demographic items. Survey results were analyzed with descriptive statistics and thematic analysis. There were 145 responses (response rate 46%). Most (53%) agreed that T18 was "incompatible w

Reactive rather than proactive: nurses' perspectives on decision-making in neonatal vascular access.

BACKGROUND: Neonatal vascular access is essential for life-sustaining therapies but is associated with frequent device failure, repeated procedures, and patient harm. Although technical aspects have been widely studied, less is known about how organisational and behavioural factors influence decision-making. METHODS: We conducted a multicentre qualitative study across three tertiary neonatal intensive care units in the Netherlands. Semi-structured interviews were conducted with 12 neonatal nurses involved in vascular access and infusion therapy. Data were analysed using reflexive thematic ana

The Space in the Middle: Attitudes of Women's Health and Neonatal Nurses in the United States about Abortion.

INTRODUCTION: Despite playing an integral part in sexual and reproductive health care, including abortion care, nurses are rarely the focus of research regarding their attitudes about abortion. METHODS: A sample of 1,820 nurse members of the Association of Women's Health, Obstetric and Neonatal Nurses were surveyed about their demographic and professional backgrounds, religious beliefs, and abortion attitudes. Scores on the Abortion Attitudes Scale were analyzed categorically and trichotomized in multinomial regression analyses. RESULTS: Almost one-third of the sample (32%) had moderately pr

Educating neonatal nurses in Brazil: a before-and-after study with interrupted time series analysis.

BACKGROUND: Preterm birth contributes significantly to infant mortality and morbidity, including blindness from retinopathy of prematurity (ROP). Access to intensive neonatal care is expanding in many countries, but care is not always optimal, one factor being that nursing is often by inadequately trained nurse assistants. OBJECTIVE: The aim of this study was to evaluate whether an educational package for nurses improves a range of outcomes including survival rates and severe ROP in 5 neonatal units in Rio de Janeiro, Brazil. METHODS: The study design included an uncontrolled before-and-afte

Reducing hazardous chemical exposures in the neonatal intensive care unit: a new role for nurses.

A wide range of toxic chemicals have been found in the umbilical cord blood of newborns, indicating the potential for health risks from chemical exposure that begin in utero. The neonatal intensive care unit (NICU) setting may also have potential chemical exposures that create health risks. Given the extreme vulnerability of this patient population, it is critical to minimize unnecessary hazardous chemicals. Neonatal intensive care unit nurses have an important role to play in making the NICU as safe as possible for their patients and themselves. This article will focus on the human health eff

Nurses' Finnegan Scoring of Newborns with Neonatal Abstinence Syndrome Not Affected by Time or Day of the Week.

OBJECTIVE: Newborns exposed to drugs in utero are at risk of developing neonatal abstinence syndrome (NAS), characterized by behavioral changes and physiologic instability. Finnegan scoring tool quantifies severity of symptoms and guides treatment. This article evaluates whether time of day and the number of shift hours affects modified Finnegan scores, and the subjective component of these scores. STUDY DESIGN: Institutional review board-approved, retrospective chart review of newborns admitted to neonatal intensive care or transitional nursery from 2011 to 2014. INCLUSION CRITERIA: > 35 we

Preventive nursing of neonatal clavicular fracture in midwifery: a report of six cases and review of the literature.

PURPOSE: To summarize and analyze the obstetric factors and medical care for neonatal clavicle fracture during delivery. MATERIALS AND METHODS: In 4,456 vaginal deliveries, only six newborns were found with a clavicle fracture in our hospital from October 2002 to October 2011. RESULTS: Clinical findings showed that dystocia and improper midwifery manoeuvres are the two major reasons which lead to newborn clavicular fractures. CONCLUSION: More attention should be paid to non-violent traction and proper treatment of shoulder dystocia.
