Slow flow and no reflow after percutaneous coronary intervention.

Primary percutaneous coronary intervention is currently the standard of care for the treatment of acute ST-segment elevation myocardial infarction (STEMI). While it can restore flow in the infarcted artery in the majority of cases, some patients experience the so-called "no-reflow" phenomenon, which consists of abnormal myocardial reperfusion occurring after the occluded coronary artery has been opened. It is caused by microvascular obstruction (MVO), and its pathogenesis is multifactorial, including myocardial ischaemia, distal embolisation, and ischaemia-reperfusion injury, combined with ind

Advances in no-reflow after stroke reperfusion therapy.

Representing a standard treatment for acute ischemic stroke, reperfusion therapy encompasses intravenous thrombolysis (IVT) and endovascular therapy (EVT). Although EVT achieves a high rate of complete large vessel recanalization, more than 50 % of patients who have successful reperfusion (modified Thrombolysis in Cerebral Infarction score of 2b-3) still don't have favorable outcomes. Therefore, futile reperfusion (FR) refers to the treatment failure to achieve favorable outcomes despite successful recanalization of occluded arteries and subsequent reperfusion of the downstream blood flow. FR

Lower Muscle Strength and Increased Visceral Fat Associated With No-reflow and High Gensini Score in STEMI.

BACKGROUND: The impact of fat distribution, muscle mass, and muscle strength on no-reflow and severity of coronary artery disease in patients with ST-segment elevation myocardial infarction (STEMI) remains unclear. OBJECTIVE: To investigate association between muscle strength and fat and muscle mass and severity of coronary atherosclerosis. METHODS: We included 218 patients with STEMI who had undergone primary percutaneous coronary intervention. We evaluated the no-reflow phenomenon in infarct-related artery and calculated Gensini scores from initial angiograms as indicative of coronary athe

The Application of Coronary Contrast Emptying Time in Diagnosing Coronary Slow Flow Phenomenon: A Serial Case Report.

The Coronary Slow Flow Phenomenon doesn't achieve as much attention as its counterpart Coronary Arterial Disease because it is considered a rather benign entity. But now it is proven that coronary slow flow phenomenon can also manifest as an acute coronary syndrome, myocardial ischemia, malignant arrhythmia, and even sudden cardiac death. This entity is usually diagnosed from coronary angiography study when a delayed coronary contrast filling time is found without the presence of significant epicardial narrowing of the related arteries. But, in our center's years of experience, we frequently fo

[Our enemy : the clot. Coronary thrombosis : strategy and therapeutic arsenal].

Intra coronary thrombus is  frequently encountered during acute coronary syndromes revascularisation procedures. It can also be encountered during angioplasty procedures in a stable angina context, although at a much lesser frequency. In both situations, it harbors a risk of poor angiographic result and poor prognosis. Intracoronnary thrombus may cause coronary occlusion at the angioplasty site or distal embolic  flow obstruction. Per procedure thrombus prevention rests on an prior optimal anti thrombotic treatment and in some circumstances the choice to defer the revascularisation procedure in

Unilateral peripheral slow flow phenomenon without significant stenosis in lower extremity artery: can primary peripheral slow flow be a new phenomenon?

Peripheral artery disease affects nearly a quarter billion of the world's population, and it is one of the most important causes of decreased quality of life. Primary peripheral slow flow without significant stenosis in peripheral arteries has not been previously reported in the literature. In this case report, we present a slow flow phenomenon that extended from the right external iliac artery to the distal peripheral bed observed during peripheral angiography in a patient who exhibited pain and claudication in the right lower extremity after walking 50 m. Medical treatment provided symptomat

Development and preliminary internal validation of a prediction model incorporating cardiac troponin I (cTnI), N-terminal pro-B-type natriuretic peptide (NT-proBNP), and thyroid function for no-reflow during percutaneous coronary intervention in patients with acute coronary syndrome.

BACKGROUND: Coronary artery disease is a leading global cause of death, and acute coronary syndrome (ACS) is its most critical acute form. Percutaneous coronary intervention (PCI) is the standard reperfusion treatment for this condition, but no‑reflow occurs in 30-40% of emergency PCI cases, and this phenomenon is associated with higher mortality and poor prognosis. Coagulation, cardiac, and thyroid function markers, reflecting thrombus burden, myocardial injury, and vascular dysfunction, are closely correlated with no‑reflow, but single biomarkers have limited predictive value. This study aim

Prediction of Major Adverse Cardiovascular Events and Slow/No-Reflow by Virtual Histology Imaging After Percutaneous Interventions on Saphenous Vein Grafts.

