Red cell transfusion benefits in oncology, haematology and palliative medicine populations: a narrative review.

Due to the heterogenous nature of the palliative medicine patient population, assessment of benefit, and thus choice of appropriate patient for consideration of transfusion, can be challenging. This can be confounded by the use of both liberal and restrictive transfusion thresholds. The multifactorial nature of many symptoms of anaemia, particularly in patients with advanced malignancy, can further complicate. As such, there is a paucity of data supporting the subjective, objective and clinical benefit of red cell transfusion in the palliative medicine setting. This narrative review summarises

Equipping future physicians to recognize a palliative approach to care: A mixed methods study on palliative care training needs in internal medicine residency.

BACKGROUND: A palliative approach to care emphasizes holistic assessment and treatment planning for individuals with life-limiting illnesses. Despite Canadian national frameworks and competency guidelines, internal medicine (IM) residents often report limited exposure and comfort with palliative care. OBJECTIVE: The purpose of this investigation is to assess the training needs of IM residents at McMaster University and inform future curriculum development. DESIGN: A sequential mixed-methods design was employed. Both quantitative survey and qualitative interview data were reviewed together to

Physicians' Attitudes Toward Defensive Medicine and Medical Errors in Palliative Care: A Cross-Sectional Study.

RATIONALE: Medical errors represent a major issue for healthcare professionals. Although they may not always cause direct harm to patients, timely reporting of such errors is an important step in improving safety and reducing malpractice risk. As the number of these lawsuits rises, physicians often resort to defensive medicine practices. AIMS AND OBJECTIVES: The aim of this study is to evaluate the attitudes, behaviours and knowledge levels of physicians working in palliative care services with resprect to defensive medicine. METHOD: This multicenter, cross-sectional and analytical study, 11

Exploring psilocybin's role in mental health and palliative medicine: a path to improved well-being.

INTRODUCTION: Although long known for their psychoactive effects, psychedelic drugs have only recently been investigated for medicinal use. Psilocybin has attracted the greatest interest with studies suggesting that it may be a useful agent in psychiatry and in palliative care. AREAS COVERED: Clinical trials that included psilocybin were searched in PubMed, Embase, and ClinicalTrials.gov, demonstrating that adult psychiatry and palliative care are the medical fields that show the greatest interest in psilocybin treatment. EXPERT OPINION: Psilocybin is a powerful drug that needs to be used wi

Immunoassay Urine Drug Testing among Patients Receiving Opioids at a Safety-Net Palliative Medicine Clinic.

BACKGROUND: Few studies have examined the use of immunoassay urine drug testing of cancer patients in palliative care clinics. OBJECTIVES: We examined the frequency of immunoassay urine drug test (UDT) abnormalities and the factors associated with aberrancy at a safety-net hospital palliative medicine clinic. METHODS: A retrospective review of the electronic medical records of consecutive eligible patients seen at the outpatient palliative medicine clinic in a resource-limited safety-net hospital system was conducted between 1 September 2015 and 31 December 2020. We collected longitudinal da

Effectiveness of Long-Term Opioid Therapy for Chronic Pain in an Outpatient Palliative Medicine Clinic.

Background: Despite widespread use of opioid therapy in outpatient palliative medicine, there is limited evidence supporting its efficacy and safety in the long term. Objectives: We sought to improve overdose risk scores, maintain pain reduction, and preserve patient function in a cohort with severe chronic pain as we managed opioid therapy for a duration of four years in an outpatient palliative care clinic. Design: Over four years, we provided ongoing goal-concordant outpatient palliative care, including opioid therapy, using quarterly clinical encounters for a patient cohort with chronic pa

Precision medicine for older adults with multimorbidity through a whole-person and palliative care approach.

As populations age globally, multimorbidity, defined as the coexistence of two or more chronic conditions, has become highly prevalent. While precision medicine holds promise for tailoring interventions based on genetics, environment, lifestyle, and other individual characteristics, its application in older adults with multimorbidity remains limited largely narrowly biomedical, disease-specific, and insufficiently aligned with palliative care principles. Such approaches are poorly suited to the complexity, vulnerability, prognostic uncertainty, and existential challenges characteristic of agin

Simulation Topics for Palliative Medicine Specialty Training: A Two-Phase Needs Assessment for Curriculum Development.

BACKGROUND: Simulation-based learning is increasingly integrated into medical education; however, the use of simulation in palliative medicine is generally limited to ad hoc communication skills and pain management. There is a recognized need for a structured approach to a simulation-based curriculum aligned with the unique challenges in complex symptom management and emergencies in palliative medicine specialty training. OBJECTIVES: To identify and prioritize key clinical topics for a simulation curriculum for palliative medicine specialty training. METHODS: Using Kern's six-step approach t

Palliative Care Competencies and Readiness for Independent Practice: A Report on the American Academy of Hospice and Palliative Medicine Review of the U.S. Medical Licensing Step Examinations.

