[Otosclerosis in children (author's transl.)].

11 children (13 ears) have been operated upon for otosclerosis. In ten cases the stapes footplate was found totally fixed, with histological verification of the diagnosis of otosclerosis. Conservative surgery of the oval window was possible in only four cases. All other patients required total removal of staples or footplate. Findings indicate that clinical otosclerosis occurring in childhood results in an otosclerotic process in the oval window region with total fixation of the footplate. Thus, correction usually requires removal of at least the entire footplate.

Temporal bone histopathologic findings in congenital anomalies of the oval window.

The histopathologic findings in five temporal bones from three cases showing various stages of oval window development arrest are presented. The anomalies ranged from complete absence of the oval window to congenital cartilaginous fixation of the stapedial footplate. Surgical approaches fro establishment of a new oval window must take into consideration the frequent association of facial nerve anomalies with anomalies of the oval window.

Analysis of middle ear mechanics and application to diseased and reconstructed ears.

OBJECTIVE: To review current concepts of the mechanical processes of the human middle ear, and to apply them to practical issues in clinical otology and tympanoplasty surgery. BACKGROUND: The wide range of conductive hearing losses associated with middle ear pathology and reconstruction cannot be adequately explained by simple models of middle ear function. METHODS: Variables used to describe the system are sound pressure, volume velocity, and acoustic impedance. The relationship between specific middle ear structures and these variables is described such that inferences can be drawn regardi

Microfiberoptic evaluation of the middle ear cavity.

Endoscopic instruments have revolutionized surgical diagnosis and treatment. Recently, a high resolution microfiberoptic endoscope has been developed that has vast potential for otologic use. This microfiberoptic endoscope was used in cadaver and human studies to visualize the middle ear cavity. The technique used involved placing a 1.0-mm or smaller microfiberoptic scope into the middle ear via a tympanic membrane perforation, through a myringotomy tube or up the eustachian tube. Using the scope, the mesotympanum and hypotympanum can be well visualized. Similarly, the round window, oval windo

A case of congenital bilateral stapes agenesis.

Congenital absence of the stapes and the oval window is an anomaly reported in only sporadic cases. We present a 17-year-old male patient with congenital bilateral conductive hearing loss. The external auditory canal and the tympanic membrane appeared normal on both sides. An exploratory tympanotomy in the right ear revealed dehiscence of the the horizontal segment of the facial nerve, which was displaced inferiorly, occupying the area of the absent oval window. The stapes was totally absent and a malformed long process of the incus was attached by a fibrous band to the promontory. Manipulatio

Dehiscences of the horizontal segment of the facial canal in otosclerosis.

Dehiscences of the bony horizontal segment of the facial canal are rather common in human adults. These facial canal dehiscences occur most often in the region of the oval window. This study presents the observations of the facial canal in the oval window region in 427 operations for otosclerosis. The incidence of dehiscences in the facial canal to the middle ear space in otosclerosis was studied. Dehiscences were observed in 14 of the 427 patients (3.27%) who had a stapedotomy. This incidence is very low compared to the anatomical studies of the facial nerve in human cadaver temporal bones. D

Congenital absence of the oval window.

From 1983 to 1987, seven patients with conductive hearing loss were found to have congenital absence of the oval window. Six of these patients underwent vestibulotomies and reconstruction with House wires or total ossicular reconstruction prostheses. Surgical findings included absent or rudimentary stapes in all cases, abnormal facial nerves in four cases, and abnormal long processes of the incus in four cases. All patients had normal external ear canals and tympanic membranes. Hearing initially improved 20 to 45 dB in four of the six patients reconstructed. Over time, however, much of this in

Intracochlear Drug Delivery Through the Oval Window in Fresh Cadaveric Human Temporal Bones.

HYPOTHESIS: Drug delivered to the oval window can diffuse to the apex of the human cochlea. BACKGROUND: We have previously demonstrated that zoledronate, a nitrogen-containing bisphosphonate, can arrest the sensorineural hearing loss in cochlear otosclerosis. We have also shown that, in animals, delivery of bisphosphonate into the cochlea can dramatically increase delivery efficiency. Intracochlear drug delivery has the potential to increase local concentration of drug while decreasing the risk of systemic toxicity. In the present study, a fluorescently labeled bisphosphonate compound (6-FAM-

Cerebrospinal fluid leak after acoustic neuroma surgery.

