Extra-mandibular Osteoradionecrosis after the Treatment of Head and Neck Cancer.

AIMS: Osteoradionecrosis (ORN) is a serious toxicity of head and neck radiotherapy. It predominantly affects the mandible. Extra-mandibular ORN is rare. The aim of this study was to report the incidence and outcomes of extra-mandibular ORNs from a large institutional database. MATERIALS AND METHODS: In total, 2303 head and neck cancer patients were treated with radical or adjuvant radiotherapy. Of these, extra-mandibular ORN developed in 13 patients (0.5%). RESULTS: Maxillary ORNs (n = 8) were a consequence of the treatment of various primaries (oropharynx = 3, sinonasal = 2, maxilla = 2, pa

Osteoradionecrosis After Mandibulotomy and Marginal Mandibulectomy in Patients With Oral Cancer.

Objectives Osteoradionecrosis is one of the most severe complications in patients with head and neck cancer, which is characterized by persistent exposed and devitalized bone without proper healing after radiation. The extent to which mandibulotomy and marginal mandibulectomy influence the occurrence of osteoradionecrosis remains unclear. This study evaluated the incidence and risk factors for developing osteoradionecrosis of the mandible after oral cancer treatments. Methods A retrospective study was performed to analyze medical records of patients who underwent surgery and postoperative radi

Osteoradionecrosis of the Temporal Bone as a Rare Cause of Facial Nerve Palsy.

We present a case of a 69-year-old male who presented with acute left facial nerve palsy, serous bloody otorrhea, otalgia, and exposed necrotic bone on the floor of his left ear canal. His medical history revealed a left canal wall-down (CWD) mastoidectomy thirty years ago. Subsequently, twenty years later, he received primary chemoradiotherapy for tonsil cancer on the same side. The patient's medical history, the typical clinical picture, and a comprehensive diagnostic workup, including imaging modalities and electrophysiology, finally led to a diagnosis of osteoradionecrosis of the temporal

[Osteoradionecrosis and bisphosphonate-related osteonecrosis of the jaw: a review of the pathogenesis and treatment].

Osteoradionecrosis of the jaw (ORNJ) and bisphosphonate-related osteonecrosis of the jaw (BRONJ) are usually caused by head and neck radio-therapy and by the usage of bisphosphonate, respectively. These diseases can lead to facial deformity and dysfunction of the mandible, and may cause severe chronic facial pain. The pathogenesis of ORNJ and BRONJ are complex, and the therapy of which is still challenged. The present article reviewed the latest literature about the pathogenesis and treatment of ORNJ and BRONJ for update. The irradiation may damage the endothelia cells and microvessels in jaw

Medication-related osteonecrosis and osteoradionecrosis of the jaws: Update and current management.

Medication related osteonecrosis of the jaws (MRONJ) and osteoradionecrosis of the jaws (ORNJ) are two different diseases of quite similar appearance. MRONJ is mainly due to antiresorptive or antiangiogenic drug therapy and ORNJ to radiotherapy. The present work aimed at presenting and comparing the current knowledge on MRONJ and ORNJ. They both present as an exposure of necrotic bone and differ in some clinical or radiological characteristics, clinical course and mostly in treatment. They share similar risk factors. A tooth extraction is more frequently found as a triggering factor in MRONJ.

Osteoradionecrosis of the hyoid bone--a novel application of the Sonopet ultrasonic aspirator.

A 66-year-old man with a history of squamous cell carcinoma of the right tonsil presented to the emergency department with two life-threatening episodes of haemoptysis 9 months after completion of chemoradiation. He was evaluated to rule out recurrent malignancy, and was determined to have osteoradionecrosis of his hyoid bone that had led to an oropharyngeal fistula. Given the proximity to branches of the external carotid, he was offered tracheostomy and resection of the protruding necrotic bone with ultrasonic bone aspirator, which was placed down the shaft of the laryngoscope to minimise dam

Is myocutaneous flap alone sufficient for reconstruction of chest wall osteoradionecrosis?

OBJECTIVES: This study was carried out to determine whether the myocutaneous flap, alone, is sufficient to reconstruct a chest wall defect after osteoradionecrosis and provide satisfactory stability to the chest wall. METHODS: This study involved five patients who were subjected to post-mastectomy radiotherapy as a treatment for breast cancer. Excision of the ulcer and all the necrotic ribs, with preservation of the parietal pleura and reconstruction with the latissimus dorsi flap, was done without the use of either an artificial prosthesis or autologous rib to reconstruct the chest wall defe

[Part III: free periosteal flaps as a treatment for mandibular osteoradionecrosis].

INTRODUCTION: The vascularized periosteal free flap transposes a non-irradiated soft tissue with neo-angiogenesis, bone induction, and osteogenesis qualities. Its technique and interest are studied for early osteoradionecrosis. MATERIAL AND METHOD: A retrospective study was made between 1992 and 2002. Thirty-four patients were selected among the 59 cases operated for osteoradionecrosis; all patients were classified stage II and I on the Epstein scale. Twenty-six patients were reconstructed using a non-periosteum free flap (bone, muscular or cutaneous flap), and eight patients were reconstruct

[Mandibular osteoradionecrosis: sword of Damocles of radiotherapy for head and neck cancers?].

