American College of Gastroenterology guideline: management of acute pancreatitis.

This guideline presents recommendations for the management of patients with acute pancreatitis (AP). During the past decade, there have been new understandings and developments in the diagnosis, etiology, and early and late management of the disease. As the diagnosis of AP is most often established by clinical symptoms and laboratory testing, contrast-enhanced computed tomography (CECT) and/or magnetic resonance imaging (MRI) of the pancreas should be reserved for patients in whom the diagnosis is unclear or who fail to improve clinically. Hemodynamic status should be assessed immediately upon

[Sepsis with acral necrosis in a patient with Sharp syndrome - case 8/2010].

HISTORY AND ADMISSION FINDINGS: We report on a patient with Sharp-Syndrome who was referred to our emergency department with sepsis. In addition, the patient showed acral necrosis, particularly of the distal phalanges of the hands and of the tip of the nose. INVESTIGATIONS: Laboratory analyses showed an elevation of the inflammatory parameters (C-reactive protein elevation, leukocytosis). Furthermore, procalcitonin and the D-dimers were increased, antithrombin III, however, was decreased. The thoracic/abdominal computed tomography (CT) showed bilateral inferior lobe pneumonia with concomitant

The Link between Alcohol Consumption and Kidney Injury.

Alcohol use disorder contributes to systemic organ dysfunction, but its direct effect on kidney health is unclear. Epidemiologic studies have shown inconsistent findings due to a reliance on conventional markers, such as serum creatinine and blood urea nitrogen, which are insensitive to early chronic kidney disease and influenced by factors such as muscle mass, diet, and hydration status. Experimental studies have indicated that alcohol may directly exacerbate renal damage through mitochondrial dysfunction, oxidative stress, and inflammation. Furthermore, indirect effects from alcohol-induced

Osmoreceptor mediation of thirst and vasopressin secretion in the dog.

Water deprivation leads to depletion of both the intracellular and extracellular compartments. The resulting cellular dehydration and extracellular dehydration stimulate thirst and the secretion of vasopressin. The cellular stimulus is thought to arise from a change in volume and hence in the hydration status of specific osmoreceptor cells located in the anterior hypothalamus. Thus, reduction in cell volume because of either water deprivation or administration of hyperosmolar solutions of solutes that cannot penetrate cell membranes would stimulate thirst and vasopressin secretion, whereas an

Lung Ultrasound B-lines Occurrence in Relation to Left Ventricular Function and Hydration Status in Hemodialysis Patients.

BACKGROUND AND OBJECTIVE: Reliable assessment of the fluid status in hemodialysis (HD) patients is often difficult. A lung ultrasound with an assessment of the B-lines ("lung comets" (LCs)) number is a novel hydration status measure. However, the occurrence of left ventricular dysfunction may have a significant effect on pulmonary congestion and further modulate the LC number. The aim of this study was to analyze to what extent left ventricular dysfunction, pulmonary hypertension, and hypervolemia affect the occurrence of LC in a cohort of prevalent HD patients. MATERIAL AND METHODS: This cro

Extract from Aronia melanocarpa, Lonicera caerulea, and Vaccinium myrtillus Improves near Visual Acuity in People with Presbyopia.

Presbyopia is a global problem with an estimated 1.3 billion patients worldwide. In the area of functional food applications, dietary supplements or herbs, there are very few reports describing the positive effects of their use. In the available literature, there is a lack of studies in humans as well as on an animal model of extracts containing, simultaneously, compounds from the polyphenol group (in particular, anthocyanins) and iridoids, so we undertook a study of the effects of a preparation composed of these compounds on a condition of the organ of vision. Concise Review: The Potential Use of Intestinal Stem Cells to Treat Patients with Intestinal Failure.

Intestinal failure is a rare life-threatening condition that results in the inability to maintain normal growth and hydration status by enteral nutrition alone. Although parenteral nutrition and whole organ allogeneic transplantation have improved the survival of these patients, current therapies are associated with a high risk for morbidity and mortality. Development of methods to propagate adult human intestinal stem cells (ISCs) and pluripotent stem cells raises the possibility of using stem cell-based therapy for patients with monogenic and polygenic forms of intestinal failure. Organoids

The role of bioimpedance and biomarkers in helping to aid clinical decision-making of volume assessments in dialysis patients.

Bioimpedance analysis (BIA) derives two main pieces of information--total tissue fluid content, which when referring to the whole patient is equivalent to the total body water (TBW), and cell mass, which in the limbs mainly reflects muscle. The relationship between these measures, expressed in different ways, is abnormal in dialysis patients due to muscle wasting combined with tissue overhydration. In both dialysis modalities this is associated with aging, comorbidity, and inflammation, and there is a conflict between achieving euvolemia to improve blood pressure control and prevent left ventr

What we know and don't know about the development of independent ingestion in rats.

Because suckling behavior differs in many ways from later ingestive behavior, the development of feeding and drinking in rats is best studied apart from the normal suckling situation. Newborn rat pups, separated from their mothers, will actively ingest diet infused into their mouths or spread on the floor beneath them. Such "independent" ingestion resembles the ingestive behavior of adult animals, but it also undergoes developmental changes in organization and control during the pre- and post-weaning periods: When young, deprived pups are fed, they show generalized, non-directed behavioral exc

Plasma kallikrein activates the epithelial sodium channel in vitro but is not essential for volume retention in nephrotic mice.

AIM: Recent work has demonstrated that activation of the epithelial sodium channel (ENaC) by aberrantly filtered serine proteases causes sodium retention in nephrotic syndrome. The aim of this study was to elucidate a potential role of plasma kallikrein (PKLK) as a candidate serine protease in this context. METHODS: We analysed PKLK in the urine of patients with chronic kidney disease (CKD, n = 171) and investigated its ability to activate human ENaC expressed in Xenopus laevis oocytes. Moreover, we studied sodium retention in PKLK-deficient mice (klkb1-/- ) with experimental nephrotic syndro

Oral Rehydration Salt Solutions for Children: A Review.

