Determinants of patient satisfaction and expectation after hip fracture surgery: a multidimensional perspective on association between clinical outcomes, subjective expectations and psychosocial factors.

BACKGROUND AND OBJECTIVES: Hip fracture represents a singular traumatic experience with substantial health and socioeconomic repercussions in the elderly. While clinical and functional outcomes are well established, patient experience has emerged as another measure of quality-of-care. However, factors influencing patient experience after osteoporotic hip fracture surgery remain poorly defined. This study aims to determine factors influencing postoperative satisfaction and patient expectation. MATERIALS AND METHODS: Retrospective analysis of a prospectively followed up cohort (n = 257) of hip f

From Expectation to Learning: A Mediational Model of Exposure Therapy Outcome in Anxiety Disorders.

INTRODUCTION: Negative expectations about treatment outcomes are associated with poorer recovery in anxiety disorders. However, it remains unclear whether patients' expectations about how effective the treatment will be (treatment-specific expectations) mediate the link between generalized negative expectations (i.e., unspecific pessimistic beliefs) and treatment outcomes, and whether these expectations affect outcomes via the learning rate during exposure therapy that is, the extent to which patients update their fear-related beliefs based on corrective experiences. Since treatment-specific e

Caesarean section scar pregnancy: expectant management with favourable outcome of an acute presentation.

A woman in her early 40s, who had two previous caesarean sections, at 10-week period of amenorrhoea presented to our hospital gynaecology clinic.A caesarean scar pregnancy (CSP) was diagnosed after performing a transvaginal scan. The diagnosis was explained to the patient, and the patient opted for expectant management. She presented to the emergency department with severe abdominal pain and vaginal bleeding after 4 days.A transvaginal scan revealed a CSP with a fetal pole and heartbeat.Since the cervical os was opened, a successful suction curettage was performed. Beta hCG became undetectable 1

Favourable maternal and neonatal outcomes following expectant management of a giant placental chorangioma.

Chorangiomas affect less than 1% of pregnancies. However, while small chorangiomas are mainly asymptomatic, large placental chorangiomas may lead to severe neonatal and maternal complications. Ultrasound and Doppler technology imaging enable appropriate prenatal diagnosis and surveillance. We present a case of a large chorangioma in a mid-20s primigravid under specialised care at a tertiary referral hospital due to her medical history. The second-trimester scan detected a placental lesion measuring approximately 40 mm, which exhibited significant growth throughout the gestational period. This

Development and initial psychometric properties of the Cannabidiol Outcome Expectancies Questionnaire (CBD-OEQ).

Cannabidiol (CBD), a nonpsychoactive cannabinoid, is used by many individuals to treat medical and mental health conditions, despite limited support for the efficacy of CBD for these conditions. Identification of CBD-related outcome expectancies (i.e., beliefs concerning the anticipated effects of CBD) could be useful in understanding the etiology and maintenance of CBD use and/or be useful in administration or clinical trial research. Although there are several measures of cannabis outcome expectancies, cannabis comprises several active compounds (e.g., tetrahydrocannabinol [THC], CBD). Thus,

Outcome of monochorionic triamniotic triplet pregnancies: Expectant management versus fetal reduction.

OBJECTIVE: To compare the outcomes of monochorionic triamniotic (MCTA) triplets managed expectantly with those reduced to twins. METHOD: This was a retrospective cohort study comparing expectant management (EM) with fetal reduction (FR) to twins in 43 consecutive MCTA triplets with 3 live fetuses at 11-14 weeks between 2012 and 2021. RESULTS: Nineteen patients managed expectantly and 24 triplets reduced to twins were included. The rate of pregnancy with at least one survivor was 84.2% in the EM group and 66.7% in the FR group (P = 0.190). Compared to the EM cases, triplets reduced to twins h

Depression Symptoms, Problem Gambling and the Role of Escape and Excitement Gambling Outcome Expectancies.

The relationship between problem gambling and depression is well documented. However, there are few studies that have explored the mechanisms that may help maintain the association between depression symptoms and problem gambling. This study tests the assumption that gambling for escape and excitement may either mediate or moderate the relationship between depression and problem gambling. To test these propositions, 282 adults who gambled at least once a month were recruited to complete an online survey that assessed depression severity, the gambling outcomes expectancies of escape and excitem

Expectant Management of PPROM Improves Neonatal Outcome-A Retrospective Study of 562 Patients.

UNLABELLED: Prelabor preterm rupture of the membranes (PPROM) refers to the rupture of the membranes before 37 weeks, but also before the onset of labor. Approximately 3% of pregnancies are complicated by PPROM, which is an important cause of neonatal morbidity and mortality. The aim of the study is to demonstrate the benefit of expectant management in PPROM, compared to immediate birth, defined in our study as birth in the first 48 h. We analyzed 562 pregnancies with PPROM by gestational age groups and short-term morbidities. MATERIAL AND METHODS: We conducted a retrospective observational a

Outcome expectancies toward adherence to antiretroviral therapy for pregnant and postpartum women with HIV.

OBJECTIVES: Adherence to antiretroviral therapy and retention in care significantly drop for women with HIV during the postpartum period. We have a limited understanding of how outcome expectancies influence maternal adherence and retention in care. METHODS: Women with HIV from an urban academic clinic completed in-depth interviews in the third trimester and at 3 to 9 months postpartum to evaluate outcome expectancies, facilitators, and barriers to antiretroviral therapy adherence and retention in care. Interviews were audio-recorded and analyzed for content. A codebook was created using dedu

Nicotine increases anterior insula activation to expected and unexpected outcomes among nonsmokers.

