BACKGROUND & AIMS: Chronic alcoholic pancreatitis occurs in only a limited number of heavy drinkers. Other factors than alcohol are necessary for the occurrence of chronic alcoholic pancreatitis. The aim of this study was to examine whether pancreatic duct obstruction resulted in increased alcohol-induced parenchymal cell damage.
METHODS: Four groups of adult mongrel dogs were used. In group A, 2. 0 g . kg-1 . day-1 of ethanol was administered via a gastric cannula. In group O, after ligation of the minor pancreatic duct, a polyethylene tube was inserted transduodenally into the major duct. I

In order to elucidate the functional states of the pancreas in the asymptomatic latent stage of chronic alcoholic pancreatitis, 45 chronic alcoholics with no obvious pancreatic structural abnormalities judged by endoscopic pancreatography were studied by pancreozymin-secretin test. We found three patterns of exocrine pancreatic function in alcoholics with or without cirrhosis: normal secretion (40%), hyposecretion (29%) and hypersecretion (31%). In the hyposecretory group, the amylase output proved to be impaired more frequently. In the hypersecretory group increases were observed in one or mo

In order to clarify the genetic factors in alcohol-related chronic pancreatitis among Japanese, we determined the genotype of two major alcohol-metabolizing enzymes, alcohol dehydrogenase (ADH) and aldehyde dehydrogenase (ALDH). The restriction fragment-length polymorphisms of the ADH2 and the ALDH2 genes were analyzed in 47 normal subjects and 31 patients with alcoholic pancreatitis. No significant difference between the patient and control groups was found in the ADH2 genotypes. A significant genetic difference between the two groups was found in the ALDH2 locus. The frequency of the ALDH2*1

Intraductal tubulopapillary neoplasm (ITPN) of the pancreas is a rare tumor that is often difficult to diagnose preoperatively because of nonspecific symptoms and atypical imaging findings. Accurate identification is important, as ITPN carries a significant risk of associated invasive carcinoma. We report a case of ITPN with carcinoma in a 30-year-old man who presented with recurrent alcoholic pancreatitis, pancreatic pseudocysts, and a pancreatic duct stricture. Distal pancreatectomy revealed ITPN with invasive carcinoma. He received adjuvant FOLFIRINOX, but developed locally recurrent adenoc

INTRODUCTION: Transcription factor EB (TFEB) deficiency contributes to insufficient autophagic degradation, resulting in zymogen granule (ZG) accumulation and subsequent premature activation, which underlies the pathogenesis of alcoholic pancreatitis (AP). Despite this mechanistic insight, pharmacological activation of TFEB via plant-derived agents remains an underdeveloped therapeutic avenue.
OBJECTIVES: This study aims to investigate Wogonin's role as a key Scutellaria baicalensis (Huangqin, HQ) component in protecting against AP, with specific focus on TFEB-mediated autophagy activation.

Among the etiologies of pyogenic liver abscess (PLA), bacterial spread from the biliary tract or portal flow is the major cause, while the onset of PLA due to arterial bacterial transmission is rare. We herein report two cases of PLA thought to be caused by arterial transmission from dental disease. In both cases, there was benign biliary stricture as a result of alcoholic chronic pancreatitis, although normal oral flora was detected as the causative bacteria and oral hygiene was poor in both patients. We presumed that the origin of PLA was dental disease and successfully treated the patients

OBJECTIVES: Smoking has been shown to affect the course of alcoholic chronic pancreatitis (ACP). However, a dose-dependent relationship between ACP course and the amount of tobacco consumption has not been studied.
METHODS: All consecutive smokers with ACP were included prospectively. Thresholds were defined at 10, 15, 20, and 30 pack-years (p.y.) to assess the relationship between tobacco intake and ACP complications. Statistical adjustment on alcohol intake was performed.
RESULTS: One hundred eight patients (male, 86%) were included. The median tobacco intake was 30 p.y. (range, 3-90 p.y.)

OBJECTIVES: Serum disialotransferrin is a specific marker of heavy alcohol consumption. We tested its accuracy and probability in detecting alcoholic cause of acute pancreatitis (AP).
METHODS: Blood samples from 271 consecutive AP patients, admitted to the Helsinki University Central Hospital emergency unit, were analyzed.
RESULTS: The median (range) disialotransferrin value was significantly higher (P = 0.001) in AP patients with alcoholic (n = 172) 1.6% (0.3%-14.4) than with biliary (n = 60) 0.7% (0.3%-1.3%) or other causes (n = 39) 0.8% (0.3%-4.1%). In receiver operating curve analysis, d

UNLABELLED: The natural history and complications of non alcoholic chronic pancreatitis (NACP) is poorly understood compared to that of alcoholic chronic pancreatitis (ACP).
PATIENTS AND METHODS: From April 1993 to April 1996, 77 patients with NACP were prospectively evaluated in 17 French centres. This population was compared to a cohort of 417 patients with ACP.
RESULTS: No significant difference was observed with respect to mean age between NACP and ACP (43 +/- 20 vs 44 +/- 11 years, respectively). The median patient follow-up time was also comparable: 7 years (1-28) and 6 years (1-34) re

