[Noninvasive ventilation in patients with persistent hypercapnia].

BACKGROUND: Chronic respiratory failure is caused by insufficiency of the inspiratory muscles, i.e. mainly the diaphragm, which represents the so-called "respiratory pump". Insufficiency of the respiratory pump causes hypercapnia. THERAPEUTIC INTERVENTION: Diseases with chronic hypercapnia are characterized by reduced survival. Mechanical ventilation-mostly applied as noninvasive mechanical ventilation (NIV)-improves ventilation and unloads the inspiratory muscles. INDICATION: Strong evidence supports the use of domiciliary NIV already in mild degrees of chronic respiratory failure caused by

A European survey of noninvasive ventilation practices.

Although noninvasive ventilation (NIV) is becoming very popular, little is known about its pattern of clinical and technical utilisation in different environments. We conducted a web-based survey in Europe to identify the perceived pattern of NIV utilisation and the reason for choosing a specific ventilator and interface type in four common clinical scenarios: acute hypercapnic respiratory failure (AHRF), cardiogenic pulmonary oedema (CPE), de novo hypoxic respiratory failure and weaning/post-extubation failure (W/PE). A response was obtained from 272 (51.3%) out of 530 selected European physi

Bronchoscopy during noninvasive ventilation: indications and technique.

Diagnostic or therapeutic flexible bronchoscopy is often necessary in severely ill patients. These patients often have comorbidities that increase the risk of bronchoscopy-related complications. Noninvasive ventilation might decrease the risk of these complications in patients with severe refractory hypoxemia, postoperative respiratory distress, or severe emphysema, and in pediatric patients. Noninvasive ventilation may prevent hypoventilation in patients with obstructive sleep apnea and obesity hypoventilation syndrome who require bronchoscopy, and may assist in the bronchoscopic evaluation o

Continuous positive airway pressure and noninvasive ventilation.

Continuous positive airway pressure (CPAP) and noninvasive ventilation (NIV) hold much promise as means to protect the lungs of newborn infants who have respiratory distress from many causes. A wide variety of options are available to the clinician, including CPAP, bilevel CPAP, and both synchronized and unsynchronized noninvasive mechanical breaths. Limited data are available regarding the best ways to use CPAP and NIV in today's NICU environment. This article reviews current information on these modalities, including available options, possible risks, and unanswered questions.

Noninvasive ventilation options in pediatric myasthenia gravis.

A 10-month-old female infant with congenital myasthenic syndrome suffering from acute respiratory failure was supported using face mask positive pressure ventilation until definitive diagnosis and specific treatment was achieved. A 12-year-old girl suffering from seronegative myasthenia gravis was treated by helmet-delivered noninvasive ventilation during recurrent myasthenic episodes. Noninvasive support was really beneficial in the myasthenic crisis with respiratory muscle weakness, whereas a shift to tracheal intubation was necessary when pulmonary infection and multiple atelectasis occurre

[Noninvasive mechanical ventilation in acute respiratory failure].

Mechanical ventilation is the essential treatment for acute respiratory failure. The methods used differ greatly depending on whether one chooses invasive or noninvasive. Noninvasive ventilation avoids orotracheal intubation, facilitates weaning and, if negative pressure ventilation is integrated with positive pressure ventilation, the majority of patients will not need intubated. The ideal setting for mechanical ventilation, where possible, is the Respiratory Intensive Care Unit.

Noninvasive ventilation in chronic obstructive pulmonary disease.

Over the past 15 years, numerous studies have explored the possibility that intermittent ventilatory assistance using noninvasive ventilation is beneficial in patients with severe stable COPD. The results are conflicting and no sustained beneficial action has been proven, although some improvement in respiratory muscle function may occur after short-term rest, and lengthening of the total duration of sleep has been shown in a few studies. Based on the observation that studies with favorable findings have generally had considerably higher PaCO2s than studies with negative findings, the current

[Noninvasive ventilation in chronic obstructive lung disease].

Noninvasive mechanical ventilation is provided without establishing an endotracheal airway. In acute respiratory failure due to chronic obstructive lung disease, noninvasive positive-pressure ventilation delivered through a nasal or face mask may eliminate the need for intubation and related complications, reduce the duration of hospitalisation and improve survival compared to assisted ventilation through an endotracheal tube. In chronic respiratory failure related to neuromuscular diseases and thoracic deformities, the advent of noninvasive intermittent longterm ventilation is well establishe

[Noninvasive ventilation in chronic obstructive lung disease].

Non-invasive ventilation refers to ventilatory support without establishing an endotracheal airway. The advent positive-pressure ventilation delivered through a nasal or face mask has greatly expanded the use of non-invasive ventilation. In patients with acute respiratory failure due to chronic obstructive lung disease, non-invasive positive pressure ventilation reduces the need for intubation and related complications, shortens the duration of hospital stay and improves survival. The role of intermittent non-invasive long-term ventilation in the management of chronic respiratory failure relat

[Noninvasive postoperative ventilation using a buccal mask].

In this preliminary study we assessed the feasibility and efficiency of non-invasive ventilation (NIV) with a buccal mask (NIV-BM), not fitted with a mouthpiece, in surgical patients in acute respiratory failure whose lungs could not be ventilated with a face mask. In the eight patients enrolled in the study, NIV-BM increased tidal volume, decreased respiratory rate and improved PaO2. In two patients the trachea had to be intubated and one of them died of septic shock. NIV-BM can act as a valuable alternative in surgical patients in acute respiratory failure in whom a face mask cannot be used

[Contra: noninvasive ventilation in acute respiratory insufficiency].

