Neuromuscular monitoring using TOF-Cuff® versus TOF-Scan®: an observational study under clinical anesthesia conditions.

BACKGROUND: Anesthesia guidelines advise objective neuromuscular monitoring. Acceleromyography, the standard technique used in clinical practice, is commonly used with the train-of-four (TOF) nerve stimulation pattern. Objective of this study was to compare the performance of two devices, TOF-Scan® and TOF-Cuff®. METHODS: This prospective, controlled observational study included patients undergoing surgery in general anesthesia with the need of neuromuscular blockade. Both neuromuscular monitoring devices were simultaneously placed on individual patients. Atracurium (0.5 mg/kg) was administer

Utilizing physical educators to monitor muscular strength and neuromuscular control among children with varied recess time.

INTRODUCTION: Inactivity levels among children are climbing at alarming rates, leading to a lack of physical activities that produce muscular strength (MusS) development, which in turn creates effective neuromuscular control (NC) development. Developing appropriate MusS during childhood decreases the chances of physical injuries and many chronic diseases such as type II diabetes and cancer, which leads to healthier, active future adults. The purpose of this study was to utilize the physical education setting to examine MusS and NC factors in the Fall and Spring (Time 1 to Time 2) of one school

Standardization of the Extent and Timing of Rectus Diastasis Repair in Abdominoplasty Cases with TOF Neuromuscular Monitoring: A Clinical Study.

Despite the close focus on the anterior abdominal wall and the classification, maneuvers and materials related to the rectus abdominis and its fascia, there is no established standard concerning the extent of plication. The anterior abdominal wall is a musculoaponeurotic structure and a dynamic motor system with innervation and tonus. Therefore, the timing of the plication is as important as the suture technique and material, since the muscle relaxant will lose its effect in varying time periods. The aims of our study were to determine the change in the amount of plication between groups with

Anesthesiologist's Concerns About Dandy-Walker Syndrome: Airway Management, Muscle Relaxants, and Train-of-Four Monitoring of Neuromuscular Blockade.

Dandy-Walker syndrome is a rare congenital malformation of the posterior fossa that presents unique anesthetic challenges, including difficult airway management, altered consciousness, hydrocephalus, and potential for prolonged postoperative ventilation. Hydrocephalus, the most common finding, can complicate ventilation and intubation, whereas brainstem involvement, along with agenesis of the corpus callosum, pontine hypoplasia, and distortion of the medullary respiratory centers, may contribute to prolonged postoperative ventilation. Anesthetic management in such cases requires thorough airwa

Intraoperative spinal cord monitoring during the surgical correction of scoliosis due to cerebral palsy and other neuromuscular disorders.

PURPOSE: Patients with neuromuscular scoliosis are at increased risk of neurological deficit post-operatively, but are a difficult population on whom to perform neurophysiological monitoring. We look here at a 7-year sample of our practice in the monitoring of neuromuscular patients. METHODS: A retrospective chart review was performed for 109 patients who underwent correction of neuromuscular scoliosis within our institution between 2005 and 2011. RESULTS: Of 109 patients who were identified, intraoperative monitoring was attempted in 66 cases. In eight cases (13 %), no reliable monitoring c

Electromyographic facial nerve monitoring during resection for acoustic neurinoma under moderate to profound levels of peripheral neuromuscular blockade.

The facial nerve is monitored intra-operatively using electromyography to identify and prevent damage during the excision of an acoustic neurinoma. In order to determine whether a profound level of peripheral neuromuscular blockade could be achieved without compromising facial electromyographic monitoring, 11 patients undergoing resection of acoustic neurinoma were studied. After induction of anaesthesia, an infusion of atracurium was administered to reduce the integrated electrically evoked muscle potential (EEMP) of the hypothenar eminence by 70-100%. The facial nerve was directly stimulated

Sugammadex versus no pharmacological reversal under quantitative neuromuscular monitoring: a propensity score-matched analysis of postoperative pulmonary complications.

PURPOSE: Sugammadex may reduce postoperative pulmonary complications (PPCs). However, its association with PPCs remains unclear when quantitative neuromuscular monitoring (NMT) is routinely used, and comparisons with spontaneous recovery are limited. We compared PPCs between cases receiving sugammadex and those receiving no reversal agent. METHODS: We included adult noncardiac surgical cases from 2010 through 2020. Eligible cases involved rocuronium administration and removal of the airway device in the operating room. The primary outcome was postoperative pneumonia within 30 days. Secondary

Updated review on the use of neuromuscular blockade during intraoperative motor-evoked potential monitoring in the modern anesthesia era.

