Specifying and Pilot Testing Quality Measures for the American Society of Addiction Medicine's Standards of Care.

OBJECTIVES: In 2013, the American Society of Addiction Medicine (ASAM) approved its Standards of Care for the Addiction Specialist Physician. Subsequently, an ASAM Performance Measures Panel identified and prioritized the standards to be operationalized into performance measures. The goal of this study is to describe the process of operationalizing 3 of these standards into quality measures, and to present the initial measure specifications and results of pilot testing these measures in a large health care system. By presenting the process rather than just the end results, we hope to shed ligh

Application of a theoretical model to evaluate COPD disease management.

BACKGROUND: Disease management programmes are heterogeneous in nature and often lack a theoretical basis. An evaluation model has been developed in which theoretically driven inquiries link disease management interventions to outcomes. The aim of this study is to methodically evaluate the impact of a disease management programme for patients with chronic obstructive pulmonary disease (COPD) on process, intermediate and final outcomes of care in a general practice setting. METHODS: A quasi-experimental research was performed with 12-months follow-up of 189 COPD patients in primary care in the

Improved cardiac arrest outcomes: as time goes by?

In a recent issue of Critical Care, Mally and colleagues reported outcomes from an observational study of out-of-hospital cardiac arrests in Slovenia. Multivariable analysis identified independent predictors for hospital discharge, including higher end-tidal carbon dioxide (ETCO2) levels, higher mean arterial pressure (MAP) and the recency (years) of the arrest. ETCO2 has been previously demonstrated to correlate with cardiac index, and predict successful resuscitation. Initial ETCO2 reflects the initial adequacy of resuscitation, and the ETCO2 on admission to hospital reflects a number of fac

A methodology for preparing 'ideal' treatment outlines in psychiatry. The Quality Assurance Project.

The methodology of preparing treatment process criteria for agoraphobia and the depressive disorders is described. To avoid bias three sources of information were used: the results of a meta-analysis of the treatment outcome literature, the views of a sample of practising psychiatrists, and the views of a small representative expert committee. When criteria for schizophrenia and the neuroses have been completed the quality assurance project should serve two main purposes: first as an aid to continuing and postgraduate education and second as a basis for clinical review criteria required by pee

Core outcomes measures in dental computer vision studies (DentalCOMS).

OBJECTIVES: To improve reporting and comparability as well as to reduce bias in dental computer vision studies, we aimed to develop a Core Outcome Measures Set (COMS) for this field. The COMS was derived consensus based as part of the WHO/ITU/WIPO Global Initiative AI for Health (WHO/ITU/WIPO AI4H). METHODS: We first assessed existing guidance documents of diagnostic accuracy studies and conducted interviews with experts in the field. The resulting list of outcome measures was mapped against computer vision modeling tasks, clinical fields and reporting levels. The resulting systematization fo

The implementation of guided Internet-based cognitive behaviour therapy for panic disorder in a routine-care setting: effectiveness and implementation efforts.

Panic disorder is a common mental disorder. Guided Internet-based cognitive behavioural therapy (Guided Internet-based cognitive behaviour therapy (ICBT)) is a promising approach to reach more people in need of help. In the present effectiveness study, we investigated the outcome of guided ICBT for panic disorder after implementation in routine care. A total of 124 patients were included in the study, of which 114 started the treatment. Large within-group effect sizes were observed on the primary panic disorder symptoms (post-treatment: d = 1.24; 6-month follow-up: d = 1.39) and moderate and l

The effects of low-intensity pulsed ultrasound on fresh fracture: A meta-analysis.

BACKGROUND: Low-intensity pulsed ultrasonography (LIPUS) is a form of mechanical stimulation that is delivered via a special device to the fracture site for the acceleration of fracture healing. We conducted a meta-analysis to assess the effect of LIPUS for fresh fractures in adults. METHODS: MEDLINE, EMBASE and the Cochrane Library searched between Jan 1980 and Nov 2016. Studies should be quasi-randomized and randomized controlled trials (RCTs) comparing treatment with LIPUS to placebo or no treatment in adults with fresh fractures, reporting outcomes such as function; time to union; delayed

Differential responses of positive affect, negative affect, and worry in CBT for generalized anxiety disorder: A person-specific analysis of symptom course during therapy.

