Introducing an osteopathic approach into neonatology ward: the NE-O model.

BACKGROUND: Several studies showed the effect of osteopathic manipulative treatment on neonatal care in reducing length of stay in hospital, gastrointestinal problems, clubfoot complications and improving cranial asymmetry of infants affected by plagiocephaly. Despite several results obtained, there is still a lack of standardized osteopathic evaluation and treatment procedures for newborns recovered in neonatal intensive care unit (NICU). The aim of this paper is to suggest a protocol on osteopathic approach (NE-O model) in treating hospitalized newborns. METHODS: The NE-O model is composed

[Bloodstream infection in a neonatology unit of Medellín-Colombia, 2008-2009].

Bloodstream infection (BSI) is one of the main causes of sepsis and death in newborns. The relative importance of nosocomial and non-nosocomial agents in developing countries is not clear. We conducted a prospective study of newborns hospitalized with a first episode of BSI in order to classify it as early, late or nosocomial, describe the clinical and microbiological characteristics, and explore risk factors between hospital-acquired vs community-acquired BSI (HA-BSI vs CA-BSI). Twenty-seven newborns with a first episode of BSI were identified. Streptococcus agalactiae and coagulase negative

[The role of S100B-protein in neonatology, pediatric intensive care, and pediatrics].

During the last years neuromonitoring with various biochemical markers such as S100B protein has been introduced into the clinical settings of neonatal and pediatric intensive care. Several investigations have been undertaken to correlate S100B protein concentrations to the diagnosis and prognosis of neonates and children with severe cerebral disorders. This articles gives a review on the current knowledge, indications and limitations on the use of S100B protein after non-traumatic and traumatic brain injury in neonates and children.

[Hirschsprung disease. Discussion of a disease picture also relevant to neonatology].

The clinical disease pattern of an "acute abdomen" is by no means rare in newborn and very young infants and always signals an emergency situation. Besides inflammatory causes, congenital anomalies are among the most prominent reasons responsible for this condition. The example of 3 own patients serves to illustrate the disease pattern of Hirschsprung's disease, which is discussed with special reference to neonatology.

Lung Ultrasound: Its Findings and New Applications in Neonatology and Pediatric Diseases.

Lung ultrasound has become increasingly used in both adult and pediatric populations, allowing the rapid evaluation of many lung and pleura diseases. This popularity is due to several advantages of the method such as the low cost, rapidity, lack of ionizing radiation, availability of bedside and repeatability of the method. These features are even more important after the outbreak of the SARS-CoV-2 pandemic, given the possibility of recognizing through ultrasound the signs of interstitial lung syndrome typical of pneumonia caused by the virus. The purpose of this paper is to review the availab

Integrating neurocritical care approaches into neonatology: should all infants be treated equitably?

To improve the neurologic outcomes for infants with brain injury, neonatal providers are increasingly implementing neurocritical care approaches into clinical practice. Term infants with brain injury have been principal beneficiaries of neurologically-integrated care models to date, as evidenced by the widespread adoption of therapeutic hypothermia protocols for hypoxic-ischemic encephalopathy. Innovative therapeutic and diagnostic support for very low birth weight infants with brain injury has lagged behind. Given that concern for significant future neurodevelopmental impairment can lead to d

[5-year evaluation of bacteriological samples (CSF, blood cultures, urine) in neonatology].

We report the result of the bacteriological samples performed for 5 years (1980-1984) in neonates, referred to the neonate unit of the hospital of Clermont-Ferrand, for infection (2,894 infants). The CSF tests show aseptic meningitis in 4% of the cases and in 0.4% meningitis with bacteria on the direct examination and in culture. We emphasize the interest of carrying out the soluble bacterial antigen assay (Strepto B, Coli K1) to find out the bacteria involved in the meningitis. 13.7% of the infants have one or several positive blood culture. The germ is a E. Coli in 30% of the cases, and less

Evaluation of a Telesimulation Program to Support Psychosocial Communication Training for Neonatology Fellows.