Transcranial electrical stimulation motor-evoked potentials (Tc-MEP) monitoring is a common practice in neurosurgery to prevent postoperative neurological damage. However, the use of neuromuscular blocking agents (NMBAs) during Tc-MEP monitoring is a subject of controversy. In addition, the effectiveness of sugammadex, a selective reversal agent, in the context of Tc-MEP monitoring requires further investigation. This review aimed to clarify the considerations involved in achieving optimal Tc-MEP monitoring while ensuring patient safety. Preoperative patient selection, comorbidity assessment,

Quantitative Neuromuscular Monitoring Permits Early Diagnosis of Abnormal Butyrylcholinestrase: Two Case Studies Demonstrating Prevention of Awareness from Premature Awakening.

Administration of succinylcholine to patients with a variant in the butyrylcholinesterase (BChE) gene increases the risk of anesthesia emergence prior to recovery from neuromuscular blockade (NMB). Application of quantitative neuromuscular monitoring (NMM) can identify residual NMB. We present two patients with abnormal BChE gene variants. In the first case, quantitative monitoring was applied too late to prevent awareness, but allowed diagnosis and prevented admission to the intensive care unit. In the second case, monitoring was applied prior to NMB, which enabled early diagnosis and prevent

To jog or to stride: A comparison between two standardised running tests to monitor neuromuscular status in team sport athletes.

This study investigated: 1) monitoring post-match neuromuscular fatigue (NMF) status in rugby union players using two submaximal running tests (SRT); and 2) the sensitivity of each SRT to locomotor variables obtained during match-play. Twenty-three male rugby players (age: 21.0 ± 1.3 years; height: 185.2 ± 6.1 cm; body mass; 97.3 ± 10.3 kg) were monitored across one season (n = 71 player-match and 159 fatigue-players-testing observations). Two different SRTs (SRT-jog [5-minute shuttle run protocol] and SRT-stride [a repeat stride effort protocol]) were used to characterise post-match NMF, with

Forty years of neuromuscular monitoring and postoperative residual curarisation: a meta-analysis and evaluation of confidence in network meta-analysis.

BACKGROUND: The reported incidence of postoperative residual curarisation (PORC) is still unacceptably high. The capacity of intraoperative neuromuscular monitoring (NMM) to reduce the incidence of PORC has yet to be established from pooled clinical studies. We conducted a meta-analysis of data from 1979 to 2019 to reanalyse this relationship. METHODS: English language, peer-reviewed, and operation room adult anaesthesia setting articles published between 1979 and 2019 were searched for on PubMed, Cochrane Central Register of Controlled Trials, ISI-WoK, and Scopus. The primary outcome was POR

Reversal of moderate and intense neuromuscular block induced by rocuronium with low doses of sugammadex for intraoperative facial nerve monitoring.

We report two cases in which moderate and intense rocuronium-induced neuromuscular block was reversed intraoperatively with low sugammadex doses in order to facilitate electromyographic evaluation of facial nerve function during surgery of the parotid gland and the middle ear. Acceleromyography was used to assess reversal of neuromuscular block before starting electromyography monitoring. Rocuronium-induced neuromuscular block was reversed with sugammadex 0.22mgkg(-1) when the TOF ratio was 0.14 in the first patient, and with sugammadex 2mgkg(-1) during intense block (PTC 0) in the second pati

Utility of Breath-Holding Time in Monitoring Acute Neuromuscular Respiratory Failure in Bulbar-Onset Myasthenia Gravis: A Case Report.

We report the management of a 64-year-old male with newly diagnosed bulbar-onset myasthenia gravis (MG) who was hospitalized with acute neuromuscular respiratory insufficiency. This case highlights the challenges in monitoring respiratory function in MG patients, especially in the presence of bulbar and nuchal weakness, and emphasizes the potential utility of single breath-hold time (SBHT) over forced vital capacity (FVC) as a reliable bedside monitoring tool. Despite initial stabilization with intravenous immunoglobulin (IVIG), the patient deteriorated, requiring escalation to the intensive c

Neuromuscular Monitoring of Rocuronium 0.6 mg/kg in Children Aged 3-24 Months With Electromyography and Acceleromyography: An Observational Study.

BACKGROUND: Objective neuromuscular monitoring is essential to prevent residual neuromuscular block in patients receiving neuromuscular blocking agents. However, the small size of pediatric hands and the lack of appropriately sized neuromuscular equipment have made quantitative neuromuscular monitoring at the ulnar nerve (UN) difficult. In such cases, the tibial nerve (TN) can serve as an alternative site. This study aimed to compare train-of-four (TOF) monitoring using acceleromyography (AMG) and electromyography (EMG) at the TN and UN in children aged 3-24 months. We hypothesized that the ti
