An isolated, closed, complex dislocation of the metacarpophalangeal joint of the long finger: a unique case.

A unique case of an isolated, closed, complex metacarpophalangeal (MP) joint dislocation of the long finger is reported. The deep transverse metacarpal ligaments play an integral role in the stability of the metacarpophalangeal (MP) joints because of their firm bilateral attachments to the palmar plate. The partial rupture of this ligament during the course of trauma prevents closed reduction. Essential correction consists of incision of the remaining fibers of the ligamentous connection to the palmar plate.

[Palmar wrist arthroscopy for evaluation of concomitant carpal lesions in operative treatment of distal intraarticular radius fractures].

Fractures of the distal radius, which currently are treated with palmar locking plates, are often accompanied by carpal lesions. Tears of the scapholunate interosseus ligament (SL) can affect the outcome. Between January 2007 and May 2008, 28 patients with distal intraarticular fractures of the radius were included in a prospective study. Preoperative CT-arthrography was performed. SL tears were found in 11 patients, with 10 partial and one complete rupture observed. A tear of the triangular fibrocartilage complex (TFCC) was detected in 16 patients. Every patient was operated with a palmar loc

A scoping review of conservative treatment approaches for volar plate injuries of the proximal interphalangeal joints in an acute health care setting.

BACKGROUND: A volar plate injury (VPI) of the proximal interphalangeal joints (PIP) is a common injury that occurs when a finger is hyperextended, rupturing or separating the thick ligament from the bone on the finger's palm side. The degree of damage determines the course of treatment. For stable injuries without dislocation, conservative treatment such as hand rehabilitation, splinting, and rest is typically recommended. It is crucial to explore the effectiveness of conservative treatment of VPIs, as evidence suggests that surgical treatment is often over-utilised in these cases. METHODS: A

Treatment of Thumb Metacarpophalangeal Joint Hyperextension by Volar Plate Advancement and Capsulodesis With Suture Tape Augmentation.

Basal joint arthritis is commonly associated with attenuation of the volar structures at the thumb metacarpophalangeal (MCP) joint, leading to an initially dynamic, and eventually passive hyperextension deformity. In surgical treatment of basilar thumb disease, intervention at the MCP joint should also be considered to correct deformity and prevent persistent dysfunction. We present a novel technique using suture tape augmentation of the thumb MCP joint volar plate advancement and capsulodesis, with the goal of preventing recurrent instability as a result of tissue attenuation and enabling ear

The minimally invasive approach for distal radius fractures and malunions.

This article reviews the author's currently used minimal invasive approach for volar plating of distal radius fractures. A single longitudinal incision of 1.5 cm is drawn on the lateral aspect of the flexor carpi radialis tendon in order to insert a plate under the pronator quadratus. With the wrist flexed, the plate is applied on the anterior cortex of the radius to reduce the fracture. This approach offers the advantage of preserving ligamentotaxis, which facilitates the reduction, and the small scar improves the cosmetics. It is mainly indicated for extra-articular and simple intra-articula

Brachioradialis Tendon Coverage in Volar Distal Radius Plating.

Volar plate fixation of unstable distal radius fractures has become the preferred technique by most surgeons for the operative treatment of displaced distal radius fractures. Flexor tendon rupture is a rare but serious complication associated with this operation that is thought to be due to irritation of the flexor tendons over the prominent distal edge of the plate. We describe a novel technique of using the brachioradialis tendon to cover the distal edge of the plate to help prevent tendon irritation and rupture.

Distal radius volar locking plate design and associated vulnerability of the flexor pollicis longus.

PURPOSE: Flexor pollicis longus (FPL) tendon rupture is a well-documented complication related to the use of distal radius volar locking plates (VLPs). The final common pathway of flexor tendon rupture appears to involve implants prominent at the watershed line. We hypothesized that significant differences in VLP prominence exist between various plate designs. METHODS: Ten fresh frozen specimens were dissected to identify the path of the FPL in relationship to the distal radius at the watershed line. Five VLP designs were fixed to each specimen based on their anatomic fit, and slid distally u

Use of standard and anatomic-tilt lateral X-rays to determine distal radius volar angulation.

PURPOSE: To determine the reliability of measurements of distal radius volar tilt using either standard lateral or anatomic tilt lateral (ATL) radiographs, and to compare the mean values obtained using each radiographic method. METHODS: We obtained standard and 23° ATL plain radiographs of the distal radii of 20 patients with a distal radius fracture treated with a volar locking plate, and of 20 healthy individuals without a history of fracture of the wrist. Three orthopedic surgeons measured volar tilt twice with an interval of 4 weeks. We analyzed intraobserver and interobserver reliability

Standardized Volar Silicone PIP Arthroplasty: Bilateral Proximal Collateral Release and a Structured Non-Repair Strategy.

Silicone arthroplasty of the proximal interphalangeal joint is traditionally performed through a dorsal approach, which requires violation of the extensor mechanism and may contribute to postoperative stiffness and extension lag. Volar approaches preserve the extensor apparatus and permit earlier active mobilization; however, previously described techniques differ in flexor sheath entry, volar plate management, and collateral ligament handling, limiting reproducibility. We describe a standardized volar technique that emphasizes controlled neurovascular mobilization; elevation of the A3 pulley

Dorsal Block Splinting of Volar Plate Injuries at Neutral Position.

PURPOSE: Volar plate injuries of the proximal interphalangeal joint (PIPJ) of a finger are common, often occurring in sporting or workplace incidents. Treatment of a stable dorsal dislocation entails a conservative approach, directed at preventing hyperextension and allowing the ligament to heal in position that does not negatively impact on healing. Current treatment regimens include dorsal block splinting (DBS) in 30° of flexion, which maintains the PIPJ in flexion in order to prevent any strain on the healing tissue while attempting to restore anatomical position. Recognized complications o

Irreducible dorsoulnar dislocation of the proximal phalanx of the thumb.

A case of irreducible complete dorsoulnar dislocation of the proximal phalanx of the thumb is presented. The pathologic anatomy included intersubstance tears of the ulnar collateral ligament and dorsal capsule and avulsions of the palmar plate and radial collateral ligament from their metacarpal attachments. The palmar plate blocked reduction, as did the radial collateral ligament that was caught on the margin of a tear in the adductor aponeurosis and on the tendon of the extensor pollicis longus. A review of the English-language literature indicates that this pathologic anatomy has not been d

How placement affects force and contact pressure between a volar plate of the distal radius and the flexor pollicus longus tendon: a biomechanical investigation.

Open reduction and internal fixation of a distal radius fracture can leave a volar plate in close proximity or touching the tendons of the wrist. This cadaveric study examines the how volar plate position changes contact pressure and force against the flexor pollicis longus (FPL) tendon in multiple wrist extension positions. This study suggests that moving the plate from an ideal position (distal edge at the watershed line) to a malposition (5 mm distal to the watershed line) significantly increased the force by 72.7% and contact pressure by 33.5% on the FPL. Multiple clinical case reports hav

External fixation for comminuted phalangeal fractures. A biomechanical cadaver study.

The mechanical rigidity obtained by external fixation in a comminuted phalangeal fracture model was assessed and the results compared with two other types of internal fixation commonly used (lateral plate and crossed Kirschner wires) in a biomechanical cadaver study. Each fixation technique was tested in apex palmar bending, compression and torsion. The results showed that lateral plating provided the best rigidity in apex palmar bending and compression and that external fixation and Kirschner wires showed the same mechanical properties. For the torque test, external fixation provided the best
