Adolescent idiopathic scoliosis: is the search for aetiology constrained by the orthosis?

While the aetiology of adolescent idiopathic scoliosis is unknown, it is widely believed that the deformity can be controlled by mechanical forces applied by a corset and acting, via skin and soft tissues, to exert pressure on vertebral growth-plates (Hueter-Volkmann Law). The reported efficacy of bracing in reducing the incidence of small degrees of progression reinforces this concept. The supposed mode of action conditioned the model of pathomechanism and precludes other hypotheses. Since the critical Cobb angle changes are usually less than the probable measurement error, another criterion

Challenges in Offloading Lesser Toe Deformities: A Case Study.

BACKGROUND: Lesser toe deformities such as hammer, mallet, claw, or cross over toe deformities are common in the elderly. They are attributed to mechanical, neuromuscular, congenital, inflammatory arthritis, or other disorders. They become clinically relevant when they produce pressure and friction at the distal ends of the digits leading to preulcerative callus or ulcer formation. These deformities can be difficult to offload with custom orthotics, accommodative footwear, and off-the-shelf devices such as crest pads and toe crutches. CASE: Mr. D was an 82-year-old man with bilateral lesser t

Contemporary technologies in wearable orthoses and their potential intervention for clubfoot treatment: A review.

The growth of assistive technology has increased the use of smart wearable orthoses. This is driven by improved sensors, actuators, cheaper integrated circuits, better data connectivity for data acquisition, and advanced manufacturing techniques. These smart devices correct, support, and monitor physical deformities and movements. This study reviews recent literature on smart wearable orthotic devices' design, application, and fabrication. The focus is on biomedical uses and advanced manufacturing methods. A total of 79 peer-reviewed articles were identified. After screening based on titles, a

An Evaluation of Orthotics on In-Toeing or Out-Toeing Gait.

Background and Objectives: In-toeing and out-toeing gait are rotational deformities commonly observed in children with neuromuscular conditions. These gait abnormalities often result from internal tibial torsion, increased femoral anteversion, and metatarsus adductus. This study was conducted to create a comprehensive evaluation of the effectiveness of lower extremity orthotics as a non-operative treatment option, given their regular use in clinical settings. The aim of this literature review was to understand the efficacy of various orthotic devices in correcting rotational deformities in the

Determining validity of the PALite and ODFS PACE activity logger for measuring step count in healthy adults.

BACKGROUND: Determining adherence with orthoses is important for clinicians prescribing devices. Measuring orthotic use often relies on patient recall which has poor agreement with objective measures. Measuring step count whilst wearing an orthosis could help objectively quantifying adherence. The Odstock Drop Foot Stimulator (ODFS) Pace, used in foot drop, has an integral activity logger which provides data on step count. The PALite, an accelerometer, measures step count and can be fixed to an ankle foot orthoses (AFO). Both have the potential to provide objective measures of adherence; howev

Long-term outcomes of a dynamic ankle-foot orthosis on gait characteristics of a service member with incomplete nerve injury to the lower extremity: a case report.

This case study reports a 5-year follow-up of a 32-year-old male service member who suffered polytrauma in 2007 following a Humvee rollover in Afghanistan. The service member's injured left lower extremity was salvaged, but severe damage to the lumbosacral plexus and significant injuries to the pelvis, hip, and femur resulted in near total paralysis and foot drop of the left lower limb. Two years of multiple substandard ankle-foot orthotic devices pushed him to investigate a dynamic ankle-foot orthotic (DAFO) with energy storing capability, which allowed him to remain on active duty and deploy

CT evaluation of the shoulder in children with Erb's palsy.

The majority of children with traumatic injuries of the brachial plexus recover with only minor sequelae. Some patients develop residual deformity, however. This deformity may consist only of internal rotation contractures, but in some patients subluxation of the humeral head may be present. CT is useful in the evaluation of: the presence or absence of subluxation, deformity of the glenoid and the proper placement of the humeral head in plaster or orthotic devices.

Static orthoses for the management of microstomia.

Microstomia is a complication of facial burns, traumatic injuries, scleroderma, or surgical reconstructions involving the oral aperture. A variety of orthoses for the correction or prevention of microstomia are offered by dentists, occupational therapists, physical therapists, and other specialists. This paper provides an overview of the structural and clinical features of 12 common tissue-borne or tooth-borne microstomia appliances. The review is intended to facilitate the selection of suitable orthoses, and to indicate the need for interdisciplinary management of microstomia patients.

[Orthotic management for patients with osteogenesis imperfecta].

Osteogenesis imperfecta (OI) is a disease caused by a genetic defect in the qualitative and quantitative synthesis of type I collagen. There is a wide variation in its clinical signs, characterized by bone fragility, resulting in a bone vulnerable to external and internal forces, determining the occurrence of frequent fractures with minimal or no trauma. The therapeutic objective is directed to improve the functional capacity of the child or adult concerned, adopting those compensatory strategies to optimise their independence. In this sense, the use of different orthoses and assistive technol

Conservative treatment of flatfoot in the newborn.

Excessive pronation of the feet is almost a universal condition among children, which is demonstrated as an abnormal weight-bearing position on hard, flat surfaces. Excessive pronation of the feet of children is closely associated with intrinsic and extrinsic skeletal deformity in adults; consequently, it must be neutralized as a basic necessity to most proper footcare, as the child grows. It must be a priority of podiatric medicine to develop a practical plan for supervision of foot health from the earliest weight bearing in infancy and to work in close cooperation with parents and pediatrici

Dynamic orthoses for the management of microstomia.

Oral wounds and burns may result in microstomia with significant management problems. An important early management decision involves the selection of an appliance capable of applying forces that will halt and reverse the condition. A selection appropriate to the patient may be based on the need for teeth suitably placed to retain and position a particular device. Limited access to the patient's mouth because of recent trauma, surgery, wiring, inability to tolerate anesthetic, etc., may further restrict choices. Thus, the process of selection involves a weighing of the specific patient's condi