Radiotherapy of head and neck cancer is efficient but may have severe late effects. Although unfrequent, mandibular radionecrosis is the most dreadful. A cascade of events leads from hyposialia to bone destruction through extensive caries and trauma linked to tooth removal. Prevention is based on oral hygiene and use of daily topical fluorides. Antibiotics and hyperbaric oxygen can be effective as a conservative management. Progressive osteonecrosis is best treated by mandible resection and reconstruction with microvascular free bone flaps. Patient compliance to prophylaxis is of major importa

Mandibular Osteoradionecrosis: Mandibular Preservation Using Humeral Periosteal Free Flap Wrapping.

BACKGROUND: Management of mandibular osteoradionecrosis (ORN) is challenging and often requires large surgical resection. Alternatively, mandibular preservation and wrapping with a periosteal free flap may achieve healing in selected patients. Our objective was to assess the success rate and morbidity of revascularization of mandibular ORN using a humeral periosteal free flap. METHODS: A retrospective study was performed using medical records of patients who had undergone humeral periosteal free flap reconstruction for mandibular ORN. Clinical data, including age, gender, ORN site, ORN grade

Combined endoscopic transnasal and transoral approach for extensive upper cervical osteoradionecrosis.

PURPOSE: Osteoradionecrosis (ORN) is a rare yet well-recognized complication following radiotherapy to the head and neck. We illustrate the only case of a spontaneous extrusion of the sequestered C1 arch through the oral cavity and discuss our experience with a combined endoscopic transnasal and transoral approach for cervical ORN. METHODS: A 56-year-old female presented with a 3-month history of blood-stained nasal discharge. She had been treated with radiotherapy for nasopharyngeal carcinoma 25 years earlier. Flexible nasal endoscopy demonstrated an exposed bone with an edematous posterior

Osteoradionecrosis of the jaw: Current concepts in prevention and management with a case report.

Osteoradionecrosis (ORN) of the jaw is a debilitating late complication of head and neck radiotherapy (RT), characterized by necrotic bone within an irradiated field, persisting without tumor recurrence. Advances in radiation techniques and preventive dental management have reduced its incidence, yet ORN remains a major cause of morbidity. This review synthesizes current understanding of ORN pathophysiology, risk factors, diagnostic strategies, and contemporary prevention and management approaches, with a focus on the updated 2024 ISOO-MASCC-ASCO guidelines. Preventive measures include pre-RT

Treatment approaches in cases of mandibular osteoradionecrosis: A systematic meta-analysis.

BACKGROUND: Osteoradionecrosis (ORN) refers to necrotic bone that remains unhealed for a minimum of 3 months following radiotherapy in an irradiated region. The aim of this systematic meta-analysis was to evaluate the efficacy of hyperbaric oxygen therapy and surgery alone or in combination in the treatment of patients with ORN. METHODS: A systematic review and analysis of literature were performed to ascertain the role of surgery and HBO in the treatment of ORN. In May 2024 we updated the searches of the Cochrane Central Register of Controlled Trials (CENTRAL), (The Cochrane Library, Issue 1

Incidence and Risk of Occult Cancer in Advanced-Stage Mandibular Osteoradionecrosis.

BACKGROUND: Advanced mandibular osteoradionecrosis (ORN) often requires segmental resection and reconstruction. Although ORN has distinct clinical and radiographic features, detecting occult cancer preoperatively remains challenging. The goal of this study was to determine the incidence and risk factors of occult cancer in patients undergoing surgery for advanced mandibular ORN. METHODS: Medical records of mandibular ORN patients who underwent compound or composite segmental mandibulectomy with flap reconstruction between 2010 and 2022 were reviewed. Data on demographics, ORN presentation, an

Research Frontiers in Oral Toxicities of Cancer Therapies: Osteoradionecrosis of the Jaws.

The deleterious effects of head and neck radiation on bone, with osteoradionecrosis (ORN) as the major disabling side effect of head and neck cancer treatment, are difficult to prevent and hard to treat. This review focuses on the current state of the science regarding the pathobiology, clinical impact, and management of ORN. With regard to the pathobiology underlying ORN, it is not yet confirmed whether the current radiation schedules by 3-dimensional conformal radiotherapy and intensity modified radiotherapy result in an unchanged, decreased, or increased risk of developing ORN when compared

Comparative osteoradionecrosis rates in bony reconstructions for head and neck malignancy.

BACKGROUND: Bony free flaps are used to reconstruct head and neck cancer defects. The most commonly used flaps are the free fibula flap and the deep circumflex iliac artery (DCIA) flap. Radiotherapy may be given post-operatively to prevent cancer recurrence. This radiotherapy can, however, destroy the bone; a complication termed osteoradionecrosis (ORN). Although there have been studies comparing free fibula and DCIA flaps in terms of success rates and complications, few have assessed the incidence of ORN in both groups. METHODS: A retrospective cohort study was conducted involving patients f
