Atypical incus necrosis: a case report and literature review.

OBJECTIVE: We report an atypical case of ossicular necrosis affecting the incus, in the absence of any history of chronic serous otitis media. We also discuss the current theories of incus necrosis. CASE REPORT: A male patient presented with a history of right unilateral hearing loss and tinnitus. Audiometry confirmed right conductive deafness; tympanometry was normal bilaterally. He underwent a right exploratory tympanotomy, which revealed atypical erosion of the proximal long process of the incus. Middle-ear examination was otherwise normal, with a mobile stapes footplate. The redundant lon

[Reconstruction of the ossicular chain--current strategies].

Besides eradication of chronic middle ear disease, the reconstruction of the sound conduction apparatus is a major goal of ear microsurgery. If available, the autogenous ossicle represents the gold standard as material for the partial ossicular reconstruction. In the event of a more extensive destruction of the ossicular chain diverse alloplastic materials are used for total reconstruction. Such prostheses must fulfil high demands in terms of biocompatibility and their acoustic-mechanic properties. In recent years, filigree titanium middle ear implants, allowing individual adaptation to anatom

Study on the Prosthesis Structural Design and Vibration Characteristics Based on the Conduction Effect of Human Middle Ear.

As a bridge from the sound signal in the air to the sound perception of the inner ear auditory receptor, the tympanic membrane and ossicular chain of the middle ear transform the sound signal in the outer ear through two gas-solid and solid-liquid conversions. In addition, through the lever principle formed by three auditory ossicle structure, the sound was concentrated and amplified to the inner ear. However, the sound transmission function of the middle ear will be decreased by disease, genetic, or trauma. Hence, using middle ear prosthesis to replace the damaged ossicles can restore the con

[The cartilage guide: a solution for anchoring a columella-prosthesis on footplate].

BACKGROUND: A torp (columella-prosthesis) is the typical ossicular reconstruction in cases of a destroyed stapedial arch. Yet, many unsatisfactory hearing results are due to the lack of a stable, reliable anchoring of the base of the prosthesis on the footplate. Some solutions have been postulated, amongst them the perforation of the footplate with a tiny spike at the lower end of the prosthesis, which, however, many otosurgeons regard as too dangerous for the inner ear. Specially designed silicone sheets cannot guarantee a permanent guide of the columella. METHODS: From our good experience w

Outcomes following ossicular chain reconstruction with composite prostheses: hydroxyapatite-polyethylene vs. hydroxyapatite-titanium.

We conducted a retrospective study to compare the results of ossicular chain reconstruction (OCR) with two types of composite prosthesis: a hydroxyapatite-polyethylene (HAPEX) implant and a hydroxyapatite-titanium (HATi) prosthesis. We reviewed the records of 222 patients-104 males and 118 females, aged 8 to 79 years (mean: 39.7)-who had undergone OCR for ossicular chain dysfunction and who met our eligibility criteria. In addition to demographic data and the type of prosthesis, we compiled information on pre- and postoperative audiometric findings, the underlying diagnosis, the timing of surg

Solving ossicular problems in stapedectomy.

Three unusual ossicular problems encountered in stapedectomy are: floating stapes footplate, partial absence of the incus long process, and otosclerosis combined with a fixed malleus. The surgical techniques advocated in solving these problems are: placing a Robinson prosthesis on the floating footplate, utilizing the Lippy modification of the Robinson prosthesis for the problem incus, and performing only a stapedectomy for malleus fixation and otosclerosis. The hearing results in 242 cases were quite satisfactory and no patient had a further sensorineural loss.

[Surgical implantation of the fully implantable middle-ear hearing device].

The new fully implantable middle ear device (CARINA) is useful in sensorineural, conductive and mixed hearing loss. Surgical implantation of this device in the different types of hearing loss is similar in some aspects but differs in others. Consequently, we describe the surgical procedure in sensorineural and in conductive hearing loss separately.

Bioglass middle ear prosthesis: long-term results.

The purpose of this study was to review the University of Florida's long-term results with Bioglass middle ear prostheses. Between April 1984 and November 1987, 37 patients were implanted with Bioglass prostheses (25 total and 12 partial ossicular replacements). Twenty-one patients had postoperative data of at least 24 months (range, 24 to 126 months; mean, 86 months; median, 100 months), and five patients had > 10 years' follow-up. In three cases, portions of fractured prostheses extruded, leaving an intact tympanic membrane. One patient with a total ossicular prosthesis was reexplored at 38

Ossiculoplasty for Trauma.

Most traumatic ossicular chain injury results from blunt head trauma associated with temporal bone fracture, though penetrating, barotraumatic, and iatrogenic causes are also encountered. Dislocation of the IS joint is the most common lesion, followed by IM separation and a stapes superstructure fracture. Management depends on the mechanism and severity. High-resolution temporal bone computed tomography aids in localizing disruption and identifying delayed ossicular fixation. Surgical reconstruction with autologous or artificial materials typically achieves postoperative air-bone gaps of 10 to

Ossiculoplasty with composite prostheses. Overview and analysis.

Aeration of a mucosa-lined tympanic cavity is essential for a functioning middle ear. Extrusions of even the best-designed prostheses occur from abnormal middle ear conditions such as atelectasis, middle ear fibrosis, recurrent cholesteatoma, tympanic membrane perforation, and otitis media. Various polymers have been developed in an attempt to maximize prosthetic biocompatibility and ease of use while minimizing the chance of extrusion. One such polymer is a composite of hydroxyapatite and Silastic, known as Flex H/A. This material has been incorporated into two ossicular prostheses, the Unive

Multiple Ossicular Dislocation Including Stapediovestibular Dislocation Presenting with Conductive Hearing Loss.

Temporal bone trauma can cause hearing loss and in case of prolonged conductive hearing loss, traumatic ossicular injury should be considered. Separation of the incudostapedial joint is the most common lesion, and stapediovestibular dislocation is relatively rare but can easily cause perilymphatic fistula. Here, we report a very rare case of external stapediovestibular dislocation after trauma, ending up with successful surgical outcome. A 27-year-old man with non-progressive hearing loss on the right side since childhood visited the clinic. Audiogram showed a conductive hearing loss with air-
