Compromised Conscience: A Scoping Review of Moral Injury Among Firefighters, Paramedics, and Police Officers.

BACKGROUND: Public Safety Personnel (e.g., firefighters, paramedics, and police officers) are routinely exposed to human suffering and need to make quick, morally challenging decisions. Such decisions can affect their psychological wellbeing. Participating in or observing an event or situation that conflicts with personal values can potentially lead to the development of moral injury. Common stressors associated with moral injury include betrayal, inability to prevent death or harm, and ethical dilemmas. Potentially psychologically traumatic event exposures and post-traumatic stress disorder c

Paramedics assessing patients with complex comorbidities in community settings: results from the CARPE study.

OBJECTIVES: The aim for this study was to provide information about how community paramedicine home visit programs best "navigate" their role delivering preventative care to frequent 9-1-1 users by describing demographic and clinical characteristics of their patients and comparing them to existing community care populations. METHODS: Our study used secondary data from standardized assessment instruments used in the delivery of home care, community support services, and community paramedicine home visit programs in Ontario. Identical assessment items from each instrument enabled comparisons of

The ability of paramedics to predict aspiration in patients undergoing prehospital rapid sequence intubation.

One of the purported benefits to invasive prehospital airway management is the prevention of aspiration; however, aspiration events may occur before the arrival of prehospital personnel. We explore the timing of aspiration in patients with severe traumatic brain injury (TBI) undergoing paramedic rapid sequence intubation (RSI). Severely head-injured (Glasgow Coma Scale [GCS] score 3-8) adults were prospectively enrolled into the San Diego Paramedic RSI Trial. As part of the prehospital data collection tool, paramedics prospectively assessed for clinical evidence of aspiration before RSI (pre-i

Psychoactive substance use among medical and paramedical undergraduate students of Lucknow.

BACKGROUND: Psychoactive drugs are substances that, when taken in or administered into one's system, affect mental processes like perception, consciousness, cognition, mood, and emotions. The use of psychoactive substances often starts at a young age. This study aimed to estimate the prevalence of psychoactive substance use among medical and paramedical undergraduate students of Lucknow. MATERIAL AND METHODS: A cross-sectional study was conducted among 492 MBBS, BDS, and BSc Nursing students of Lucknow. A predesigned pretested semistructured self-administered questionnaire was used for data c

Paramedic Understanding of Tension Pneumothorax and Needle Thoracostomy (NT) Site Selection.

Introduction Tension pneumothorax is an immediate threat to life. Treatment in the prehospital setting is usually achieved by needle thoracostomy (NT). Prehospital personnel are taught to perform NT, frequently in the second intercostal space (ICS) at the mid-clavicular line (MCL). Previous literature has suggested that emergency physicians have difficulty identifying this anatomic location correctly. We hypothesized that paramedics would also have difficulty accurately identifying the proper location for NT. Methods A prospective, observational study was performed to assess paramedic ability

Representations of Research among Newly Graduated Paramedical Professionals: A Qualitative Study.

Despite a keen interest in clinical research, most paramedical professionals are unwilling to play an active role. Our objective was to explore paramedical professionals' representations of research. Using an existing database of final year paramedical students (speech therapy, occupational therapy, psychomotricity, audiometry, physiotherapy, orthoptics), we deployed a qualitative approach composed of two successive steps: (1) a free word association task, and (2) semi-structured individual interviews. Out of the 54 students who agreed to be contacted, we received 21 responses to the free word

Survival in Patients with Paramedic-Identified ST-Segment Elevation Myocardial Infarction.

Field identification and treatment of ST-segment elevation myocardial infarction (STEMI) by paramedics is an important component of care for these patients. There is a paucity of studies in the setting of paramedic-identified STEMI. This study investigated mortality and factors associated with mortality in a large state-wide prehospital STEMI sample. Methods: Included were adult STEMI patients identified and treated with reperfusion therapy by paramedics in the field between January 2016 and December 2018 in Queensland, Australia. 30-day and one-year all-cause mortality was compared between tw

[Predonation interview by a trained and authorized paramedical staff: feasibility, reliability and safety].

INTRODUCTION: Predonation interview accounts for a major step in transfusion safety. In France, it must be performed by a physician, following a methodical questioning and a standardized questionnaire. Faced with this evolution, the value of a strictly medical expertise has been progressively losing importance. In many countries, blood donor selection is being organized by non medical trained staff (Québec, Switzerland, e.g.). A decree of April 30, 2006 allowed the Établissement français du sang to experiment a predonation interview by an authorized paramedical staff in the form of a two-phase

Status asthmaticus in a paramedic following exposure to a roadside flare: a case report.

Highway flares are commonly used by police and emergency service workers to divert traffic away from roadside accidents. We report a case of a 35-yr-old paramedic who developed a sudden, severe asthmatic reaction following a brief exposure to such a flare, necessitating intubation and a prolonged hospitalization.

