Hypothalamic opioid mechanisms controlling oxytocin neurones during parturition.

The influences of opioids on oxytocin secretion and parturition were investigated in the rat. Morphine, administered centrally or peripherally, severely delays the course of established parturition. This delay is accompanied by reduced plasma oxytocin levels and is overcome by treatment either with the opioid antagonist naloxone, or by infusion of oxytocin. An endogenous opioid regulatory mechanism inhibiting oxytocin secretion becomes activated immediately prior to and during parturition. This mechanism does not operate earlier in pregnancy or during normal lactation and is not seen in nonpre

Multiphasic prolactin secretion during parturition in human subjects.

Prolactin (PRL) secretion in the periparturitional period in patients undergoing labor and vaginal delivery follows a remarkable multiphasic pattern not found in patients who underwent elective cesarean section without labor. There is a highly significant decline in PRL levels during active labor which reaches a nadir about two hours prior to delivery. Immediately after delivery, a surge of PRL is noted, reaching peak levels within two hours post partum. Thereafter, PRL levels fall, reaching a second nadir about nine hours post partum, and this low level is maintained for nine to 24 hours afte

Maternal and fetal plasma cortisol levels at parturition.

Fetal adrenal function during pregnancy has a probable role in parturition. Ninety-five mothers and fetuses were evaluated to ascertain maternal or fetal plasma cortisol interrelationships under various clinical situations. When mode of delivery was evaluated, maternal cortisol levels showed no differences. However, the fetuses from vaginal delivery (mean, 43.7 mug/100 ml) had higher levels than those from cesarean section (mean, 34.7 mug/100 ml). Induction of labor showed a rise in maternal cortisol from preinduction levels (mean, 40.2 mug/100 ml) to delivery (mean, 49.6 mug/100 ml), probably

Hormonal profiles and biomarkers leading to parturition in cattle†.

This study aimed to understand the physiological mechanisms regulating parturition and to identify potential biomarkers to predict onset of birth. Additionally, we compared hormone profiles between cows with shorter and longer gestation lengths. Twenty-eight days before due date until 3d postpartum, cows (n = 18) were blood-sampled daily. Circulating concentrations were measured for progesterone and estradiol by RIA, testosterone, prostaglandin F2α metabolite, cortisol, pregnancy-specific protein B by enzyme-linked immunosorbent assay, and lactate concentrations by colorimetric assay. At end o

The dual role of glucocorticoid regeneration in inflammation at parturition.

INTRODUCTION: Fetal membrane inflammation is an integral event of parturition. However, excessive pro-inflammatory cytokines can impose threats to the fetus. Coincidentally, the fetal membranes express abundant 11β-hydroxysteroid dehydrogenase 1 (11β-HSD1), which generates biologically active cortisol to promote labor through induction of prostaglandin synthesis. Given the well-recognized anti-inflammatory actions of glucocorticoids, we hypothesized that cortisol regenerated in the fetal membranes might be engaged in restraining fetus-hazardous pro-inflammatory cytokine production for the safe

A single-cell atlas of the myometrium in human parturition.

Parturition is a well-orchestrated process characterized by increased uterine contractility, cervical ripening, and activation of the chorioamniotic membranes; yet, the transition from a quiescent to a contractile myometrium heralds the onset of labor. However, the cellular underpinnings of human parturition in the uterine tissues are still poorly understood. Herein, we performed a comprehensive study of the human myometrium during spontaneous term labor using single-cell RNA sequencing (scRNA-Seq). First, we established a single-cell atlas of the human myometrium and unraveled the cell type-s

Parturition and Its Relationship with Stillbirths and Asphyxiated Piglets.

The transition from an intra- to extrauterine existence is extremely challenging for the pig. This is evidenced by the fact that conservative estimates place intrapartum piglet death at between 5% and 10%. The main cause of this loss is the parturition process itself, with a long farrowing duration resulting in reduced oxygenation to the piglet via uterine contractions stretching, and in some cases, causing rupture of the umbilical cord. Sows that experience a long expulsive stage of parturition are likely compromised before the birth of the first piglet, and so any strategy to reduce stillbir

Body Mass Index and the Spontaneous Onset of Parturition.

OBJECTIVE: To investigate the relationship between body mass index (BMI) and the onset of parturition throughout gestation. METHODS: This was a secondary analysis of the Maternal-Fetal Medicine Units Network Preterm Prediction Study. Time-to-spontaneous-birth-event (ie, "survival") methods were used to study the association of BMI with the timing of spontaneous onset of labor throughout gestation with indicated births censored at delivery. A Kaplan-Meier estimate of the probability of spontaneous labor was compared with a log rank test across five categories of BMI (kg/m): underweight (less t

Parturition dysfunction in obesity: time to target the pathobiology.

