Cryptococcus gattii infection in solid organ transplant recipients: description of Oregon outbreak cases.

Cryptococcus gattii was recognized as an emerging infection in the Pacific Northwest in 2004. Out of 62 total infections in Oregon since the outbreak, 11 were in solid organ transplant (SOT) recipients. SOT recipients were more likely to have disseminated disease and higher mortality than normal hosts, who mostly had isolated mass lesions. The median time from transplantation to C. gattii diagnosis was 17.8 months. The primary sites of infection were lung (n = 4), central nervous system (n = 3), or both (n = 4). The Oregon-endemic strain, VGII (subtypes IIa and IIc) was present in 10 of 11 pat

Ocular plague (Yersinia pestis) in mule deer (Odocoileus hemionus) from Wyoming and Oregon.

Although plague is relatively rare in wild ungulates, this report describes ocular lesions associated with Yersinia pestis infection in three free-ranging mule deer (Odocoileus hemionus) from Wyoming and Oregon, USA. All deer were observed antemortem and seemed to be blind. Post-mortem examination revealed gross lesions of bilateral keratoconjunctivitis and/or panophthalmitis in the first two deer, but only partial retinal detachment in the third deer. Microscopically, all deer had moderate-to-severe necrotizing and fibrinopurulent endophthalmitis and varying degrees of keratoconjunctivitis wi

A new species of glossiphoniid leech from Rana pretiosa (Amphibia: Ranidae) in Oregon.

A new species of ectoparasitic glossiphoniid leech was found feeding on frogs in the Nature Center Pond and elsewhere in Deschutes County, Oregon. The new species of Placobdella resembles the southern alligator leech, Placobdella multilineata Moore, 1953, notwithstanding their vast geographic separation in North America. The new species is readily distinguished by possessing subdivided annuli, by its papillation and pigmentation patterns as well as by the arrangement of ovarian tissues. There is strong evidence of nocturnality and of the potential for parasitizing humans.

The nematode Anisakis simplex in American shad (Alosa sapidissima) in two Oregon rivers.

This paper represents the first report of the nematode Anisakis simplex in the American shad (Alosa sapidissima) in its introduced range in the American Pacific Northwest. All the adult shad sampled from spawning populations in the Willamette (n = 9) and Umpqua (n = 12) rivers were infected with A. simplex with intensities ranging from 6 to 89 worms per fish. This preliminary investigation contrasts sharply with previous studies in the native range of American shad and confirms that this fish may be an important intermediate host for A. simplex in the Pacific Northwest. It is suggested that th

Legionnaires' Disease associated with potting soil--California, Oregon, and Washington, May-June 2000.

Since Legionnaires' Disease (LD) was first reported in 1976, outbreaks have been associated with airborne transmission of Legionella bacteria through cooling towers, showers, and other aerosolizing devices (1). However, most LD cases are sporadic, and the source and mode of infection in many cases are unknown. Infections with one species, Legionella longbeachae, have been associated with gardening and use of potting soil in Australia and Japan (2,3). This report summarizes the findings of LD investigations in California, Oregon, and Washington, that suggest that transmission from potting soil

Chronic Opioid Use After Implementation of Oregon's Medicaid Back Pain Policy.

INTRODUCTION: In 2016, Oregon developed an innovative policy to improve care for Medicaid patients with back pain. The objective of this study was to identify the factors associated with dose reduction and discontinuation among Medicaid patients using chronic opioid therapy after implementation of this policy. METHODS: Using Medicaid administrative claims data, this was a retrospective cohort of patients on chronic stable opioid therapy between July and December 2016. Outcomes assessed were (1) 30% reduction in opioid dose and (2) an absolute discontinuation between January and December 2017.

Mercury levels and relationships in water, sediment, and fish tissue in the Willamette Basin, Oregon.

