Although parathyroid neoplasms are common and cause primary hyperparathyroidism, parathyroid carcinoma is a rare entity. At times it can be difficult to diagnose. Patients with parathyroid carcinoma usually present with profound symptoms of hyperparathyroidism and highly elevated serum calcium and parathyroid hormone (PTH) levels. At the time of neck exploration, a large, gray-white, locally invasive tumor is commonly encountered. The course of patients with parathyroid carcinoma is variable; unfortunately, more than 50% have persistent or recurrent disease due to regional or distant disease.

Water-clear cell adenoma (WCCA) of the parathyroid gland is an exceedingly rare neoplasm. To date, 17 cases have been reported in the literature, with only one of them being intrathyroidal. Here we report a case of a 34-year-old woman who presented for evaluation of a goiter and was found to have a thyroid nodule and abnormal thyroid function tests (TFT). Fine needle aspiration biopsy of the nodule revealed thyroid follicular cells without atypia and subsequent Afirma® Gene Expression Classifier (GEC) testing results were suspicious for malignancy. As a result, the patient underwent a right th

A case of basosquamous or so-called transitional cloacogenic carcinoma of the sigmoid colon, which arose above the pelvic brim at the peritoneal reflection, is reported. We were not able to find a report of this histologic type of tumor arising this far from the pectinate line of the anus, which is the most common primary site of this neoplasm. Possibilities as to the histogenesis of this tumor at this site are stated. The neoplasm also produced parathyroid hormone (PTH) and also possibly adrenal corticotrophic hormone (ACTH), which had not been previously reported for this specific neoplasm.

An 11-year-old, male, neutered crossbred Collie dog was presented for a history of polydipsia and polyuria. Diagnostic investigations revealed total and ionized hypercalcemia and an increased concentration of parathyroid hormone-related peptide. Abdominal ultrasonography and contrast-enhanced computed tomography of the abdomen revealed a right-sided, cystic-appearing renal mass. Cytological examination of ultrasound-guided aspirates of the mass revealed high numbers of spindle cells. The mass was removed en bloc via an ureteronephrectomy. Histopathological examination of the mass revealed neop

Parathyroid carcinoma (PC) is most often linked to germline or somatic CDC73 mutations. Previously, we identified three CDC73-wildtype PCs with massive chromosomal losses and genome near-haploidization, with or without endoreduplication/genome doubling - features also seen in rare tumor types such as oncocytic thyroid carcinoma. To assess whether similar alterations occur in other parathyroid neoplasms, we analyzed fourteen parathyroid adenomas (PA; thirteen oncocytic), three atypical parathyroid tumors (APTs; one oncocytic), and one additional PC, the latter four all CDC73-wildtype. Histopath

Chromogranin A, the protein that is co-stored and co-released with catecholamines from the adrenal medulla, has recently been identified in a variety of human endocrine tissues, both normal and neoplastic. We investigated the secretion of chromogranin A by peptide hormone-producing human tumors in studies of patients with the following neoplastic disorders: pheochromocytoma, parathyroid adenoma, primary parathyroid hyperplasia, medullary thyroid carcinoma, thyroidal C-cell hyperplasia, carcinoid tumor, oat-cell lung carcinoma, pancreatic islet-cell tumor, and aortic-body tumor. All these patie

Currently, the treatment of choice for refractory tertiary hyperparathyroidism is a total parathyroidectomy with autotransplantation of the parathyroid tissue into the forearm. Recurrent hyperparathyroidism after autotransplantation requiring surgical excision of the autograft is uncommon. We describe a female patient with recurrent hyperplasia who underwent an excision of her parathyroid autograft. The excisional specimen was submitted to pathology with the only provided history of a 'left arm mass'. The lack of clinical history led to a preliminary impression of an adnexal neoplasm by the pr

Parathyroid carcinoma is the rarest endocrine malignancy. Definitive diagnosis is challenging as it is difficult to distinguish malignant from benign disease. A 71-year-old man presented with weight loss and hypercalcaemia. CT scans revealed multiple lung nodules and lytic bone lesions that were consistent with metastatic dissemination. Technetium-99m-sestamibi-single-photon emission computed tomography scan showed an abnormal uptake in the right thyroid lobe. Fine-needle aspiration (FNA) was performed on three occasions. The patient underwent parathyroidectomy with ipsilateral hemithyroidecto

Atypical parathyroid tumors (APTs) are an uncommon subset of parathyroid neoplasms that carry substantial clinical relevance because of their histological and clinical resemblance to parathyroid cancer. Despite this importance, the diagnosis of APTs remains persistently challenging, even for experienced pathologists. Their histopathological features overlap extensively with those of parathyroid cancer, and at present, no specific immunohistochemical markers are available that can reliably distinguish between these entities. Moreover, the clinical manifestations of APTs are indistinguishable fr

Human multiple endocrine neoplasia subtype 2A (MEN 2A) is characterized by medullary thyroid carcinoma, pheochromocytoma and parathyroid hyperplasia or adenoma in the same individual. In this report, a case of a female Rottweiler with medullary thyroid carcinoma, bilateral pheochromocytoma and parathyroid adenoma was described. Clinical manifestations of muscle weakness, polydipsia, polyuria, diarrhea and weight loss were observed. Two adrenal neoplasms were identified incidentally by ultrasonography, and tumor in the left thyroid lobe was identified by palpation. Primary hyperparathyroidism w