Parents of infants in the Neonatal Intensive Care Unit (NICU) frequently experience significant emotional distress, yet neonatology fellows receive limited training in psychosocial support. This study evaluates the use of online standardized-patient simulation to improve fellows' communication skills addressing parent concerns. Fellows (n = 13) completed two virtual interactions with standardized parents, with feedback from a neonatal mental health professional (NMHP), a former NICU parent, and a neonatologist using narrative comments and the Gap-Kalamazoo Communication Skills Assessment Scale

ABCs of base therapy in neonatology: role of acetate, bicarbonate, citrate and lactate.

Metabolic acidosis is common in preterm and term newborn infants and may be attributed to a variety of etiologies, potentially requiring base therapy such as acetate or bicarbonate. However, concerns exist regarding potential harm of sodium bicarbonate due to intracellular acidosis, fluctuations in cerebral blood flow, and osmolar load with rapid infusions, with no improvement in survival when used during resuscitation. Alternative approaches to correct metabolic acidosis include the addition of acetate in parenteral nutrition, intravenous lactated Ringer's (LR) solution, and use of oral citra

Profile of antimicrobial use and potential use of the defined daily dose in neonatology.

One of the measures for monitoring microbial resistance is the calculation of the defined daily dose of antimicrobial agents. For this calculation, the weight of an adult of 70 kg is used as a standard, so that application in neonatology is not possible. The aim of this study is to describe the use profile and calculate the defined daily dose (DDD) of antimicrobials in a neonatal intensive care unit (NICU) of a public hospital in the interior of Bahia, Brazil. From March 2020 to December 2021, the medical records of 712 newborns admitted to a NICU between September 2018 and June 2020 were anal

Core outcomes in neonatology: development of a core outcome set for neonatal research.

BACKGROUND: Neonatal research evaluates many different outcomes using multiple measures. This can prevent synthesis of trial results in meta-analyses, and selected outcomes may not be relevant to former patients, parents and health professionals. OBJECTIVE: To define a core outcome set (COS) for research involving infants receiving neonatal care in a high-income setting. DESIGN: Outcomes reported in neonatal trials and qualitative studies were systematically reviewed. Stakeholders were recruited for a three-round international Delphi survey. A consensus meeting was held to confirm the final

Moral dilemmas in neonatology as experienced by health care practitioners: a qualitative approach.

During the last two decades there has been an enormous development in treatment possibilities in the field of neonatology, particularly for (extremely) premature infants. Although there are cross-cultural differences in treatment strategy, an overview of the literature suggests that every country is confronted with moral dilemmas in this area. These concern decisions to initiate or withhold treatment directly at birth and, later on, decisions to withdraw treatment with the possible consequence that the child will die. Given that the neonate cannot express his or her own will, who will decide?

[Transcutaneous bilirubin determination in the newborn infant. Recommendations of the Swiss Neonatology Group].

The clinical usefulness of transcutaneous bilirubinometry with the Minolta-Air Shield bilirubinometer was evaluated at four obstetrical neonatology units (Basle, Berne, Lucerne and Zurich). Transcutaneous bilirubin (TcB), expressed as bilirubin index, was compared with simultaneous serum bilirubin estimations in healthy Caucasians babies born at term with a birth weight of more than 2500 g. TcB measured over the forehead: 926 determinations in 629 newborns. TcB measured over the sternum: 404 estimations in 260 newborns. TcB estimations over the sternum produced a closer correlation (r = 0.93)

Improving Communication Competencies in Neonatology: A Multimodal Simulation-Based Palliative Care Workshop.

BackgroundEffective communication between providers and parents is fundamental in critical care provision. Particularly when discussing goals of care and delivering bad news, which is often complicated by differing values and beliefs. Many trainees lack formal education in communication skills for end-of-life care. Implementing a formal curriculum may improve physician-patient relationships, parents' mental health, and overall patient care.ObjectiveTo assess the impact of an integrative multimodal simulation-based palliative care workshop experience combining simulated complex medical cases wi
