Thinking beyond the bladder: antidiuretic treatment of nocturia.

Nocturia is a bothersome and highly prevalent urological condition characterised by the need to wake to void at night. Contrary to popular misconception, nocturia is equally common in men and women, and although its prevalence increases with age, a significant proportion of younger people are also affected. Nocturia leads to repeated fragmentation of sleep and consequently to a serious decline in daytime functioning and in overall quality of life and health. As such, its impact should not be underestimated by clinicians. Traditionally, nocturia has been regarded as a symptom of benign prostati

Nocturia: an Austrian study on the multifactorial etiology of this symptom.

AIMS: To identify the different factors contributing to nocturia in a clinical setting. PATIENTS AND METHODS: Three hundred twenty-four patients (133 women, 191 men; mean age 63 years) were entered into this multi-institutional study. When presenting with nocturia we obtained detailed medical history and performed urine analysis, post-void residual volume and renal ultrasonography. Bothersome score and quality of life were evaluated using visual analogue scale and Kings' Health Questionnaire (KHQ), respectively. Patients were asked to complete a 48-hr voiding diary (VD). Nocturia and its asso

Effect of naftopidil on nocturia after failure of tamsulosin.

OBJECTIVES: The clinical usefulness of naftopidil was evaluated in 122 patients with benign prostatic hyperplasia for urinary tract symptoms and signs, focused in particular on nocturia. METHODS: A total of 122 patients with BPH whose symptoms did not improve after 6 weeks of tamsulosin administration were enrolled. After the treatment was followed by a washout period with placebo, patients were prescribed 75 mg of naftopidil to be taken after dinner for 6 weeks, and the efficacy was re-evaluated. All the drugs used were unidentified, and attention was given to not have the patients recognize

Causes of frequency and nocturia after renal transplantation.

OBJECTIVE: To explore the role of bladder capacity, bladder pain, dysfunctional voiding, urgency, urinary tract infections (UTIs), and urinary output as potential causes of frequency and nocturia after renal transplantation. PATIENTS AND METHODS: Data were gathered from 52 adult renal transplant patients (35 men and 17 women, mean age 49 years), using a written questionnaire, medical records, frequency/volume charts, and urinary cultures. The mean time between transplantation and data collection was 5 months. Structural equation modelling (SEM) was used for the simultaneous assessment of dire

Nocturia and the burning mouth syndrome (BMS) in the elderly.

The aim of this study was to assess the relationship between BMS and nocturnal micturition in a group of elderly men and women. The study comprised 6103 elderly men and women recruited from a group of pensioners by means of a questionnaire (n=10,216; response rate 61.3%). The mean (+/-S.D.) ages of the men and women were 73.0+/-6.0 years and 72.6+/-6.7 years, respectively. The questionnaire included questions on their health, diseases and symptoms, drugs, sleep habits and the number of nocturnal voiding episodes. BMS was reported by 2.4% of the men and 8.5% of the women (p<0.0001). The occurre

Impact of the International Continence Society (ICS) report on the standardisation of terminology in nocturia on the quality of reports on nocturia and nocturnal polyuria: a systematic review.

OBJECTIVE: To systematically review and evaluate the impact of the International Continence Society (ICS)-2002 report on standardisation of terminology in nocturia, on publications reporting on nocturia and nocturnal polyuria (NP). In 2002, the ICS defined NP as a Nocturnal Polyuria Index (nocturnal urine volume/total 24-h urine volume) of >0.2-0.33, depending on age. MATERIALS AND METHODS: In April 2013 the PubMed and Embase databases were searched for studies (in English, German, French or Dutch) based on original data and adult participants, investigating the relationship between nocturia

Is it effective to do mathematical analysis for the etiology of nocturia using the nocturia indices derived from the frequency volume chart?: A retrospective observational study.

This study aims to determine the problems involved in using a mathematical calculation to analyze the pathophysiology of nocturia using a frequency volume chart (FVC). In this retrospective study, we reviewed 253 patients who completed the FVC during ≥ 3 days for nocturia from January 2017 to February 2019. The etiology of nocturia was defined as a combination of 4 pathophysiologies, including nocturnal polyuria (NP), decreased bladder capacity (dBC), decreased nocturnal bladder capacity, and global polyuria. To analyze the differences according to diagnostic criteria for NP and dBC, 2 definit

Analysis of changes in the pathophysiology of nocturia according to the number of nocturia episode, age, and gender using frequency volume charts: A retrospective observational study.