Mechanical ventilation via a tracheal tube is an invasive measure whose complications may prevent recovery from respiratory failure. Today, noninvasive positive pressure ventilation via mouthpiece or mask is an economically and medically successful alternative for the treatment of chronic respiratory failure and acute exacerbation of COPD, respectively. Within certain limits, noninvasive ventilation may take over inspiratory work of breathing as well as elevate mean airway pressure and inspiratory oxygen concentration. This does not at all question the absolute indications to maintain a patent

Noninvasive high frequency oscillatory ventilation versus noninvasive positive pressure ventilation in preterm neonates after extubation: A randomized controlled trial.

BACKGROUND: Weaning from mechanical ventilation is a challenging phase of neonatal respiratory support [1]. Choosing efficient and safe noninvasive modality to prevent re-intubation and choosing the optimal time for weaning are key points for weaning success. The aim of the study is to compare the efficiency and safety of noninvasive high frequency oscillatory ventilation (NHFOV) versus noninvasive positive pressure ventilation (NIPPV) as respiratory support after extubation in preterms with respiratory distress syndrome (RDS). Also, the study compared the lung ultrasound findings between thes

Filters Alter the Performance of Noninvasive Ventilators.

BACKGROUND: Noninvasive ventilation is recommended in hypercapnic respiratory failure secondary to ventilatory failure. Noninvasive ventilation may contribute to aerosol dispersion, which may increase the risk of transmission of COVID 2019. The addition of filters to the ventilator circuit has been recommended to reduce this risk. The aim of this benchtop study was to investigate the impact of adding filters to a ventilator circuit. METHODS: In this benchtop study, a breathing simulator was used with 4 commonly used ventilators. Ventilators were set to approximate the typical settings that ar

Noninvasive Ventilation in the Age of Surfactant Administration.

Numerous advances in neonatal care have improved outcomes in preterm infants. Antenatal steroids, through their ability to promote lung maturation and function, have led to significant improvements in death, intraventricular hemorrhage, necrotizing enterocolitis, and respiratory distress syndrome. For years, exogenous surfactant administration has been used in conjunction with antenatal steroids to further improve outcomes for preterm infants. However, as continuous positive airway pressure has been shown to be effective in treating respiratory distress syndrome, it has become less clear how e

Amyotrophic Lateral Sclerosis and Noninvasive Positive Pressure Ventilatory Support: "Nasal Noninvasive Ventilation" or "Noninvasive Ventilatory Support"?

Many studies suggest a brief statistical benefit on survival and quality of life by using nasal noninvasive ventilation for patients with amyotrophic lateral sclerosis and other neuromuscular conditions. Indeed, nasal noninvasive ventilation has become synonymous with continuous positive airway pressure and lo-span bilevel positive airway pressure. Nasal noninvasive ventilation, however, may not normalize CO2 levels and continuous positive airway pressure and O2 exacerbate hypercapnia and often lead to CO2 narcosis, intubation, and ultimately tracheostomy or palliative care death. However, a t

[Influence of nurses on the experience of noninvasive ventilation].

Noninvasive ventilation (NIV) is established as an integral part of intensive care therapy for respiratory insufficiency. However, it is associated with restrictions on patient autonomy and comfort. A successful course of therapy is largely dependent on the acceptance and adherence of the patients concerned. Intensive care nurses can have a significant influence on the success of the therapy by dealing with the subjective experience of the patients and by maintaining close contact with them. The prerequisite for this is profound knowledge of the different technical aspects of therapy and equip

Acute Parotitis as a Complication of Noninvasive Ventilation.

Several conditions, including oropharyngeal dryness, pressure sores, ocular irritation, epistaxis, or gastric distension, have been described during noninvasive ventilation (NIV). Although this technique has been widely used in intensive care units and emergency wards, acute swelling of the parotid gland remains a scarcely reported complication. We describe herein the case of an 82-year-old man who developed unilateral parotitis during prolonged NIV for acute heart failure. Intravenous antibiotics, corticosteroids, and adjusting the mask laces' position allowed rapid resolution of clinical sym

[Home mechanical ventilation: Invasive and noninvasive ventilation therapy for chronic respiratory failure].

Home mechanical ventilation represents a valuable therapeutic option to improve alveolar ventilation in patients with chronic respiratory failure. For this purpose both invasive ventilation via tracheostomy and noninvasive ventilation via facemasks are available. The primary goal of home mechanical ventilation is a reduction of symptoms, improvement of quality of life and in many cases reduction of mortality. Elective establishment of home mechanical ventilation is typically provided for noninvasive ventilation in respect to clinical symptoms and partial pressure of carbon dioxide depending on

The growing role of noninvasive ventilation in patients requiring prolonged mechanical ventilation.

For many patients with chronic respiratory failure requiring ventilator support, noninvasive ventilation (NIV) is preferable to invasive support by tracheostomy. Currently available evidence does not support the use of nocturnal NIV in unselected patients with stable COPD. Several European studies have reported benefit for high intensity NIV, in which setting of inspiratory pressure and respiratory rate are selected to achieve normocapnia. There have also been studies reporting benefit for the use of NIV as an adjunct to exercise training. NIV may be useful as an adjunct to airway clearance te

Prediction of noninvasive ventilation failure: a narrative review.

With the increasing clinical application of noninvasive ventilation (NIV) and the accumulation of evidence-based medical data, NIV has become an important therapeutic intervention for various types of respiratory failure. However, a subset of patients still experience NIV failure. Compared with those with successful NIV, patients who develop NIV failure tend to have a poorer prognosis and an elevated risk of mortality. Therefore, the accurate identification and early prediction of NIV failure risk are crucial for guiding clinical decision-making and optimizing treatment strategies. This articl
