Nasal Reconstruction: Defects that Cross Anatomical Subunits.

The nose plays a crucial role in the human experience, both aesthetically and functionally. The biomechanics of covering flaps, the importance of a firm framework, and the anatomical basis for aesthetic subunits are imperative concepts as one proceeds through the algorithm of reparative options. The subunits consist of three paired units-the ala, soft tissue facets (also known as soft tissue triangles), and sidewall, as well as three unpaired units-the dorsum, tip, and columella. Because many defects cross aesthetic subunits, it is critical to understand when to adapt the subunit principle and

Trends in Head and Neck Cancer: Oral Cavity Carcinoma and What the Radiologist Needs to Know.

Decreased tobacco use has resulted in substantial declines in the prevalence of upper aerodigestive tract malignancies. However, the prevalence of oral cavity squamous cell carcinomas has been steadily increasing despite decreases in tobacco-related malignancies both within the United States and worldwide. The cause driving the increasing prevalence is unknown and may reflect a combination of viral, environmental, and genetic mechanisms. Radiologists must be familiar with the imaging appearance of oral cavity carcinomas to achieve proper staging and to guide surgical and/or radiation therapy m

Selective use of radiation therapy for neoplasms of the skin.

Radiation therapy is preferable treatment for a minority of basal cell and epidermoid carcinomas of the skin. Proper use exploits the inherent advantage of preservation of function and cosmesis. Therefore, many cancers involving the eyelid, canthus, nose, nasolabial fold, pinna, ear canal, vermilion surface of the lower lip and skin of the chin can be advantageously treated by radiation therapy as compared to surgery, if pretreatment destruction of normal tissue is minimal. Although irradiation is equally effective, surgery is more expeditious for small lesions and cancers at other sites, whic

Possibilities of intranasal reconstruction in complex nasal defects.

BACKGROUND: Complex nasal defects most often arise due to oncological resection or severe trauma. Traditional methods of two-stage nose reconstruction using a forehead flap with a skin graft have often resulted in collapse and deformity of the nose with a very compromised outcome over time. These techniques were gradually replaced by new procedures consistently reconstructing the intranasal lining, most often with flaps from the nasal septum. These methods reconstruct the cartilaginous and bony support of the nose as well, while the skin cover of the nose is, nowadays, in large defects, recons

Implications of Malignancy, Radiation, and Timing of Major Nasal Reconstruction.

Owing to the complex, multilayered anatomy of the nose in the central face, major nasal reconstruction can pose a significant challenge for reconstructive surgeons. It is the responsibility of reconstructive surgeons to have an understanding of the most common cutaneous malignancies and excisional techniques that may lead to complex nasal defects. The purpose of this article is to discuss these malignancies, excisional techniques, and impacts of radiation on tissue that has implications for reconstructive surgeons.

[Nasal schwannoma with skull base destruction in a child (a case report and literature review)].

Schwannomas (neurinomas) are benign, encapsulated, and slow-growing neoplasms developing from any peripheral, cranial, or vegetative nerve that has the Schwann sheath. Schwannomas of the nasal cavity and paranasal sinuses in adults occur rarely, less than 4% of cases. The world literature lacks similar data for the pediatric age group because these tumors in children are unique and extremely rare. The most common clinical symptoms of sinonasal schwannoma are nasal breathing disorders, hyposmia or anosmia, and snoring at night. Other symptoms are associated with intranasal schwannoma invasion o

[The clinical features and prognosis of nasal Schwannoma].

Objective:To investigate the experience of nasal Schwannoma in order to provide guidance for the diagnosis and treatment of the disease. Method:Clinicopathological data and follow-up results of nine patients, which histopathology proved nasal Schwannoma were collected and analyzed. Result:The patients were referred to our clinic due to the space-occupying symptoms or signs of tumour compression. There were no specific findings in imaging examination. The patients were treated by operation except a multiple Schwannoma patient. The recurrence after operation were rare. The main features of pathol

Renal cell carcinoma presenting as a cutaneous horn and nodules on the gingiva and scalp.

A 63-year-old man presented with a pulsatile cutaneous horn on the nose and multiple angiomatous nodules on the gingiva and scalp, which appeared over 2 months. He had severe hypercalcaemia, lytic lesions in multiple bones and acute kidney injury. Excision biopsy from the gingival nodule showed a clear cell neoplasm. The bone marrow showed atypical cells with similar morphology. Imaging showed a 7 cmx7.5 cm mass at the upper pole of the left kidney with metastases to the bones, liver and lung. Immunohistochemistry was consistent with metastatic renal cell carcinoma. Renal cell carcinoma presen

Voluminous Myoepithelioma of the Minor Salivary Glands Involving the Base of the Tongue.

Myoepithelioma is an extremely rare tumour subtype and diagnosis is based on a wide variation of cellular morphology. FNAC specimens do not always suffice for a definitive differential diagnosis which depends on histology and immunohistochemistry of the lesion. Case Presentation. A 54-year-old female came to our attention with dysphagia and dyslalia of 6-month standing. Ear, Nose, and Throat (ENT) examination revealed a voluminous mass on the right portion of the base of her tongue, where postcontrast T2-weighted Magnetic Resonance Imaging (MRI) evidenced a hyperintense lesion. The fine-needle

Malignant melanoma of the nose.

Malignant Melanoma of the nose is a rare neoplasm, with primary mucosal melanoma being more aggressive than its cutaneous counterpart (Land,1982). The presentation is often not as dramatic as compared to its progression. While cutaneous melanomas typically present with obvious and consistent features, melanomas of the mucosal form are often diagonised late due to their hidden location and relatively non-specific features. This tumour can metasiasise by lymphasies or bloodsream. Moreover the tumour is often quite resistent to complete cure especially due to reccurence and metastotis. We wish to

[Giant laterocervical lipoma in a 6-month-old infant: a case report].

Lipomas are mesenchymal neoplasms that affect the head and neck region in about 13% of cases. However, they are rarely reported in the pediatric population. We here report the case of a 6-month-old infant with no notable medical history, admitted for the management of a large left lateral cervical mass that had been progressing for 4 months. Ear, nose and throat (ENT) examination revealed a large left lateral cervical mass extending to the submental area, approximately 7cm in length, with no signs of compression. Cervical CT scan showed a fat-density mass, consistent with a lipoma. The patient

Medical comorbidity of sleep disorders in children and adolescents.

PURPOSE OF REVIEW: Sleep disorders in children and adolescents often present with various medical comorbidities. The aim of this review is to present the recent literature findings on this issue. RECENT FINDINGS: The major medical comorbid conditions that accompany insomnia or short sleep duration in children and adolescents are obesity and metabolic syndrome, growth hormone deficiency, allergic conditions, various disorders accompanied by chronic pain, neoplasms and blood malignancies, and genetic and congenital disorders. Hypersomnia is mainly related to malignancies. Children and adolescen

Role of skull base surgery for local control of sarcoma of the nasal cavity and paranasal sinuses.

Sarcomas of the nose and paranasal sinuses are rare neoplasms and comprise less than 1% of the malignancies arising in the sinonasal tract. From 1977 to 1989, we had the opportunity to treat 15 patients presenting with these tumors at The Eye & Ear Institute of the University of Pittsburgh. The clinical charts of these patients were reviewed retrospectively for demographic data, characteristics of disease, treatment considerations and clinical outcome. Data were also analyzed with special attention to the role of skull base surgery for the local control of tumors. Seven patients underwent cran