Parathyroid carcinoma is a rare form of cancer. We describe a 45-year-old man diagnosed with parathyroid carcinoma long after initial surgery. At age 24, the patient had elevated serum calcium and intact parathyroid hormone (PTH). Ultrasonography revealed a left lower parathyroid tumor, which led to the diagnosis of primary hyperparathyroidism. Parathyroidectomy was performed, and the pathological diagnosis was parathyroid adenoma. At age 32, hypercalcemia and elevated intact PTH recurred, and computed tomography revealed a right lung tumor with focal uptake on 99mTc-methoxyisobutylisonitrile

BACKGROUND: Familial forms of hyperparathyroidism are responsible for approximately 10% of the cases of primary hyperparathyroidism, and their management is different from the sporadic forms. Our objective was to study the gene sequence and expression of HRPT2 and clinical outcome regarding recurrence or persistence rates in three Brazilian kindreds with familial hyperparathyroidism after up to 30 years of follow-up.
METHODS: Clinical and biochemical data, direct sequencing of germline DNA of the HRPT2 gene, and analysis of parafibromin expression (HRPT2 gene product) using RT-PCR and immunoh

OBJECTIVE: Parathyroid gland malignancies are considered rare. The most common of these tumor types is primary parathyroid carcinoma. Metastatic spread from other cancers may also occur with up to 10% of cancers from other sites showing parathyroid involvement at autopsy. Tumor-to-tumor metastases (metastatic spread to parathyroid neoplasm) from remote cancers to the parathyroid gland have been described.
METHODS: We did a PubMed literature review and analysis of our own experience of 392 consecutive parathyroidectomies.
RESULTS: Primary and secondary parathyroid malignancies can be grouped

Functional oxyphil adenomas of parathyroid gland are rare. Several studies have suggested that these tumors consist of a mixed cell population and that the transitional oxyphil cells and/or chief cells are the main source of parathormone, while the fully developed oxyphil cells have minimal or no hormonal activity. This report describes light, ultrastructural, and immunocytochemical features of six parathyroid adenomas, and confirms that these neoplasms are composed of a mixed cell population. It also demonstrates that the oxyphil cells have strong hormonal activity and therefore can play a si

Inherited thyroid and parathyroid diseases represent a small but clinically significant subset of endocrine neoplasms, often associated with identifiable genetic mutations. Familial medullary thyroid cancer is linked to RET mutations and frequently occurs with multiple endocrine neoplasia syndromes, requiring early diagnosis and prophylactic intervention. Nonmedullary familial thyroid cancers are more heterogenous with unclear genetic drivers and variable inheritance patterns. Syndromic forms carry increased thyroid cancer risk necessitating targeted surveillance. Familial parathyroid disease

The reticulin framework, composed mainly of type III collagen, is an essential structural component of biological tissues. Reticulin stains, particularly silver-based methods, enable detailed visualization of reticulin framework alterations, which have been proven to be quick, low-cost, and reliable solutions for highlighting quantitative and qualitative changes of reticulin framework and have been variably associated with neoplastic and non-neoplastic conditions. This review provides an updated overview of reticulin stain applications and reticulin framework assessment in endocrine and neuroe

Primary hyperparathyroidism is a common disorder characterized by aberrant growth and function of solitary or multiple parathyroid glands. Many, if not all, parathyroid adenomas are examples of benign clonal neoplastic growth. The molecular events associated with the development of parathyroid neoplasia have not been well characterized. We examined benign and malignant parathyroid tissues for structural abnormalities of the p53 tumor suppressor gene. To screen for mutations in the p53 gene, we analyzed polymerase chain reaction-amplified DNA by denaturing gradient gel electrophoresis. DNA was

We describe the clinical and morphological features of a most unusual functional mediastinal tumor that combines features of thymolipoma and lipoadenoma of the parathyroid gland. The lesion was unique in that microscopic elements of benign thymic, adipose, and parathyroid tissue were admixed intimately in one partially encapsulated mass. To the best of our knowledge, there has not been a previous description of this tumor. It was excised from a 54-year-old woman with a 10-year history of persistent hyperparathyroidism. The controversial relationship of embryological malformation, hamartoma, an

Parathyroid carcinoma is one of the least frequent malignant neoplasms, diagnosis and treatment are often complex; however, the prognosis is not bad and, although recurrences are frequent, the mortality rate is low. We describe the clinical case of a 42-years-old patient, the evolution of the natural history and treatment.

Polycythemia vera (PV) is a chronic myeloproliferative neoplasm characterized by increased erythrocytes and commonly associated with JAK2 mutations. Primary hyperparathyroidism (PHPT), which is often caused by a parathyroid adenoma, is a common cause of hypercalcemia. Although unrelated, a potential association between PV and PHPT has been described in the literature. We report the case of a 41 year-old male presenting with concurrent PV and PHPT due to a parathyroid adenoma. Following parathyroidectomy, the patient's hemoglobin and hematocrit levels normalized without further treatment, sugge