Purtscher's retinopathy (PR) is a rare ischaemic retinopathy associated with systemic disorders and trauma. Few cases have been reported, and prognosis remains unclear. We present a case of Purtscher's-like retinopathy (PLR) secondary to alcoholic pancreatitis and document visual acuity improvement but persistent visual field deficits. A man in his 50s presented to the hospital with vomiting, seizures and severe abdominal pain on the background of alcohol excess. One day after symptom onset, he developed a sudden, painless loss of vision bilaterally. He was diagnosed with acute alcoholic pancr

BACKGROUND: Alcoholic chronic pancreatitis (ACP) is the dominant cause of chronic pancreatitis (CP). As a main complication of CP, the formation of pancreatic duct stones (PDS) compromises pancreatic function and symptomatic patients are often subjected to aggressive treatments. The present study aimed to identify PDS risk factors in patients with ACP.
METHODS: A retrospective analysis of 93 ACP patients was performed; patients were divided into two groups: ACP with PDS (n = 48) and ACP without PDS (n = 45). Fourteen potential factors were analyzed by univariate and multivariate analyses to i

The etiology of an attack of acute pancreatitis (AP) is highly variable. It is important to identify the causing factor, because it can have an effect on the specific treatment of AP. Furthermore, in some cases relapses of AP can be prevented when the causing factor is eliminated. The most common etiology of AP is alcohol abuse and gallstone disease (>80% of all cases). What diagnostic measures should be undertaken before an attack of AP is considered as idiopathic? Important factor in the etiologic diagnostic work-up is the availability, safety, and cost-effectiveness of suitable methods. Fur

We report 2 patients with an unusual form of chronic pancreatitis, both of whom were treated for clinical suspicion of pancreatic malignancy. The surgical specimens revealed a dense lymphoplasmacytic infiltration of the main and interlobular branches of the pancreatic duct, causing sclerosis of the duct wall, diffuse irregular lumenal narrowing, extensive parenchymal fibrosis, and organ enlargement. Neither case showed calcifications, fat necrosis, or cyst formation, features usually seen in alcoholic pancreatitis, nor was there any evidence of neoplasia. One patient had an unusual form of aca

The major complications of chronic pancreatitis are malabsorption, diabetes mellitus, pancreatic calcification and pseudocysts. Sinistral portal hypertension due to splenic vein thrombosis, obstructive jaundice and duodenal stricture have also been reported as complications of chronic pancreatitis. However, a case having all these three complications at the same time is relatively rare. We present a case of chronic alcoholic pancreatitis complicated with simultaneous multiple severe complications. Although biliary drainage is usually a useful treatment for reducing the bilirubin level in the p

BACKGROUND AND STUDY AIMS: Biliary obstruction in chronic pancreatitis is frequently treated by endoscopic insertion of a plastic stent into the common bile duct, a therapy regarded as having a low complication rate. The aim of this study is to analyze the frequency and severity of complications caused by biliary stents in patients with chronic alcoholic pancreatitis.
PATIENTS AND METHODS: We retrospectively analyzed all our patients with chronic pancreatitis (n = 14) who were provided with a plastic stent for biliary stenosis between June 1993 and December 1997. Stent exchanges were followed

Distinguishing true cysts from pseudocysts can be challenging in clinical practice. We report a patient with a pancreatic pseudocyst secondary to alcoholic chronic pancreatitis that closely mimicked a mucinous cystic neoplasm. A 64-year-old woman with a medical history of alcoholic liver cirrhosis had a progressively enlarging pancreatic cyst in the pancreas body associated with elevated serum carbohydrate antigen 19 - 9 levels. Computed tomography indicated a multilocular cystic lesion with calcification, thickened septa, and protruded lesion. Endoscopic ultrasonography revealed a protruded l

Hypertriglyceridemia is a less frequent yet clinically significant cause of acute pancreatitis, accounting for approximately 4-10% of cases, and is often overlooked when routine laboratory analysis is hindered by lipemia-related analytical interference. We describe two cases of severe hypertriglyceridemia-induced pancreatitis presenting with acute abdominal pain and markedly elevated triglyceride levels. The first case involved polygenic hypertriglyceridemia triggered by undiagnosed diabetes. The second case represented secondary, alcohol-related hypertriglyceridemia. In both cases, lipemia-re

OBJECTIVES: Alcohol and nicotine are the two most important risk factors of chronic pancreatitis, and they often occur together. It is still unclear how much they influence the severity of the disease and which of the two addictions should be treated with priority.
METHODS: We performed a single-center, retrospective, cross-sectional study in a mixed medicosurgical cohort of 870 patients diagnosed with chronic pancreatitis (CP). We analyzed the impact of the drinking pattern and abstinence for alcohol and nicotine on the course of the disease. Patients with alcoholic CP were subdivided into 1
