Improving accuracy of wound measurement in clinical practice.

Wound measurement is an important component of wound assessment that has the potential to provide baseline measurements to predict treatment outcomes. This review critically evaluates the efficacy of current wound measurement techniques. Findings indicate that true area measurement and percentage of area reduction of wounds are useful parameters for differentiating between wounds that are/are not responding to treatment. Two stages of error have been found to result during tracing and calculation of wound surface area. Studies demonstrate that the greatest error occurs when identifying the wou

Cause of death certificates in nursing homes: Does quality matter? A retrospective review from two counties in Norway.

AIMS: One half of Norwegians die in nursing homes, where death certificates (DCs) are completed by two types of physicians: in-house physicians or physicians on call. The aims of this study were to examine differences in the quality of DCs due to type of physician and to uncover possible implications of errors for the public statistics. METHODS: DCs from the year 2013 from nursing homes in the catchment area of Akershus University Hospital were examined with regard to logical deficiencies, garbage code diagnoses and type of certifying physician. In one third of cases, the registered causes of

Evaluating effect of implementing Gibbs' reflective model on quality of nurses' documentation in emergency departments: a clinical trial.

BACKGROUND: Emergency nurses are responsible for providing and documenting immediate patient care. Documentation is a fundamental component of their professional practice; however, evidence indicates that documentation standards are often not adequately met. Therefore, innovative training approaches should be employed to improve the quality of nurses' documentation. The present study aims to evaluate the effect of implementing Gibbs' reflective model on the quality of documentation by emergency department nurses. METHODS: This cluster-randomized clinical trial was conducted on 68 nurses worki

Pain assessment by patients and nurses, and nurses' notes on it, in early acute myocardial infarction. Part I.

Pain is a multi-dimensional phenomenon unique to each individual. Most of the research to date on acute pain has focused on oncology, medical and surgical patients. This pilot study was designed to explore the issues of pain assessment by patients and nurses in acute myocardial infarction, and the nurses' subsequent documentation of the pain assessment. Three research instruments were chosen to elicit the information required: A demographic questionnaire was constructed to elicit relevant data on patients and nurses. The Short Form McGill Pain Questionnaire (SFMPQ) was used to measure the qual

Integrating a Large Language Model to Streamline Nursing Handover Documentation Across Multiple Hospitals in Taiwan: Development and Implementation Study.

BACKGROUND: The global nursing shortage, exacerbated by heavy workloads and high turnover rates associated with the COVID-19 pandemic, continues to undermine care quality and nurse well-being. Although digital health technologies have enhanced coordination, improved communication, and reduced clinical errors in nursing practice, they have also increased nurses' documentation burden. Advances in large language models (LLMs) and other generative artificial intelligence (GenAI) tools facilitate the generation of accurate reports from electronic medical records (EMRs), thereby streamlining documen

Patterns of dialysis-related complications and their association with baseline characteristics in outpatient hemodialysis: a retrospective analysis of nursing documentation.

OBJECTIVE: To describe complication patterns documented in routine nursing records and explore their association with baseline demographic and diagnostic characteristics among outpatients with end-stage kidney disease (ESKD) on maintenance hemodialysis. The goal is to inform the development of nurse-led, documentation-informed observation protocols rather than to establish definitive predictive markers. METHODS: A single-center, retrospective observational study was conducted using electronic nursing records from January 1, 2024, to April 30, 2025. Two reviewers independently extracted data.

Snapshots of simulation: creative strategies used by Australian educators to enhance simulation learning experiences for nursing students.

Simulation in nursing is a flourishing area for nurse educators' practice. Defined as learning that amplifies, mimics or replaces real-life clinical situations, simulation aims to give students opportunity to reason through a clinical problem and make decisions, without the potential for harming actual patients. Educators in nursing are contributing to simulation learning in diverse and creative ways. Yet much of their craft is not being widely disseminated because educators are not always confident in publishing their work. This paper aims to stimulate creative development in simulation by pr

APIM-guided dyadic nursing after colorectal cancer surgery: a propensity score-matched cohort study.

BACKGROUND: Recovery after colorectal cancer surgery is influenced not only by patient symptoms but also by caregiver capacity, psychological distress, self-efficacy, and dyadic coping. However, real-world evidence on Actor-Partner Interdependence Model (APIM)-guided dyadic nursing in routine postoperative care remains limited. OBJECTIVE: To examine the association between documented exposure to APIM-guided dyadic nursing and 6-month postoperative outcomes among colorectal cancer patient-caregiver dyads, and to explore potential actor-partner pathways. METHODS: This propensity score-matched

[Patient classification systems. Nursing workload from the perspective of applied research (part I)].

This article describes in two parts the basic principles of categorising nursing data into Patient Classification Systems (PCS) and related findings of nursing research. PCS are used to group patients into classes by using some specified criteria or indicators, e.g. needs of a patient, functional status or different nursing activities. They are often applied to constitute staff requirements. Since the large number of available classification systems differs in terms of development procedures and use of resulting data, it was rarely possible to compare different PCS with each other until now.
