Obesity paradox in heart failure patients - Female gender characteristics-KAMC-single center experience.

BACKGROUND/INTRODUCTION: The correlation between low body mass index (BMI) and congestive heart failure (obesity paradox) has been described in the literature; However, the association between BMI and clinical outcome measures is not well characterized. Little is known about CHF in the Middle Eastern female population; most of the gender-specific information on heart failure comes from higher income "Western" countries. OBJECTIVES: We aimed to identify the correlation between heart failure patients especially those with low BMI and clinical/safety outcome measures with focusing on female pati

An Obesity Paradox: Increased Body Mass Index Is Associated with Decreased Aortic Atherosclerosis.

Brodsky et al. (Cardiovasc Pathol 25(6), 515-520, 2016) recently have reported that there was an unexpected and highly significant inverse correlation between body mass index (BMI) and atherosclerosis of the aortas of morbidly obese decedents (BMI >40 kg/m2). In a series of 304 decedents, 65 of whom were morbidly obese, minimal or no atherosclerosis was seen in 46 of them (70%) versus 20 (30%) who had severe atherosclerosis (P = 0.008). This obesity paradox was unexpected and raises important questions about the etiology and pathogenesis of atherosclerosis, which will be the subject of this co

Weight Loss, the Obesity Paradox, and the Risk of Death in Rheumatoid Arthritis.

OBJECTIVE: In contrast to what is observed in the general population, a low body mass index (BMI) has been associated with accelerated mortality in patients with rheumatoid arthritis (RA). The aim of this study was to assess whether weight loss might explain these seemingly paradoxical observations. METHODS: Our study included patients identified from the Veterans Affairs (VA) RA Registry. Dates of death were abstracted from VA electronic medical records. The BMI at each study visit and the change from the previous visit were determined. The maximum BMI of each patient was also obtained from

Metabolic syndrome reduces the survival benefit of the obesity paradox after infrainguinal bypass.

BACKGROUND: Metabolic syndrome, having risen to epidemic proportions in the United States, is associated with future cardiovascular disease and mortality and increased postoperative complication rates. However, the impact of metabolic syndrome on outcomes after infrainguinal bypass surgery remains poorly defined. METHODS: Using the American College of Surgeons-National Surgical Quality Improvement Program database from 2005-2011, patients undergoing infrainguinal bypass were identified. Data on preoperative risk factors, operative details, and 30-day outcomes were collected. Metabolic syndrom

The influence of polyvascular disease on the obesity paradox in vascular surgery patients.

BACKGROUND: Obesity is a risk factor for atherosclerosis, a polyvascular process associated with reduced survival. In nonvascular surgery populations, a paradox between body mass index (BMI) and survival is described. This paradox includes reduced survival in underweight patients, whereas overweight and obese patients have a survival benefit. No clear explanation for this paradox has been given. Therefore, we evaluated the presence of the obesity paradox in vascular surgery patients and the influence of polyvascular disease on the obesity paradox. METHODS: In this retrospective study, 2933 co

The impact of obesity and adiponectin signaling in patients with renal cell carcinoma: A potential mechanism for the "obesity paradox".

Although obesity increases the risk of renal cell carcinoma (RCC), obese patients with RCC experience longer survival than non-obese patients. However, the mechanism of this "obesity paradox" is unknown. We examined the impact of preoperative BMI, serum total adiponectin (sAd) level, total adiponectin secretion from perinephric adipose tissue, and intratumoral expression of adiponectin receptors on RCC aggressiveness and survival. We also investigated the mechanism underlying enhanced cancer aggressiveness in RCC cells stimulated with exogenous adiponectin. Overweight and obese patients had si

Genetic Dissection of the Obesity Paradox in Carotid Atherosclerosis Using a Hyperlipidemic Mouse Cohort.

