Validation method of a middle ear mechanical model to develop a surgical simulator.

Ossicular surgery requires a high dexterity for the manipulation of the fragile and small middle ear components. Currently, the only efficient technique for training residents in otological surgery is through the use of temporal bone specimens, where any existing surgical simulator does not provide useful feedback. The objective of this study was to develop a finite-element model of the human ossicular chain dedicated to surgical simulation and to propose a method to evaluate its behavior. A model was developed based on human middle ear micromagnetic resonance imaging. The mechanical parameter

Reconstruction of the entire ossicular conduction mechanism.

OBJECTIVES/HYPOTHESIS: Stapes fixation combined with fixation, absence, or malformation of the malleus-incus complex requires an uncommon surgical reconstruction and offers a unique combination of challenges and hazards. This situation may occur in the presence of severe tympanosclerosis, otosclerosis, congenital ossicular malformations, and revision surgery for either stapedectomy or chronic ear disease. In previous reports, this procedure has been grouped with total ossicular reconstruction without much distinction. However, the challenges unique to this problem deserve special consideration

An intact stapes superstructure independently predicts ossiculoplasty success: analysis of 100 titanium reconstructions.

OBJECTIVES: This study aimed to evaluate whether the presence of an intact stapes superstructure is a significant factor in the success of an ossiculoplasty. Secondary factors assessed included presence of cholesteatoma and revision surgery. METHODS: A retrospective cohort study in a Scottish tertiary centre analysed audiological outcomes from 100 consecutive adult patients undergoing single-stage ossiculoplasty with titanium prostheses (partial ossicular replacement prosthesis; total ossicular replacement prosthesis). Patient demographics and pre- and post-surgery audiometric data were colle

Primary ossiculoplasties provide better hearing results than revisions: a retrospective cohort study.

PURPOSE: To evaluate the efficacy of ossicular chain reconstruction (OCR) in primary and revision surgeries, and to investigate the impact of the number of previous surgeries on hearing outcomes. METHODS: Retrospective analysis of cases with OCR due to chronic otitis in a tertiary center between January 2018 and September 2021. RESULTS: Altogether, 147 cases of ossicle involvement were assessed. In 91.83% (n = 135) OCR was performed, 96.26% of them with titanium TORP/PORP (n = 130), two cases with autologous prosthesis and three with piston. Mean follow-up was 8.8 months. The ABG significant

Reconstructive ossiculoplasty options in primary cholesteatoma surgeries with an intact stapes.

PURPOSE: In primary cholesteatoma patients, incus destruction with an intact and mobile stapes is a frequent finding. Different techniques have been described to restore the ossicular chain, including incus interposition, stapes augmentation and type III tympanoplasty. Controversy about postoperative hearing results in open versus closed surgical techniques exist. METHODS: We performed a retrospective analysis of clinical, surgical and audiometric data of patients with primary cholesteatoma surgery operated between 2010 and 2020, and a mobile stapes and one-stage ossicular reconstruction. Postinflammatory ossicular fixation: CT analysis with surgical correlation.

Postinflammatory ossicular fixation is a common problem encountered by the otologic surgeon upon exploration because of conductive hearing loss in patients with chronic otitis media. These nonotosclerotic noncongenital lesions take three pathologic forms: fibrous tissue fixation (chronic adhesive otitis media), hyalinization of collagen (tympanosclerosis), and new bone formation (fibro-osseous sclerosis). Fibrous tissue fixation appears on CT as nonbony, noncalcific soft-tissue debris encasing some or all of the ossicular chain. Tympanosclerosis appears as unifocal or multifocal punctate or we

[Ear ossicle transplants for reconstruction of sound transmission in the middle ear. A histologic long-term study].

In this study 130 ossicular middle ear transplants were studied. 33 remained in the middle ear for 10 years or longer, one for 21 years. The majority of the grafts show 5% to 40% bone new formation. Absence of vital bone or subtotal host bone replacement is exceptional. The amount of bone new formation appears independent the implantation time. 75% of the grafts show no bone resorption. In 15% sound transmitting might be impaired due to resorption. Grafts with a high degree of bone new formation show less resorption. 21% of the ossicles have signs of inflammation. Inflammation boosts bone turn

Differentiating Causes of Post-Surgical Conductive Hearing Loss with Optical Coherence Tomography Vibrometry.

