COVID-19-The impact of variable and "low normal" pulse oximetry scores on Oximetry@Home services and clinical pathways: Confounding variables?

COVID-19 Oximetry@Home services have been commissioned nationally. This allows higher-risk patients with mild COVID-19 symptoms to remain at home, being supplied with a Pulse Oximeter to measure their oxygen saturation (SpO2 ) two to three times daily for two weeks. Patients record their readings manually or electronically which are monitored by a clinical team. Clinical decisions, using an algorithm, are based on SpO2 readings in a narrow range with 1-2 point changes potentially affecting care. In this article, we discussed the problem that multiple factors affect SpO2 readings, and that some

Ear oximetry: a noninvasive method for detection of patent foramen ovale: a study comparing dye dilution method and oximetry with contrast transesophageal echocardiography.

BACKGROUND AND PURPOSE: Patent foramen ovale (PFO) may play an important role as a risk factor for ischemic stroke and some other neurological conditions. There is a need for low-cost and noninvasive methods for the detection of PFO. This study evaluates the accuracy of two simple bedside tests, the dye dilution method and ear oximetry, in the detection of PFO. METHODS: Dye dilution curves and ear oximetry recordings with a noninvasive ear densitometer were obtained from consecutive cryptogenic stroke patients referred for contrast transesophageal echocardiography (TEE). All test results were

Generalized self-calibrating probe approach for CW tissue oximetry.

SIGNIFICANCE: Functional near-infrared spectroscopy (fNIRS) and oximetry are important noninvasive methods to investigate physiological processes and states in the human brain and muscle tissue. Self-calibrating probe methods combine two light sources with typically two detectors to improve the stability of continuous-wave (CW) devices, but so far are limited to fully symmetric geometries. AIM: We developed a generalized self-calibrating probe approach to improve accuracy and stability in CW oximetry for arbitrary configurations of two light sources and detectors. APPROACH: We used the diffu

Feasibility of Obtaining Pulse Oximetry Readings From the Oropharynx.

BACKGROUND: As access to monitoring sites on the hands and feet may be limited due to surgical procedures, burns, trauma, or injuries, alternative sites for the measurement of pulse oximetry may be needed. AIMS: The current study assesses the feasibility of obtaining pulse oximetry measurements from the oropharynx, using a pulse oximeter probe attached to a standard oral airway. METHODS: Two cohorts were studied, each including 50 patients, with the oximeter facing toward the palate (cohort 1) or the tongue (cohort 2). RESULTS: The oropharyngeal oximeter functioned effectively with an absol

Critical discrepancies in oximetry among patients with Raynaud's phenomenon.

INTRODUCTION: Systemic sclerosis (SSc) is associated with a high frequency of pulmonary complications, making oxygen saturation critical for monitoring purposes. However, there are significant difficulties, such as skin thickening and edema, digital ulcers, and Raynaud's phenomenon (RP), which may lead to inaccurate and unreliable conventional digital readings. The objective of the present study is to assess the concordance of oxygen saturation at different anatomical sites, including the forehead, which has been proven to be unaffected by vascular changes, such as RP. METHODS: Patients with

SARS-CoV-2 infection, pulse oximetry, and interpretive caveats.

Hypoxaemia in COVID-19 does not necessarily imply COVID pneumonia or post-COVID lung fibrosis, and the caveats of finger pulse oximetry should be remembered. Drug-induced methaemoglobinemia should be considered in individuals with unexplained cyanosis, refractory hypoxaemia, or the presence of a saturation gap. Here, we share our recent encounter of 'spurious hypoxia' in a patient with COVID-19 and methaemoglobinemia.

A novel sleep oximetry scoring tool for pediatric laryngomalacia.

OBJECTIVE: Despite being a more accessible and less resource intensive modality than polysomnography, the utility of sleep oximetry (SO) in pediatric laryngomalacia (LM) is not well understood or validated. We aimed to retrospectively evaluate the utility of overnight home based SO in children with LM by developing and internally validating the Modified Laryngomalacia Oximetry Score (MLOS) scoring system to triage severity and guide clinical decision making. METHODS: We evaluated pediatric patients with a diagnosis of LM at our tertiary referral centre. Data from initial and post-treatment SO

Retinal vessel oximetry in children with inherited retinal diseases.

BACKGROUND: Retinal vessel oximetry (RO) has been used to show altered metabolic function in patients with inherited retinal diseases (IRDs). The aim of this study was to investigate RO parameters of children with IRDs and presumed IRD carriers (pIRDc) and to compare them to controls. METHODS: In this cross-sectional cohort study, 142 eyes from 71 Caucasian subjects were included: 40 eyes with IRDs, 26 eyes with pIRDc and 76 control eyes. The oxygen saturation was measured with the Retinal Vessel Analyser (IMEDOS Systems UG, Jena, Germany). Mean oxygen saturations in the peripapillary retinal

HIGH LIFE: High altitude fatalities led to pulse oximetry.

