Age-related increases of macroautophagy and chaperone-mediated autophagy in rat nucleus pulposus.

OBJECTIVE: Excessive apoptosis plays an important role in the progression of intervertebral disc degeneration. However, the effect of autophagy, another type of programmed cell death, on the pathogenesis of disc degeneration is still unclear. Macroautophagy and chaperone-mediated autophagy (CMA) change and intervertebral disc degeneration aggravates with age. This study aims at examining the expression changes of light chain 3 (LC3), lysosome-associated membrane protein 2A (LAMP-2A), and Hsc70, the indicator substrates of macroautophagy and CMA, in rat nucleus pulposus (NP) to prove that macro

Herniated nucleus pulposus in isthmic spondylolisthesis: higher incidence of foraminal and extraforaminal types.

BACKGROUND: The purpose of this study was to evaluate the pattern of disc herniation and to investigate the associated symptoms in cases of isthmic spondylolisthesis. It is well known that the pathogenesis of degenerative spondylolisthesis associates with disc degeneration, followed by facet laxity and ligamentum flavum hypertrophy, which result in severe spinal canal stenosis. But isthmic spondylolisthesis is known to have a different pathogenesis. In isthmic spondylolisthesis, pseudodisc bulging is easily identified, and canal stenosis is comparatively rare. Therefore, we propose that isthmi

Early effects of nucleus pulposus application on spinal nerve root morphology and function.

It is known that 24 h or more after epidural application of autologous nucleus pulposus, functional and structural changes are established in adjacent nerve roots. It is, however, not known how soon after the application these changes appear. The aim of this study was to reduce the exposure duration to 3 h and to evaluate nerve function and histological changes in the nerve tissue during this time period. A total of 12 pigs was used. In ten pigs, autologous nucleus pulposus (NP) was applied epidurally on the cauda equina. Nerve function was then monitored for 3 h by measurements of muscle acti

Dynamic adaptive coassembled sericin protein orchestrating stem cell development for nucleus pulposus regeneration.

The zonal structure of natural extracellular matrix (ECM) creates a dynamic microenvironment that regulates mechanical and biochemical signals to support stem cell development. Here, we engineered a dynamic adaptive protein gel by coassembling bioactive sericin with an amphiphilic tripeptide, forming a nucleus pulposus (NP)-like viscoelastic network with rapid stress relaxation. This coassembled protein gel sequentially activated integrin β3-mediated mechanotransduction, promoted cytoskeletal remodeling and Yes-associated protein nuclear translocation, and subsequently up-regulated bioactive f

Poly(vinyl alcohol)-hyaluronic acid injectable hydrogel as a nucleus pulposus substitute candidate.

Based on the Global Burden of Disease (GBD) study, low back pain is the top 10 highest prevalence diseases in the world. As many as 30%-80% of cases are induced by herniated nucleus pulposus (HNP). HNP is a protrusion of the nucleus pulposus (NP) caused by the rupture of the annulus fibrosus. Injectable hydrogel as an NP substitute is an alternative HNP treatment through hydrating and restoring disc thickness with minimally invasive surgery. A poly(vinyl alcohol)-hyaluronic acid hydrogel was developed as an injectable material for the treatment of HNP. Poly(vinyl alcohol) or PVA has good mecha

Injectable Thermoresponsive Microgels Encapsulate and Precondition Nucleus Pulposus Cells for Intervertebral Disc Regeneration.

Chronic lower back pain (LBP), a leading cause of disability affecting over half a billion people globally, is often linked to intervertebral disc (IVD) degeneration. Current treatments, like surgery and pain management, fail to address the IVD degeneration that is associated with the deterioration of the nucleus pulposus (NP). We aim to develop an injectable cell therapy for LBP using human nucleus pulposus cells (NPCs) encapsulated in hydrogel microparticles (microgels) as a cell delivery vehicle. The thermoresponsive hydrogels enabled cell encapsulation and preconditioning without the use o

Taurine can restrict MtdsRNA mediated pyroptosis by enhancing mitophagy in nucleus pulposus cells.

Mitochondrial double-stranded RNA (mtdsRNA), a long-stranded RNA molecule generated by bidirectional transcription of the circular mitochondrial genome, can trigger the type 1 interferon response by activating pattern recognition receptors in the cytoplasm during mitochondrial dysfunction. However, its regulatory and pathogenic mechanisms in intervertebral disc degeneration are poorly understood. Here, we showed that mtdsRNA was abnormally elevated in degenerative nucleus pulposus tissues. Furthermore, we found that mtdsRNA leakage into the cytoplasm could cause MAVS oligomerization by binding

Safety Evaluation of Intradiscal Delivery of Nucleus Pulposus Allograft for Lumbar Discogenic Pain.

BackgroundPersistent concerns remain about the deleterious pathological effects of minimally invasive transannular puncture, such as occurs during discography and therapeutic intradiscal procedures. The objective of this study was to estimate the safety profile associated with fluoroscopically guided intradiscal delivery of nucleus pulposus (NP) allograft under clinical trial and real-world conditions.MethodsThis was a retrospective pooled analysis of adverse events (AEs) and clinical complaints captured from 4 different treatment populations (n = 392) and a database of commercial cases (n = 1

Single-cell sequencing reveals cellular heterogeneity of nucleus pulposus in intervertebral disc degeneration.

The nucleus pulposus (NP) plays a vital role in intervertebral disc degeneration (IVDD). Previous studies have revealed cellular heterogeneity in NP tissue during IVDD progression. However, the cellular and molecular alterations of diverse cell clusters during IVDD remain to be fully elucidated. NP tissues were isolated from patients with different grades of IVDD undergoing discectomy, and then subjected to single-cell RNA sequencing (scRNA-seq). Cell subsets were identified based on unbiased clustering of gene expression profiles. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes

Comparative metagenomic analysis of human intervertebral disc nucleus pulposus and cartilaginous end plates.

