Monitoring of optic nerve function in Neurofibromatosis 2 children with optic nerve sheath meningiomas using multifocal visual evoked potentials.

Monitoring optic nerve sheath meningiomas (ONSM) in Neurofibromatosis type 2 (NF2) patients remains difficult. Other ocular manifestations of NF2 may obscure ophthalmic assessment of optic nerve function in these patients. Serial magnetic resonance imaging (MRI) used to assess the optic nerve is not without limitations, being expensive and often requiring general anaesthetic in children, with associated risks. This study was undertaken to describe the use of multifocal visual evoked potentials (multifocal VEP, mfVEP) in the regular monitoring of NF2 patients with ONSM. This study involved thre

External ophthalmoplegia due to ocular myositis in a patient with ophthalmic herpes zoster.

External ocular muscle palsies in patients with ophthalmic zoster are traditionally interpreted as diseases of III, IV or VI cranial nerves. Orbital myositis associated with zoster ophthalmicus has been diagnosed only rarely. We describe a patient with ophthalmic zoster and external ophthalmoplegia due to ocular myositis demonstrated by MR imaging. Treatment with acyclovir and cortisone resulted in a rapid improvement of the ophthalmoplegia. In ophthalmic herpes zoster associated with external ocular muscle palsies, ocular myositis is an important differential diagnosis to inflammatory involve

Neuro-ophthalmology in horses.

A complete neuro-ophthalmologic assessment is relatively simple, requires minimal instrumentation and should be performed as part of every complete ophthalmic and neurologic examination. This article has summarized the tests that comprise and the species-specific details of the complete neuro-ophthalmologic of the equine patient. Selected causes of sudden blindness in the horse were summarized and some common neuro-ophthalmic conditions with significant ophthalmic consequences, such as facial nerve paralysis, were discussed. Split-lid tarsorrhaphies, which are indicated but rarely used in case

The lateral sellar nerve plexus and its connections in humans.

OBJECT: The aim of the present study was to elucidate the systematic topography of the lateral sellar (cavernous sinus [CS]) nerve plexus and its connections in humans. METHODS: Seven specimens of human CS and adjacent regions were dissected in steps and stained as whole-mount preparations by using a sensitive acetylcholinesterase method. Another specimen was frozen, cut on a frontal plane, and stained for acetylcholinesterase. The human CS contains an extensive nerve plexus with small ganglia. The plexus is composed of a main part, the lateral sellar plexus proper, which is located around th

Cadaveric Anatomical Study of Orbit-Preserving Ophthalmic Neurectomy via Endoscopic and Exoscopic Techniques.

BACKGROUND: Perineural spread of cutaneous head and neck squamous cell carcinoma is most effectively managed by surgical resection with a clear central margin at the affected nerve. For ophthalmic nerve disease, there is no clear consensus regarding the extent of spread which is resectable via an orbit-preserving neurectomy, either in regard to oncological outcome or technical feasibility. METHODS: Cadaveric anatomical study of 10 human orbits, with endoscopic and exoscopic dissection of the frontal division of the ophthalmic nerve, to quantify the maximal extent of resection via orbit-preser

Optic Foraminotomy for Clipping of Superior Carotid-Ophthalmic Aneurysms.

Background: Carotid-ophthalmic aneurysms usually cause visual problems. Its surgical treatment is challenging because of its anatomically close relations to the optic nerve, carotid artery, ophthalmic artery, anterior clinoid process, and cavernous sinus, which hinder direct access. Despite recent technical advancements enabling risk reduction of this complication, postoperative deterioration of visual function remains a significant problem. Therefore, the goal of preserving and/or improving the visual outcome persists as a paramount concern. Objective: We propose optic foraminotomy as an alte

Immunohistochemical determination of the sympathetic pathway in the orbit via the cranial nerves in humans.

OBJECT: The present study was undertaken to elucidate the extent and precise distribution of the postganglionic sympathetic fibers in the cranial nerves projecting to the orbit and to reconstruct sympathetic routes in the orbit in humans. For this purpose, the authors made an immunohistochemical determination of the sympathetic fibers by using an antibody against norepinephrine-synthetic enzyme, tyrosine hydroxylase (TH). METHODS: Specimens containing the orbit and the cavernous sinus were obtained from formalin-fixed human cadavers. First, it was confirmed that the superior cervical ganglion

Indocyanine Green Videoangiography Transoptic Visualization and Clipping Confirmation of an Optic Splitting Ophthalmic Artery Aneurysm.

OBJECTIVE: Ophthalmic artery aneurysms with medial and superior projection in exceptionally rare cases can split the optic nerve. Treatment of these aneurysms is challenging, because the aneurysm dome is hidden from the optic nerve, rendering its visualization and clipping confirmation difficult. In addition, optic nerve function should be preserved during surgical maneuvers. Preoperative detection of this growing feature is usually missing. CASE DESCRIPTION: We illustrate the first case of indocyanine green videoangiography (ICG-VA) application in an optic penetrating ophthalmic artery aneur

Supraorbital Nerve and Cavernous Sinus Invasion with Poorly Differentiated Carcinoma of Unknown Primary.

