Congenital cyclic oculomotor palsy and spasms: a review of the literature and presentation of two new cases.

BACKGROUND: Cyclic oculomotor palsy and spasms (COPS) is a rare neurological syndrome characterized by alternating involuntary phases of paresis and spasm of muscles supplied by the oculomotor nerve. Its etiology, management, and long-term prognosis are poorly understood. The purpose of this study was to provide a systematic review of the published literature and present 2 new cases. METHODS: We systematically reviewed the literature on congenital COPS following PRISMA guidelines (PROSPERO CRD42021286740). Searches were conducted in MEDLINE, EMBASE, Web of Science, KoreaMed, KMBase, KISS, KCI

Extraocular muscles: basic and clinical aspects of structure and function.

Although extraocular muscle is perhaps the least understood component of the oculomotor system, these muscles represent the most common site of surgical intervention in the treatment of strabismus and other ocular motility disorders. This review synthesizes information derived from both basic and clinical studies in order to develop a better understanding of how these muscles may respond to surgical or pharmacological interventions and in disease states. In addition, a detailed knowledge of the structural and functional properties of extraocular muscle, that would allow some degree of predicti

Congenital oculomotor nerve synkinesis associated with fetal retinoid syndrome.

INTRODUCTION: Isotretinoin (RA), used for the treatment of cystic acne, is a powerful teratogen, causing craniofacial dysmorphisms and neural tube defects. We present two patients with RA embryopathy and oculomotor nerve synkinesis. METHODS: Retrospective review of patient records. RESULTS: Two patients presented with third nerve synkinesis and fetal RA exposure. Both had marked elevation of the upper eyelids on adduction such that the lid fissures alternately opened and closed on gaze from side to side. Both patients showed typical dysmorphisms of RA embryopathy. The first patient had compl

The role of intraoperative monitoring of oculomotor and trochlear nuclei -safe entry zone to tegmental lesions.

OBJECTIVE: A safe entry zone to tegmental lesions was identified based on intraoperative electrophysiological findings, the compound muscle action potentials (CMAP) from the extraocular muscles, and anatomic considerations. This entry zone is bordered caudally by the intramesencephalic path of the trochlear, laterally by the spinothalamic tract, and rostrally by the caudal margin of the brachium of the superior colliculus. METHODS: Four intrinsic midbrain lesions were operated upon via the safe entry zone using the infratentorial paramedian supracerebellar approach. All lesions involved the t

Abducens-oculomotor synkinesis associated with internuclear ophthalmoplegia and acquired abducens nerve palsy.

We report a case of acquired abducens-oculomotor synkinesis, internuclear ophthalmoplegia, and abducens nerve palsy following brainstem hemorrhage. Weakening surgery on the medial rectus muscle did not eliminate the synergistic movement. The mechanism responsible for the abducens-oculomotor synkinesis is discussed.

Magnetic Resonance Imaging Findings in Patients With Duane Retraction Syndrome.

BACKGROUND: Duane retraction syndrome (DRS) is known to relate to the absence of the abducens nucleus, with abnormal innervation of the lateral rectus (LR) muscle by branchesof the oculomotor nerve (CN III). The purposes of this study were to investigate the morphological characteristics of the oculomotor nerve (CN III), the abducens nerve (CN VI), and the extraocular muscles in patients with clinically diagnosed Duane retraction syndrome (DRS) using MRI. In addition, we assessed the association between ocular motility, horizontal rectus muscle volumes, and CN III/VI in patients with Duane ret

Oculomotor functions in focal dystonias: A systematic review.

Focal Dystonia (FD) is a chronic neurological disorder, which causes twisting and repetitive movements and abnormal postures induced by involuntary sustained contractions of agonist and antagonist muscles. Based on the hypothesis that several dystonia-related brain regions, including cerebellum, are implicated in oculomotor disturbances (OCD), a number of studies investigated oculomotor function in patients with dystonia. However, conceptual clarity with respect to the used assessment tools and interpretation of the findings is lacking in the literature. This is the first article to systematic

Isolated oculomotor nerve palsy related to sinusitis?

The association of sinusitis with ocular motility disorders is a seductive theory due to their close anatomical vicinity. Typically, sinusitis can influence ocular motility by affecting single muscles or a combination of muscles and/or cranial nerves due to a local inflammatory tissue reaction. Although rare, sinusitis has been suggested at least as a cause for superior-branch oculomotor palsy. We report a case of progressive, isolated, third-cranial-nerve palsy caused by an aneurysm of the posterior-communicating artery that initially was thought to be related to pansinusitis.

