Cardiovascular medication adherence testing in patients living with HIV: A single-centre observational study.

INTRODUCTION: People with HIV (PWH) are at an increased risk of developing cardiovascular disease (CVD) compared to HIV-negative individuals. We sought to evaluate the adherence to medications for CVD in PWH and identify factors associated with non-adherence to these medications. METHODS: We conducted a cross-sectional study at the University Hospitals of Leicester NHS Trust between 16 April 2019 and 8 November 2022. We recruited consecutive PWH, who were attending a routine follow-up outpatient appointment and were prescribed at least one medication for CVD. In addition, we included urinary

Directly Measured Adherence to Treatment in Chronic Heart Failure: LEVEL-CHF Registry.

BACKGROUND: Currently, most available data on the medication adherence of patients with chronic heart failure are based on indirect methods. We examined the level of adherence to medical therapy using a direct method - serum drug level testing. METHODS: We carried out a prospective single-centre registry of patients with chronic heart failure (LEVEL-CHF registry), in whom we analysed serum levels of the medications prescribed for the treatment of heart failure: angiotensin converting enzyme inhibitors, angiotensin receptor blockers, beta-blockers and mineralocorticoid receptor antagonists. We

Non-prescribed drug use and its associated factors among pregnant women in Southwest Ethiopia.

Non-prescribed drug utilization is the act of using medication to treat self-diagnosed problems without consulting a healthcare provider. Pregnant women are among the most vulnerable population groups for self-medication to treat pregnancy-related problems. The use of non-prescribed drugs, however, has numerous detrimental effects on both the growing fetus and the mother. Besides, community-based information regarding the pattern of non-prescribed drug use is limited in Ethiopia. Hence, this study aims to investigate non-prescribed drug use and its associated factors among pregnant women in Ji

Provider barriers to prescribing HAART to medically-eligible HIV-infected drug users.

We aimed to identify factors associated with a medical provider's resistance to prescribing HAART to medically-eligible HIV-infected illicit drug users. In four US cities, a mailed, self-administered survey queried 420 HIV care providers about patients' characteristics and barriers to care. Adjusted odds ratios (AOR) and 95% confidence intervals (CI) were estimated using logistic regression. Providers identified as resistant to prescribing HAART to medically-eligible HIV-infected illicit drug users were more likely to be non-physicians (AOR=1.89, 95% CI: 1.04-3.46), to work in populations with

A Cross-Sectional Survey of Fixed-Dose Combination Antihypertensive Medicine Prescribing in Twenty-Four Countries, Including Qualitative Insights.

BACKGROUND: Treatment inertia, non-adherence and non-persistence to medical treatment contribute to poor blood pressure (BP) control worldwide. Fixed dose combination (FDC) antihypertensive medicines simplify prescribing patterns and improve adherence. The aim of this study was to identify factors associated with prescribing FDC antihypertensive medicines and to understand if these factors differ among doctors worldwide. METHODS: A cross-sectional survey was conducted online from June 2023 to January 2024 to recruit doctors. We collaborated with an international network of researchers and cli

Buprenorphine prescribing is increasingly delivered by primary care nurse practitioners to Medicaid beneficiaries.

BACKGROUND: Medicaid beneficiaries have disparate access to buprenorphine, a lifesaving medication for opioid use disorder (OUD) that can be prescribed in primary care settings. Nurse practitioners (NPs) are a growing primary care workforce, but their buprenorphine prescribing habits compared with physicians and the impact of recent policy changes on their practice is unknown. PURPOSE: To characterize buprenorphine prescribing by primary care NPs and physicians to Medicaid beneficiaries with OUD and to examine the impact of the removal of the buprenorphine waiver on prescribing behavior. Medication misuse and overuse in community-dwelling persons with dementia.

BACKGROUND: Persons with dementia (PWD) have high rates of polypharmacy. While previous studies have examined specific types of problematic medication use in PWD, we sought to characterize a broad spectrum of medication misuse and overuse among community-dwelling PWD. METHODS: We included community-dwelling adults aged ≥66 in the Health and Retirement Study from 2008 to 2018 linked to Medicare and classified as having dementia using a validated algorithm. Medication usage was ascertained over the 1-year prior to an HRS interview date. Potentially problematic medications were identified by: (1

Observational UK cohort study to describe intermittent oral corticosteroid prescribing patterns and their association with adverse outcomes in asthma.

INTRODUCTION: Oral corticosteroids (OCS) for asthma are associated with increased risks of developing adverse outcomes (adverse outcomes); no previous study has focused exclusively on intermittent OCS use. METHODS: This historical (2008-2019) UK cohort study using primary care medical records from two anonymised, real-life databases (OPCRD and CPRD) included patients aged≥4 years with asthma receiving only intermittent OCS. Patients were indexed on their first recorded intermittent OCS prescription for asthma and categorised by OCS prescribing patterns: one-off (single), less frequent (≥90 da

Prescribing Patterns of End-of-Life Medication for Compassionate Discharge: A Retrospective Observational Cohort Study.

ObjectiveIn Singapore, where home is often the preferred place of death, compassionate discharge enables acutely deteriorating patients in hospital to die in their location of choice. Anticipatory prescribing ensures medication access at home for timely relief of distressing symptoms. Without standardized guidelines, however, prescribing patterns remain unclear. We investigate medication use in this unique setting by evaluating types and dosages of drugs prescribed for adult patients on terminal care at home.MethodsA retrospective observational cohort study was conducted on home hospice patien

Potentially Harmful Medication Use Among Medicare Patients with Heart Failure.

BACKGROUND: The unintentional prescribing of medications harmful for patients with heart failure (HF) remains an ongoing problem. The American Heart Association published a scientific statement detailing a list of medications that may worsen or exacerbate a patient's HF. The use of potentially harmful medications has not been studied in Medicare patients with HF. OBJECTIVE: The aim of this study was to assess the prevalence of prescribing these harmful medications in a Medicare-enrolled medication therapy management (MTM)-eligible population with HF and to identify the characteristics associa

Decade-Long Trends (2001 to 2011) in the Use of Evidence-Based Medical Therapies at the Time of Hospital Discharge for Patients Surviving Acute Myocardial Infarction.

Optimization of medical therapy during discharge planning is vital for improving patient outcomes after hospitalization for acute myocardial infarction (AMI). However, limited information is available about recent trends in the prescribing of evidence-based medical therapies in these patients, especially from a population-based perspective. We describe decade-long trends in the discharge prescribing of aspirin, angiotensin-converting enzyme inhibitors/angiotensin receptor blockers, β blockers, and statins in hospital survivors of AMI. The study population consisted of 5,253 patients who were d