Dehydration continues to be a major contributor to morbidity and mortality in children globally. Oral rehydration salts (ORS) solutions can successfully treat mild to moderate dehydration. However, the uptake of this simple and cost-effective intervention remains low in both high-income and low-to-mid-income countries. The low-osmolality ORS solution recommended by the World Health Organization (WHO) contains an appropriate balance of electrolytes and glucose that helps effectively absorb water and electrolytes, irrespective of the cause of gastroenteritis. ORS solution can be administered ora

Review article: Management of hyperemesis gravidarum and nausea and vomiting in pregnancy.

Nausea and vomiting in pregnancy (NVP) are common in early pregnancy but there is a wide spectrum of severity in terms of the duration and acuity of symptoms throughout gestation. Adverse maternal and fetal outcomes have been seen in women who experience severe symptoms, also known as hyperemesis gravidarum (HG). Evidence-based, assessment and management can reduce symptom severity, avoid physical and psychological deterioration and minimise the impact on quality of life and function. A pathway for assessment and management of NVP and HG in the emergency room is presented based on the Society

Diagnosis and management of fluid overload in heart failure and cardio-renal syndrome: the "5B" approach.

Cardio-Renal syndrome may occur as a result of either primarily renal or cardiac dysfunction. This complex interaction requires a tailored approach to manage the underlying pathophysiology while optimizing the patient's symptoms and thus providing the best outcomes. Patients often are admitted to the hospital for signs and symptoms of congestion and fluid overload is the most frequent cause of subsequent re-admission. Fluid management is of paramount importance in the strategy of treatment for heart failure patients. Adequate fluid status should be obtained but a target value should be set acc

Dynamic and distinct physiological responses by a soil bacterium promote survival along a desiccation continuum.

Soil bacteria play a central role in global biogeochemical cycles and are critical for soil health and agricultural productivity. The dynamic nature of soil hydration status affects bacterial habitats by changing the energy state of soil water and disrupting aqueous connections critical for nutrient diffusion. To study how soil bacteria respond to desiccation, we used the rhizobacterium Pseudomonas synxantha 2-79 as a model organism and quantified its response to co-occurring water and nutrient limitations at the single-cell level. We hypothesized that the relative importance of osmolyte synth

Hemoconcentration: a major risk factor for neurological involvement in hemolytic uremic syndrome.

BACKGROUND: Shigatoxin-associated hemolytic uremic syndrome (STEC-HUS) is a common thrombotic microangiopathy (TMA) in which central nervous system (CNS) involvement is responsible for the majority of deaths and for severe long-term sequelae. We have analyzed the role of hemoconcentration in disease severity. METHODS: This was a retrospective review of the records and laboratory data at presentation of all patients with STEC-HUS cases (n = 61) over a 10-year period. The patients were grouped into three severity classes: group A, comprising patients who did not require dialysis; group B, patie

Rhabdomyolysis and Acute Kidney Injury: Exploring the Potential Causes in a Hospitalized Patient.

Rhabdomyolysis refers to the breakdown of skeletal muscle with subsequent release of intracellular contents into blood, causing elevated creatine kinase (CK) and renal damage. The four main etiological categories are traumatic, non-traumatic, non-traumatic exertional, and non-traumatic non-exertional. We report a case of a young female who presented with rhabdomyolysis due to agitation, increased motor activity, and venlafaxine use, resulting in severe organ dysfunction. A 30-year-old female on venlafaxine, pregabalin, and fentanyl patch presented to our hospital with a sudden onset of general

A double-blind placebo-controlled trial of azithromycin to reduce mortality and improve growth in high-risk young children with non-bloody diarrhoea in low resource settings: the Antibiotics for Children with Diarrhoea (ABCD) trial protocol.

BACKGROUND: Acute diarrhoea is a common cause of illness and death among children in low- to middle-income settings. World Health Organization guidelines for the clinical management of acute watery diarrhoea in children focus on oral rehydration, supplemental zinc and feeding advice. Routine use of antibiotics is not recommended except when diarrhoea is bloody or cholera is suspected. Young children who are undernourished or have a dehydrating diarrhoea are more susceptible to death at 90 days after onset of diarrhoea. Given the mortality risk associated with diarrhoea in children with malnutr

Prolonged requirement for ventilatory support in a patient with Eskalith overdose.

Lithium (Eskalith) is commonly used in the treatment of depressive and bipolar affective disorders, in a population at relatively high risk for overdose. Lithium may help correct a chemical imbalance in the brain; however, it has a comparatively narrow therapeutic index. Thus, lithium intoxication is a frequent complication of chronic lithium therapy. The central nervous system (CNS) is the major organ system affected, although the renal, gastrointestinal (GI), endocrine, and cardiovascular (CV) systems also may be involved. Here we present a forty-two-year-old Caucasian female with altered me

Pathophysiological changes of the gastrointestinal tract in ischemic stroke.

OBJECTIVE: Dysphagia is common after stroke and represents a marker of poor prognosis. After ischemic stroke, dysphagia represents only one part of the clinical spectrum of changes in the gastrointestinal (GI) tract and includes GI hemorrhage, delayed GI emptying, and colorectal dysfunction. State-of-the-art imaging techniques have started to revolutionize to study the cortical and brainstem control of these GI symptoms. It has become increasingly obvious that GI alterations after stroke are complex and its recovery following stroke is even more so. METHODS: In this review, an electronic data