A 50-year-old man presented with a 4-month history of right-sided trigeminal neuropathy in the V1 and V2 distribution, right sixth nerve palsy and Horner syndrome. Magnetic resonance imaging (MRI) showed enhancement and thickening along the right ophthalmic nerve and supraorbital nerve and fullness at the right cavernous sinus extending to Meckel's cave. Evaluation for a primary tumour was negative. Cavernous sinus biopsy showed infiltrating poorly differentiated carcinoma; the patient underwent radiation therapy. To our knowledge this is the only case of poorly differentiated carcinoma involv

Probabilistic tractography of the extracranial branches of the trigeminal nerve using diffusion tensor imaging.

PURPOSE: The peripheral course of the trigeminal nerves is complex and spans multiple bony foramen and tissue compartments throughout the face. Diffusion tensor imaging of these nerves is difficult due to the complex tissue interfaces and relatively low MR signal. The purpose of this work is to develop a method for reliable diffusion tensor imaging-based fiber tracking of the peripheral branches of the trigeminal nerve. METHODS: We prospectively acquired imaging data from six healthy adult participants with a 3.0-Tesla system, including T2-weighted short tau inversion recovery with variable f

Herpes zoster in neuro-ophthalmology: a practical approach.

Herpes Zoster (HZ) or shingles is the reactivation of the Varicella Zoster Virus (VZV), usually along a single sensory nerve, but can affect both sensory and motor cranial nerves. Major risk factors for HZ include immunosuppressed status and age older than 60 years. In the United States, the lifetime risk of HZ is approximately 30%. Worldwide, the median incidence of HZ is 4-4.5 per 1000 person-years across the Americas, Eurasia, and Australia. HZ ophthalmicus, occurring in 10-20% of patients, is an ophthalmic emergency characterized by VZV reactivation along the V1 branch of the trigeminal ne

Intraorbital aneurysm of the ophthalmic artery: a rare cause of apex orbitae compression syndrome.

PURPOSE: The purpose of this study was to report the case of a patient with a saccular, intraorbital aneurysm of the ophthalmic artery and to analyze the correlation between clinical symptoms and aneurysm localization with regard to the literature. METHOD: We correlated the patient's clinical findings with anatomical substrates on magnetic resonance imaging and angiographic studies. RESULTS: A 64-year-old woman presented with a rapidly progressive loss of visual acuity in her right eye, temporo-basal visual field defects, a temporal pallor of the optic disc and third and sixth nerve palsies.

Carotid cavernous fistula complicated by contralateral abducens palsy and optic neuropathy.

Carotid-cavernous fistulas (CCFs) occur rarely in children. We present the second known case of a pediatric CCF complicated by contralateral cranial nerve palsies. A 9-year-old boy developed a left CCF after falling onto a plastic stick. He underwent stenting of the internal carotid artery and transvenous coil embolization of the left superior ophthalmic vein and left sides of the cavernous and circular sinuses. Six weeks after repair, he demonstrated right periorbital edema, proptosis, and engorged conjunctival vessels, a right abducens nerve palsy, and a right relative afferent pupillary def

Fourth and sixth cranial nerve injury after halo traction in children: a report of two cases.

BACKGROUND: Spinal traction is the application of a longitudinal force to the spinal column as a means of stabilizing a damaged or abnormal spine. Although not well documented in the ophthalmic literature, complications include cranial nerve palsies, with the sixth nerve being most commonly affected. Fourth nerve palsies have not previously been reported to our knowledge. We present 2 cases of combined fourth and sixth palsies after cervical traction. METHODS: Retrospectively, we reviewed the ophthalmic findings in 2 children with diplopia after spinal traction. RESULTS: Case 1 suffered a tr

Olfactory navigation in homing pigeons: the last challenge.

The olfactory navigation hypothesis of pigeon homing was recently challenged by the discovery in the upper beak of putative magnetoreceptors innervated by the ophthalmic branch of the trigeminal nerve. To assess whether the nature of the cues used for navigation is determined by the kind of stimuli to which the birds are preferentially exposed during development, we tested the navigational performance of pigeons subjected when young to section of either the ophthalmic branch of the trigeminal nerve (V1) or the olfactory nerve and then subjected to training flights after the surgery. Section of

Ophthalmic terminals in the iris and the ciliary body of monkeys. An electron microscopical study.

Recent studies have indicated that sensory terminals can be recognized by the presence of an accumulation of mitochondria. By inducing changes in uveal ophthalmic nerve fibers it was shown that an accumulation of mitochondria also occurs in autonomic terminals. Although an aggregation of mitochondria may occur in sensory terminals, it is not a feature unique to them. The ophthalmic fibers in the iris and the ciliary body appeared to terminate as free nerve endings without characteristics peculiar to them.

Neuro-Ophthalmic Manifestations of Mollaret Meningitis.

Mollaret meningitis (MM) refers to benign recurrent aseptic meningitis usually following herpes simplex virus 2 (HSV-2) infection. Neuro-ophthalmic manifestations associated with MM are rarely reported. We present a case of recurrent HSV-2 meningitis with the neuro-ophthalmic presentation of papilledema and sixth nerve palsy. To our knowledge, this is the first such description in the English language ophthalmic literature.