In Oregon's Willamette River Basin (the Basin), health advisories currently limit consumption of fish that have accumulated methylmercury (MeHg) to levels posing a significant human health risk. These advisories created the requirement for a mercury total maximum daily load for the Basin, which required a greater understanding of the behavior, distribution, and levels of mercury and MeHg in the Basin. In 2002, the Oregon Department of Environmental Quality initiated a study to measure (using ultraclean techniques) mercury and MeHg levels in water, sediment, and fish samples collected throughou

Timing and sequence of eruption of permanent teeth in a longitudinal sample of children from Oregon.

A longitudinal sample of children was examined for timing and sequence of eruption of permanent teeth. Separate consideration was given to 124 boys and 163 girls from the Child Study Clinic longitudinal growth study. Girls show more variability in age at eruption than boys, and eruption is generally earlier in girls. The degree of variation in sequences of eruption of the first seven permanent teeth is distinct. The most common sequence in girls' maxillas occurred in only 11.4% of subjects; in boys, the most common sequence appeared in 13.4% of subjects. Predictability efficiency of eruption o

Epidemiological Study of Cryptococcus gattii Complex Infection in Domestic and Wild Animals in Oregon.

The members of the Cryptococcus gattii species complex are the etiologic agents of potentially fatal human infection. C. gattii causes disease in both immunocompetent and immunocompromised hosts. In the early 2000s, infection caused by C. gattii emerged in the Pacific Northwest of the US. While many studies have been published about the human infection, the epidemiological characteristics of the infection in animals, with a possible role in human infection, have not been in investigated. Cases of C. gattii diagnosed in animals in Oregon from 2008 to 2019 were cataloged by county, species of an

An Overview of Pathogens Associated with Biotic Stresses in Hemp Crops in Oregon, 2019 to 2020.

Hemp (Cannabis sativa) acreage in Oregon has increased by approximately 240 times in the last 5 years, and a greater number of hemp diseases have been observed. This special report documents pathogens, particularly those causing virus and virus-like diseases, that have been detected from field and greenhouse-grown hemp crops in Oregon, based on plant samples submitted to the Hermiston Agricultural Research and Extension Center Plant Clinic of Oregon State University in 2019 and 2020. Symptoms and signs were used to evaluate disease types and determine diagnostic assays used on each submission.

Automating Case Reporting of Chlamydia and Gonorrhea to Public Health Authorities in Oregon Clinics.

BACKGROUND: Optimizing sexually transmitted disease (STD) reporting to state public health authorities is important to reduce incidence and manage outbreaks of STDs. Electronic laboratory reporting (ELR) is the standard through which local clinics report STDs to state public health authority. Electronic case reporting (eCR) is an alternative approach which automates transmission of case reports to public health jurisdictions using electronic health record (EHR) data. METHODS: Working with 3 community health centers in Oregon between February 3, 2020 and May 15, 2020, we piloted an automated e

Investigating the prevalence of three medically important pathogens in Ixodes pacificus from southern Oregon.

INTRODUCTION: In the far western United States of America, Ixodes pacificus is the primary vector of several pathogens of public health and veterinary importance including the Lyme disease spirochete Borrelia burgdorferi sensu lato (s.l.), as well as Borrelia miyamotoi and Anaplasma phagocytophilum. Ixodes pacificus is common in southern Oregon yet there are few published studies on the distribution of tick-borne pathogens in this region. METHODS: Using real-time quantitative PCR, we assessed the prevalence of B. burgdorferi s.l., B. miyamotoi, and A. phagocytophilum among 2,463 unfed I. paci

The Oregon Child Absenteeism Due to Respiratory Disease Study (ORCHARDS): Rationale, objectives, and design.

BACKGROUND: Influenza viruses pose significant disease burdens through seasonal outbreaks and unpredictable pandemics. Existing surveillance programs rely heavily on reporting of medically attended influenza (MAI). Continuously monitoring cause-specific school absenteeism may identify local acceleration of seasonal influenza activity. The Oregon Child Absenteeism Due to Respiratory Disease Study (ORCHARDS; Oregon, WI) implements daily school-based monitoring of influenza-like illness-specific student absenteeism (a-ILI) in kindergarten through Grade 12 schools and assesses this approach for ea
