The current status of neonatal extracorporeal membrane oxygenation.

Marked changes have occurred in the practice of neonatal extracorporeal membrane oxygenation (ECMO) since the first survivor in 1975. Coagulation management has been markedly refined, new catheters allow ECMO to be done either in a venoarterial or venovenous (VV) mode, depending on cardiac function in the infant. A new design of the VV catheter will allow this technique to be used in more infants in the future. New therapies for respiratory failure have changed the complexion of the population being treated with ECMO. The 34 to 36 week gestation infant with respiratory distress syndrome and/or

Extracorporeal membrane oxygenation in the treatment of inhalation injuries.

Smoke inhalation is a leading cause of death in burns patients. Conventional ventilation cannot always maintain adequate tissue oxygenation. Extracorporeal membrane oxygenation (ECMO) has rarely been used in the treatment of smoke inhalation injuries. ECMO is a proven therapy in severe neonatal respiratory failure and has also been used to good effect in the treatment of children and adults. We report the cases of two children who developed severe respiratory failure refractory to maximal ventilation, one case followed smoke inhalation alone, the other followed smoke inhalation and burns. Extracorporeal membrane oxygenation for postcardiotomy cardiogenic shock.

BACKGROUND: Extracorporeal membrane oxygenation circuits have recently been introduced for extracorporeal life support (ECLS) in adult patients in cardiogenic shock and have been shown to provide excellent oxygenation and hemodynamic support. Heparin coating of the extracorporeal circuit provides a more biocompatible surface, which has been shown to minimize early surface-induced complement activation and platelet dysfunction and hence may improve patient survival. This report reviews our experience with extracorporeal membrane oxygenation to treat postcardiotomy cardiogenic shock using minima

[Extracorporeal membrane oxygenation in newborn infants and children].

The purpose of this report is to describe our experience with veno-arterial (VA) and veno-venous extracorporeal membrane oxygenation (VV-ECMO) for neonates and children with severe acute respiratory or cardiocirculatory failure. From 1990 to 1994 20 neonates and 12 children were treated at the ECMO center in Graz. Indications for ECMO were acute respiratory failure in 27 patients and cardiocirculatory failure in 5 patients. Mean duration of ECMO was 228 +/- 30 hours. Fifteen neonates were weaned from ECMO and were subsequently extubated. Of the 12 children 7 had severe acute respiratory failur

Use of extracorporeal membrane oxygenation in the septic neonate.

Extracorporeal membrane oxygenation (ECMO) is widely used for cardiopulmonary support in neonates with cardiopulmonary failure secondary to overwhelming sepsis. The purpose of this study was to examine the effects of culture status on the eventual outcome of septic neonates requiring ECMO support. Data from the Extracorporeal Life Support Organization (ELSO) for the tears 1990 through 1992 inclusive were collected and analyzed for all neonates with a primary diagnosis of sepsis. Records were reviewed for gestational age, birth weight, culture status and isolated organism, last arterial blood g

Blood use during extracorporeal membrane oxygenation.

An analysis of the transfusion records of 91 neonatal patients subjected to extracorporeal membrane oxygenation (ECMO) is reported. Mean daily blood usage was 250 mL of red cells (RBCs), 80 mL of fresh-frozen plasma, and 2 units of platelets. Average time on ECMO was 4.6 days. Group O or ABO type-specific RBCs and group AB or ABO type-specific plasma products and platelets were transfused. RBCs were not washed, and neither RBCs nor other components were tested for anticytomegalovirus (CMV) or irradiated. No cases of posttransfusion CMV infection or graft-versus-host disease were observed. Hemo

[Present status and future observation of membrane oxygenator].

The oxygenator has been developed as the most important tools for open heart surgery as a part of pump-oxygenator. Recently, two kinds of them are routinely selected. One is the bubble oxygenator (BO) and the other is the membrane oxygenator (MO). The latter is recognized as the most physiological and similar to the human lung. It is proved by many researchers that the denaturation of the blood elements is less in the MO compare with the BO even it is used for short period such as open heart surgery. The more hospitals and institutes have been taking more MO than BO. Microporous membrane and d

Tension pneumothorax during extracorporeal membrane oxygenation.

Life-threatening tension pneumothorax in neonates on extracorporeal membrane oxygenation (ECMO) has been associated with an increase in arterial oxygen tension and a decrease in peripheral perfusion, followed by a decrease in ECMO flow with progressive hemodynamic deterioration. To investigate this triad, chest tubes were placed bilaterally in 9 dogs to allow injection of air to produce tension pneumothorax. Six dogs were subsequently placed on standard venoarterial ECMO before the reinduction of tension pneumothorax. Measured values included arterial pulse pressure, inferior vena cava pressur

Extracorporeal membrane oxygenation experience at the University of Pittsburgh.

Between January, 1981, and May, 1985, 33 infants suffering from acute cardiorespiratory failure were treated with extracorporeal membrane oxygenation (ECMO) when all other forms of conventional management had failed. Only the patients with respiratory failure that was thought to be reversible were treated. Prolonged conventional respiratory management (more than five days) was considered a contraindication to ECMO support because of irreversible damage to the lungs caused by the barotrauma associated with conventional ventilation. Eighteen of the 33 patients (54%) survived and were discharged

Extracorporeal Membrane Oxygenation Ethics: A Call for Action.

