PURPOSE: We evaluated the clinical characteristics of men with uniform testicular maturation arrest and nonobstructive azoospermia or severe oligospermia, including the frequency of genetic defects and outcome of intracytoplasmic sperm injection with or without testicular sperm extraction.

OBJECTIVE: This study aimed to investigate the genetic etiology of male infertility patients.
METHOD: A total of 1600 male patients with infertility, including 1300 cases of azoospermia and 300 cases of severe oligozoospermia, underwent routine semen analysis, chromosomal karyotype analysis and sex hormone level testing. The Azoospermia factor (AZF) on the Y chromosome was detected using the multiple fluorescence quantitative PCR technique. Additionally, copy number variation (CNV) analysis was performed on patients with Sertoli-cell-only syndrome who had a normal karyotype and AZF.

Investigation of infertile couples at Goroka Base Hospital during 1972 showed that tubal pathology, apart from any other factor, accounts for the majority of infertile unions in the Highlands of Papua New Guinea. There is also a high incidence of oligospermia. The poor results from tuboplasty are confirmed in this limited series.

We report two cases of Kennedy's disease (muscle weakness, amyotrophy, intentional tremor, endocrine abnormalities, and denervation signs at electromyography). This entity must be differentiated from other motor neuron disorders by the genetic pattern (X-liked recessive), gynecomastia, testicular atrophy, oligospermia and good prognosis. A discussion about the clinical pattern and evolution is made.

The case of an infertile man with oligospermia and symptoms of urethritis-prostatitis, whose spouse had a vaginal discharge, is reported. Microbiological analysis of appropriate specimens revealed a strain of tetracycline-resistant ureaplasma urealyticum in both patients. Using the transmission electron microscope, it was possible to demonstrate spermatozoal heads "infected" with microorganisms strongly resembling ureaplasma urealyticum.

Some weeks after an episode of fever and diarrhea, a 33-year-old male presented with sacroiliitis, Achilles tendinitis and aphthous stomatitis. Chromosome studies were conducted on 117 men with primary infertility (54 with oligospermia and 63 with azoospermia) and 25 fertile volunteers who acted as controls. No chromosomal abnormalities were observed except for nine cases of Klinefelter's syndrome. From this study, it is apparent that chromosomal abnormalities other than Klinefelter's syndrome are not etiologic factors in the infertility of oligo- or azoospermic men.

The age at surgical intervention, always bilateral, was 11-12 years, following the same technique and in 85% by the same surgeon. Out of the total of 285 men, 190 submitted to a follow-up examination and 141 sperm analyses were obtained; 25% azoospermia, 55% oligospermia and 20% normospermia. Out of a total of 107 married men, 39 had fathered one or more children.
