Clinical overview of the synucleinopathies.

The term synucleinopathies is used to name a group of neurodegenerative disorders characterized by fibrillary aggregates of alpha-synuclein protein in the cytoplasm of selective populations of neurons and glia. These disorders include Parkinson's disease (PD), dementia with Lewy bodies (DLB), pure autonomic failure (PAF), and multiple system atrophy (MSA). Clinically, they are characterized by a chronic and progressive decline in motor, cognitive, behavioural, and autonomic functions, depending on the distribution of the lesions. Because of clinical overlap, differential diagnosis is sometimes

Ultrastructure of diffuse plaques in senile dementia of the Alzheimer type: comparison with primitive plaques.

We compared the ultrastructure between diffuse and primitive plaques in the brains of senile dementia, using pairs of routine electron microscopic ultrathin sections and adjacent semithin sections, which were immunolabeled for beta protein. In the frontal cortex, amyloid fibrils were rarely seen in a minority of diffuse plaques, suggesting an initial stage of the diffuse plaques. A majority of the diffuse plaques had electron-dense material and/or amyloid fibrils between cell processes in part of but not the entire beta/A4-immunoreactive areas. Small degenerating neurites were often seen with

Evidence for a diffusional model of Alzheimer amyloid A4 (beta-amyloid) deposition during neuritic plaque formation.

A recent study reported that Alzheimer senile plaques immunostained with monoclonal antibodies against the A4 (beta-amyloid) region of the amyloid precursor protein show gradients of density (Majocha R. E., Benes F. M., Reifel R. L., Rodenrys A. M. and Marotta C. A., Proc. natn. Acad. Sci. U.S.A. 85, 6182-6186, 1988). Although more than one explanation was suggested for this observation, the possible involvement of a diffusional process during plaque maturation was considered. In order to examine this hypothesis, specimens from prefrontal cortex, entorhinal area and hippocampal formation were

Porencephaly and hydranencephaly: a neuropathological study of four autopsy cases.

Four autopsy cases of porencephaly and hydranencephaly were evaluated clinico-pathologically. The patients showed profound mental retardation, convulsive seizures and spastic quadriplegia. The life spans were much shorter in the hydranencephalic cases. Pathologically, the cerebral cortex in the vicinity of the parenchymal defects often showed an abnormal cytoarchitecture, which appeared to be closely correlated with superficial glial proliferation. Several neurofibrillary tangles were found in the nucleus basalis of Meynert and the locus ceruleus in a 23-year-old porencephalic patient, and so-

System degeneration of the thalamus. A clinico-neuropathological study.

A case of essential degeneration of the thalamus is reported. The patient was a 43-year-old Japanese male, who, a few weeks after mild head trauma, suffered from forgetfulness, psychomotor slowing, and Korsakoff's syndrome. Four to five months later, there were optical hallucinations and delirium and he died 19 months after the onset of symptoms. Neuropathological examination revealed symmetrical thalamic degeneration, whose distribution corresponded to phylogenetically younger subunits of the thalamus. In addition, there was olivovermian degeneration. These findings are identical to those of

Presenile dementia with Lewy bodies and neurofibrillary tangles.

A 55-year-old man showed progressive mental deterioration over a five-year period along with a syndrome of "normal pressure hydrocephalus." At autopsy the main finding was the presence of two age-related neurocellular changes, the neurofibrillary tangle and the Lewy body, in limbic structures and in the pigmented nuclei of the brain stem. Cortical changes were mild, and senile plaques were not present. Electron microscopy occasionally showed an intimate relationship between the paired helical filaments of the neurofibrillary tangle and Lewy bodies in nerve cell processes in the hypothalamus. T

Antemortem diagnosis of diffuse Lewy body disease.

Using the presence of widespread cortical Lewy bodies (LB) as the pathologic criteria of diffuse Lewy body disease (DLBD), we describe serial neurologic and mental status examinations in 6 patients with DLBD, 3 patients with Alzheimer's disease (AD), and 1 patient with Parkinson's disease (PD). The 6 patients with DLBD included 3 with neocortical neurofibrillary tangles (NFT) consistent with coincident AD. Most patients with DLBD had gait impairment concurrent with mild to moderate dementia. Abnormalities of tone or resting tremor were also prominent early symptoms in the subjects with DLBD, b

Congo red birefringent structures in the hypothalamus in senile dementia of the Alzheimer type.

Serial cryostat sections of hypothalamus from four cases of Alzheimer-type dementia and four controls were stained with Congo red and examined for birefringence. Green birefringence and dichroism was associated with (a) neurofibrillary tangles, which were most numerous at the level of the tuberomammillary nucleus where they were found in large acetylcholinesterase-positive neurons; (b) neuropil processes within tangle fields; (c) a few plaque cores in the mammillary body; (d) spicules at the ependyma of the third ventricle, and (e) blood vessels. Birefringence (e) had different properties from

Familial Alzheimer's disease with myoclonus and 'spongy change'.

"Spongy change," or vacuolar change, was observed in the neocortex of temporal and frontal lobes in four of 27 patients with clinical and pathologic characteristics of Alzheimer's disease. All four cases had the autosomal dominant form of the disease, and aside from severe dementia, all developed myoclonus and became mute before death. The degeneration observed was unlike that seen in Creutzfeldt-Jakob disease because it lacked astrocytosis and had a different distribution of lesions. Moreover, the abundance of neurofibrillary changes and senile plaques was consistent with Alzheimer's disease.