Overweight and obese individuals often exhibit lower mortality rates or better prognoses than lean or normal-weight individuals with stroke and other diseases, a phenomenon called the "obesity paradox". Carotid atherosclerosis is a major cause of ischemic stroke, and body weight serves as a reliable surrogate for adiposity in adult mice. Phenotypic and genetic connections of carotid atherosclerosis with body weight were evaluated in 299 F2 mice (154 females, 145 males) derived from BALB/cJ and LP/J Apoe knockout (Apoe-/-) mice. F2 mice were fed a Western diet for 12 weeks. Atherosclerotic lesi

"Obesity Paradox" as a new insight from long-term survivors in pancreatic cancer patients.

BACKGROUND: Obesity is traditionally associated with poor outcomes. However, the recently emerging concept of the "obesity paradox" suggests that obese cancer patients have better survival rates than non-obese patients. While this phenomenon has been confirmed in several cancers, its relevance to pancreatic cancer remains unclear. This retrospective study explores whether the obesity paradox applies to pancreatic cancer (PC) after pancreatectomy. METHODS: A total of 404 PC patients who underwent surgery between 2004 and 2022 were studied. Patients were classified into the non-obese (BMI <25.0

The 'obesity paradox' in patients undergoing transcatheter aortic valve implantation. A specialized narrative review.

Obesity has long been identified as a noteworthy risk factor for cardiovascular diseases, including aortic stenosis. Nevertheless, an unexplained phenomenon known as the obesity paradox has arisen in the context of transcatheter aortic valve implantation (TAVI), inquiring conventional beliefs. The obesity paradox refers to better clinical outcomes observed in obese individuals undergoing TAVI. These include reduced all-cause mortality, lower rates of procedural complications, and improved long-term survival rates compared to their lean counterparts. Several theories have been proposed to expla

Partial obesity paradox regarding mortality in patients hospitalized with diastolic or systolic heart failure.

INTRODUCTION: A phenomenon known as the obesity paradox has been reported in patients with heart failure (HF). The goal of this study is to characterize this observation in systolic (SHF) and diastolic (DHF) HF. METHODS AND RESULTS: We used the National Inpatient Sample (NIS) database for 2016-2020. We evaluated mortality based on body weight. A total of 7,364,023 with SHF and 10,064,223 with DHF were found in the NIS database. All-cause inpatient mortality was lowest in overweight followed by obesity and morbid obesity, whereas mortality was highest in cachexia for SHF and DHF (mortality: ov

Obesity paradox of sepsis in long-term outcome: The differential effect of body composition.

BACKGROUND: The obesity paradox has been widely studied recently; however, its impact on the long-term prognosis of sepsis and the protective mechanism of body mass have not yet been sufficiently revealed. METHODS: We retrospectively evaluated the association between obesity and 1-year survival after sepsis in a single university-affiliated hospital and examined the differential effects of muscle mass and adiposity. RESULTS: Adult patients with sepsis or septic shock (n = 1492)were classified into obese (n = 300) and nonobese (n = 1192) groups. One-year mortality due to sepsis was lower in t

Potential confounders of the obesity paradox in older patients following transcatheter aortic valve replacement.

PURPOSE: A higher body mass index (BMI) is associated with lower mortality in older patients following transcatheter aortic valve replacement (TAVR) for severe aortic valve stenosis. The current study aimed to investigate potential confounders of association between BMI and prognosis. METHODS: The retrospective single-center study included consecutive patients following TAVR and excluded those in whom subcutaneous fat accumulation (SFA), visceral fat accumulation (VFA), and major psoas muscle (MPM) volume were not assessed by computed tomography. Cachexia was defined as a combination of BMI <

Impact of Gait Speed on the Obesity Paradox in Older Patients With Cardiovascular Disease.

