Analysis of Isolated Patella Fractures: Do they Affect Walking?

The patella is an important part of the extensor mechanism of the knee. Due to its subcutaneous location, it can be easily exposed to trauma. In this study, we aimed to clarify the effect of patella fractures on walking by examining the results of gait analysis of patients with patella fractures.Patients who were treated due to a patella fracture in the last 10 years were selected from the hospital archive. A total of 18 patients and 36 healthy control subjects were included. Clinical, radiological, and functional results were collected for the fracture group retrospectively, and gait analysis

Operative management of periprosthetic patella fractures: A case series.

BACKGROUND: Periprosthetic patella fractures (PPPFs) following total knee arthroplasty (TKA) are rare but challenging complications associated with poor outcomes and high failure rates. Optimal surgical fixation strategies remain unclear, particularly in the setting of compromised bone stock and extensor mechanism disruption. QUESTIONS/PURPOSES: We aimed to (1) evaluate clinical and radiographic outcomes following operative fixation of PPPFs, (2) compare results across fixation methods, and (3) assess functional recovery using extension lag, ambulation status, and PROMIS Physical Function sco

Developments in the epidemiology and surgical management of patella fractures in Germany.

BACKGROUND: Patella fractures account for approximately 1% of all skeletal injuries. Treatment options are vast and no definitive conclusion on what option is the most beneficial could be made so far. Plate osteosynthesis appears to gain in importance. We aim to give insight into the more recent trends and developments as well as establish the epidemiology of patella fractures in Germany by analysing treatment and epidemiological data from a national database. METHODS: Anonymised data was retrieved form a national database. In the period of 2006 to 2020, all patients with patella fractures as

Crossing Suture Technique for the Osteochondral Fractures Repair of Patella.

Osteochondral fracture of the patella is a common concomitant injury of the knee, especially in lateral patellar instability, and the importance of early stable fixation with minimal complication and early mobilization should be emphasized. Screws and Kirschner wires both absorbable and nonabsorbable have been the common mode of fixation of these fractures. Nevertheless, these fixation techniques require larger osteochondral fragments and are associated with cartilage abrasion, hardware prominence, synovitis, and foreign body reaction. In contrast, suture fixation can adequately stabilize smal

Sleeve fractures of the patella in children: a report of three cases.

The clinical characteristics of sleeve fractures of the patella are described and the methods of treatment are reviewed. Attention is drawn to the fact that the diagnosis may be missed because the distal bony fragment may be so small as not to be detectable by radiography. This is important because a large fragment of articular cartilage also separates. The best result was obtained from reconstitution of the extensor apparatus by internal fixation of the patellar fracture with repair of the quadriceps expansion.

A new technique for the treatment of comminuted, transverse fractures of the patella.

In an attempt to re-establish continuity of the quadriceps mechanism following partial patellectomy more securely in patients with comminuted, transverse fractures of the patella, a technique whereby the patellar ligament is inserted into a transverse groove created in the proximal patellar fragment and this anastomosis is then protected by a tension band wiring, is described and depicted. Using this method, it is possible to provide a larger surface area fro more satisfactory tendon to bone healing and to initiate relatively early motion of the knee, without compromising the site of reconstit

The fractured patella: a long-term follow-up study.

One hundred and three patients with 104 patellar fractures were examined over a follow-up period ranging from three to 11 years. Patients were assessed subjectively and objectively. The fractures were classified into three groups, transverse, comminuted and polar. Four methods of treatment were used, conservative, internal fixation, partial excision and total excision. In general, the results in the majority of patients were satisfactory, those patients who underwent total patellectomy faring at least as well as any others requiring surgical intervention.

Revision Patella Tendon Repair With Hamstring Tendon Autograft Augmentation Following Failed Inferior Pole Patella Fracture Open Reduction and Internal Fixation.

A 46-year-old man presents with left knee bucking 3 months after open reduction and internal fixation of a left knee inferior pole patella fracture. Radiograph and magnetic resonance imaging evaluation reveal displacement and comminution of the inferior pole of the patella confirming failure of the repair. The purpose of this video is to demonstrate the surgical management of failed inferior pole patella fractures using suture anchors for repair of the patella tendon and augmentation with ipsilateral autologous hamstring grafts.

