The aim was to improve the prediction rate of hyposalivation after using a modern 3D-conformal-radiotherapy-technique (3D-CRT) by sparing of the contralateral parotid gland. Between June 2002 and October 2006, 107 patients (90 male, 17 female, average age: 58 years) with squamous cell carcinoma of the head and neck were included in a prospective, non-randomized study. All patients were treated using 3D-CRT. Parotid function was assessed by measuring stimulated salivary flow before, during and at the end of radiotherapy, as well as 1, 6 and 12 months after radiotherapy. Measurements were conver

Basaloid squamous cell carcinoma (BSCC) in the head and neck area is extremely rare. World Health Organization defined BSCC as a high-grade variant of squamous cell carcinoma composed both of basaloid and squamous components. Due to its histologic similarity to other parotid gland malignancies it can be easily misdiagnosed. We report first case of BSCC arising from parotid gland in Europe and second in the world literature. A 72-year-old female patient was referred to the Department of Cranio-Maxillofacial Surgery of the Jagiellonian University in Poland due to the tumor arising from the right

The present paper considers some of the problems linked to the surgical treatment of parotid metastases. The authors base these considerations on their limited personal cases and on the most accredited literature. In the presence of cutaneous malignancies of the upper portion of the face, accurate clinical-radiological evaluation must be made of the parotid and neck region. Elective parotidectomy is performed when the cutaneous carcinoma is quite extensive or located in proximity of the gland or when the case involves a melanoma. Depending on the oncological margins, a sub-total parotidectomy

Synovial sarcoma (SS) is a malignant mesenchymal proliferation most commonly found near the large joints and bursae of the lower extremities. It is rarely found in the head and neck region. The objective of our study is to report a female infant with SS of the right parotid gland. She did not present any particular pathological history and presented with a right laterocervical tumefaction rapidly increasing in size. Clinical examination showed a non-pulsatile, non-tender, and firm mass in the right parotid region measuring 7 cm x 5 cm with no local signs of aggression. Facial CT and MRI scans

BACKGROUND: The irradiation of sub-regions of the parotid has been linked to xerostomia development in patients with head and neck cancer (HNC). In this study, we compared the xerostomia classification performance of radiomics features calculated on clinically relevant and de novo sub-regions of the parotid glands of HNC patients.
MATERIAL AND METHODS: All patients (N = 117) were treated with TomoTherapy in 30-35 fractions of 2-2.167 Gy per fraction with daily mega-voltage-CT (MVCT) acquisition for image-guidance purposes. Radiomics features (N = 123) were extracted from daily MVCTs for the w

PURPOSE: In this retrospective study, two approaches to preserve the parotid function after radiotherapy (RT) were compared: application of the radioprotective agent amifostine during RT and parotid-sparing intensity-modulated radiotherapy (IMRT).
PATIENTS AND METHODS: Patients were qualified for this analysis if (1) both parotid glands received a radiation dose of >or=50Gy using conventional radiotherapy techniques (cRT) or if they received a parotid-sparing IMRT as alternative, if (2) salivary gland scintigraphies before and after RT were performed, and if (3) a normal parotid function was

PURPOSE: The practice of deliberately sparing the ipsilateral parotid gland with intensity modulated radiation therapy (IMRT) in patients with node-positive head and neck cancer is controversial. We sought to compare the clinical outcomes among consecutive cohorts of patients with head and neck cancer who were treated with differing strategies to spare the parotid gland that is ipsilateral to the involved neck using IMRT.
METHODS AND MATERIALS: A total of 305 patients were treated with IMRT for node-positive squamous cell carcinoma of the head and neck. The first 139 patients were treated wit

Background: Lymphatic malformations of the parotid gland and of the adjacent parotideo-masseteric region are rare and technically demanding, because the malformation infiltrates the glandular parenchyma and grows around the branches of the facial nerve rather than displacing them, and published experience consists almost entirely of isolated case reports. Case Presentation: A 16-year-old boy was treated for an extensive lymphatic malformation of the left parotid and parotideo-masseteric region, present since childhood and progressive over three years, with facial asymmetry, no pain and normal

Parotid malignancy may occur as a primary neoplasm of the salivary tissue or as metastatic involvement of the parotid lymph nodes. Primary tumors of squamous cell carcinoma and malignant melanoma involving the skin of the head and neck have the potential to spread to lymph nodes of the parotid gland. Metastatic malignant melanoma to the back was exceptionally rare and no such reports have been noted in the literature. We reported an exceptional case of intraparotid lymph nodes metastasis of the right scalp junctional nevus with malignant transformation to malignant melanoma in a 48-year-old ma