PURPOSE: The purpose of this study was to determine whether gait speed affects the obesity paradox in older patients with cardiovascular disease. METHODS: The study population consisted of 2224 patients ≥60 years old with cardiovascular disease admitted to hospital between May 1, 2006, and January 31, 2018. Body mass index (BMI) and gait speed before hospital discharge were determined, and patients were divided into two groups: slow and preserved gait speed (≤0.8 and >0.8 m/s, respectively), according to the algorithm for sarcopenia diagnosis. The slow and preserved gait speed groups were als

"Obesity Paradox" Holds True for Patients with Hip Fracture: A Registry-Based Cohort Study.

BACKGROUND: Hip fractures are associated with high mortality and reduced quality of life. Studies have reported a high body mass index (BMI) as being positively associated with survival when linked to old age and some chronic diseases. This phenomenon is called the "obesity paradox." The association between BMI and survival after hip fracture has not been thoroughly studied in large samples, nor has to what extent the association is altered by comorbidities, sex, and age. The objective of this study was to investigate the association of BMI with survival after hip fracture and with the probabi

Time to redefine body mass index categories in chronic diseases? Spotlight on obesity paradox.

Obesity is a complex condition classically characterised by excessive body fat accumulation and represents one of the most important public health problems worldwide. Although several epidemiological studies have shown that elevated BMI is associated with higher morbidity, and with increased rate of death from all causes and from cardiovascular disease, accumulating evidence suggests that being overweight or obese may be protective (the so-called obesity paradox), at least in chronic diseases. These observations, not only question the validity of the BMI system, but also raise the intriguing q

Collider Bias Is an Insufficient Explanation for the Inverse Obesity Paradox in Prostate Cancer.

BACKGROUND: Collider bias is often considered a potential explanation when the association between obesity and disease diagnosis differs from that with disease outcome, as seen in the "obesity paradox." For prostate cancer (PCa), in particular localized PCa, an "inverse" obesity paradox has been observed, where body mass index (BMI) is negatively associated with diagnosis (hazard ratio [HR] ~0.9 per 5-kg/m2 increase), but positively associated with PCa-specific death (HR ~ 1.2). However, collider bias in this context remains unexplored. METHODS: We simulated binary disease diagnosis and outco

Revisiting the Obesity-Anaemia Paradox: Inflammation and Iron Homeostasis in the BMI-Haemoglobin Relationship.

BACKGROUND: Obesity and anaemia are global epidemics with complex, overlapping pathophysiology. While excess adiposity is known to induce chronic inflammation that disrupts iron homeostasis, multiple population studies paradoxically report higher haemoglobin levels and lower anaemia prevalence among obese individuals. The nonlinear and potentially suppressive role of inflammation in this relationship remains understudied. METHODS: We analysed adults aged 18-64 from the 2015-2023 National Health and Nutrition Examination Survey (NHANES). Haemoglobin was modelled as a function of body-mass inde

Obesity Paradox among Heart Failure with Reduced Ejection Fraction Patients: A Retrospective Cohort Study.

Background and Objectives: There is consensus on the negative effects of obesity on the development of heart failure. However, several studies have suggested that obesity may have paradoxical survival benefits in heart failure patients. Therefore, the aim of this study is to investigate whether the obesity paradox exists in heart failure with reduced ejection fraction (HFrEF) patients in Jordan. Materials and Methods: In this retrospective cohort study, data were retrieved from electronic hospital records of heart failure patients admitted to King Abdullah University Hospital between January 2

BMI and acute kidney injury post transcatheter aortic valve replacement: unveiling the obesity paradox.

AIMS: Acute kidney injury (AKI) may complicate transcatheter aortic valve replacement (TAVR) leading to higher mortality. The relationship between AKI, obesity, and mortality, however, is controversial. We sought to investigate the impact of body habitus on the prognostic value of AKI in TAVR. METHODS: Among the 645 patients who underwent successful TAVR in a single high-volume centre, we retrospectively evaluated the association between AKI-TAVR and 30-day, 6-month, and 1-year mortality, and whether this relationship was affected by BMI. AKI was defined according to the Valve Academic Resear
