Peripheral nerves of the lumbar plexus share a common origin but exhibit distinct anatomical and clinical features. Although the gross anatomy of these nerves has been described, differences in cross-sectional structure remain unclear. This study compared the histological architecture of the obturator nerve (ON) with that of the femoral nerve (FN) and lateral femoral cutaneous nerve (LFCN). Eighteen nerves (ON, FN, and LFCN) from six cadavers were examined using Masson's trichrome-stained cross-sections. Morphometric analysis included the short-to-long axis ratio and fascicular area ratio. Epi

The hip is the location for many disorders, such as osteoarthritis, femoroacetabular impingement syndrome, avascular necrosis of the hip, and cartilage injuries, all of which cause chronic pain, disability, and limitation. It is estimated that 10% of the population ≥40 years of age will present with hip pain. The line of treatment goes from nonsteroidal anti-inflammatory drugs, physical therapy, lifestyle modification, intra-articular injections, and hip surgery. This will depend on the pathology, age, lifestyle, and sport activity of each patient. Currently, hip joint intra-articular injectio

BACKGROUND: The proximal course of the obturator nerve as related to the psoas major has been described differently among various authors. Because this relationship is important for better understanding of clinical presentations and during surgical approaches, this study aimed to elucidate this anatomy via cadaveric dissection.
METHODS: Twenty obturator nerves from 10 white cadaveric specimens underwent dissection. Observations were made of the relationship between the nerve and psoas major muscle.
RESULTS: On all sides, the obturator nerve descended posterior to the psoas major and never th

PurposeChronic hip pain is a common symptom experienced by many people. Often, surgery is not an option for patients with multiple comorbidities, and conventional drugs either have many side effects or are ineffective. Pulsed radiofrequency (PRF) is a new method in the treatment of pain. We attempt to compare the efficacy of PRF relative to conservative management for chronic hip pain.Patients and methodsBetween August 2011 and July 2013, 29 patients with chronic hip pain were divided into two groups (PRF and conservative treatment) according to consent or refusal to undergo PRF procedure. A 76-yr-old male presented for leg amputation above the knee. The patient complained of dyspnea due to pulmonary embolism occurring 3 weeks before operation. In addition, the patient could not report paresthesias because he had suffered from a cerebral infarction. Anesthesia was performed with combined block of femoral, sciatic, obturator nerves and the lateral cutaneous nerve of the thigh. The nerves were anesthetized with 0.75% ropivacaine solution 31 ml by use of an electrical nerve stimulator and an insulated needle. Nerve stimulation technique is the best choice for patients who are unabl

Transurethral resection of bladder tumors (TURBT) is a pivotal procedure in the management of bladder cancer, essential for both diagnosis and treatment. Effective anesthesia is crucial in TURBT to ensure a stable and pain-free operative field, facilitate precise tumor resection, and minimize complications such as the obturator reflex, which can lead to involuntary leg movement and bladder injury. The obturator nerve block (ONB) is a regional anesthesia technique designed to prevent the obturator reflex by blocking the obturator nerve, which innervates the adductor muscles of the thigh. This c

OBJECTIVES: The objective of the study is to clarify potential risks to the dorsal nerve of the clitoris (DNC) and obturator canal using different minimally invasive slings.
STUDY DESIGN: Ten embalmed hemipelves were dissected to demonstrate the course of the DNC and the obturator canal. On each cadaver, tension-free vaginal tape (TVT), transobturator in-out (TVT-O) and transobturator out-in (Monarc) procedures were performed. Distances between the DNC and the obturator canal to the different devices were measured.
RESULTS: The DNC passes beneath the pubic bone at a distance of 14.3 +/- 4.7

BACKGROUND: Pelvic lymph-node dissection is performed across multiple surgical specialties. However, inconsistent terminology and unclear anatomical boundaries hinder standardization. This study established a multidisciplinary team with a shared anatomical understanding, with the aim to prospectively evaluate standardized pelvic lymph-node dissection, focusing on the dorsal region of the obturator nerve.
METHODS: A prospective observational study was conducted at a single institution from November 2022 to 2025. A team of urology, gastrointestinal surgery, gynecology, and pathology specialists