PURPOSE: To evaluate whether optical coherence tomography (OCT) vibrometry can differentiate causes of conductive hearing loss (CHL) following ossiculoplasty, specifically distinguishing among effusion, soft-tissue fixation, and prosthesis disconnection. METHODS: We simulated three post-surgical CHL conditions, effusion, soft-tissue fixation, and prosthesis disconnection, in a cadaveric temporal bone model (N = 10 per condition), with a partial ossicular replacement prosthesis (PORP) and cartilage graft implanted via a retrotympanic approach. A custom-built swept-source middle-ear OCT system

Virtual Quality Control in Middle Ear Surgery: Is Image-guided Tympanoscopy a Valuable Tool for Depicting Borderline Situations?

HYPOTHESIS: Before modern imaging was introduced, revision surgery was the only way to evaluate possible reasons for inadequate improvement in hearing after ossicular replacement during reconstructive middle ear surgery. BACKGROUND: The aim of this study was to evaluate freely navigable virtual tympanoscopy using different computed tomographic modalities. We compared cone-beam computed tomography (CBCT), flat panel computed tomography (FPCT), and conventional computed tomography in helical mode (CTH), volume mode (CTV), and ultra high resolution mode (CTD). METHODS: Four temporal bone specim

Influence of backside loading on the floating mass transducer: An in vitro experimental study.

HYPOTHESIS: The vibration of the floating mass transducer (FMT) of a single active middle-ear implant (AMEI) is distinctly influenced by the properties of the material coupled to its back side. BACKGROUND: In round window vibroplasty, the FMT needs to be padded against the surrounding bone opposite from the round window membrane. This represents one factor influencing its performance as a round window driver. Therefore, we examined the effects of different materials linked to the back side of an FMT on its vibration range. METHODS: The back side of an FMT was glued to a silicone cylinder 1.0

Experimental middle ear surgery in rabbits: a new approach for reconstructing the ossicular chain.

This experimental animal study aimed at evaluating a new prosthesis to replace the ossicular chain; we developed a new technique for surgical implantation into the middle ear of rabbits. The rabbit middle ear is, owing to the relative anatomical dimensions involved, an ideal environment for implantation procedures involving the ossicles, as the surgical conditions are similar to those of the human middle ear. This study included a total of 34 approximately six-month-old female white rabbits (New Zealand) weighing between 3.2 and 4.4 kg. The implants used were constructed of ceramic materials (

Modification of closed tympanoplasty in middle ear cholesteatoma.

BACKGROUND: Classic mastoidectomy and modified mastoidectomy are traditional surgical procedures for middle ear cholesteatoma with goals of eradicating diseases, creating dry ears and preventing severe complications. However, the drawback of these procedures is the lack of hearing improvement. Therefore, our study aimed to investigate the effects and safety of a modification of closed tympanoplasty for middle ear cholesteatoma. METHODS: Eighty-three patients were recruited in this study based on the following two criteria: each patient had middle ear cholesteatoma in one ear; the affected ear

Laser applications in ossicular surgery.

In the past 8 years, the author has used the laser in more than 2800 cases of tympanomastoid surgery. Of these, more than 600 involved middle ear ossicular surgery including otosclerosis, congenital deformities, and fixations of the ossicular mass for various reasons. During this time, there have been no complications, either short-term or long-term, associated with the use of the laser. There are many tasks the laser can perform with less trauma, more precision, and greater speed than conventional hand-held tools. The laser does not replace these tools in otologic surgery, but it does add an

The Audiometric and Mechanical Effects of Partial Ossicular Discontinuity.

OBJECTIVES: Ossicular discontinuity may be complete, with no contact between the disconnected ends, or partial, where normal contact at an ossicular joint or along a continuous bony segment of an ossicle is replaced by soft tissue or simply by contact of opposing bones. Complete ossicular discontinuity typically results in an audiometric pattern of a large, flat conductive hearing loss. In contrast, in cases where otomicroscopy reveals a normal external ear canal and tympanic membrane, high-frequency conductive hearing loss has been proposed as an indicator of partial ossicular discontinuity.

Management of tympanosclerosis with ossicular fixation: review and presentation of long-term results of 30 new cases.

OBJECTIVE: Surgery for tympanosclerosis has always been challenged with the risk of deterioration of hearing or the recurrence of the disease. If surgery is indicated, controversy exists regarding staging versus nonstaging and whether to perform stapedectomy or mobilize the fixed ossicles by removal of plaques. The aim of this study was to review the previous reports and to analyze the long-term surgical outcome of 30 patients who have been operated on owing to tympanosclerosis. METHODS: Nine hundred twelve cases with chronic otitis media with or without cholesteatoma that have undergone eith