Outcomes of Variable-Angle Locking Anterior Patella Plating for the Treatment of OTA 34-C3 Patella Fractures.

The gold standard for patella fracture fixation is tension band wiring; however, achieving stable anatomic fixation can be challenging in comminuted patterns. The "star" variable-angle locking patella plate is an alternative fixation construct that is meant to address these limitations by providing multiple fixation points and dorsal cortical stability. The purpose of this study was to analyze the outcomes of patients treated with the variable-angle locking "star" patella plate. A total of 358 patients who underwent repair of a displaced patella fracture over a 10-year period were reviewed. Pa

Management of patella fractures associated with central third bone-patella tendon-bone autograft ACL reconstructions.

Patella fractures are an acknowledged complication of cruciate ligament reconstructions that use bone-tendon-bone autografts. Two fracture patterns are seen: a nondisplaced longitudinal patellar fissure fracture recognized interoperatively and the classic displaced transverse fracture that disrupts the extensor mechanism. Rigid fixation techniques are described for each fracture type that minimally affect rehabilitative efforts.

Open lateral femoral condyle fracture concomitant with a vertical patella fracture in an adolescent: A case report.

We present the rare case of an open femoral condyle fracture (Gustilla IIIA) combined with a vertical patella fracture due to sharp injury after a traffic collision in a 17-year-old female. During subsequent operation, a locking compression plate was used for definitive fixation of the distal femur and the patella was reduced and fixated with two screws. Four months after surgery, the patient had practically full range of motion again and no secondary complications occurred. On control X-ray, there was consolidation of the fracture.

Surgical Outcomes Following Patella Fracture Repair: A Single-Center Retrospective Cohort Study.

Introduction Patella fractures are an uncommon yet complex injury, accounting for approximately 1% of presentations to orthopedic departments. Surgical management is indicated for unstable, displaced, or comminuted fracture patterns, with the aim of restoring the extensor mechanism and preserving knee function. Various surgical techniques are available, each with differing complication profiles. This study evaluated surgical outcomes following patella fractures over a 15-year period within a single-center orthopedic department. Methods A retrospective cohort study was conducted. Open Reduction and Internal Fixation of Patella Fracture With Contoured Dorsal Plating.

In this article, we present a novel patella fixation technique using a low-profile highly contoured dorsal mini-fragment locking plate. This procedure is ideally indicated in patients with fracture patterns that are transverse without significant comminution. Long-term clinical studies are being performed to evaluate the effectiveness of this surgical technique.

Patella fracture fixation with suture and wire: you reap what you sew.

INTRODUCTION: Operative fixation of displaced inferior pole patella fractures has now become the standard of care. This study aims to quantify clinical, radiographic and functional outcomes, as well as identify complications in a cohort of patients treated with non-absorbable braided suture fixation for inferior pole patellar fractures. These patients were then compared to a control group of patients treated for mid-pole fractures with K-wires or cannulated screws with tension band wiring. METHODS: In this IRB approved study, we identified a cohort of patients who were diagnosed and treated s

Atraumatic Bilateral Patella Fracture in Middle-Aged Female: A Rare Case Report.

The patella is the largest sesamoid bone in the body and an important structure of the extensor apparatus which is under undue stress during flexion and extension of the knee. Bilateral fracture of the patella without trauma is a very rare event and may be multifactorial without a single cause. A repetitive stress reaction in a previously predisposed bone can be an important cause. We report a case of a 45-year-old female with a nontraumatic bilateral transverse patella fracture with loss of extensor mechanism. A stable surgical fixation for such a displaced fracture with a good rehabilitation

Non-operative management of a patella fracture: environmental considerations in the Subantarctic.

Injuries occurring in the remote environment present particular challenges to healthcare professionals, and decisions need to be carefully made on an individual basis. This report describes the successful management of a patella fracture sustained on the Subantarctic island of South Georgia. A 36-year-old boating officer presented to the island surgery after sustaining an isolated closed injury to his left knee. On physical examination there was a large effusion palpable within the joint, and on subsequent radiography an undisplaced transverse fracture of the patella was apparent. The patient
