Mechanism of Chronic Pain of Symptomatic Hip Osteoarthritis by Association of its Distribution, Nociceptive, Neuropathic, Nociplastic, or Mixed-pain Screening, and the Prevalence of Lumbar Spinal Stenosis: A Cross-sectional Study.

OBJECTIVES: The pain of hip osteoarthritis (OA) is generally recognized as nociceptive in origin because of the local pathology. However, some patients with OA experience a neuropathic pain component as an essential part of some nociplastic pain subtype. Here, we sought to examine the mechanism of chronic pain of symptomatic hip OA by the association of its distribution, complex pain mechanism screening, and the prevalence of lumbar spinal stenosis. MATERIALS AND METHODS: We conducted a prospective cross-sectional study of 100 hips in 100 patients with symptomatic hip OA in a chronic state. W

Melatonin Mitigates Central Sensitization and Nociplastic Pain in Spinal Cord and Dorsal Root Ganglia of FM Rat Model: Modulation of SIRT1/PGC-1α/MAPK/NF-κB Signaling.

Fibromyalgia (FM) is a central sensitization syndrome characterized by neuroinflammation and synaptic hyperexcitability, which amplifies pain signals in the spinal cord (SC) and dorsal root ganglia (DRG). Melatonin (MEL) has demonstrated analgesic and antinociceptive properties in experimental models, supporting its clinical application in various pathological conditions. This study aimed to elucidate the impact of Melatonin on neuroinflammation and glutamatergic dysregulation in the spinal cord and DRG using a Reserpine-induced fibromyalgia model (RIFM). RIFM was induced in female rats by adm

Neuroinflammatory activation in sensory and motor regions of the cortex is related to sensorimotor function in individuals with low back pain maintained by nociplastic mechanisms: A preliminary proof-of-concept study.

BACKGROUND: Chronic pain involves communication between neural and immune systems. Recent data suggest localization of glial (brain immune cells) activation to the sensorimotor regions of the brain cortex (S1/M1) in chronic low back pain (LBP). As glia perform diverse functions that impact neural function, activation might contribute to sensorimotor changes, particularly in LBP maintained by increased nervous system sensitivity (i.e., nociplastic pain). This preliminary proof-of-concept study aimed to: (i) compare evidence of neuroinflammatory activation in S1/M1 between individuals with and w

The complexity of pain in inflammatory arthropathies beyond pain intensity and impact: An OMERACT initiative.

INTRODUCTION: People with IA may suffer from pain of differing aetiologies and subtypes including nociceptive joint pain, neuropathic pain of carpal tunnel syndrome or nociplastic pain from concomitant fibromyalgia. Lack of precise measurement tools to identify nociplastic pain influences as a contextual factor potentially all outcomes in collected clinical trials as residual pain might impact various measurements in IA. The OMERACT 2025 pain SIG discussed, developing a scoping review from protocol, to identify an instrument to measure nociplastic pain in IA and a contextualised domain definit

Exploring Pain Phenotyping in Cervicogenic Headache Management.

PURPOSE OF REVIEW: Intervention trials for cervicogenic headache (CGH) often yield equivocal results with marked treatment effect heterogeneity, possibly reflecting variations in underlying pain mechanisms throughout the CGH population. Pain phenotyping or classifying patients into subgroups based on their predominant pain mechanism may facilitate more precise CGH treatment. This review aims to explore the role of pain phenotyping in CGH management. RECENT FINDINGS: Clinical evidence suggests two predominant pain mechanisms in the CGH population: nociceptive and nociplastic. Arguably, treatme

Application of the IASP Grading System to Identify Underlying Pain Mechanisms in Patients With Knee Osteoarthritis: A Prospective Cohort Study.

