Factors affecting forced eruption duration of impacted and labially displaced canines.

INTRODUCTION: Patients with impacted maxillary canines typically endure long treatment time, and reliable pretreatment estimates of orthodontic treatment duration would be beneficial to patients and clinicians. The purpose of this study was to identify factors that affect the duration of forced eruption for impacted maxillary canines. METHODS: Data from treatment records and radiographic scans of 27 patients with 29 impacted and labially displaced maxillary canines (25 unilateral, 2 bilateral) were collected. The patients underwent surgical and orthodontic interventions to bring the canine in

Forced orthodontic eruption of fractured teeth before implant placement: case report.

The treatment of esthetic areas with single-tooth implants represents a new challenge for the clinician. Placement of a single-unit implant is indicated for fractured or periodontally compromised teeth. In 1993, a modification of the forced eruption technique, called "orthodontic extrusive remodelling," was proposed as a way to augment both soft- and hard-tissue profiles at potential implant sites. This case report describes augmentation of the coronal soft and hard tissues around a fractured tooth, which was achieved by forced orthodontic extrusion before implant placement. This technique may

Orthodontic considerations in restorative management of hypodontia patients with endosseous implants.

The use of implant-supported restorations in patients with hypodontia remains challenging and requires a multistage treatment that begins in late mixed dentition and continues into late adolescence. The aim of this article is to review the role of orthodontics in endosseous implant rehabilitation of patients with hypodontia. The MEDLINE, Web of Science, Scopus, Cochrane databases, and necessary scientific textbooks were searched for relevant studies and reviews, and as far as possible, they were only included if they had been cited at least once in the literature. Dental implants are susceptib

Intra-alveolar surgical uprighting of impacted teeth: a case report.

Although orthodontic repositioning of impacted teeth is widely used, the treatment has its limitations. Autotransplantation or intra-alveolar surgical repositioning of teeth is an alternative therapy that may be used in selected cases of desperate impactions, when orthodontic extrusion is unsuccessful or when orthodontic treatment is rejected by the patient. A case report is presented to stress both the indications and limitations of a modified technique of intra-alveolar surgical uprighting of impacted teeth.

Biomechanically and Periodontally-Based Orthodontic Treatment of a Patient with Upper Canine Affected by External Cervical Resorption (ECR): A Case Report.

(1) Background: Orthodontic treatment may be a potential predisposing factor for ECR. The affected tooth goes to ankylosis, which could lead to a malocclusion. Although teeth severely affected by ECR (class IV Heithersay) are usually extracted, this case report aims to present the use of an ECR class IV upper canine, both as ankylosed to solve the malocclusion and the occlusal plane canting, as well as not ankylosed to correct its ridge defect with orthodontic extrusion. (2) Methods: A 14-year-old male, complaining of an ugly smile and a failed orthodontic attempt to recover an impacted canine

Effect of three different attachment designs in the extrusive forces generated by thermoplastic aligners in the maxillary central incisor.

INTRODUCTION: Orthodontic aligners use have increased in dentistry. The resolution of complex movements such as extrusion demands the use of attachments to reach the aimed force, but just a few studies have been developed to evaluate the biomechanical performance of the aligners and their accessories. OBJECTIVE: The objective of this study was to evaluate on the three axes (X, Y and Z) the forces generated by three different attachment designs for the extrusion of the maxillary central incisor using esthetic orthodontic aligners. METHODS: Three prototypes of maxillary models were developed,

An Analysis of the Stress Induced in the Periodontal Ligament during Extrusion and Rotation Movements: A Finite Element Method Linear Study Part I.

BACKGROUND: Orthodontic tooth movement occurs due to various biomechanical changes in the periodontium. Forces within the optimal range yield maximum tooth movement with minimum deleterious effects. Among various types of tooth movements, extrusion and rotational movements are seen to be associated with the least amount of root resorption and have not been studied in detail. Therefore in this study, the stress patterns in the periodontal ligament (PDL) were evaluated with extrusion and rotational movements using the finite element method FEM. MATERIALS AND METHODS: A three-dimensional (3D) FE

Open surgical approach to a maxillary impacted canine: a case report.

Palatally impacted permanent maxillary canines are a relatively common dental anomaly mandating surgical exposure before moving them orthodontically into correct alignment. The open technique for exposing these palatal canines involves their orthodontic movement into its correct position above the palatal mucosa in contrast to the closed approach in which canine is moved beneath the palatal mucosa. This case report describes the orthodontic eruption of a palatally impacted canine in a 22-year-old female using open surgical approach.