PURPOSE: To evaluate the pathophysiology of nocturia based on the frequency volume chart, and determine the risk factors for nocturia occurring ≥2 times per night. MATERIALS AND METHODS: In this retrospective study, we reviewed 311 patients with complaints of nocturia from January 2017 to February 2019 at our institution. Nocturnal polyuria (NP) and global polyuria (GP) were defined as NP index >0.35 regardless of age and 24 h urine volume >2.5 L/day, respectively. Decreased bladder capacity (dBC) was when the maximal voided volume was <325 mL. Decreased nocturnal bladder capacity (dNBC) was

First voided volume: A novel approach to characterize nocturia.

AIMS: Nocturnal polyuria syndrome (NPS) denotes nocturnal polyuria (NP) in the absence of identifiable contributory factors. The trajectory of nocturnal urine production (NUP; typically expressed as ml/hour) may be useful in delineating between NP patients with versus without NPS, but changes in absolute urine volume, the directly measured substrate for behavioral and pharmacologic interventions targeting nocturnal urine production, have not been well characterized. This study compares the ratio of the first nocturnal voided volume (FNVV) to the nocturnal average voided volume (NAVV) in patien

Nocturia: when the heart wakes you up at night.

BACKGROUND: Nocturia is a common and burdensome symptom that may reflect underlying cardiovascular dysfunction. While previous studies have linked nocturia to heart failure and fluid overload, direct hemodynamic evidence supporting this association remains limited. OBJECTIVE: To evaluate the relationship between left- and right-sided filling pressures, particularly pulmonary artery wedge pressure (PAWP) and right atrial (RA) pressure, and nocturia severity in a cohort of patients undergoing right heart catheterization. METHODS: We conducted a prospective observational study of 74 adult patie

Disease-Related Etiologies of Nocturia: A Focused Literature Review.

PURPOSE OF REVIEW: Nocturia is a highly prevalent symptom that affects multiple aspects of health. However, it is often underrecognized and undertreated, leading to problems both within the genitourinary system and systemically. The goal of this paper is to further characterize the pathophysiology behind the most common medical factors that contribute to nocturia by performing a focused literature review in a structured format. This will aid clinicians when evaluating and treating patients with nocturia. RECENT FINDINGS: Based on expert opinion, we recognized six main common pathologies assoc

Improving Nocturia Management Through Sleep Apnea Diagnosis and Treatment.

INTRODUCTION/BACKGROUND: Obstructive sleep apnea (OSA) has an established association with nocturia, but even when referred, patients presenting with nocturia may not undergo a full evaluation or begin treatment for underlying OSA. At our institution, patients with nocturia (≥ 2 episodes per night) are often referred for at-home sleep studies. This study aims to assess if patients presenting with nocturia undergo evaluation for OSA and the impact of continuous or automatic positive airway pressure (CPAP/APAP) on nocturia severity. METHODS: We conducted a retrospective chart review of patients

Nocturia due to nocturnal polyuria (NP). A common disorder.

To the Editor, Nocturia is a significantly underestimated medical problem that affects seriously patients' quality of life, work engagement, productivity, and overall life conditions per se. Nocturia is a common condition, proven to be the most bothersome for patients with Lower Urinary Tract Symptoms (LUTS). People with two or more events of micturition per night have a significant increase in mortality rate and an increased risk of fall-related fractures if they visit the toilet two or more times per night [...].

Sleep disordered breathing and nocturnal polyuria: nocturia and enuresis.

Although nocturnal voiding is frequently attributed to urologic disorders, nocturia and enuresis are also important symptoms of sleep-disordered breathing. However, polyuria can be elicited by obstructive sleep apnea as well as bedrest, microgravity and other experimental conditions where the blood volume is shifted centrally to the upper body. The nocturnal polyuria of sleep apnea is an evoked response to conditions of negative intrathoracic pressure due to inspiratory effort posed against a closed airway. The mechanism for this natriuretic response is the release of atrial natriuretic peptid
