OBJECTIVE: The use of virtual surgical planning (VSP) in orthognathic surgery is increasing. The aim of this study was to investigate which type of dentofacial deformity makes the surgeon choose VSP and if VSP influences the choice of surgical techniques. Furthermore, we sought to evaluate if use of VSP affects surgical time and blood loss during surgery.
MATERIAL AND METHODS: Patients having undergone orthognathic surgery and registered in the Swedish quality registry for orthognathic surgery (NROK) between 2018 and 2019 were eligible for inclusion in this study. An evaluation of the usage o

INTRODUCTION: More than 15 years of experience in orthodontic-surgical collaboration has allowed the authors to identify some situations in which a new perspective is needed. Although it may seem easy to refer a patient to a maxillo-facial surgeon in cases of major dysmorphoses, this can lead to yet other dilemmas: a loss of results at the end of a developmental growth stage, an adult requesting a return to treatment after a camouflage orthodontic treatment or a non-cooperative child in an interceptive and preventive treatment phase. Then, a comprehensive process of reassessment becomes compul

Orthognathic surgery is a well-recognized method to correct dentofacial deformities. The main goal of orthognathic surgery is to improve soft tissue change. Soft tissue changes to the nose have been well documented. Simultaneous rhinoplasty during orthognathic surgery can be performed to correct existing inherent nasal deformities and also the unfavorable changes that arose from the maxillary surgery. Challenges for concurrent nasal surgery with jaw surgery include preoperative, perioperative, and postoperative which can be overcome with meticulous planning and experience. In complex cases, rh

A previously healthy 26-year-old man presented for elective Le Fort I osteotomy, bilateral mandibular osteotomy, and genioplasty. During the intraoperative course, the patient developed sudden bradycardia and asystole on placement of a bite block. Asystole is an extremely rare consequence of the trigeminocardiac reflex, and awareness of this potentially life-threatening complication is pivotal in its management.

Well-controlled clinical trials supporting orthognathic surgery as the primary management for temporomandibular disorders (TMDs) are lacking. Most published studies lack an adequate experimental design to minimize biases. Studies that did minimize some biases do support an overall reduction in the frequency of TMD signs and symptoms in some Class III and Class II patients who had orthognathic surgery. However, Class II correction with counter-clockwise rotation of the mandible increased TMD. Individual variability precludes the ability to predict TMD outcome after surgery. Irreversible therapi

The aim of this study was to report the orthodontic-surgical approach of a 21-year-old female patient diagnosed with cleidocranial dysplasia. An orthognathic surgery was performed in the maxilla and mandible during the same procedure to correct an existing dentofacial deformity (class III malocclusion). In addition, malar prostheses were used to correct midface deficiency. After surgical intervention, orthodontic treatment continued in order to promote stability, function, and aesthetics. Cases of cleidocranial dysplasia treated with the defined criteria can bring aesthetic and functional bene

PURPOSE: The aim of this study was to evaluate the impact of orthognathic surgical correction of facial asymmetry and maxillary hypoplasia on the magnitude and pattern of facial expressions.
PATIENTS AND METHODS: This study was carried out on 2 cohorts of patients: in group 1, 10 patients had surgical correction of facial asymmetry; in group 2, 13 patients had Le Fort I osteotomy to correct maxillary hypoplasia. The patients were asked to perform 3 facial expressions (maximal smile, lip purse, and cheek puff) that were recorded using the Di4D image-capture system before and after surgery. The

The role of orthognathic surgery for the correction of dentofacial deformities is widely accepted. However, its role in the treatment of temporomandibular joint disorders (TMD) is quite controversial. TMD symptoms should be treated independently of dentofacial deformities. Understanding that TMD can potentially worsen following orthognathic surgery, it is important to modify the surgical treatment plan to minimize the risk of exacerbation of TMJ pain, dysfunction, and condylar resorption.

OBJECTIVE: To evaluate the effect of orthognathic surgery in the treatment of condylar osteochondroma.
METHODS: A total of 12 cases of condylar osteochondroma were treated with Le Fort I osteotomy, intraoral vertical ramus osteotomy, sagittal split ramus osteotomy and genioplasty.
RESULTS: No recurrence of condylar osteochondroma was observed in all 12 cases two years after the treatments. Facial asymmetry was obviously corrected.
CONCLUSION: The orthognathic surgery methods are advantageous in improving facial figure without scar development in the treatment of condylar osteochondroma.