OBJECTIVES: This study aimed to apply the International Association for the Study of Pain (IASP) grading system for identifying nociplastic pain in knee osteoarthritis (KOA) awaiting total knee arthroplasty (TKA) and propose criteria to fine-tune decision-making. In addition, the study aimed to characterize a "probable" versus "no or possible" nociplastic pain mechanism using biopsychosocial variables and compare both groups in their 1-year post-TKA response. METHODS: A secondary analysis of baseline data of a longitudinal prospective study involving 197 patients with KOA awaiting total TKA i

Characterizing chronic pain phenotypes in multiple sclerosis: a nationwide survey study.

Chronic pain is highly prevalent in multiple sclerosis (MS). Pain heterogeneity may contribute to poor treatment outcomes. The aim of this study was to characterize pain phenotypes distributions in persons with MS and compare pain phenotypes in terms of pain intensity, frequency of chronic overlapping pain conditions, and use and analgesic effects of different classes of pain medications. Data were collected through a national web-based survey with measures of neuropathic (painDETECT) and nociplastic pain (Fibromyalgia Survey Criteria), chronic overlapping pain conditions, and pain medication

At the intersection of anger, chronic pain, and the brain: A mini-review.

Chronic pain remains one of the most persistent healthcare challenges in the world. To advance pain treatment, experts have recently introduced research-driven subtypes of chronic pain based on proposed underlying mechanisms. Nociplastic pain (e.g., nonspecific chronic low back or fibromyalgia) is one such subtype which may involve a greater etiologic role for brain plasticity, painful emotions induced by life stress and trauma, and unhealthy emotion regulation. In particular, correlational and behavioral data link anger and the ways anger is regulated with the presence and severity of nocipla

Clinical Phenotyping for Pain Mechanisms in Urologic Chronic Pelvic Pain Syndromes: A MAPP Research Network Study.

Three categories of pain mechanisms are recognized as contributing to pain perception: nociceptive, neuropathic, and nociplastic (ie, central nervous system augmented pain processing). We use validated questionnaires to identify pain mechanisms in Urologic Chronic Pelvic Pain Syndrome (UCCPS) patients (n = 568, female = 378, male = 190) taking part in the Symptom Patterns Study of the Multidisciplinary Approach to the study of chronic Pelvic Pain Research Network. A cutoff score of 12 on the painDETECT questionnaire (-1 to 38) was used to classify patients into the neuropathic category while t

Mirogabalin and pregabalin alleviate nociplastic sensitization induced by chemogenetic activation of the central amygdala neurons in rodents.

Nociplastic pain represents the third mechanistic descriptor of pain, alongside neuropathic and nociceptive pain, as proposed in 2017 by the International Association for the Study of Pain (IASP). It describes pain occurring in the absence of nociceptor activation, tissue damage, or neuropathy. The underlying brain mechanisms of nociplastic pain remain poorly understood. Despite the potentially large patient population with chronic pain of this class, effective pharmacological treatments for nociplastic pain are still limited, highlighting the urgent need for drug development using appropriate

Adapting Tension-Type Headache to the International Association for the Study of Pain (IASP) Taxonomy: A Conceptual Proposal.

Tension-type headache (TTH) is a primary headache characterized by treatment effect heterogeneity, possibly reflecting variations in underlying pain mechanisms. Identifying pain phenotyping or classifying patients into subgroups based on their predominant pain mechanism may provide targeted treatment options. This paper proposes a conceptual adaptation of the International Association for the Study of Pain (IASP) clinical criteria to identify the predominant phenotype in individuals with TTH. It has been found that pain referrals elicited by trigger points (TrPs) reproduce headache symptoms in

Proposal to refine the mechanistic classification of chronic pain and corresponding clinical symptomatology.

The management and treatment of patients suffering from chronic pain must be based on a precise understanding of the pathophysiological mechanisms underlying the syndrome. However, it cannot be defined solely by the presence or absence of disease or injury affecting either non-neuronal tissues or the neuronal structures of the somatosensory system, as these elements do not always accurately or causally reflect the underlying mechanisms of the pain experienced by patients. The current taxonomy of chronic pain classically includes three main mechanisms: nociceptive, neuropathic, and nociplastic.