Over a third of women of childbearing age in the United States are obese, and during pregnancy they are at increased risk for delayed labor onset and slow labor progress that often results in unplanned cesarean delivery. The biology behind this dysfunctional parturition is not well understood. Studies of obesity-induced changes in parturition physiology may facilitate approaches to optimize labor in obese women. In this review, we summarize known and proposed biologic effects of obesity on labor preparation, contraction/synchronization, and endurance, drawing on both clinical observation and e

Roles of glucocorticoids in human parturition: a controversial fact?

The pivotal role of glucocorticoids in the initiation of parturition has been very well documented in several domestic mammalian animal species. However the role of glucocorticoids in human parturition remains controversial mainly because of the absence of effect of synthetic glucocorticoids, given to promote fetal organ maturation in pregnant women with threatened preterm delivery, on the length of gestation. This article will review studies of glucocorticoids in human parturition and provide evidence for an important role of glucocorticoids in human parturition as well but a simultaneous hig

Pregnancy, parturition and preeclampsia in women of African ancestry.

Maternal and associated neonatal mortality rates in sub-Saharan Africa remain unacceptably high. In Mulago Hospital (Kampala, Uganda), 2 major causes of maternal death are preeclampsia and obstructed labor and their complications, conditions occurring at the extremes of the birthweight spectrum, a situation encapsulated as the obstetric dilemma. We have questioned whether the prevalence of these disorders occurs more frequently in indigenous African women and those with African ancestry elsewhere in the world by reviewing available literature. We conclude that these women are at greater risk o

A molecular signature of an arrest of descent in human parturition.

OBJECTIVE: This study was undertaken to identify the molecular basis of an arrest of descent. STUDY DESIGN: Human myometrium was obtained from women in term labor (TL; n = 29) and arrest of descent (AODes; n = 21). Gene expression was characterized using Illumina HumanHT-12 microarrays. A moderated Student t test and false discovery rate adjustment were applied for analysis. Confirmatory quantitative reverse transcription-polymerase chain reaction and immunoblot were performed in an independent sample set. RESULTS: Four hundred genes were differentially expressed between women with an AODes

Myometrial progesterone responsiveness and the control of human parturition.

OBJECTIVE: The goal of this review is to assess the body of literature addressing the mechanism of progesterone withdrawal in the control of human parturition and in particular the recent advances in testing the hypothesis that human parturition is initiated by decreased myometrial responsiveness to progesterone, ie, functional progesterone withdrawal. METHODS: Published studies of progesterone responsiveness of the pregnant human myometrium in the context of parturition control were reviewed. RESULTS: Advances in understanding the molecular basis for progesterone receptor (PR)-mediated cont

The effects of intramuscular administration of corticosteroids on the induction of parturition.

Our purpose was to examine the hypothesis that corticosteroids, when administered intramuscularly, can enhance the labor process and reduce the time interval between the induction and the active phase. A randomized, controlled study was conducted on 66 women with gestational age of 41 weeks and over and favorable cervix (bishop score > or = 7). The study group (n = 32) received 10 mg of dexamethasone phosphate intramuscularly in two doses at an interval of 12 hours, and the day after the enrolling administration intravenous oxytocin was given. The control group (n = 33) received only intraveno

The timing of parturition in the pig is altered by intravenous naloxone.

This experiment tested the hypothesis that opioid antagonists could influence the timing of the onset and progress of parturition in the pig. Primiparous pigs (gilts) received a jugular catheter on Days 104 to 106 of pregnancy. At 1400 h on Day 112 the gilts received 10 mg PGF2alpha, i.m. to induce parturition. At 1000 h on Day 113 (i.e., 20 h later) gilts received either saline (n=6), 1 mg/kg, i.v. naltrexone (n=4) or 1 mg/kg, i.v. naloxone (n=5). Blood samples were taken daily from Days 108 to 116. On Day 113, blood samples were taken hourly from 0500 to 0900 h and then every 30 min until 24

Effect of lichen sclerosus on sexual function and parturition.

Women attending a dermatologic vulvar clinic were asked to reply by mail to an anonymous questionnaire asking simple questions about sexual function and pregnancy following the development of lichen sclerosus. Additional comments were invited from respondents. The majority of women of all ages reported that lichen sclerosus had a detrimental effect on sexual function, with problems including dyspareunia, apareunia and difficulty achieving orgasm. The symptoms may relate to continuing inflammatory disease as well as to anatomic changes and scarring from long-standing active disease. Disease-sup