[Organ donation for transplantation. Survey of opinions of physicians and paramedics in the province of Vaud].

In 1987, two media campaigns were organized in Switzerland to promote donor cards. The purpose of this study was to test their impact on the medical community. Questionnaires were sent to qualified practitioners, doctors in training and intensive care nurses. It is concluded that the campaigns for organ donation must be repeated and that their quality can be improved. It is however more urgent to improve the level of knowledge among doctors and other health professionals.

Bacterial contamination of mask inner surfaces during prolonged use by paramedics in prehospital emergency care.

Paramedics are routinely exposed to respiratory secretions and aerosols in prehospital settings, necessitating prolonged or repeated mask use for infection prevention. However, evidence regarding time-dependent bacterial contamination of masks worn during extended prehospital duty remains limited. This within-subject repeated-measures study investigated bacterial contamination on the inner surface of KF94 masks worn by 24 Korean paramedics during routine duty. Mask samples were collected after 6, 12, and 24 h of wear. Bacterial contamination was quantified using colony-forming unit (CFU) count

A repeated measures investigation of post-traumatic stress symptoms among Australian paramedics over six weeks.

BackgroundParamedics have a higher prevalence of probable post-traumatic stress disorder than the general population. However, some paramedics may experience significant distress and impairment on a daily or weekly basis while never meeting post-traumatic stress disorder diagnostic criteria, as post-traumatic stress reactions often resolve within days or weeks following potentially psychologically traumatic event exposure.ObjectiveTo evaluate post-traumatic stress symptom severity in a cohort of paramedics following exposure to a single potentially psychologically traumatic event (i.e., post-t

Factors affecting paramedics situational awareness in out of hospital emergencies: A mixed methods systematic review.

INTRODUCTION: Out-of-hospital emergency care is complex, time-critical, and demands rapid decisions that directly affect patient safety. Paramedics deliver life-saving treatment in unpredictable environments, requiring swift judgment and adaptability under pressure. Despite expertise, errors occur-especially under stress. Situational awareness (SA), the ability to perceive, interpret, and anticipate key elements, is vital for safe, effective care and coordination. Yet research on how paramedics develop and maintain SA in these challenging settings remains limited, leaving critical knowledge ga

Paramedics' Experiences in Managing Obstetric Emergencies: A Qualitative Study Based on Reason's Swiss Cheese Model.

OBJECTIVES: This study explores paramedics' experiences in managing childbirth-related emergencies and examines challenges through Reason's Swiss Cheese Model, which frames obstetric errors as the result of multiple system-level vulnerabilities. METHODS: A qualitative phenomenological design was employed to gain an in-depth understanding of paramedics' knowledge, experiences, and perceived barriers in obstetric emergencies. Purposive sampling recruited 18 paramedics who had encountered obstetric cases in prehospital settings. Semi-structured, in-depth interviews were conducted between March a

Mental Disorder Symptoms and the Relationship with Resilience among Paramedics in a Single Canadian Site.

There is growing recognition in research and policy of a mental health crisis among Canada's paramedics; however, despite this, epidemiological surveillance of the problem is in its infancy. Just weeks before the emergence of the COVID-19 pandemic, we surveyed paramedics from a single, large, urban paramedic service in Ontario, Canada to assess for symptom clusters consistent with post-traumatic stress disorder (PTSD), major depressive disorder, and generalized anxiety disorder and to identify potential risk factors for each. In total, we received 589 completed surveys (97% completion rate) an

Paramedics' Perspectives on the Hospital Transfers of Nursing Home Residents-A Qualitative Focus Group Study.

Emergency department (ED) visits and hospital admissions are common among nursing home residents (NHRs). Little is known about the perspectives of emergency medical services (EMS) which are responsible for hospital transports. The aim of this study was to explore paramedics' experiences with transfers from nursing homes (NHs) and their ideas for possible interventions that can reduce transfers. We conducted three focus groups following a semi-structured question guide. The data were analyzed by content analysis using the software MAXQDA. In total, 18 paramedics (mean age: 33 years, male n = 14

Acute Myocardial Infarction Identified by Paramedics Using the Smith-Modified Sgarbossa Criteria: A Case Report.

Identifying acute MI in the setting of left bundle branch block (LBBB) is challenging because ST-segment elevation is often found at baseline. When faced with LBBB and ischemic symptoms, identifying which patients require urgent reperfusion therapy is critical. Sgarbossa et al. derived three criteria that can be used to help identify these patients, Smith et al. refined these criteria creating the Smith-modified Sgarbossa criteria. We present a case of LBBB meeting the Smith-modified Sgarbossa criteria, recognized by paramedics and used to activate the catheterization laboratory after normal b
