Successful prevention of severe allergic transfusion reactions with omalizumab.

BACKGROUND: Allergic transfusion reactions (ATRs) are a common adverse reaction to transfusion therapy and can be potentially fatal. Washing blood products is the most effective strategy for preventing ATRs; however, washed products, especially platelets, are not available at many blood centers. STUDY DESIGN AND METHODS: A 29-year-old female patient with an advanced myelodysplastic/myeloproliferative neoplasm, unclassifiable, developed severe ATRs after four platelet transfusions in a week. She showed no response to premedication with histamines and steroids and still had severe ATRs with the

Omalizumab Treatment for Allergic Bronchopulmonary Aspergillosis in Cystic Fibrosis.

BACKGROUND: Allergic bronchopulmonary aspergillosis (ABPA) in cystic fibrosis (CF) is characterized by destructive changes in the airways. Long-term treatment with oral corticosteroids is often required for repeated exacerbations. Because elevated total IgE is a cardinal abnormality of ABPA, omalizumab has been used sporadically to decrease corticosteroid dose or totally replace corticosteroids. OBJECTIVE: The aim of this report is to describe our experience with omalizumab treatment in patients with CF and ABPA. METHODS: We conducted a review of 6 CF patients with ABPA receiving omalizumab.

Predictors of relapse after omalizumab withdrawal in chronic spontaneous urticaria: Role of baseline autoimmunity and hematologic indices: CSU Relapse After Omalizumab Withdrawal.

Background: There is currently no accepted biomarker for predicting which patients will experience relapse after discontinuation of omalizumab therapy. Objective: This retrospective study aimed to identify clinical and laboratory markers associated with chronic spontaneous urticaria relapse. Methods: "Relapse" was defined as a urticaria control test score of <12 during the 6-month follow-up after treatment interruption after 24 weeks of therapy. Baseline demographic characteristics and laboratory values, including total immunoglobulin E (IgE), anti-thyroid peroxidase (TPO), C-reactive protein

Biomarkers of response to omalizumab in patients with chronic spontaneous urticaria.

BACKGROUND AND OBJECTIVES: Managing chronic spontaneous urticaria (CSU) resistant to anti-histamines remains challenging, and predictors of omalizumab efficacy are not fully understood. This study evaluated omalizumab's effectiveness, response patterns, and predictors in CSU patients. MATERIALS AND METHODS: A retrospective analysis was conducted on 72 patients receiving omalizumab for at least six months. Factors influencing response to anti-IgE therapy were examined. RESULTS: Among the 72 patients with chronic spontaneous urticaria (CSU), 86.1% responded to omalizumab: 58.3% had a good resp

Biopredictors for Omalizumab Response in Patients With Chronic Spontaneous Urticaria.

BACKGROUND: Proposed negative biopredictors of omalizumab response in patients with chronic spontaneous urticaria are low baseline IgE (≤40 IU/mL), positive CU Index (CUI) test result, and basopenia defined by blood histamine content (BHC) (≤8 ng/mL). OBJECTIVE: To test the hypothesis that patients with these negative biopredictors will have a poorer response to omalizumab. METHODS: We performed a retrospective analysis of 3 phase III studies of antihistamine-refractory subjects with chronic spontaneous urticaria who received omalizumab 300 mg every 4 weeks for 12 weeks. The relationship bet

Predictive cytokines of omalizumab in the treatment of chronic spontaneous urticaria.

BACKGROUND: Omalizumab, an anti-IgE biological agent, is commonly prescribed as a second-line therapy for Chronic Spontaneous Urticaria (CSU). However, there is a lack of biomarkers to predict which CSU patients will respond favorably to omalizumab. OBJECTIVE: Our study aims to identify cytokine markers associated with the efficacy of omalizumab in treating CSU. METHODS: We conducted a prospective study, enrolling antihistamine-resistant CSU patients from Tongji Hospital between Feburary and August 2023. All patients received a 16-week course of omalizumab (300 mg, administered every 4 weeks

Omalizumab for the reduction of allergic reactions to foods: a narrative review.

The frequency of food allergies varies between 2% and 10%, depending on characteristics including age, region, race, and method of diagnosis self-reported by patients or oral food challenges (OFCs). The most common allergies reported are tree nuts (1.2%), milk (1.9%), peanuts (2.2%), and shellfish (1.3%). Omalizumab injection has now been approved by the FDA for the treatment of immunoglobulin E-mediated food allergies in specific adults and children aged one year or older. This medication reduces the risk of allergic reactions (Type I), which can include anaphylaxis, when an individual accide

The relationship between allergen immunotherapy and omalizumab for treating asthma.

Allergen-specific immunotherapy (AIT) is considered the only treatment capable of modifying the natural history of allergic respiratory disorders. The possible adverse events related to AIT have, until now, limited its use to mild and controlled asthma. The pre-administration or concomitant treatment of AIT and omalizumab (an anti-IgE humanized antibody), recommended for the treatment of severe allergic asthma, could be useful in reducing the adverse events due to AIT and to allow its use in patients with more severe or uncontrolled asthma. AIT/omalizumab combination has been explored in a few