Clinical Study of Eleven Patients with Midbrain Infarction-Induced Oculomotor Nerve Palsy.

BACKGROUND: Midbrain lesion-induced oculomotor nerve palsy can be divided into the nuclear and infranuclear types. In the infranuclear type, the degree of each subtype of ocular muscle palsy usually varies. METHODS: The neuroradiological findings of 11 patients with midbrain infarction-induced oculomotor nerve palsy were analyzed. Their infarcts were grouped into rostral and caudal lesions. Each group was then divided into lesions that occurred in the paramedian and lateral regions before being subdivided further into lesions that occurred in the tegmental, central, and ventral areas. The pupillary and ciliary components of the cat Edinger-Westphal nucleus: a transsynaptic transport investigation.

The distribution of preganglionic motoneurons supplying the ciliary ganglion in the cat was defined both qualitatively and quantitatively. These cells were retrogradely labeled directly, following injections of wheat germ agglutinin conjugated to horseradish peroxidase (WGA-HRP) into the ciliary ganglion, or were transsynaptically labeled following injections of WGA into the vitreous chamber. Almost half of the cells are distributed rostral to the oculomotor nucleus, both in and lateral to the anteromedian nucleus. Of the remaining preganglionic motoneurons, roughly 20% of the total are locate

Recurrent oculomotor neuritis related to autoimmune hypothyroidism. Case Report.

Ophthalmopathy related to thyroid disease is due mainly to diffuse periorbital or eye muscle inflammation. It is more common in Grave's hyperthyroidism and rare in Hashimoto's hypothyroidism. Here we report a case of recurrent oculomotor nerve palsy associated with autoimmune hypothyroidism. Brain MRI revealed enhancement of the oculomotor nerves. Despite thyroid hormone replacement therapy, oculomotor nerve palsy recurred at the side contralateral to the initially involved nerve and the autoimmune antibody titer remained high. The symptom was responsive to high-dose steroid therapy.

[A case of bilateral blephaloptosis resulting from midbrain lesions caused by diffuse large B-cell lymphoma].

We report the case of a patient with bilateral blephaloptosis associated with a recurrence of diffuse large B-cell lymphoma (DLBCL) in the midbrain. A 70-year-old man experienced acute onset bilateral blephaloptosis; the other ocular movements, except for medial rectus muscle in the right eye, were not impaired. Pupils were isocoric and light reflexes were prompt. Other cranial nerves were intact. Gadolinium-enhanced MRI revealed abnormal enhancement in the midbrain and peri-ventricular regions. FDG-PET revealed an abnormal positive signal in the midbrain, similar to the distribution seen in t

Evidence for a chronic axonal atrophy in oculopharyngeal "muscular dystrophy".

We report on morphometric investigations of peripheral nerves in a woman, who died at the age of 69, presenting the classical symptoms of oculopharyngeal muscular dystrophy (OPMD) and a typical family history with several members (males and females) affected over three generations. Evidence for chronic axonal atrophy was found in peripheral nerves and especially in oculomotor nerves with severe axon loss in endomysial nerve twigs of extraocular, laryngeal, and tongue muscles. Whereas limb muscles presented features of neurogenic atrophy, severe changes of "myopathic" type were evident in extri

Intramuscular Nerve Distribution in the Medial Rectus Muscle and Its Clinical Implications.

PURPOSE: The intramuscular nerve distribution in the extraocular muscles may be crucial for understanding their physiological and pathological responses. This study aimed to determine the oculomotor nerve distribution in the medial rectus muscle (MR) using Sihler's staining. METHOD: Thirty-seven MRs from 23 cadavers were investigated. The MR including the oculomotor nerve was finely dissected from its origin to its insertion point into the sclera. The total length of the muscle-belly, tendon length and maximum width of the muscle were measured. We evaluated the pattern of distribution and the

Non-nystagmus hyperkinetic eye movement disorders.

BACKGROUND: While nystagmus is an involuntary ocular movement relatively frequent, well described, and easily recognized by most practitioners, non-nystagmus hyperkinetic eye movement disorders (NHEMD) are less obvious and can be overlooked. NHEMD may arise from intrinsic abnormalities in ocular muscles and oculomotor nerves, brain and brainstem lesions, systemic diseases, dysimmune disorders, drugs or can even be functional in nature. Given that some treatable disorders initially manifest with NHEMD, their knowledge becomes crucial. AIMS: This review aims to offer a practical and comprehensi