STUDY DESIGN: The diversity of microflora inhabiting endplate (EP) and nucleus pulposus (NP) tissues of human intervertebral disc (IVD) was profiled through NGS-supported 16S rRNA amplicon sequencing. Sixteen EP and their corresponding NP were excised from the brain-dead voluntary organ donors with no clinical history of low back pain, and 12 herniated and 8 degenerated NP tissues isolated from the patients undergoing spinal surgery were subjected to study the alteration in the microbial diversity. OBJECTIVES: To understand in normal IVD, whether the colonization of bacteria to the NP is thro

Fas ligand expression on human nucleus pulposus cells decreases with disc degeneration processes.

BACKGROUND: The intervertebral disc has been reported to be an immunologically privileged environment, possibly mediated by Fas ligand (FasL) expression. On the other hand, recent studies have shown the infiltration of host immune cells into the degenerated disc, which may indicate the failure of the immune-privilege feature of the disc with degeneration. However, the relationship between FasL expression and disc degeneration is still unclear. Therefore, the purpose of this study was to clarify the relationship between FasL expression and disc degeneration. METHODS: Ten human degenerated disc

Acute necrotizing myelopathy from nucleus pulposus embolism in dogs with intervertebral disk degeneration.

Two dogs that suddenly developed neurologic signs indicating a severe cervical spinal cord lesion were found at necropsy to have necrotizing myelopathy secondary to nucleus pulposus embolism. One dog had embolism of arteries and veins; in the other dog, veins alone were embolized. Each dog had early degeneration and extrusion of the C6--7 disk, from which emboli originated. In one dog, arterial emboli apparently entered inflammatory vasculature in the anulus fibrosus and were forced by extrusion back through the arterial system to the radicular artery and then to the cord. Venous embolism occu

Mechanism of the NAMPT-SIRT2-Lactate Axis in Senescence of Human Nucleus Pulposus Cells.

BACKGROUND: Intervertebral disc degeneration (IDD) is a primary cause of low back pain, closely associated with the senescence of nucleus pulposus cells (NPCs). Energy metabolism disorders and lactate accumulation are observed in degenerated intervertebral discs. However, the molecular mechanism through which lactate accumulation induces IDD and its upstream regulatory factors remain unclear. METHODS: Clinical degenerated intervertebral disc tissues were collected. Lactate content was measured using the water-soluble tetrazolium salt-8 (WST-8) method, and the expression of nicotinamide phosph

The Inhibitory Effect of RADKPS on Pyroptosis of Nucleus Pulposus-Derived Mesenchymal Stem Cells.

Intervertebral disc (IVD) degeneration (IDD) is a primary cause of low-back pain in people, which is associated with nucleus pulposus-derived mesenchymal stem cells (NPMSCs). In this study, the involvement of lipopolysaccharide (LPS) in the pyroptosis of NPMSCs was investigated. The effect of RADKPS on the pyroptosis of NPMSCs and the underlying mechanism behind the impact of RADKPS on the proliferative capacity of NPMSCs were also studied. Pyroptosis of NPMSCs was induced with 10 μg/mL LPS and its effects on the downstream signaling pathways were explored. The protective effect of RADKPS on N

Achilles' Heel-The Significance of Maintaining Microenvironmental Homeostasis in the Nucleus Pulposus for Intervertebral Discs.

The dysregulation of intracellular and extracellular environments as well as the aberrant expression of ion channels on the cell membrane are intricately linked to a diverse array of degenerative disorders, including intervertebral disc degeneration. This condition is a significant contributor to low back pain, which poses a substantial burden on both personal quality of life and societal economics. Changes in the number and function of ion channels can disrupt the water and ion balance both inside and outside cells, thereby impacting the physiological functions of tissues and organs. Therefor

Time- and dose-dependent cytotoxicities of ioxitalamate and indigocarmine in human nucleus pulposus cells.

BACKGROUND CONTEXT: Ioxitalamate (Telebrix 300) is an ionic iodinated contrast medium commonly used for discography or percutaneous endoscopic lumbar discectomy (PELD), though it has side effects such as anaphylactic shock and renal toxicity. Indigocarmine is an organic compound dye with a distinctive blue color that is commonly used during PELD to stain the acidic, degenerated nucleus pulposus (NP). Although ioxitalamate and indigocarmine are widely used in spinal surgery, there have been no reports on their effects on NP cells. We studied the toxicities of both ioxitalamate and indigocarmine

Rheological characterization of the nucleus pulposus and dense collagen scaffolds intended for functional replacement.

Lumbar discectomy is an effective therapy for neurological decompression in patients suffering from sciatica due to a herniated nucleus pulposus (NP). However, high numbers of patients suffering from persisting postoperative low back pain have resulted in many strategies targeting the regeneration of the NP. For successful regeneration, the stiffness of scaffolds is increasingly recognized as a potent mechanical cue for the differentiation and biosynthetic response of (stem) cells. The aim of the current study is to characterize the viscoelastic properties of the NP and to develop dense collag

TonEBP/OREBP is a regulator of nucleus pulposus cell function and survival in the intervertebral disc.

The nucleus pulposus is an aggrecan-rich hydrated tissue that permits the intervertebral disc to resist compressive loads. Adaptation to loading is achieved through an elevation in disc osmolarity mediated by the numerous charged glycosoaminoglycan side chains of the aggrecan molecule. The goal of this investigation was to determine the functional role of the osmo-regulatory protein, TonEBP, in cells of the nucleus pulposus. We found that TonEBP and its downstream target genes were robustly expressed in the tissues of the disc. Above 330 mosmol/kg, cultured nucleus pulposus cells up-regulated