BACKGROUND: CSF rhinorrhea is a common complication of acoustic neuroma surgery. METHODS: Twenty cases of persistent cerebrospinal fluid rhinorrhea, 2 of transient leaks, and 2 of late meningitis occurring in a series of 156 posterior fossa acoustic neuroma removals were studied. Prophylaxis of leaks was usually attempted by filling opened air cells in the drilled internal auditory canal (IAC) with bone wax. Communications were repaired by rewaxing the IAC, or by fat packing of either the mastoid or the eustachian tube, middle ear, and mastoid. RESULTS: Most leaks were diagnosed within 1 wee

Combined Endoscopic/Microscopic Cochlear Implantation Through the Oval Window.

Standard round window (RW) cochlear implantation is a well-described technique. Implantation might be difficult in patients with inner and middle ear anomalies, in some cases because of not achieving adequate exposure to the RW, with a related higher risk of complications such as facial nerve injury. It is proposed a combined microscopic/endoscopic oval window approach in a 63 year old man affected by bilateral Menière disease, with bilateral severe sensorineural hearing loss, speech discrimination score for bysillabic words under 40% and a hidden RW by anomalous facial nerve course. Perilymphatic fistulas in children: rationale for therapy.

We report 26 consecutive patients (32 ears) who were identified in a 2 year period (July 1, 1985-June 30, 1987) with unexplained sudden, fluctuating, or progressive sensorineural hearing loss (SNHL). All patients underwent an exploratory tympanotomy and a perilymphatic fistula was identified in 13 patients (14 ears). The mean change of 14 +/- 27 dB in speech reception threshold before and after surgery was significant at p = 0.08 among children with fistula and ranged from -30 to 80 dB. In children with sudden, progressive or fluctuating SNHL and multiple sensory deficits, including blindness

[Recurrent meningitis in inner ear malformations].

Authors present two cases of children with reccurent meningitis and unilateral deafness. Implemented diagnostics (CT, NMR, ABR) revealed one side inner ear congenital malformation in one case and anterior fossa bony defect accompanied by labirynthine deformation in the other case. The presence of perilymphatic fistulae in oval and round windows and cerebrospinal fluid leakage has been confirmed in both cases during surgery. Carefull obliteration of the Eustachian tube and both windows has been performed. Non- complicated postoperative course (2 months and 6 years - respectively) has prooved th

[Temporal bone histopathology in a patient suspected of inner ear extension of otitis media].

We report the histopathological findings in the temporal bone of a 30-year-old female who died of cervical esophageal carcinoma. The temporal bone sections revealed severe bilateral suppurative labyrinthitis and otitis media that presumably occurred immediately before her death. Many inflammatory cells were present in the middle ear, particularly around the stapes and the round window niche. They had also infiltrated the inner ear via the annular ligament of the stapes and the round window membrane. Inflammatory cell accumulation was also observed in the peri- and endolymphatic spaces, and it

Conductive hearing loss induced by experimental middle-ear effusion in a chinchilla model reveals impaired tympanic membrane-coupled ossicular chain movement.

Otitis media with effusion (OME) occurs when fluid collects in the middle-ear space behind the tympanic membrane (TM). As a result of this effusion, sounds can become attenuated by as much as 30-40 dB, causing a conductive hearing loss (CHL). However, the exact mechanical cause of the hearing loss remains unclear. Possible causes can include altered compliance of the TM, inefficient movement of the ossicular chain, decreased compliance of the oval window-stapes footplate complex, or altered input to the oval and round window due to conduction of sound energy through middle-ear fluid. Here, we

[Intraoperative application of optical coherence-tomography (OCT) for visualization of the oval window niche].

BACKGROUND: CT and MRI of the windows of the temporal bone have become an important tool in the analysis of malformation, trauma and chronic otitis media. Optical Coherence-Tomography (OCT) provides optical cross-sections of the tissue, comparable to ultrasound. In a study on temporal bone specimens and during middle ear surgery we tested, whether OCT provides information about the oval window niche. MATERIAL AND METHODS: OCT was performed ex vivo on five human temporal bone preparations, in which the oval window niche was exposed and in vivo in eight patients who underwent middle ear surgery