RATIONALE: Tobacco has a higher rate of dependence than other drugs of abuse. However, the psychopharmacological effects of nicotine are incongruent with the tenacity of tobacco addiction since nicotine does not produce robust euphoria in humans or self-administration in rodents. A potential explanation is that nicotine amplifies the salience of other stimuli that have some incentive value, which could influence the initiation and persistence of smoking. However, the neural mechanisms of this process are unknown. OBJECTIVES: One way that nicotine may amplify the salience of other stimuli is b

Predicting gambling problems from gambling outcome expectancies in college student-athletes.

While previous research has suggested the potential importance of gambling outcome expectancies in determining gambling behaviour among adolescents, the predictive ability of gambling outcome expectancies has not yet been clearly delineated for college-aged youth. The current study aims to explore the relationships between gender and outcome expectancies in the prediction of gambling severity among college student-athletes. Data from the National Collegiate Athletic Association (NCAA) study assessing gambling behaviours and problems among U.S. college student-athletes were utilized. Complete d

[Integrated care and expected outcomes over the progression of CKD (chronic kidney disease)].

Among hypertensive cardiovascular complications only end-stage renal failure (ESRD) is progressively increasing, while the increases in the incidence of stroke and coronary heart disease have been suppressed. On the other hand, it is well established that CKD, especially due to diabetic nephropathy and IgA nephropathy, can be arrested into complete remission if treated in the early stages. Therefore, early detection program is highly and urgently required. To achieve this goal, we must consider what we can do now in nation-wide scale including patients, doctors and governments. There are remar

Expectant parents' preferences for mode of delivery and trade-offs of outcomes for breech presentation.

OBJECTIVE: To assess patients' preferences and trade-offs for mode of delivery in case of breech presentation at term. METHODS: Eighty women (40 with a foetus in breech presentation and 40 with a foetus in cephalic presentation) with a gestational age from 36 weeks onwards were offered scenarios of vaginal and caesarean breech delivery in which 1-month and 2-year neonatal and maternal complication rates were varied; expectant fathers (when present) were interviewed separately. Thresholds for complication rates where patients switch preferences were visualised graphically in trade-off curves.

The influence of gallbladder function on the symptomatology in gallstone patients, and the outcome after cholecystectomy or expectancy.

The purpose of this study was to examine whether gallbladder function as assessed by a hepatobiliary scintigraphy was related to the symptomatology in gallstone patients and to the outcome 1 year after either cholecystectomy or watchful waiting. The study included 100 patients with uncomplicated gallstone disease. Fifty-six patients had a functioning gallbladder and 44 had a nonfunctioning gallbladder. Patients with a nonfunctioning gallbladder had significantly more vomiting and received more pain-killing injections during pain attacks. Otherwise, there were no differences in pain patterns or

Contamination vs. harm-relevant outcome expectancies and covariation bias in spider phobia.

There is increasing evidence that spiders are not feared because of harmful outcome expectancies but because of disgust and contamination-relevant outcome expectancies. This study investigated the relative strength of contamination- and harm-relevant UCS expectancies and covariation bias in spider phobia. High (n=25) and low (n=24) spider fearful individuals saw a series of slides comprising spiders, pitbulls, maggots, and rabbits. Slides were randomly paired with either a harm-relevant outcome (electrical shock), a contamination-related outcome (drinking of a distasting fluid), or nothing. Sp

Differential aversive outcome expectancies for high- and low-predation fear-relevant animals.

There is now considerable evidence that phobic responding is associated with a bias towards expecting aversive or traumatic outcomes following encounters with the phobic stimulus (e.g. Behavioural Brain Sci. 18 (1995) 289-325; Phobias: A Handbook of Theory, Research and Treatment. Wiley, Chichester, 1997). In terms of conditioning contingencies, this can be described as a bias towards expecting an aversive unconditioned stimulus (UCS) following a phobic conditioned stimulus (CS). The disease-avoidance model of animal fears (Anxiety Res. 4 (1992a) 314; Matchett and Davey, 1991) suggests that co

Outcome Expectancies for Gambling: Empirical Modeling of a Memory Network in Federal Prison Inmates.

Outcome expectancies for gambling were explored in a group of 316 male medium security federal inmates with the multidimensional scaling statistical technique. Sixty-six possible outcomes for gambling were reduced to 16 after a factor analysis determined that 75% of the total variance was accounted for by the first 16 factors (eigenvalues > 0.75). The highest loading item for each factor was utilized in all subsequent analyses. Multidimensional scaling of these 16 items revealed the presence of two primary dimensions (positive-negative and arousing-sedating), corroborating previous research on

Leveraging expectation effects to improve outcomes in the context of digital mental health interventions.

Patients' expectations about treatment benefits are robust predictors of clinical outcomes across medical and psychological contexts. Recent evidence suggests that expectancy effects in digital mental health interventions (DMHIs) are comparable in magnitude to those observed in face-to-face treatments. Despite their relevance, systematic guidance on how expectations can be shaped and optimized within DMHIs is limited. This narrative review summarizes theoretical and empirical work on expectations, including placebo and nocebo mechanisms, and digital intervention design to outline how expectati