CONTEXT: It is unknown whether the palliative care (PC) content tested in the U.S. Medical Licensing Examination (USMLE) step examinations reflects the consensus-developed PC competencies. OBJECTIVES: To review the USMLE step examinations to determine whether they test the PC knowledge necessary for graduating medical students and residents applying for licensure. METHODS: Eight PC physicians reviewed three complete examination forms and a focused 509-item bundle of multiple-choice questions (MCQs) identified by the USMLE content outline as potentially assessing PC content. Integrated models of palliative medicine and pain management for the treatment of cancer-related pain: a review.

PURPOSE OF THE REVIEW: Cancer-related pain remains one of the most common and distressing symptoms experienced by people with cancer. Increasing cancer prevalence, longer survival, and greater symptom complexity have highlighted the limitations of traditional, fragmented approaches to cancer pain management. This review examines contemporary models of integrated palliative medicine and pain management services, with particular emphasis on interdisciplinary care pathways and interventional pain techniques. RECENT FINDINGS: Recent literature supports the earlier integration of palliative medici

[Comparative study of end-of-life care in an internal medicine ward and a palliative care unit].

INTRODUCTION: Dued to ageing population and growing of chronic advanced illnesses, nowadays, the majority of deaths take place in hospitals. Terminal patients with cancer and other non-malignant diseases share the same type of problems which, once correctly accessed, may have similar approach. Inappropriate terminal care can lead to unnecessary suffering of patients and their families. METHODS: A retrospective chart review was done for the first 96 patients who died in 2005 due to advanced chronic illnesses in a Internal Medicine ward (48 patients) and in a Palliative Care unit (48 patients).

The South Had The Lowest Rates Of Hospice And Palliative Medicine-Certified Providers In The US, 2024.

To improve access to care for serious illness, policy makers need evidence on how workforce capacity aligns with the need for palliative care. This study evaluated the palliative care workforce and policy environments at the state level, using a new data source: the 2024 Center to Advance Palliative Care's comprehensive Serious Illness Scorecard. We describe variation in the palliative care workforce relative to serious illness deaths across states and by five policy types related to serious illness: serious illness task forces, continuing medical education (CME), pediatric Medicaid waivers, a

A National Core Community Palliative Care Medicines List for managing end-of-life symptoms.

BACKGROUND: Australians with life-limiting conditions can, with appropriate support, have a home death, if that is their choice. General practitioners (GPs) are essential for coordinating care and managing symptoms. Necessary medicines must be quickly available to provide responsive symptom management in the terminal phase. GPs can facilitate prompt availability of medicines by working collaboratively with pharmacists who have a shared understanding of the required medicines. OBJECTIVE: This article explains the development of, and provides information about, the National Core Community Palli

Pancreatic Cancer-Associated Pain Management Beyond Opioids: Integrating Oncological Treatment, Palliative Care, and Interventional Pain Medicine.

Pain is one of the most frequent and burdensome symptoms of pancreatic cancer and may substantially impair quality of life throughout the disease trajectory. Its complex and evolving etiology, involving visceral, neuropathic, somatic, and treatment-related mechanisms, frequently makes adequate pain control challenging. This critical narrative review provides a multidisciplinary overview of pancreatic cancer-associated pain management, integrating oncological treatment, palliative and pharmacological care, and interventional pain management into a practical clinical framework. Emergencies in palliative medicine: novel simulation-based curriculum for end-of-life crises and symptom management.

Hospice and palliative medicine (HPM) simulation training primarily focuses on communication skills, rather than symptom management. Simulation can be an effective educational tool in high stakes clinical settings and provides a safe environment to improve skills in crisis management. HPM providers are often called to manage refractory symptoms in emotionally charged situations, especially at end-of-life care. These scenarios may present infrequently during their 1-year fellowship training. We describe a novel two-part simulation series 'End of Life Symptom Management: Emergencies in Palliative

[Clinical ethics support with CURA. Acceptability and feasibility among medical practitioners in geriatric and palliative medicine].

INTRODUCTION: Moral conflicts are common within geriatric and palliative care and can lead to moral distress. Clinical ethics support helps healthcare providers in managing moral conflicts and moral distress. CURA is a low-threshold clinical ethics support instrument. The feasibility for nurses and nurse-assistants in palliative care has already been investigated. However, the acceptability and feasibility of CURA within medical practitioners is unclear. METHODS: Prospective cohort study, using mixed methods. Participants are medical practitioners working in geriatric or palliative care, who
