OBJECTIVES: Involvement of the pancreas in systemic lupus erythematosus is rare. The purpose of this article is to provide a detailed review of lupus-associated pancreatitis.
METHODS: We describe 3 patients with lupus-associated pancreatitis and review the English literature of the last 30 years, including the demographic, clinical, therapeutic, and prognostic aspects of this disorder.
RESULTS: There were detailed descriptions of 77 patients, 88% were females. Median age was 27 years. In 44% of the patients pancreatitis developed within 1 year of the diagnosis of lupus, and 84% had active lu

Lymphoplasmacytic sclerosing pancreatitis is a rare entity that has been described under many different names and constitutes a diagnostic challenge as it may simulate a neoplastic process. Herein, we report a case of a 61-year-old woman who presented to our institution complaining of left flank pain and was found to have normal levels of amylase and lipase. An abdominal magnetic resonance image showed thickening of the pancreatic tail and compression of the pancreatic duct. The radiographic differential included both chronic pancreatitis and a neoplastic process. She underwent an exploratory

Lesions of the pancreas induced by viral infection have drawn relatively little attention because of their low incidence, and the histopathologic features of viral pancreatitis have not been fully elucidated. We report the autopsy findings of 2 patients, a 59-year-old woman with allergic granulomatous angiitis and a 73-year-old man with invasive pulmonary aspergillosis who had a disseminated visceral herpes simplex virus (HSV) infection. In both cases, the liver was the organ most severely affected by the viral infection. The pancreas showed multiple small foci of hemorrhagic necrosis, which w

OBJECTIVE: To report a case of acute reversible pancreatitis associated with metronidazole-treated aspiration pneumonia.
CASE SUMMARY: A 61-year-old white woman requiring coronary artery bypass surgery developed acute pancreatitis following treatment with metronidazole for suspected postsurgical aspiration pneumonia. The patient developed moderate to severe bilateral upper quadrant abdominal pain; laboratory studies revealed elevated amylase and lipase concentrations four days following the initiation of metronidazole therapy. After discontinuation of metronidazole, the patient's abdominal pa

The most important consideration in preventing ERCP-induced pancreatitis is patient selection. If you want to avoid pancreatitis, avoid performing ERCP in young patients for sphincter of Oddi dysfunction. Sphincter of Oddi manometry, difficult biliary cannulations (repeated pancreatic duct cannulations/injections), and precut and pancreatic sphincterotomy are associated with increased risk of pancreatitis. Pancreatic endotherapy, precut sphincterotomy, and Sphincter of Oddi manometry should be reserved for expert endoscopists. Short-term pancreatic stenting appears to decrease the risk of panc

The treatment of severe, complicated pancreatitis requires: Rapid assessment and recognition of severity, determined by using APACHE or equivalent scoring systems. Aggressive resuscitation with crystalloid and colloid solutions using central venous monitoring for guidance, and to help identify and treat early organ failure. Nutritional support including total parenteral feeding, which can assist in maintaining nutrition during the prolonged period that may ensue. Nasojejunal feedings, bypassing the duodenum, supply nutrition and may decrease infectious complications. Judicious use of imaging s

Biliary pancreatitis is a major complication of gallstones (6-8%) and mainly affects patients with microlithiasis (22%) and cholesterolosis (29%) of the gallbladder. Transient or prolonged obstruction of the ampulla represents the accepted cause and severe forms of acute pancreatitis are more frequently associated with microlithiasis (21.3% vs. 9.6%) with higher incidence of mortality (6.5% vs. 3.2%) as compared with patients with cholelithiasis. The treatment of cholelithiasis and choledocholithiasis performed electively during the same admission, after manifestations of acute pancreatitis ha

Recent information about drugs implicated in causing pancreatitis is summarized. Although the frequency of drug-induced acute pancreatitis is generally low, the disease is associated with substantial morbidity and mortality, which makes timely identification of the offending agent important. Mechanisms suggested for drug-induced pancreatitis include pancreatic duct constriction; immunosuppression; cytotoxic, osmotic, pressure, or metabolic effects; arteriolar thrombosis; direct cellular toxicity; and hepatic involvement. Agents reported to have a definite association with pancreatitis are aspa

In most patients presenting with acute pancreatitis, the cause can be established on the basis of initial history, physical examination, laboratory studies, and abdominal sonography. Patients with unexplained pancreatitis at that point are often considered to have idiopathic disease. However, a cause and, often, effective treatment to prevent recurrent pancreatitis are possible in many of these patients if an aggressive diagnostic approach is taken to discover undiagnosed hyperlipidemia, occult gallstones, abnormalities of the bile and pancreatic ducts, sphincter of Oddi dysfunction, pancreati

Review of all deaths from acute pancreatitis recorded at Glasgow Royal Infirmary between 1974 and 1984 identified 126 patients, 53 (42%) of whom had pancreatitis first diagnosed at necropsy. Aetiologies of the fatal attacks of pancreatitis included gall stones (30%), alcohol (15%), other identified aetiological factors (17%), and was unknown (38%). Overall mortality fell from 14.9% in the early half of the study to 10.8% in the latter half although in the 73 patients in whom the diagnosis of acute pancreatitis was made during life, the mortality rate was unchanged throughout. Within the group

Hypercalcaemia is considered to be a rare cause of pancreatitis but the true cause-and-effect relationship between hyperparathyroidism and pancreatic inflammatory disease remains controversial. Over 100 patients have been reported in whom both processes have occurred concurrently, but doubts have been expressed as to whether or not this association is due to chance. We report 10 new cases of hypercalcaemic hyperparathyroidism associated with different types of pancreatitis. Seven patients had primary hyperparathyroidism and three had hyperparathyroidism after renal transplantation. Two experie

Pancreatitis in teenagers is rare and has not received attention in the medical literature. A retrospective study was conducted to identify unique factors in this age group. Clinical data, cause, radiographic evaluation, operative procedure, and outcome were reviewed in 21 teenagers. Endoscopic retrograde cholangiopancreatography and computerized axial tomography proved to be valuable in determining the etiology and the role and type of operation. Causes of disease were varied but were established in 86 percent of patients; as a group, teenagers were unlike children or adults in their disease

Thirty cases of pancreatitis in children are reviewed. Diagnosis is based on the clinical findings of pancreatic inflammation, laboratory confirmation with elevated serum and/or urinary amylase values, and in some cases surgical or postmortem tissue substantiation. Four major classes of pancreatitis are defined in children: (1) traumatic; (2) systemic-disease associated; (3) drug-induced, caused by drug therapy for various life-threatening diseases, and (4) chronic pancreatitis, with or without an anatomic abnormality. If cases of traumatic pancreatitis are excluded, females outnumber males 5

Necrotising pancreatitis is a rare but serious complication of acute pancreatitis in children. We report a case of a toddler who presented with a 1 day history of persistent non-bilious vomiting, intermittent pallor and irritability, without fever or diarrhoea. Initial examination was non-specific, but progressive abdominal tenderness and new onset of fever prompted imaging and laboratory investigations. Elevated inflammatory markers and serum lipase, along with contrast-enhanced CT findings, confirmed a diagnosis of necrotising pancreatitis. The patient was transferred to a tertiary hospital,
