Advanced practice nursing in child maltreatment: practice characteristics.

INTRODUCTION: Child maltreatment is a problem of epidemic proportions in the United States. Pediatric nurse practitioners and other advanced practice nurses (APNs) have been caring for maltreated children for decades, yet to date no comprehensive assessment of their practice characteristics or their clinical and academic contributions to the field has been performed. The purpose of this study is to describe the practice characteristics of APNs who care for maltreated children. METHOD: A descriptive design was used for this study. Child advocacy centers and children's hospitals were contacted

Nursing update on retinopathy of prematurity.

One of the problems that plagues premature infants is retinopathy of prematurity, a potentially blinding disease that occurs because the retina is immature before 34 weeks gestation and must develop in a suboptimal environment when a baby is born early. Prevention by minimizing oxygen exposure has been somewhat effective, but survival of the tiniest babies has led to a recent resurgence in cases. Oxygen targeting and early surgery show promise to reduce the risk of blindness in the smallest premature infants. Nurses play an important role in oxygen management and parental support.

Narratives by district nurses about elder abuse within families.

Twenty-one district nurses (DNs) narrated 44 cases of elder abuse within families. A phenomenological-hermeneutical analysis revealed that the experiences were complex and often included families providing care for an elderly person. The abuse seemed to be related to the inability of one party to meet the care demands required by the elderly, by him- or herself, or by the situation. It also seemed to be related to an inhibiting dependency between the parties, a negative execution of power over the weaker party, and a history of violence. When the experiences were considered within the perspect

Nursing's contribution to the literature on preterm labor and birth.

Although preterm labor and birth continues to plague us as an unsolved mystery in maternal/child health, much has actually been learned over the past few decades. Most reviews of the preterm labor literature examine the research in an interdisciplinary manner; this article instead looks at the active role that nurses have taken in the research concerning preterm labor and birth. This article will categorize research conducted by nurses over the past 30 years on preterm labor and birth, emphasizing some of the changes in nursing practice engendered by nursing's important research effort.

A new understanding of the second stage of labor: implications for nursing care.

A reconceptualization of the second stage of labor is proposed, with an early phase of descent and a later phase of active pushing, as the basis for nursing care related to direction or support of expectant mother's bearing-down efforts. This reconceptualization challenges the rules that have accompanied second stage by providing criteria for the obstetric conditions optimal for fetal descent that develop during the initial phase of second stage as the fetal head rotates to an anterior position and descends to at least a 1+ station. The phase of active pushing is accompanied by a decline in fe

Health-related quality of life and chronic venous leg ulceration: Part 2.

Leg ulceration is a debilitating condition which compromises the quality of life of the sufferer, owing to factors such as pain, exudate, odour and social isolation. As nurses, much of the daily care provided for such patients focuses on the provision of wound care; often failing to fully address the wide ranging effects that the ulceration is having on the life of the sufferer. This article reviews the quantitative studies that have explored the health-related quality of life of patients with chronic venous leg ulceration and presents a synthesis of their findings.

The use of hope as a coping mechanism in abused women.

The purpose of this study was to identify how abused women use hope and to determine whether hope is a coping mechanism in their relationship with the abusers. Four focus group sessions were conducted with a sample of 24 women using a phenomenological methodology. The data analysis indicated that abused women maintain hope in four major dimensions: hope for change in their partner's behavior, hope for survival, hope as something to cling to, and hope for control of the situation. The subjects stated that hope was used as a coping mechanism to help them keep their sanity; but in this context, t

Does continuity of carer matter to women from minority ethnic groups?

OBJECTIVE: To explore the maternity care views and experiences of minority ethnic women who did not respond to a postal survey of mothers' responses to care and to assess whether the concept of continuity mattered to them. DESIGN: A semi-structured narrative interview, at about six months following birth, designed to encourage women to describe their experiences of pregnancy, birth and maternity care in their own words and according to their own perspectives. SETTING: Maternity care in a London NHS Trust with two teaching hospital units, where women in a specific neighbourhood received casel

An evidence-based approach to urinary catheter changes.

Many people in the community have a urinary catheter that is routinely changed by a community nurse. People with urinary catheters can experience a number of problems including catheter blockages. A blocked catheter causes great distress and inconvenience to the person and places a strain on the resources of community nursing organizations because of the need for unscheduled visits to change the catheter. A procedure that enabled catheter changes to be planned before they blocked would reduce patient distress and better assist community nursing organizations with resource management. The plann

Support needs of children's nurses involved in child protection cases.

AIM: This qualitative study explored the experiences of nurses working in a hospital paediatric department, who had direct involvement in child protection cases, to discover their support needs and suggest developments in training and support. BACKGROUND: The study was inspired by an awareness of increasing anxiety amongst nurses involved in child protection cases. METHOD: A phenomenological approach was used with fifteen nurses during semi-structured interviews, which were taped, transcribed and analysed thematically. RESULTS: Involvement in child protection has a lasting impact; nurses ne

An investigation into competence in TCM of BSN graduates from technological universities in Taiwan.

The purpose of this study was to determine what competences in Traditional Chinese Medicine (TCM) should be possessed by Bachelor of Science in Nursing (BSN) graduates from technological universities in Taiwan. There were two phases in this study: focus groups and expert panel discussion. First, we held four focus groups, and interviewed 31 participants, including consumers, nursing staff and clinical nursing faculty. The interview tapes were transcribed verbatim, were proofed for accuracy and the content was analyzed. The results were then discussed by four academic experts in a panel discuss

The adolescent research participant: strategies for productive and ethical interviewing.

Nurse researchers who seek to study the experiences of adolescents have limited resources to assist them with the process. Although some elements of interviewing are standard practice, special approaches are needed for the adolescent participating in research. Interviews are especially challenging. The purpose of this article is to present strategies to assist researchers as they engage this cohort in research. These strategies include evaluating the adolescent's developmental level, designing developmentally appropriate questions, and refining interviewing techniques to optimize the experienc

[Prevention of burnout and incidences by coping strategies].

The object of our research is centred around burnout in nurses confronted by the successive and repeated deaths of patients in their care. We propose to study the theoretical links between burnout, coping and alexithymie. Our initial hypothesis is that palliative care formation tends to diminue the characteristics of burn-out, the negative and defensive strategies of coping. On the other hand, the process of palliative and adaptative care education tends to develope more efficient, positive and adaptative strategies of coping. The comparative results of two test groups of nurses, formed and no

Red alert: perinatal hemorrhage.

The purpose of this article is to help nurses understand how to quickly and effectively manage the nursing care of patients with perinatal hemorrhage. The etiology, symptoms, medical management, and nursing care of the patient experiencing a perinatal hemorrhage are discussed. Hemorrhage during the antepartum, intrapartum, or postpartum period is a life-threatening emergency for the mother and/or fetus. Early antepartum hemorrhage (before 20 weeks gestation) can be related to abortion/miscarriage, ectopic pregnancy, or gestational trophoblastic disease; late antepartum hemorrhage (after 20 wee

Moving beyond biomedical understanding of lipodystrophy in people living with HIV/AIDS.

The purpose of this article is to move beyond the biomedical standpoint in the field of HIV/AIDS in order to contribute to the recognition oflipodystrophy as a phenomenon that reaches far beyond its current definition as an unfortunate side effect of highly active antiretroviral therapy (HAART). This article hopes to demonstrate how theory, while remote to the clinical setting, can allow nurses to understand the experience of lipodystrophy as a social phenomenon and address it likewise in their practice. The specific aim of this article is to apply the concept of stigma to the experience of li
