Location of the nasolacrimal canal in relation to the high Le Fort I osteotomy.

The positional relationship between the high-level Le Fort I osteotomy and the nasolacrimal canal was studied in standard posteroanterior and lateral cephalograms made of 100 adult dry skulls after orienting the Frankfort horizontal of each skull to the natural horizontal plane. The simulated high-level Le Fort I osteotomy in all of the 100 skulls was under the level of the inferior orifice of the nasolacrimal canal by a mean distance of 5.2 mm (range, 0.5 to 11.5 mm). The average distance between the canal orifice and a line which extended from the lacrimal fossa to the anterior attachment of

Facial growth subsequent to Le Fort I osteotomies in adolescent monkeys.

The study goal was to assess qualitatively and quantitatively the craniofacial adaptations following total maxillary osteotomy with and without mandibular autorotation. Six adolescent female Macaca fascicularis monkeys were randomly divided into two surgical groups, and eight others served control. Prior to surgery, metallic implants were inserted in the anterior part of the cranial base, on opposite sides of craniofacial sutures, and in multiple sites of the maxilla and mandible. Group I animals underwent a Le Fort I advancement of 4 mm, while Group II animals had a 5-mm advancement and a 2.5

The behaviour of the maxilla in vertical movements after Le Fort I osteotomy.

In 15 cases of ordinary Le Fort I osteotomy with simultaneous vertical displacement, the behaviour of the maxilla in respect to vertical movement was examined. It was observed that out of 12 maxilla which had been tilted downwards, 10 moved upwards again. One maxilla remained stable and another one moved further downwards. Of 3 cases with upward movement, two remained stable whereas one moved further upwards. Therefore, the authors conclude that the downward displacement especially is unstable and that one has either to overcorrect or search for possibilities of stabilization other than wiring

Three-dimensional evaluation of the alar cinch suture after Le Fort I osteotomy.

Orthognathic surgery has an influence on the overlying soft tissues of the translated bony maxillomandibular complex. Improvements in both function and facial appearance are the goals of surgery. However, unwanted changes to the soft tissues, especially in the nose region, frequently occur. The most common secondary change in the nasolabial region is widening of the alar base. Various surgical techniques have been developed to minimize this effect. The purpose of this study was to evaluate the changes in the nasal region due to orthognathic surgery, especially the alar width and nasal volume,

Blindness after Le Fort I osteotomy: a possible complication associated with pterygomaxillary separation.

INTRODUCTION: Visual loss after Le Fort I osteotomy is a devastating complication the mechanism of which is not always clear. METHODS: A case report of blindness following Le Fort I osteotomy is presented. The literature on the various skull base complications associated with Le Fort I osteotomies is reviewed and the mechanisms of these complications discussed. CONCLUSION: The radiological findings in this case are similar to those previously reported. They strongly support the hypothesis that an adverse transmission of forces associated with pterygomaxillary separation via the sphenoid bone

Long-term stability of surgical open-bite correction by Le Fort I osteotomy.

Skeletal changes greater than those observed in untreated adults have been noted beyond 1 year post-surgery in adult patients who had surgical correction of a long face deformity. The stability of skeletal landmarks and dental relationships from 1 to >3 years post-surgery was examined in 28 patients who had undergone surgery of the maxilla only, and in 26 patients who had undergone 2-jaw surgery to correct >2 mm anterior open bite. Although the average changes in almost all landmark positions and skeletal dimensions were less than 1 mm, point B moved down >2 mm and face height increased >2 mm

Le Fort type III osteotomy for midface deficiency in selected cleft palate patients.

The comprehensive treatment of cleft lip and palate continues to evolve as understanding of the pathogenesis of this malformation and refinement of surgical techniques for its treatment have improved. The malformation, although varying in severity from individual to individual, is now considered an abnormality of the entire maxilla. Our experience indicates that a finite minority of cleft patients exists with significant malar and midfacial projection deficiency in addition to class III malocclusion who will benefit from a more extensive midfacial advancement. We have treated 10 patients with

Tooth pulp changes following Le Fort I maxillary osteotomy in a primate model.

Le Fort I maxillary 'down-fracture' osteotomy with 8 mm advancement was performed in 15 adult rhesus monkeys. Forty-five tooth pulps were examined histologically at intervals from 0 to 24 weeks after surgery. Cell degeneration occurred in 31% of pulps examined, necrosis in 16% of pulps and osteo-dentine was found in 7% of pulps. Almost half of the teeth examined (47%) showed marked cellular changes, more frequently found in posterior teeth. Other features noted were inflammation (13%) and reactive dentine in pulps (24%). Axons degenerated initially but recovered by 24 weeks. It is concluded th

Severe late-onset epistaxis following Le Fort I osteotomy: angiographic localization and embolization.

Epistaxis following maxillofacial trauma or maxillofacial surgery is uncommon. It usually occurs within 24 hours of the injury and can usually be controlled by packing. Rarely internal maxillary artery ligation is necessary and embolization has been used in some cases of severe trauma. We present a case of severe late-onset epistaxis following Le Fort I osteotomy. The diagnostic approach and treatment are discussed.

Surgical-orthodontic correction of maxillary retrusion by Le Fort I osteotomy and proplast.

The combined efforts of the orthodontist and surgeon are necessary for successful treatment of most patients with maxillary retrusion. Both the aesthetic and functional manifestations of this dento-facial deformity can usually be effectively treated by maxillary advancement, augmentation of the midfacial region and orthodontics. Treatment planning, pre- and post-surgical orthodontic treatment principles, and surgical considerations in treating 25 adults with maxillary retrusion are described and illustrated. Surgical advancement of the maxilla by Le Fort I osteotomy, proper alignment of the te

Does preoperative joint space affect accuracy in splint-based Le Fort I osteotomy?

In splint-based Le Fort I osteotomy (LFI), variations in condyle-fossa relationship can influence surgical accuracy. The aim of this study was to investigate the impact of the preoperative joint space (JS) on surgical errors. This retrospective study included patients status post splint-based maxilla-first LFI at a single institution in a 7-month period. Preoperative JS was measured on preoperative cone-beam computed tomography (CBCT), while angular and linear surgical movements at the maxillary dental midline (U1) were measured using regional voxel-based registration. In 26 eligible subjects,