BACKGROUND: Stress urinary incontinence is a frequent condition in female patients. Surgical treatment with tension-free vaginal tape (TVT) insertion is a minimally invasive option with immediate improvement of symptoms. Different possible complications have been described in the literature. Obturator nerve entrapment is a very rare complication of retropubic (rp) TVT insertion.
OBJECTIVES: To show the feasibility of robotic-assisted laparoscopic mesh removal in a case of rpTVT-related entrapment of the left obturator nerve.
MATERIALS AND METHODS: We present the case of a 55-year-old patient

BACKGROUND: Obturator nerve injury may occur in gynecologic surgery, particularly in cases in which extensive pelvic sidewall retroperitoneal dissection is performed. The lack of tactile feedback from the robotic surgical system may contribute to obturator nerve injury. If surgical division occurs, microsurgical end-to-end anastomosis of the obturator nerve may be performed.
CASE: A 76-year-old woman with stage IA endometrial adenocarcinoma sustained a left obturator nerve transection during pelvic lymphadenectomy that was recognized immediately. Robotic-assisted laparoscopic repair was perfo

A female patient was successfully treated surgically for obturator nerve palsy resulting from extrapelvic extrusion of cement during total hip arthroplasty. Obturator nerve palsy developed because of the extrapelvic extrusion of cement medially in the anterior part of the incisura acetabuli in the zone of the extrapelvic exit of the obturator tunnel. Compression of the obturator nerve was confirmed by electromyography. After surgical removal of a piece of cement, 2.5 cm in length, which was done 11 months after total hip arthroplasty, and after 6 months of intensive postoperative rehabilitatio

INTRODUCTION: Urinary bladder tumors are one of the most common urological malignancies. Traditionally, it has been managed with trans-urethral resection of urinary bladder tumor (TURBT) for both diagnostic and therapeutic purposes. During TURBT of lateral wall tumors, there is risk of obturator nerve reflex (ONR), which can lead to serious complications such as inadvertent bleeding and urinary bladder perforation. To prevent this, obturator nerve block is given after spinal anesthesia. In this study, we have used the transvesical approach to block the obturator nerve.

The sensory innervation of the hip joint is complex. The joint and sensory branch block of the obturator and femoral nerves is effective for treating the pain caused due to different hip diseases. This could be an option to be considered in certain circumstances such as, being a surgical-anaesthetic high risk, or if there is significant overweight, It could also be useful on other occasions if the traumatoligist considers that it is better to delay hip replacement for a limited period.

Internal hernia beneath the vascular structures after pelvic lymphadenectomy is a rare condition. Herein, we report a case of a strangulated internal hernia beneath the obturator nerve 38 months after laparoscopic radical cystectomy with extended pelvic lymphadenectomy. Computed tomography revealed dilated small bowels and a closed loop in the pelvis. The emergency laparotomy was performed, and a strangulated internal hernia beneath the obturator nerve was observed. It is necessary to consider the possibility of internal hernia beneath the vascular structure, including the obturator nerve, aft

BACKGROUND AND OBJECTIVES: A substantial group of patients suffer from moderate to severe pain following elective total hip arthroplasty (THA). Due to the complex innervation of the hip, peripheral nerve block techniques can be challenging and are not widely used. Since the obturator nerve innervates both the anteromedial part of the joint capsule as well as intra-articular nociceptors, we hypothesized that an obturator nerve block (ONB) would decrease the opioid consumption after THA.
METHODS: Sixty-two patients were randomized to receive ONB or placebo (PCB) after primary THA in spinal anes

We report successful management of anesthesia in two cases of knee arthroscopic surgery of meniscal lesions using ultrasound-guided combined femoral-obturator nerve block with inhalation anesthesia. The blocks were performed with 30 ml of 0.5% ropivacaine under ultrasonographic visualization. The perioperative courses were uneventful and there was no complaint about postoperative pain. Unlike spinal or epidural anesthesia, combined femoral-obturator nerve block has advantages of no muscle weakness in healthy lower limbs, no urinary retention, and no post dural puncture headache. Our technique