Saphenous vein grafts disease (SVGD) is a common complication after coronary artery bypass graft (CABG) and usually treated by percutaneous coronary intervention (PCI). In this prospective cohort study, we performed virtual histology-intravascular ultrasound to investigate whether plaque composition and morphological characteristics were associated with post-PCI major adverse cardiac events (MACEs) and slow/no-reflow in patients with SVGD. Patients (n = 90) were studied (76.7% men, mean age 64.9 ± 8.2 years and mean duration of SVG 8.0 ± 3.6 years). There were 77.8% lesions with a plaque burde

Novel Guidewire Design and Coating for Continuous Delivery of Adenosine During Interventional Procedures.

Background The "no-reflow phenomenon" compromises percutaneous coronary intervention outcomes. There is an unmet need for a device that prevents no-reflow phenomenon. Our goal was to develop a guidewire platform comprising a nondisruptive hydrophilic coating that allows continuous delivery of adenosine throughout a percutaneous coronary intervention. Methods and Results We developed a guidewire with spaced coils to increase surface area for drug loading. Guidewires were plasma treated to attach hydroxyl groups to metal surfaces, and a methoxy-polyethylene glycol-silanol primer layer was covale

No Reflow at Perfusion Imaging: Thresholds That Distinguish Functional and Tissue Outcome Groups in Acute Stroke.

Background Poor outcome after successful endovascular thrombectomy (EVT) may reflect microvascular dysfunction, or no-reflow phenomenon, but operational definitions are lacking. Purpose To identify the most informative candidate no-reflow definition at 24-hour post-EVT perfusion imaging for outcome assessment. Materials and Methods This secondary analysis used data from two prospective multicenter imaging registries of patients with acute stroke who were recruited between June 2016 and July 2022, achieved modified thrombolysis in cerebral infarction grade 2c or 3 recanalization after EVT, and

Cerebral No-Reflow after Thrombectomy: International Delphi Consensus on Definition and Imaging Assessment.

BACKGROUND: The operational definition and imaging assessment of the no-reflow phenomenon, a potential contributor to poor clinical outcome despite successful thrombectomy, remains debated. We aimed to develop expert-based international consensus on the definition, assessment, and reporting standard of cerebral no-reflow post-thrombectomy. METHODS: Three-rounds of a modified Delphi web-based survey was conducted among corresponding authors of peer-reviewed clinical studies on cerebral no-reflow between 2020 and 2025. Responses at each round were used to develop structured questionnaires for s

Growth differentiation factor 15 may protect the myocardium from no‑reflow by inhibiting the inflammatory‑like response that predominantly involves neutrophil infiltration.

The aim of the current study was to investigate the time course of the expression of growth differentiation factor‑15 (GDF‑15) in rat ischemic myocardium with increasing durations of reperfusion, and to elucidate its physiopathological role in the no‑reflow phenomenon. Wistar rats were randomly divided into ischemia reperfusion (I/R) and sham groups, and myocardial I/R was established by ligation of the left anterior descending coronary artery for 1 h followed by reperfusion for 2, 4, 6, 12, 24 h and 7 days whilst rats in the sham group were subjected to a sham operation. The expression levels

Pathophysiology, prevention, and management of coronary microvascular obstruction.

Although prompt primary percutaneous coronary intervention (PCI) reduces mortality in patients with ST-elevation myocardial infarction (STEMI), the burden of post-infarction heart failure remains considerable and is expected to increase. A major contributory factor is suboptimal myocardial reperfusion, which persists in up to 60% of cases even with timely revascularization. This is largely driven by microvascular obstruction and ischaemia-reperfusion injury, culminating in the no-reflow phenomenon, a critical prognostic factor associated with impaired infarct healing, adverse left ventricular

Long-term outcomes of the modest stent expansion strategy for the culprit lesion of acute myocardial infarction.

Slow flow during primary percutaneous coronary intervention (PCI) is a common complication. Our group showed that the stent (or post-balloon) diameter-to-vessel diameter ratio was inversely associated with slow flow phenomenon. We advocated the utility of modest stent expansion strategy, which was defined as the stent (or post-balloon) diameter-to-culprit vessel diameter ratio < 0.71, for prevention of slow flow phenomenon. This study aimed to compare the long-term outcomes in patients with acute myocardial infarction (AMI) between the modest stent expansion strategy and the aggressive stent e