Alzheimer's disease and the pivotal role of the hypothalamus and the intrinsic opioid system.

Although the "cholinergic hypothesis" of the pathophysiology of Alzheimer's disease has received great attention during the past years, it has recently come under increasing criticism specifically owing to failure of therapeutic endeavors based on this premise. As the potential broad role of the intrinsic opioids in the neurochemical modulation of diverse brain functions emerges, the realization that these data may be reconciled to a unifying hypothesis underlying the nature of some chronic dementing diseases (including Alzheimer's disease, Korsakoff disease and Parkinson's disease) occurs. Immunocytochemical studies of neurofilament antigens in the neurofibrillary pathology induced by aluminum.

Intrathecal administration of aluminum salts induces accumulation of neurofilaments in axons and perikarya of motor neurons and is associated with impaired axonal transport of neurofilament proteins. Because phosphorylation of the 200-kilodalton (kd) neurofilament protein, thought to be a major component of the sidearms, seems to be important in interactions of neurofilaments with other cytoskeletal elements, we have postulated that aluminum may produce neurofibrillary pathology by altering patterns of neurofilament phosphorylation. To test this hypothesis, antibodies against phosphorylated an

[Immunohistochemical study of the basic lesions of Alzheimer's disease].

Alzheimer's disease is characterized by the presence of two argentophilic histopathologic brain lesions: neurofibrillary tangles (NFT) and senile (neuritic) plaques (SP). NFT consist of large perikaryal masses of abnormal cytoplasmic fibers, most of which have the ultrastructural appearance of pairs of intermediate-sized (10 nm) filaments wound into a double helix and named Paired Helical Filaments (PHF). PHF also occur within the degenerating neurites of SP. The insolubility of PHF in strong detergents is turned to account for their isolation. We have isolated these structures from an Alzheim

Immuno-electron-microscopic localization of complements in amyloid fibrils of senile plaques.

The presence of components of immunoglobulins (Ishii et al. 1975) and complements (Eikelenboom and Stam 1982) in senile plaques suggests that the immunologic mechanisms are involved in the causation of pathologic processes in the brain of patients with Alzheimer's disease. Senile plaques consists of amyloid degenerated neuritis and glia, and exact localization of complements among these tissue elements will provide an important clue to the pathogenesis of the Alzheimer brain. This report deals with light- and electron-microscopic localization of complements in amyloid fibrils of senile plaques

Evidence that paired helical filaments originate from neurofilaments--electron microscope observations of neurites in senile plaques in the brain in Alzheimer's disease.

Neurites in senile plaques in the cerebral cortex from an autopsied case of Alzheimer's disease were examined by electron microscope. Swollen axons containing vastly increased numbers of neurofilaments (NF) together with paired helical filaments (PHF) were common. In some of these axons the continuity of NF with PHF was frequently discernible. In one, there were numerous figures of continuity in a bundle, indicating transition from NF to PHF. Moreover, some of these figures showed that two NF seemed to join to form one PHF. No straight 15 nm filament was noted anywhere in the sectioned plane o

Progressive dementia with "diffuse Lewy-type inclusions' in cerebral cortex. A case report.

A 69-year-old male suffering from progressive dementia died 3 years after the beginning of his disease. The neuropathology of this case revealed the coexistence of senile changes, typical for Alzheimer's disease, and the characteristics of Parkinson's disease, namely, numerous senile plaques and neurofibrillary tangles in the cerebrum and neuronal loss with depigmentation in the substantia nigra and locus caeruleus. Lewy-type inclusions were distributed not only in the pigmented brain stem nuclei, but also diffusely in the CNS. The close nosological relationship between paralysis agitans and A

Relative roles of irritant, type-J and pulmonary stretch receptors in lung reflexes.

Bilateral anodal block of both cervical vagus nerves of anaesthetized rabbits has been used to assess the relative role of afferent pathways with myelinated fibres (primarily from lung irritant receptors) and with nonmyelinated fibres (from J-receptors) in respiratory responses to some lung conditions. With inhalation of histamine aerosol the increase in breathing frequency is due to myelinated afferent fibres from irritant receptors. The similar response with inhalation of halothane is due to non-myelinated fibres from J-receptors. The reflex breathing response in lung oedema involves both pa

Differential susceptibility to neurofibrillary pathology among patients with Down syndrome.

Individual differences in the development of neurofibrillary changes were examined in eight cortical regions in the brains of 43 subjects with Down syndrome (DS; age range, 15-69 years) using sections stained with monoclonal antibodies (mAb) tau-1 and 3-39. Neurofibrillary pathology was found in 4 cases below 36 years of age and in all 20 cases above that age. In the 24 positive cases, numerical density of pretangles stained with tau-1 and 3-39, respectively, was 6.1/mm2 and 0/mm2; early tangles, 5.0/mm2 and 5.3/mm2; mature tangles, 4.0/mm2 and 5.0/mm2 (p < 0.01); and end-stage tangles, 0.04/m