TOPIC IMPORTANCE: Extracorporeal membrane oxygenation (ECMO) is a relatively novel, intensive medical intervention. It is known to raise important yet unsettled ethical concerns. OVERVIEW: We established the ECMO Ethics Workgroup in 2021 to improve our understanding of these ethical issues and their possible resolutions. This paper explains the methods used to produce our insights and recommendations, which additional papers from the project elaborate on in detail. Initially, over 6 months, the 37 group members described the full range of ethical issues in ECMO care they were familiar with fr

Condensation in a Membrane Oxygenator Analyzed by Bronchoscopy and Ultrasonography.

OBJECTIVE: To elucidate the mechanism of condensation-related oxygenator dysfunction (CROD) and explore preventive strategies, we directly visualized the interior of a membrane oxygenator using bronchoscopy and ultrasonography and evaluated the effect of external warming. METHODS: The CROD model consisted of a circuit incorporating a membrane oxygenator perfused with water at 3 L/min and supplied with gas (50% O2 in air) at 2 L/min. The hollow fiber bundle was observed bronchoscopically at the outlet surface and ultrasonographically from the lateral aspect. The percentage droplet area was qua

Litigation associated with the use of extracorporeal membrane oxygenation.

IntroductionExtracorporeal membrane oxygenation (ECMO) is a high-risk treatment but the legal risk surrounding its use is unknown.MethodsRetrospective review of the Westlaw legal database was used to identify eligible cases. The reason for litigation (directly related to ECMO or not directly related to ECMO) and other case details were abstracted. Descriptive statistics were calculated.ResultsA total of 28 cases were identified, nine related to ECMO and 19 not related to ECMO. Eight cases were settled out of court, while seven had a plaintiff verdict and 13 had a defense verdict. The median pa

Severe cholestasis in neonatal extracorporeal membrane oxygenation.

INTRODUCTION: Cholestasis is a common complication of Extracorporeal Membrane Oxygenation (ECMO) secondary to patient physiology and circuit-induced factors. In our institution's Neonatal Intensive Care Unit (NICU), we noted cases of severe cholestasis, with peak conjugated bilirubin levels much higher than previously reported in the literature. The objective of our study was to identify the contributing factors to the development of severe cholestasis in neonatal ECMO cases. METHODS: Using our institutional ECMO database, all neonates who received ECMO at our institution were identified. A r

Thromboelastography in Children Undergoing Extracorporeal Membrane Oxygenation.

BACKGROUND: Children undergoing extracorporeal membrane oxygenation (ECMO) are at risk of hematologic and coagulation complications, including intracranial hemorrhage. Thromboelastography is used to monitor anticoagulation in patients receiving ECMO, but no data for its use in children are available. OBJECTIVE: To evaluate the incidence of complications when managing anticoagulation with thromboelastography or with conventional coagulation tests (activated clotting time, plasma fibrinogen level, and platelet count) among children receiving venoarterial or venovenous ECMO. METHODS: This singl

Research progress of portable extracorporeal membrane oxygenation.

INTRODUCTION: Extracorporeal membrane oxygenation (ECMO) is primarily used for the supportive treatment of patients suffering from severe cardiopulmonary failure. With the continued development of ECMO technology, the relevant scenarios also extend pre-hospital and inter-hospital. In order to meet the needs of emergency treatment in communities, disaster sites and battlefields, inter-hospital transfer and evacuation; miniaturized and portable ECMO has become a current research hotspot. AREA COVERED: The paper first introduces the principle, composition and common modes of ECMO and summarizes

Hemostatic disorders associated with extracorporeal membrane oxygenation.

Hemostatic disorders are common during extracorporeal membrane oxygenation (ECMO)-therapy. This includes both bleeding and thrombotic complications. Particularly bleeding is often associated with fatal outcome. The early identification of hemorrhagic diathesis and the diagnosis of the underlying pathology are essential. A distinction into device-, disease-, and drug-related disorders appears reasonable. However, both correct diagnosis and therapy can be challenging and sometimes counterintuitive. Since bleeding seems to be more frequent and dangerous compared to thrombosis, the understanding o

The Hematological Effects of Extracorporeal Membrane Oxygenator Exchange.

Membrane oxygenator failure during venovenous (V-V) extracorporeal membrane oxygenation (ECMO) can lead to life-threatening hypoxia, high replacement costs, and may be associated with a hyperfibrinolytic state and bleeding. The current understanding of the underlying mechanisms that drive this is limited. The primary aim of this study therefore is to investigate the hematological changes that occur before and after membrane oxygenator and circuit exchanges (ECMO circuit exchange) in patients with severe respiratory failure managed on V-V ECMO. We analyzed 100 consecutive V-V ECMO patients usin

Extracorporeal Membrane Oxygenation in Acute Respiratory Failure.

Venovenous (VV) extracorporeal membrane oxygenation (ECMO) is a form of mechanical life support that provides full respiratory bypass in patients with severe respiratory failure as a bridge to recovery or lung transplantation. The use of ECMO for respiratory failure and capable centers offering ECMO has expanded over the years, increasing its availability. As VV-ECMO provides an artificial mechanism for oxygenation and decarboxylation of native blood, it allows for an environment in which safer mechanical ventilatory care may be provided, allowing for treatment and resolution of underlying res