In 1875, Paul Bert linked high altitude danger to the low partial pressure of oxygen when 2 of 3 French balloonists died euphorically at about 8,600 m altitude. World War I fatal crashes of high altitude fighter pilots led to a century of efforts to use oximetry to warn pilots. The carotid body, discovered in 1932 to be the hypoxia detector, led to most current physiologic understanding of the body's respiratory responses to hypoxia and CO2. The author describes some of his UCSF group's work: In 1963, we reported both the brain's ventral medullary near-surface CO2 (and pH) chemosensors and the

Harnessing color vision for visual oximetry in central cyanosis.

Central cyanosis refers to a bluish discoloration of the skin, lips, tongue, nails, and mucous membranes, and is due to poor arterial oxygenation. Although skin color is one of its characteristic properties, it has long been realized that by the time skin color signs become visible, oxygen saturation is dangerously low. Here we investigate the visibility of cyanosis in light of recent discoveries on what color vision evolved for in primates. We elucidate why low arterial oxygenation is visible at all, why it is perceived as blue, and why it can be so difficult to perceive. With a better unders

Intra-operative pulse oximetry can help determine intestinal viability.

The intra-operative assessment of intestinal viability when dealing with ischaemic bowel remains a challenge. Reliable healing of small-bowel anastomoses, using pulse oximetry to exclude critical ischaemia, has been shown in the canine model. In this study, intra-operative pulse oximetry (PO) was used to help determine intestinal viability 48 h after de-torsion of a volvulus. Approximately one-half of the ischaemic, volved bowel was able to be preserved. Intra-operative PO can thus help preserve bowel of doubtful viability.

Regional oximetry for diagnosing compartment syndrome: a scoping review.

PURPOSE: Diagnosis of compartment syndrome remains challenging, as intracompartmental pressure (ICP) monitoring measures mechanical pressure rather than tissue perfusion. Near-infrared spectroscopy (NIRS) enables non-invasive, continuous assessment of tissue oxygen saturation (StO2), potentially identifying ischemia earlier. However, its diagnostic accuracy remains uncertain. METHODS: Following PRISMA-ScR guidelines, PubMed, EMBASE, Cochrane Library, ClinicalTrials.gov, and WHO-ICTRP were searched to April 2025 for studies evaluating NIRS in acute (ACS) or chronic exertional (CECS) compartmen

Retinal oximetry: new insights into ocular and systemic diseases.

Retinal oximetry, which refers to the measurement of the oxygen saturation of haemoglobin in retinal blood, has emerged as a promising tool for understanding ocular and systemic diseases over the past few decades. Advances in traditional dual-wavelength measurement techniques, as well as their integration with more advanced technologies, have driven significant progress in the field. Researchers have utilised commercially available devices to explore the applications of retinal oximetry in both healthy individuals and various disease states. To provide a comprehensive overview of the current s

Can Oximetry-Derived Parameters Predict 3-Year Cardiovascular Mortality?

As obstructive sleep apnoea (OSA) is associated with cardiovascular disease (CVD), we are keen to investigate whether future CVD outcomes can be predicted by measuring OSA-associated hypoxemia. The apnoea-hypopnoea index (AHI), the gold standard for OSA diagnosis, has been shown to be inadequate for predicting CVD outcomes, and studies have explored oximetry-derived parameters as potential predictors. While many studies have proposed individual oximetry-derived parameters as promising predictors, the combined predictive performance has not been thoroughly investigated. This study aims to evalu

Retinal oximetry and microvascular assessment after hypertensive pregnancy complications.

PURPOSE: Women with hypertensive disorders of pregnancy (HDP) are at increased risk of developing premature cardiovascular disease (CVD). The mechanisms behind this are not fully understood, but microvascular alterations have been documented in retinal arterioles and venules. The aim of this study was to use non-invasive retinal imaging to investigate the structural and functional properties of arterioles, venules and capillaries in this patient group. METHODS: We examined 27 women with previous HDP and 23 controls at 3 years postpartum. The retinal microvasculature was assessed by vessel cal

Newborn Pulse Oximetry for Infants Born Out-of-Hospital.

BACKGROUND AND OBJECTIVES: Conventional timing of newborn pulse oximetry screening is not ideal for infants born out-of-hospital. We implemented a newborn pulse oximetry screen to align with typical midwifery care and measure its efficacy at detecting critical congenital heart disease. METHODS: Cohort study of expectant mothers and infants mainly from the Amish and Mennonite (Plain) communities with limited prenatal ultrasound use. Newborns were screened at 1 to 4 hours of life ("early screen") and 24 to 48 hours of life ("late screen"). Newborns were followed up to 6 weeks after delivery to
