Periodontal and restorative considerations with clear aligner treatment to establish a more favorable restorative environment.

Although standard fixed appliances are better suited to control difficult tooth movement in three planes of space, clear aligner orthodontic treatment may be requested by patients because of the superior esthetics compared with conventional fixed appliances when clear aligners can be considered. Several clinical trials have shown improved periodontal status during orthodontic treatment with clear aligners while fixed appliances have almost always been associated with increased inflammation and mild periodontal breakdown despite preventive measures performed during treatment. There have been no

Rare earth magnets in orthodontics: an overview.

Magnets have been used in dentistry for many years. They can be made to push or pull teeth. The force they deliver can be directed, and they can exert their force through mucosa and bone, as well as within the mouth. In orthodontics they are used for intrusion of teeth, tooth movement along archwires, expansion, retention, in functional appliances, and in the treatment of impacted teeth. New 'high energy' magnets are capable of producing very high forces relative to their size. Although magnets are potentially very useful there are a number of problems that severely affect their performance; t

The Lynn Maxilla Rotator Combination Appliance and Lynn Archial Face Bow.

This combination orthopedic traction appliance offers a new dimension in treating long faced, mouth breathing patients, who generally exhibit pre-maxillary gingival excess and lip incompetency or a short upper lip. The dynamics of excessive clockwise growth can be reversed if the physical pressures placed on the craniofacial skeletal tissues are reversed. In many instances extraction therapy or orthognatic surgery may be unnecessary to achieve acceptable occlusion and facial esthetics. Dr. Ullrich Teuscher, M.D., D.M.D., of Zurich, Switzerland published an article in the American Journal of Or

A comparative assessment of the BioBiteCorrector® and Herbst appliances combined with fixed appliances in class II patients: A retrospective cohort study on PAR and IOTN indices.

PURPOSE: The BioBiteCorrector® (BBC) appliance was introduced as a further development of the Herbst appliance; however, there is no research comparing their treatment outcomes. MATERIALS AND METHODS: Caucasian healthy patients with at least 5mm overjet were included and treated with either Herbst or BBC, both combined with fixed appliances, by one experienced orthodontist at a private practice in Switzerland. Treatment effectiveness was evaluated in terms of occlusal outcomes via the Peer Assessment Rating (PAR) and Index of Orthodontic Treatment Need (IOTN) indices, for which groups were ma

Orthodontic retainers: are they all the same?

INTRODUCTION: It is known that the stability of the results obtained with orthodontic treatment depends, in addition to the functional and aesthetic aspects, also on the adequate planning of the retention devices, the patient's compliance with this new phase, and the physiological changes that the human body experiences over the years, throughout the craniofacial aging process. OBJECTIVE: This article discusses the importance of the orthodontic retention phase and the influence of diagnosis, planning and execution of corrective treatment of malocclusions, in order to achieve the expected succ

[Are self-ligating brackets more efficient than conventional brackets ? A meta-analysis of randomized controlled and split-mouth trials].

The aim of this systematic review and meta-analysis was to compare self-ligating brackets (SLBs) considered as a whole to conventional brackets (CBs). An electronic search was performed in three databases (PubMed, MEDLINE via Web of Science, Cochrane Library) from their origin up to June 2017. Additional articles were hand searched from January 2006 to June 2017. This meta-analysis was restricted to randomized controlled trials (RCTs) and split mouth design studies (SMDs). No distinction was made between active and passive SLBs. The following variables were investigated : treatment duration, n

Effect of Different Bracket Prescriptions on Orthodontic Treatment Outcomes Measured by Three-dimensional Scanning.

Various types of bracket are currently available, and different prescriptions are provided for the same type of tooth. There are no definite criteria, however, on which to base bracket selection. The purpose of this study was to investigate differences in the angulation and inclination of orthodontically aligned teeth when using different prescription brackets. Thirty patients undergoing orthodontic treatment for maxillary protrusion and crowding were enrolled. After orthodontic treatment with a pre-adjusted appliance, angulation and inclination were measured on dental casts obtained from thes

Fixed orthodontics in a Class II Division 2 young adult with Brodie's occlusion and three-dimensional anomalies using a Herbst appliance and miniscrew: A case report.

In this case report, we described the successful treatment of a 17-year-old male with Class II Division 2 and a Brodie occlusion, with bilateral scissor bite, maxillomandibular transverse discrepancy, deep overbite and Class II molar relationship, using a fixed Herbst appliance. The treatment was carried out in two phases. In the first phase, the Herbst appliance was used to advance the mandible, correcting the sagittal relationship while simultaneously improving transverse and vertical discrepancies, with minimal reliance on patient compliance. In the second phase, fixed appliances combined w

Class II subdivision treatment and stability with asymmetric Herbst appliance: A case report.

Non-surgical treatment of Class II subdivision may involve complex mechanics or asymmetric tooth extraction in its resolution. This report demonstrates the result and the short-term stability of Class II subdivision treated with asymmetrically installed Herbst appliance followed by conventional fixed orthodontic appliance. The approach allowed the correction of the unilateral Class II molar relationship and increased overjet, as well as the deviation of dental midlines, with improvement in lip posture and facial profile. The results remained stable two years after treatment, confirming the tre

From Twin Blocks to Quad Blocks: A Two-Phase Approach to Class II Correction.

Twin Block, a functional orthodontic appliance, has a major impact on the dentoalveolar structures with limited skeletal effects. In specific clinical scenarios, particularly in growing patients such as class II division 1 where the mandible is retropositioned, these appliances can effectively address the malocclusions. Patient compliance plays a crucial role in the success of these appliances, which also streamline subsequent phases of fixed appliance treatment. In the current case report, a Twin Block appliance followed by a Quad Block was given to a 12-year-old boy to refine the occlusion.

Comparison of anchorage reinforcement with temporary anchorage devices or a Herbst appliance during lingual orthodontic protraction of mandibular molars without maxillary counterbalance extraction.

BACKGROUND: Orthodontic protraction of mandibular molars without maxillary counterbalance extraction in cases of aplasia or extraction requires stable anchorage. Reinforcement may be achieved by using either temporary anchorage devices (TAD) or a fixed, functional appliance. The objective was to compare the clinical effectiveness of both methods by testing the null-hypothesis of no significant difference in velocity of space closure (in mm/month) between them. In addition, we set out to describe the quality of posterior space management and treatment-related factors, such as loss of anchorage