PURPOSE: To assess the diagnostic potential of the synthetic MRI (SyMRI) for differentiating Warthin's tumors (WT) from pleomorphic adenomas (PA).
MATERIALS AND METHODS: Forty-nine individuals with parotid gland tumors (PA, n = 23; WT, n = 26) were recruited. Using two distinct regions of interest (ROI), SyMRI quantitative parameters of lesions were calculated, including mean and standard deviation (T1, T2, PD, T1sd, T2sd, and PDsd). Meanwhile, T1ratio, T2ratio, and PDratio (lesion/masseter muscle) were calculated based on the mean SyMRI quantitative parameters of masseter muscle (T1, T2, PD)

BACKGROUND: For nasopharyngeal carcinoma (NPC) patients with bilateral parotids near high - dose areas, more research is needed on how radiation to the parotid stem cell region (SCR) impacts xerostomia. This multicenter study aims to explore SCR radiation dosimetric parameters and clinical factors that influence recovery from radiation-induced xerostomia in long-term survivors.
METHODS: We analyzed 648 NPC patients from four Chinese centers who underwent radical radiotherapy. Recovery was assessed via patient-reported (PRO) and physician-assessed (PAO) outcomes. The Cox proportional hazards m

The murine parotid secretory protein (PSP) gene is expressed selectively at high levels in parotid and sublingual salivary glands. Previously, the transcriptional activity of a PSP mini-gene, called Lama, was shown to be dependent on a 1.5 kb region located 3 kb upstream of the transcription start site. Here, functional studies in transgenic mice demonstrate that this proximal regulatory region has properties of a parotid and sublingual gland specific enhancer. Protein-binding experiments identify multiple sequence-specific binding complexes spanning the entire 1.5 kb enhancer region. Several

Various skin incision for access to the parotid region are described by different authors, which differ in extension and localization of the scar. Therefore after parotidectomy, patients are often left with a retromandibular and cervical depression. Maintaining normal facial contour may be difficult in parotidectomy defects, but a good symmetry can and should be achieved when reconstructing the parotid region with a specific approach. The authors show their experience in parotid gland surgery using a new skin incision to improve the aesthetic and functional results and three different techniqu

Cutaneous spindle cell malignancy is associated with a broad differential diagnosis, particularly in the absence of a known primary melanocytic lesion. We present an unusually challenging patient who presented with clinical symptoms involving cranial nerves VII and VIII and a parotid-region mass, which was S100-positive while lacking in melanocytic pigment and markers. Over a year after resection of the parotid mass, both a cutaneous primary lentigo maligna melanoma and a metastatic CP angle melanoma were diagnosed in the same patient, prompting reconsideration of the diagnosis in the original

The anatomy of the buccal region of the face is complex and injuries in this region can lead to significant complications. Trauma in this region can easily injure the parotid duct and facial nerve. The management of parotid duct injuries is usually by microsurgical anastomosis, diversion of salivary flow by creating an oral fistula, or suppression of salivary gland function. The authors present a case of parotid duct injury treated by using an epidural catheter.

BACKGROUND: Reporting the clinical course and management of a skull base malignant triton tumor in a child.
METHODS: An 11-year-old female patient initially presented at the age of 7 with left parotid gland swelling, exhibiting symptoms similar to parotitis. Magnetic resonance imaging (MRI) of the head performed at an external hospital suggested a neoplastic lesion in the left parotid gland and the left parasellar region. Six months later, a parotid gland biopsy was conducted. Postoperative pathology confirmed the diagnosis of embryonal rhabdomyosarcoma. Following the biopsy, the patient unde

INTRODUCTION AND IMPORTANCE: Hydatid cysts are often found in the liver or lungs. As for the parotid localization, it remains exceptional even in endemic areas and presents as a tumor of the parotid thus representing a challenge to the practitioner.
CASE PRESENTATION: A 54-year-old male farmer presented with a swelling in the left parotid gland region evolving over the past 2 years. Physical examination revealed a non-tender and mobile swelling in the left parotid region, the overlying skin was normal. Magnetic resonance imaging showed a cystic lesion of the left parotid gland. The patient presenting with a mass lesion of the parotid gland is frequently found to have a mixed tumor of salivary tissue origin. However, less common lesions occur in the anatomical region of the parotid gland. These pathological entities deserve consideration in the differential diagnosis. This report presents nine patients with unusual lesions occurring in the parotid region. These regions include cervicofacial actinomycosis, branchial cleft cyst, parapharyngeal tumors, bony lesion of the mandible, non-parotid origin malignant tumor, and metastatic malignant tumors. Each class of lesions

We reported a case of parotid sialolithiasis. A man, aged 72, complained of swelling of the left buccal region accompanied by a sensation of inflammation. On diagnosis of sialolithiasis of the parotid gland, a surgical operation was performed with a small incision in the left cheek region. About one month after the operation, a salivary calculus was discharged spontaneously from the left buccal region. He recovered satisfactory without the formation of a parotid salivary fistula. Morphological observation on the salivary calculus was carried out using the scanning electron microscope (SEM).
