Clinical and radiological factors in traumatic open-globe injuries operated via primary enucleation, evisceration versus primary repair in Somalia.

BACKGROUND: Traumatic eye injuries are an important cause of visual impairment and can significantly affect patients’ quality of life. Accounting for approximately 7% of all bodily injuries and 10–15% of ocular diseases, traumatic eye injuries pose a substantial medical and socioeconomic burden. This study aimed to evaluate the clinical and radiological characteristics of traumatic open-globe injuries (OGIs) that resulted in either primary enucleation/evisceration or primary repair at a tertiary hospital in Mogadishu, Somalia. METHODS: This retrospective study was conducted at Mogadishu Somali

Pyoderma gangrenosum of the orbit.

Pyoderma gangrenosum is an uncommon chronic ulcerative condition, the aetiology of which is poorly understood. Ophthalmic involvement is rare. The case presented in this paper involves destruction of the orbital contents with subsequent perforation of the eye despite conventional therapy. Evisceration was performed with the patient being subjected to hyperbaric oxygen therapy pre- and post-operatively, with resultant cessation of the disease process.

Biomaterials in ophthalmic plastic and reconstructive surgery.

Over the past several years, new biomaterials have been developed or modified for use in ophthalmic plastic and reconstructive surgery. Hydroxyapatite and high-density porous polyethylene are extensively utilized in enucleation and evisceration surgery as well as in orbit reconstruction. Absorbable plates and screws as well as cyanoacrylate glue are at the forefront of fracture repair. Silicone and polytetrafluoroethylene eyelid implants are important adjuncts for adult and congenital ptosis repair. As technology advances, new materials will be developed and new applications identified. An und

Comparison of the accuracy between guided and freehand placement of periorbital implants-a cadaveric split-face study.

BACKGROUND: Orbital exenteration (exenteratio orbitae) is a disfiguring procedure performed with tumor resection. Rapid realization of an implant-retained craniofacial prosthesis with a secure fit for an active life is essential to restore quality of life. Osseointegrated implants are commonly used for maxillofacial rehabilitation. The precise positioning of these implants is more difficult in cases of reduced bone availability, but it enables anaplastologists to achieve an unobtrusive restoration. METHODS: After computed tomography (CT) scans of 13 cadaver heads, 104 craniomaxillofacial (CMF

Orbital and paranasal sinuses changes after neonatal orbital exenteration for massive teratoma management.

A neonate at 13 days of age underwent a lid and conjunctiva-sparing orbital exenteration for a massive right orbital teratoma. To reduce the degree of orbital contraction, the entire temporalis muscle was rotated into the orbit along with a dermis-fat graft. Sequential postoperative orbital imaging showed that hyperostosis developed in the orbital apex at the age of two months. Despite a significant expansion of the fat graft, by the age of 4 years, hyperostosis had progressed to the anterior portion of the orbit associated with over-pneumatization of the paranasal sinuses. This case demonstra

Orbital administration of fillers for optimization of symmetry in patients with microphthalmos or acquired anophthalmic sockets: A review.

The treatment of facial asymmetry in patients with microphthalmos or acquired anophthalmic sockets usually requires aggressive reconstructive surgeries. In recent years, studies have been published on the use of fillers to optimize orbital tissue symmetry, as minimally invasive techniques. For this reason, we performed a systematic review of the literature published to date on the use of fillers for the treatment of volume loss in acquired anophthalmic or microphthalmic cavities. Fourteen articles were reviewed in which the material used, the injection technique, the anatomical study of the pa

[Evisceration, enucleation and exenteration-Indications, techniques, and postoperative care].

Various surgical techniques are available for the extirpation of an eye, depending on the underlying disease. Typical indications encompass tumors, inflammatory and infectious processes, glaucoma, trauma as well as congenital malformations. Surgical procedures include evisceration (resection of the intraocular space with preservation of the sclera), enucleation (extirpation of the entire eyeball including sclera and cornea) and exenteration (resection of the complete orbital soft tissue). The early detection of postoperatively manifesting complications, such as implant extrusion, conjunctiviti

[Conjunctival melanoma and xeroderma pigmentosum, a rare association].

Ocular involvement in the xeroderma pigmentosum (XP) can be located on the eyelids, conjunctiva or cornea. Malignant melanoma of the conjunctiva is a rare tumor, and even more so in children. Only five cases associated with XP have been published since 1960. We report a sixth case occurring in a girl aged 14. Surgical treatment consisted of an orbital exenteration extended to the eyelids and the evolution is marked by a good healing of the orbital cavity without local recurrence after four months.

Arrow injuries to the eye.

Two male siblings aged 10 and 17 years, respectively, presented to our hospital with two days history of being shot in the right eyes with arrows. The patients presented with severe pains, bleeding, swelling, discharge and loss of vision. There was positive history of application of traditional eye medicine and an unsuccessful attempt was made to remove the arrows. Both patients presented with panophthalmitis. X-rays of the orbits and para nasal sinuses indicated the tip of the arrow was lodged to the apex of the orbit in the younger patient, and the arrow vertically traverses the eye/ orbit a

Klebsiella pneumoniae endogenous endophthalmitis presenting as orbital cellulitis.

This report describes the first documented case of Klebsiella pneumoniae endogenous endophthalmitis with concurrent orbital cellulitis in a patient without a liver abscess. A 34-year-old, caucasian male with a history of intravenous drug abuse and type 1 diabetes mellitus was transferred from an outside hospital for progressively worsening pain and swelling in the right eye. Careful history, physical examination, and imaging were consistent with a diagnosis of endogenous endophthalmitis with concurrent orbital cellulitis. Vitreous cultures were positive for Klebsiella pneumoniae. Despite aggre

A case of delayed oculocardiac reflex induced by an intraorbital foreign body.

A 56-year-old male presented with a history of left periorbital pain, nausea, vomiting, dyspnea, sweating, and pallor. His left eye was injured by a bomb explosion approximately 40 years ago. Evisceration was performed. There were no cardiopulmonary and gastroenterological findings except mild bradycardia (54/min). CT evaluation revealed a 4-cm-sized, sharp, metallic, intraorbital foreign body in the left orbit. After the removal of the foreign body, the symptoms completely resolved with a normal heart rate (69/min). The authors present a case of delayed oculocardiac reflex induced by an old i