INTRODUCTION: Research indicates that individuals with generalized anxiety disorder (GAD) may experience deficits in positive affect (PA), and tend to dampen or intentionally suppress PA. Despite the presence of PA-related pathology in GAD, little is known about change in PA during GAD treatment. OBJECTIVE: This study examines changes in PA, negative affect (NA) and worry in seven participants during cognitive behavioral therapy (CBT) for GAD. METHOD: Intensive repeated measures (i.e., time series) data were subjected to person-specific regression analysis to delineate individual change traj

The Intersession Process in Psychotherapy for Anorexia Nervosa: Characteristics and Relation to Outcome.

OBJECTIVE: The "inter session process" (ISP) is defined as therapy-related conscious thoughts, memories, and emotions that patients and therapists experience between psychotherapy sessions. It indicates how the participants process and use treatment. The main aim of this study is to describe the ISP characteristics of patients in outpatient treatment for anorexia nervosa (AN). An additional aim is to explore the relation between patients' ISP and treatment outcome. METHOD: Patients taking part in a randomized controlled trial on outpatient psychotherapy for AN (ANTOP) filled in the Intersessi

Fontan Operation: Indications, Short and Long Term Outcomes.

Fontan operation, since its original description, has undergone a number of modifications so that it is now a staged, total cavo-pulmonary connection with fenestration. Stage I is palliation, depending upon the pathophysiology of the defect complex in early life, Stage II is bidirectional Glenn at about the age of 6 mo and Stage III is transfer of inferior vena caval blood to the pulmonary circuit along with fenestration between 2 to 4 y. Any patient that has only one functioning ventricle is a candidate for Fontan surgery. The morbidity and mortality have remarkably improved since the institu

Inside the black box: measuring addiction treatment services and their relation to outcomes.

The adoption of performance-based management has been under consideration by addiction treatment funding agencies, and, recently, many state and county agencies have developed performance-based measurement/management systems in an attempt to improve their treatment system. This article describes one such effort in Los Angeles County, California. The Performance-Based Pilot Project linked treatment encounters (counseling sessions, drug testing, case management, and methadone dosing) with client outcomes (abstinence or reduced drug use at discharge) and longer lengths of stay in treatment. Eleve

Outcome of cataract surgery. II. Visual functioning in aphakic patients.

The degree rehabilitation obtained by cataract surgery was evaluated by visual acuity and by Visual Functioning Index, which measures the patients' ability of performance of every-day tasks that are dependent on vision. Examinations and Index-scoring were done pre- and post-operatively and at one year follow-up in a non-selected, consecutive series of 114 cataract patients. 82% of the patients obtained a final visual acuity of 0.5 or more. Index scoring at follow-up was significantly improved and indicated that 74% had achieved normal or near-normal visual functioning. Furthermore, we compared

An operant reinforcement method of treating depression.

A method of treating depression based on an operant reinforcement perspective was evaluated with 29 depressed persons. The procedure included increased reinforcer utilization, behavioral contrast, overcorrection, reinforcer sampling and stimulus control by positive events. Also, training was given in skills related to the cause of the depression such as communication, job-finding, marital and study skills. A behavioral measure of individual goal attainment was taken in addition to three common subjective measures. Improvement occurred after the first two sessions, increased with successive ses

Evaluation of outcomes of non-nurse midwives: matched comparisons with physicians.

The purpose of this study was to assess the practice of a group of non-nurse midwives by comparing the outcomes of their deliveries to the standard of hospital-based physicians. This was done by means of a matching procedure designed to equate the populations on major medical risk factors. The first phase of the study used all physicians and found major differences favoring the midwives. The second phase of the study used the least interventionist half of the physician group and found minimal differences between midwives at home and physicians in the hospital. It is suggested that excessive in