Palatally impacted canines: the double traction technique.

When closed eruption is used to recover palatally impacted canines, patients risk a second surgical operation because the traction attachment could come off or the wire ligature, which is connected to the attachment, could break. In order to reduce this risk, a new simple technique has been developed. Instead of the classic single one, two attachments are bonded to the impacted canine. During the orthodontic finishing, the technique includes a mandibular premolar bracket bonded to the ectopic canine. This device should produce a more negative torque (Torque -17 degrees rather than -7 degrees)

A conservative multidisciplinary approach for improved aesthetic results with traumatised anterior teeth.

A subgingival crown-root fracture presents a restorative problem to the clinician because restoration is complicated by the need to maintain the health of the periodontal tissues. If the remaining portion of the root is thought to be enough to support a definitive restoration, the root may be extruded by orthodontic forced eruption after root canal treatment. Extrusion enables the remaining root portion to be elevated above the epithelial attachment. Endodontic posts may be useful in exerting vertical forces to the root for extrusion without buccal tipping. The following case shows multidiscip

Esthetic enhancement of a traumatized anterior tooth with a combination of forced eruption and tooth alignment: a case report.

Exposing sound structure of a subgingivally fractured tooth using orthodontic extrusion is considered to be a conservative way to re-establish biologic width without sacrificing esthetics or jeopardizing periodontal support of neighboring teeth. When a misaligned tooth is traumatically involved, a more comprehensive approach combining tooth extrusion and re-alignment may be necessary for a successful restorative outcome. This case report describes a successful esthetic management of a patient with complicated crown-root fracture on the maxillary right central incisor and pre-existing malocclus

Immediate Orthodontic Repositioning of Traumatically Extruded Incisors in a Patient With Cleft Lip and Alveolus: A Case Report.

This case report describes a 15-year-old boy born with a cleft lip and alveolus who sustained severe extrusive and lateral luxation trauma to multiple incisors during the final phase of orthodontic treatment. The displacement caused incisal interference and prevented normal jaw closure. Deformed beta titanium wires were removed, loosened brackets were rebonded, and immediate orthodontic repositioning was initiated using light forces with 0.014-inch CuNiTi round wires combined with manual repositioning. This approach resolved the incisal interference and restored functional occlusion within 1 w

Guiding unerupted teeth into occlusion: case report.

Delayed eruption of a permanent tooth may be a significant concern for children in the mixed dentition stage and their parents. This report describes the orthodontic management of a case in which an unerupted tooth was guided into occlusion during the mixed dentition stage, rather than waiting until the permanent dentition stage to perform comprehensive treatment. The article includes a description of the treatment mechanics for this process, which can be readily accomplished by general dentists.

How to...mechanically erupt a palatal canine.

Management of ectopic permanent maxillary canines represents one of the greatest challenges to orthodontists. This paper outlines a variety of techniques and mechanics which may facilitate expedient, predictable and safe eruption of palatal canines. While each method may be useful in isolation, the varying presentations of palatal canines ensure that the ability to apply an array of techniques is essential if successful outcomes are to be consistently achieved.

Forced eruption of a solitary nonrestorable tooth using mini-implants as anchorage: rationale and technique.

PURPOSE: The need to place a restoration's margins on sound tooth material may require crown lengthening or forced eruption. The latter intervention may benefit from a severance of the tooth's circumferential fibers during the orthodontic process in an effort to stabilize the bone level. Orthodontic appliance design for the extrusive action, especially in the absence of neighboring teeth, may require the use of mini-implants, which are a useful adjunct in such situations. MATERIALS AND METHODS: This case history describes the management of a maxillary left canine that lacked sound tooth mater

Restoring the unrestorable! Developing coronal tooth tissue with a minimally invasive surgical extrusion technique.

Surgical extrusion is a recognised treatment option for teeth that have insufficient coronal tooth structure remaining due to deep caries, resorption or traumatic injury. However, the technique has not been widely adopted, arguably because extraction of a severely compromised tooth may be difficult to achieve in a gentle and predictable way. In this paper, we present our novel approach to surgical extrusion and subsequent management of teeth using a vertical extraction system (Benex), which has become the method of choice in the authors' practice for many teeth that would otherwise be deemed u