INTRODUCTION: Osteopetrosis is a metabolic bone disease that leads to bone malformation and bone healing disorders. We report the case of a patient who underwent orthognathic surgery to treat a class III skeletal malocclusion.
OBSERVATION: A 21-year-old patient had presented with osteopetrosis in his childhood, which had been treated by bone marrow transplantation. He presented with esthetic and functional sequels, a class III skeletal malocclusion, lateral mandibular deviation, and dental agenesis. This dysmorphism was corrected by bimaxillar osteotomy. The postoperative outcome was uneventf

Modern orthognathic surgery is said to be clean contaminated due to the intraoral means of access. Complications after orthognathic surgery, a common operation, occur about 10% of the time. Actinomycosis, a rare specific infection, plays a negligible role. Diagnostically, it should be differentiated from other infections that occur a long time after the operation. Three cases of actinomycosis that occurred after orthognathic surgery were observed, and therapeutic measurements are described case by case.

The field of orthognathic surgery offers ever-improving technology to better rehabilitate patients with dentofacial deformities. Hospital stays have been reduced due to improved surgery and anesthesia. Rigid internal fixation has increased comfort for many patients by eliminating the inconvenience of having the jaws wired together. Most important has been the realization that teamwork between the general dentist and the various specialty disciplines is indispensable for good patient care and the attainment of the very best results.

In patients with orthognathic problems, in whom a combination of surgery and orthodontics is required, it seems to us better to defer orthodontic treatment until after surgery has been completed and a possible relapse tendency has petered out. This conclusion is reached on the basis of arch instability and the incidence of root resorption as reported in the orthodontic literature. The soundness of "permanent retention" after orthodontic therapy is questioned. The author's concepts are illustrated with a few case reports.

This study aimed to analyse the spectrum of unsatisfactory outcomes requiring secondary surgery, the technical challenges involved, and the results achieved in a cohort of patients undergoing reoperative orthognathic surgery. A retrospective, single-centre study was conducted on all patients who underwent secondary orthognathic surgery at the Face Surgery Center (Parma, Italy) between January 2014 and June 2024. All patients had previously undergone orthognathic procedures by other surgeons abroad and were dissatisfied with the aesthetic outcomes. The final sample included 72 patients (50 wome

BACKGROUND: Peripherally induced movement disorders are hyperkinetic conditions triggered by peripheral trauma or surgical intervention. Oromandibular dystonia (OMD), a focal dystonia affecting the masticatory and lingual muscles, is the most common peripherally induced movement disorder within the stomatognathic system. However, OMD following orthognathic surgery has rarely been reported.
OBJECTIVE: This study aimed to describe the clinical characteristics, latency, and treatment outcomes of patients who developed OMD after orthognathic surgery in a retrospective single-center case series of

Orthognathic surgery has long been successfully utilized to correct dentofacial deformities to help facilitate improved oral function and facial esthetics. The treatment, however, has been associated with a high degree of complexity and severe postoperative morbidity. More recently, minimally invasive orthognathic surgical procedures have emerged that offer potential long-term benefits such as less morbidity, a lower inflammation response, improved postoperative comfort, and better esthetic outcomes. This article examines the concept of minimally invasive orthognathic surgery (MIOS), discussin

BACKGROUND: The standard procedure for managing skeletal class III malocclusion is maxillary advancement with mandibular setback. Occlusal plane-altering orthognathic surgery, such as jaw rotation, is useful as well. Although clockwise jaw rotation is a common procedure, its mechanism has not been well investigated. With this study, the authors aim to introduce the gearing effect to correct class III malocclusion in Asians by maxillary posterior impaction using clockwise rotation without advancing the maxilla.
METHODS: Patients with class III correction with clockwise rotation of the maxillom

Surgical ciliated cysts, also known as implantation cysts or postoperative maxillary cysts, are uncommon cystic formations predominantly arising in the posterior maxillary region after radical maxillary sinus surgery. Herein, we present the case of a 28-year-old male patient who had previously undergone orthognathic surgery and later experienced a minor car accident without major consequences. Approximately four years after surgery, the patient sought dental care due to the presence of a painless swelling in the maxillary region. Upon intraoral examination, the swelling extended from the area

Conventional model surgery planning for bimaxillary orthognathic surgery can be laborious, time-consuming and may contain potential errors; hence three-dimensional (3D) virtual orthognathic planning has been proven to be an efficient, reliable, and cost-effective alternative. In this report, the 3D planning is described for a patient presenting with a Class III incisor relationship on a Skeletal III base with pan facial asymmetry complicated by reverse overjet and anterior open bite. A combined scan data of direct cone beam computer tomography and indirect dental scan were used in the planning
